RESUMO

Objective: To compare the sedation induced by target-controlled infusion of propofol with that by propofol-remifentanil in third molar exaction surgery. Methods: 60 patients for third molar exaction were divided randomly into 2 groups(n = 30): group P(propofol group) and group PR(propofol-remifentanil group). In group P,a titrated infusion of propofol was started until the OAA/S score had reached level 3 in the patients,then the surgery began. In group PR,a infusion of remifentanil with a target plasma concentration of 1 ng /ml and a titrated infusion of propofol was started until the OAA/S score had reached level 3,then the surgery began. In all patients,the heart rate,blood pressure,respiratory rate,oxyhemoglobin saturation and narcotrend index were recorded during the operation. The reactions of the patients in the operation were recorded. The satisfaction of the patients and surgeons was asked. Results: The respiratory rate and the oxyhemoglobin saturation in group PR was lower than those in group P(P < 0. 05). No obvious adverse reaction was observed in the 2 groups. The satisfaction of the patients in the 2 groups was 30 /30 and 30 /30(P> 0. 05). Conclusion: The sedation induced by target-controlled infusion of propofol or propofol-remifentanil in third molar extraction is safe. The sedation under target-controlled infusion of propofol-remifentanil is better than that by propofol when inhalating oxygen.

RESUMO

Bilateral mandibular first molars of 36 rabbits were extracted.The tooth extraction wounds were treated by:platelet-rich fibrin +acellular dermal matrix(group A),platelet-rich fibrin(group B),acellular dermal matrix (group C) and without treatment (group D,the control) (n =9) respectively.The measurments of alveolar bone height and width showed that there were no significant differences among groups at different times(P > 0.05).Bone histomorphometry showed that at the 2th and 4th week,the best result was found in group A(P<0.01).While,at the 8th week,the result of group A was still better than that of other 3 ones (P < 0.01),but group B and C showed no significant difference(P > 0.05).The combination of PRF and ADM shows the most significant effect.

Objective:To explore the influence of buccal corridor of Han people on smile esthetics.Methods:An attractive adult male and a adult female were selected as the models.Buccal corridor was altered digitally with slider technology of Adobe Flash CS4 to obtain a continuous range of buccal corridors(0% -25%).96 orthodontists aged 35.1 ±7.2 years and 96 laypersons aged 37.3 ± 5.1 years were chosen as the raters.The minimum tolerable value(A%),the ideal value(B%)and the maximum tolerable value (C%)of buccal corridor of the models were statistically analyzed.Results:In the orthodontist groupA,B and C of the male model were 5.00 ±0.1 7,9.75 ±2.77 and 1 5.00 ±2.84,in layperson group were 4.79 ±1 .00,9.20 ±3.08 and 1 5.05 ±2.91 ,respec-tively;in orthodontist group,A,B and C of the female model were 3.92 ±0.1 7,1 1 .87 ±2.77 and 1 5.82 ±2.84,in layperson group were 4.00 ±1 .00,1 2.05 ±3.08 and 1 5.1 1 ±2.91 ,respectively(all data between groups,P >0.05).The ideal buccal corri-dor value(%)of the male and female models were 9.48 ±2.73 and 1 1 .96 ±1 .99 respectively(P <0.05).Conclusion:There is no difference between orthodontists and laypersons for buccal corridor esthetic judgment.The ideal esthetic buccal corridor size of male and female is different.

Objective:To analyze the dental and mandibular asymmetry of adults with Class Ⅱ subdivision malocclusion.Methods:The jaw bones of 30 adults with Class Ⅱ subdivision malocclusion(case group)and 30 with normal-occlusion(control group)were scanned by CBCT.Linear and angular comparison was conducted between the two groups.Results:Dental midline deviation was ob-served in case group,mostly in mandibular arch (60%).The development of Class Ⅱ molar relationship correlated mainly to distally positioned mandibular molar on Class Ⅱ side.Conclusion:In the adults with Class Ⅱ subdvision malocclusion odontogenic asymme-try is the major factor,bony asymmetry is the miner.

<p><b>OBJECTIVE</b>To evaluate the morphology and position of condylar processes in skeletal Class III adult patients with mandibular deviation.</p><p><b>METHODS</b>Twenty skeletal Class III adult patients with mandibular deviation were selected as the deviated group and twenty adults with normal occlusion were selected as the control group. Cone- beam CT (CBCT) scans of their temporomandibular joints were taken. The original data were reconstructed and rebuilt. A series of measurements of the condylar morphology and location were performed. The collected data were analyzed using paired t- test.</p><p><b>RESULTS</b>In the deviated group, the deviated sides were smaller than the contralateral sides in the perpendicular height, volume, maximum area of axial plane and the height of condylar head. The contralateral condyle was positioned more posteriorly and laterally [(0.35 ± 0.10) and (0.58 ± 0.18) mm] than the deviated condyle (P < 0.05).</p><p><b>CONCLUSIONS</b>Adult skeletal Class III patients with mandibular deviation had some extent of condyle asymmetry in both morphology and location.</p>

Objective:To analyse the asymmetry of mandibular of adults with skeletal classⅢmalocclusion and mandibular deviation using cone-beam computerized tomography(CBCT).Methods:20 adults with skeletal classⅢmalocclusion and mandibular deviation (deviation group)and 20 adults with individual normal occlusion(normal group)were included.CBCT slices were taken for each sub-ject.INVIVO 5 software was used to make 3D reconstruction and location marks for line distance and volume measurements of mandi-ble and condyle.Results:The measurements of the mandibular ramus length,mandibular total length,hemi-mandibular volume, mandibular ramus volume,posterior part of mandibular body length,condylar unit length,angular unit length,body unit length and an-gular angle in deviation group were significantly different between 2 sides(P<0.05).Compared with the normal group,differences of the asymmetry of mandibular ramus length,mandibular total length,hemi-mandibular volume,mandibular ramus volume,posterior part of mandibular body length,angular angle,condylar unit length and body unit length between 2 sides in deviation group were sig-nificantly different(P<0.05).Conclusion:In adults with skeletal classⅢmalocclusion and mandibular deviation deviated condylar and mandibular ramus are smaller than those of the nondeviated;the posterior part of mandibular body is slightly asymmetric while the anterior part of mandibular body is almost symmetric.

Objective: To understand the effects of different expansion ways on collagen and actin of skin and to provide an experiment foundation for the suitable expansion method clinically. Methods: The rabbits were divided into five groups. The expanders were embedded in experimental group. The sirius-red specific staining method was applied to observe the changes of type I collagen and type III collagen and their proportion. Immunochemistry method was used to detect the expression of actin. Results: 1: In group A, B and C, type I collagen arranged turbulently and was thicker and type III collagen decreased relatively at earlier maintaining period. After 4 week, type I collagen arranged regularly and type III collagen was relatively increased. 2: Comparisons of the area about type I, III collagen: At the same maintaining period, there were no significant difference among the group A,B and C. With the extension of maintaining period, the area of type I, III collagen increased gradually, but there was inclined to stabilize at the fourth week in group A, B and at the third week in group C. Comparison of the proportion of collagen I and collagen III: There were significant difference between group A, B and group D, but no significant difference was observed after the fourth week. There were significant difference between group C and control group, but no significant difference was observed after three week. 3. With the extension of maintaining period, the positive expression rates of actin decreased gradually in group A, B, C. At the same maintaining period, significant difference were observed among group A, B, C. The positive expression rates of actin in group C were lower than that of group A at the same maintaining period. Conclusion: The repeated rapid expansion can not only shorten the clinical period but also promote the quality of skin flap. It is a safe and feasible method in clinic.

Objective:To explore the role of MVD, bFGF and VEGF in OMFS and the relationship between them. Methods: Immunochemistry streptavidin peroxidase method was used to detect the MVD and the bFGF, VEGF expressions in 57 cases of OMFS samples. Results: MVD, bFGF and VEGF expressions were different between OMFS and benign. The close correlations were found among the expressions of VEGF, bFGF and MVD. Conclusion: The expressions of VEGF and bFGF are associated with MVD which indicated that VEGF and bFGF may play important roles in angiogenesis.

OBJECTIVE: With the prolongation of life span, more and more people care their health. In this paper we summarized the studies about correlative factors of mandible osteoporosis so as to investigate the cause of osteoporosis and how to inhibit or postpone the occurrence of mandible osteoporosis and improve people's quality of life.DATA SOURCES: We searched Qsinghua Tongfang Database for papers about mandible osteoporosis published between January 1991 and May 2005 with the key words of "osteoporosis, mandible bone, alveolar bone"in Chinese. Meanwhile, related English literatures were also searched in Medline between January 1975 and May 2005 with the same key words.STUDY SELECTION: The literatures included all the related papers of correlative factors of mandible osteoporosis. Inclusive criteria were: the study types were randomized controlled trials, drug stress test and clinical drug effect test; the samples were both human and animals. Exclusive criteria: Reviews and literatures without controls were excluded.DATA EXTRACTION: Totally 182 literatures related to the correlative factors of mandible osteoporosis were collected, and 36 were accorded with the inclusive criteria. The excluded were 146 papers of repeated drug effect study.DATA SYNTHESIS: A total of 36 papers including 5 321 human and 152 animals analyzed the data of correlative factors of mandible osteoporosis in age, sex, hormones, biting force, physical training and living habit,etc., respectively, which included 6 interfere-test papers of ovariactomized rats.CONCLUSION: It is concluded that mandible osteoporosis can result in a part of bone mine loss, which is related to age, sex, hormones, biting force,physical exercise and living habits, and so on.

Objective:To determine the effects of angiogenesis in c ervical lymph node metastasis and progression of squamous cell carcinomas of gin giva (GSCC), and the distribution and expression of vascular endothelial growth factor (VEGF) in GSCC.Methods: Paraffin sections of surg ically obtained GSCC samples from 42 patients were stained with CD34 monoclon al antibody by immunohistochemical S P method to demonstrate blood vessels. Exp ression of VEGF was tested with the S P method, and the microvessel density (M VD) was determined according to the percent of the neoplastic cells showing VEG F immunoreactivity and the degree of staining.Results: The MVD in GSCC was significantly higher than that in normal gingival tissue( P 0.05). Objective:To observe the expression of PCNA and morphological changes of submandibular gland of db/db mutant diabetic mice. Methods:Fifty C57BL/Ksj-db/+m mice were randomly divided into 2 groups, db/db diabetic group and control group. The morphological changes of submandibular gland and the expression of PCNA were observed by using light microscope (LM) and immunohistochemistry. Results:Submandibular gland atrophy and the significant morphological changes in acini and ducts were observed in db/db diabetic mice. Objective:To investigate the relationship between the expression of P16 protein and the clinico-pathology of squamous cell carcinoma(SCC) of buccal mucosa and its precursor lesions. Methods:The immunohistochemical stain against p16 was performed in 30 cases of SCC of buccal mucosa, 32 cases of buccal leukoplakia and buccal lichen planus and 10 cases of normal buccal mucosa. All data were analyzed quantitatively by imag analysis technique. Then, the results were compared with clinico-pathological parameters.Results:The positive expression of P16 protein was found in all normal buccal and hyperplasia mucosa (100.0%), in 9 out of 10 cases of atypical hyperplasia (90.0%), in 12 out of 30 cases of SCC of buccal mucosa (40.0%). RESUMO

Objective:To investigate the effects of survivin antisense oligonucleotide (ASODN) on expression of survivin and ACC-M cell apoptosis. Methods: A phosphorothioate antisense oligonucleotide (ASODN) of specific target survivin was designed and synthesized and then transferred to ACC-M cell line by lipofectin. At the same time blank control group, sense oligonucleotide (SODN) group were set up for comparison. MTT assay was used to detect cytotoxicity. Apoptosis was observed by flow cytometry. Survivin expression was determined by RT-PCR and Western-Blotting. Results: Compared with control group and SOND group, in ASODN groups, the expression of survivin mRNA and protein were obviously weak, apoptosis rate apparently increased, cells growth was inhibited. There was no obviously difference in SODN and control groups.Conclusion:ASODN can down-regulate the expression of survivin gene in ACC-M cell line specifically. It plays an important role in inducing tumor apoptosis and suppressing cell proliferation.
