RESUMO

Nevus sebaceus is a hamartoma of both epithelial and non-epithelial components, which that undergoes age-related change. It commonly affects the head and neck, particularly the scalp. In 10-20% of nevus sebaceus cases, secondary benign or malignant neoplasms such as syringocystadenoma papilliferum, trichoblastoma or basal cell carcinoma occur between 40-80 years of age. In only rare instances does a sebaceous carcinoma develop within a nevus sebaceus. To our knowledge, only 3 cases of sebaceous carcinoma in nevus sebaceus have been reported in the Korean dermatologic literature. We present a case of a sebaceous carcinoma arising from nevus sebaceus in a 46-year-old Korean man.

RESUMO

Eosinophilic pustular folliculitis (EPF) is a rare dermatosis of unknown cause. It is characterized by recurrent crops of sterile, pruritic, follicular papulopustules on the face, trunk, and extremities. Although various treatments have been attempted for EPF, including systemic corticosteroid dapsone, cetrizine, UVB phototherapy, as well as cyclosporin, no uniformly effective treatment has been found. We describe a 20-year-old man with long standing EPF which was resistant to various treatments, but who responded dramatically to indomethacin.

RESUMO

Neonatal lupus erythematosus(NLE) is a distinct subset of lupus characterized by cutaneous findings, cardiac conduction defects, hepatic or hematologic abnormalities. These manifestations are associated with the presence of maternal auto-antibodies such as anti-SSA/Ro, anti-SSB/La, and rarely anti- RNP(U1RNP) antibodies. Cases of U1RNP antibody-positive NLE have somewhat atypical cutaneous manifestation without cardiac or systemic abnormalities. We report a case of cutaneous NLE associated with U1RNP antibodies.

BACKGROUND: In accordance with the increase of the old age group, the incidence of actinic keratosis (AK) also increases. But there has been no comparative study between the clinical and histopathological margins. OBJECTIVE: The purpose of this study was to compare the clinical margin with histopathological margin of AK in Korean patients. METHOD: All cases of AK confirmed by biopsy from February 1, 1993 to May 31, 2003 were reviewed by hospital records and biopsy slides. Total 50 AK cases excised with 2mm surgical free margin were selected and analyzed clinically and histopathologically. RESULTS: 1. Histopathologically, hypertrophic type of AK was noted in 18 cases (36%), atrophic type in 8 cases (16%), bowenoid type in 5 cases (10%), acantholytic type in 3 cases (6%), pigmented type in 2 cases (4%), and combined form of hypertrophic and atrophic types was noted in 7 cases (14%), combined form of atrophic and pigmented types in 5 cases (10%), and combined form of acantholytic and pigmented types in 2 cases (4%). 2. The total proportion of surgical margin involvement was 44% (22/50): hypertrophic type in 55.5% (10/18), atrophic type in 87.5% (7/8), bowenoid type in 20% (1/5), acantholytic type in 33.3% (1/3), pigmented type in 0% (0/2), combined form of hypertrophic and atrophic types in 28.5% (2/7), combined form of atrophic and pigmented types in 20% (1/5), and combined form of acantholytic and pigmented types in 0% (0/2). CONCLUSION: In 50 cases of AK excised with 2mm surgical free margin, 22 cases (44%) showed tumor cells on the surgical margin. Especially hypertrophic and atrophic types showed significant proportion of marginal involvement. In respect that hypertrophic and atrophic types are more common than other subtypes of AK in Korean patients, skin biopsy and check-up for surgical margin are recommended in all AK.

RESUMO

Neonatal lupus erythematosus(NLE) is a rare disease characterized by the transplacental passage from the mother to the fetus of autoantibodies, particularly autoantibodies of Ro family. The patient with NLE exhibits one or more of the following findings: congenital heart block, cutaneous lupus lesions, hepatobiliary disease and hematologic disorders(thrombocytopenia, anemia). We report a case of NLE in a 2-week-old male infant, born of a clinically asymptomatic mother, presenting multiple, round, target-like lesions which have not been reported in the English and Korean literature. Both infant and mother were positive for anti-SSA/Ro and anti-SSB/La.

RESUMO

Eosinophilic ulcer of the tongue, which is characterized by an ulcer with indurated and elevated border that is usually covered by a pseudomembrane, is a benign reactive lesion. It rapidly develops, and spontaneously resolves in a few weeks. It occurs mostly on the tongue, and has a peak onset in the fifth to sixth decades of life. We experienced an eosinophilic ulcer of the tongue in a 65 years old man. The patient showed a single ulcer on right side of his tongue, but had neither subjective symptom nor previous oral or genital ulcers, arthralgia, and erythema nodosum-like skin lesions. The ulcer was 1.2 cm sized with indurated border. Histopathologically, the lesion exhibited ulceration, and a mixed, dense cellular infiltrate composed of eosinophils, lymphocytes, and neutrophils extended from the dermis into muscular layer. The lesion had completely disappeared in two weeks, and did not recur yet for follow-up period of nine months. Only one case of eosinophilic ulcer of oral mucosa have been reported in the Korean dermatological literature, so it is the second case of eosinophilic ulcer of the tongue in Korea.

RESUMO

Nevus lipomatosus superficialis is a fairly uncommon disease characterized by ectopic fat tissue in the dermis. Lesions most commonly occur on the lower trunk, especially on the back, buttocks or abdomen. Rarely, lesions also occur on the knee, axilla, arm, ear and scalp. To our knowledge this is the first case of nevus lipomatosus superficialis developing on the dorsal foot.

RESUMO

Cutaneous or subcutaneous metastases occur in 2% to 9% of visceral malignancies. Skin metastases are seen most commonly from carcinomas of the breast, lung, large intestine. Skin metastases of prostatic origin are quite uncommon and preferentially localized to the lower abdomen and genital area. Two cases of cutaneous metastases of prostatic origin have been reported in the Korean dermatological literature in our review. We report a case of cutaneous metastases from prostatic cancer

RESUMO

Lichen striatus is an uncommon, self-limited, unilateral, linear dermatosis of unknown etiology, ordinarily seen in children from 5 to 15 years of age. Clinically lichen striatus consists of a continuous or interrupted band of discrete or confluent, flat-topped, smooth or scaly papules. Multiple lesions are so rare that only 3 cases have been reported in the Korean literature. We reported a rare case of multiple lichen striatus in a 10-year-old girl. She had linear erythematous papules and brownish macules on the lower back, left shoulder, left chest, and left axillar area. The biopsy revealed that spongiosis and vacuolar degeneration in the epidermis, a lichenoid inflammatory infiltrate in the upper dermis and a dense lymphocytic infiltrate around the eccrine glands, ducts and hair follicles. The patient was treated with topical steroid. The lesions disappeared with minor hypopigmentation after 4 months.

RESUMO

Florid cutaneous papillomatosis(FCP) is a disease consisting of multiple acuminate keratotic papules in association with an underlying malignancy. Most common underlying malignancy is gastric adenocarcinoma. Other associated malignancies are breast cancer, bladder cancer, hepatobiliary cancer, ovarian cancer and uterine cancer. An association of FCP with other signs of internal cancer is common, with malignant acanthosis nigricans usually appearing many times with sign of Leser-Trelat. We experienced a florid cutaneous papillomatosis associated gastric adenocarcinoma in an 84 years old woman. The patient had a severe itching and showed numerous tiny papules on the trunk(FCP) and brown verrucose papules on the axillae(acanthosis nigricans) and hyperkeratotic yellowish tiny papules on the palms(tripe palm). Histopathologically, the lesion of the trunk exhibited papillomatosis, hyperkeratosis, and acanthosis in the epidermis. Gastroscopic biopsy revealed gastric adenocarcinoma.

RESUMO

The cutis marmorata telangiectatica congenita(CMTC) is characterized by the presence of purplish, reticulated vascular network with a localized or generalized distribution. The condition is present at birth and tends to improve with age, although some patients have lesion that remains relatively fixed throughout life. Multiple birth defects have been found in 50% of patients with CMTC. We experienced a case of CMTC in a 5-days-old infant. The patient had purplish reticulated patches with telangiectasia on the both arms, chest, abdomen, back, and both legs. The area of involved skin showed an atrophic change. There were no other associated anomalies. Also, we reviewed cases previously reported in Korea.
