RESUMO

Androgen receptor (AR) plays a key regulatory role in the development of castration resistant prostate cancer (CRPC), and the level of constitutive active variants represented by androgen receptor variant 7 (AR-V7) is increasing during the progress of CRPC, which can be used as a molecular marker of disease progress and prognosis of patients with CRPC. It is an important target to overcome castration resistance and improve the quality of life and survival of patients. In this paper, the function of AR-V7 and its molecular regulation mechanism in CRPC are reviewed. The research shows that the generation of AR-V7 is related to the structural rearrangement of AR gene, gene amplification and the selective splicing of AR gene transcripts, and it is affected by the coordinated regulation of multiple signal pathway molecules such as TGF-β; AR-V7 changes the transport and nuclear localization mechanism of AR protein, and further affects the transcriptional expression of downstream target genes. AR-V7 antagonizes AR activity and blocks the differentiation process driven by AR and androgen, and inhibits the expression of tumor suppressor genes to stimulate the proliferation of tumor cells, thus promoting the progress of Pca. Related targeting studies have revealed AR-V7 targets and CRPC treatment strategies. Currently, they mainly focus on AR-V7 protein degradation, mRNA expression inhibition and N-terminal domain targeting intervention. With the development of in-depth research, the molecular mechanism of AR-V7 in the progress of Pca will be gradually clarified, which will certainly play a greater role in the prevention and treatment of CRPC.

Objective To study the relationship between the individualized nutritional intervention during pregnancy with childbirth way and neonatal birth weight , provide clinical data for nutrition intervention treatment . Methods The outpatient pregnant women were randomly divided into intervention group and control group according the nutritional intervention intentions from January to June in 2014.The incidence of pregnancy specific disease , childbirth way and neonatal birth weight were recorded .Results The incidence of special diseases during pregnan-cy, cesarean section and abnormal neonatal birth weight in intervention group were lower than the control group , and the differences were statistically significant (p<0.05).Conclusion Individualized nutritional intervention during pregnancy can reduce the incidence of pregnancy specific disease , macrosomia and cesarean section .

Objective:To explore the impact of clinical pharmacists involved in the treatment team on the reasonable drug use rate in medical orders. Methods:Totally 782 medical records including hypertension and 1-3 kinds of hypertension associated diseases in our hospital from January 2011 to December 2011 were used as the pre-intervention group, and 1 061 ones with the same conditions from September 2012 to August 2013 were used as the post-intervention group. The patients of the two groups were divided into 8 dis-ease subgroups, clinical pharmacists performed timely intervention on the medical orders in the post-intervention group, and the rational drug use rate between the two groups was compared. Results:In hypertension, hypertension associated coronary heart disease, hyper-tension with heart failure and hypertension with diabetes subgroups,the difference in the rational drug use proportion before and after the intervention was statistically significant(P0. 05). After the medical records in the 8 sub-groups were aggregated, the difference in the rational drug use proportion before and after the intervention was statistically significant( P<0. 01). Conclusion:Clinical pharmacists should involve in the clinical treatment to intervene irrational drug use,which can signifi-cantly improve the medical record rationality, avoid the health risks as much as possible and ensure the medication safety and treatment effectiveness for patients.

Objective To explore the prognostic significance of bone marrow examination at different time points by using different methods during induction therapy.Methods From Feb.2004 to Jan.2013,268 newly diagnosed (B acute lymphoblastic leukemia,B-ALL) pediatric patients in Peking University People's Hospital,were enrolled for the study.In this study,the overall survival (OS) ratio and event-free survival(EFS) ratio of patients with different bone marrow morphology on day 8 and day 15 were analyzed.Based on different cut off value of minimal residual disease (MRD) on day 15 and end-of-induction,the OS ratio and EFS ratio of the higher patients and the lower patients were compared.Results Patients with M1,M2 or M3 marrows on day 8 had no significant difference in OS ratio (P =0.372) or EFS ratio (P =0.393).When it came to day 15,patients with M1,M2 or M3 marrows had no significant difference in OS ratio (P =0.050) or EFS ratio (P =0.324).Patients with MRD > 10.00% on day 15 had lower OS ratio than those with MRD ≤ 10.00%,and it had significant difference(P =0.022).But there was no significant difference in EFS ratio (P =0.191).As for MRD on the end-of-induction,when using the MRD level of 0.0l %,0.10%,1.00% as cut-off values,the lower group of end-of-induction MRD was significantly associated with a higher probability of OS ratio and it had significant differences(P =0.018,0.006,0.002),and it showed the same results at EFS ratio (P =0.002,0.000,0.000).Conclusions The bone marrow morphology on day 8 and day 15 during induction therapy had no prognostic significance.The MRD of day 15 had prognostic significance when using 10.00% as the cut off value.The critical value of MRD on the end-of-induction MRD should be 0.01% for the prognosis.

Objectives To compare the efficacy and adverse effects of combining all-trans retinoic acid and arsenic triox-ide with or without anthracyclines on the treatment of childhood acute promyelocytic leukemia (APL) patients. Methods The retrospective study included 46 children as newly diagnosed APL from January 1st, 2001 to December 31st , 2012. Efficacy and adverse effects for different induction therapies and in high and low white blood cell (WBC) count subgroups were studied. Results In the non antharcycline containing group, 2 patients died during remission induction, and in the antharcycline containing group none of the patients died. No statistical difference was observed between the antharcycline containing group and the non antharcycline containing group in complete remission, the length of time to achieve molecular complete remission and minimal residual disease quantitative analysis at the end of the induction. The mean duration of high WBC count subgroup in the anthar-cycline containing group was shortened than that of the non antharcycline containing group (P<0.05). The recovery time of the abnormal coagulation was found similar between these two groups. Conclusions The use of antharcycline in induction therapy could shorten the duration of high WBC count and reduced the WBC count peak , thus reduces the risk of early death.

Objective To analyze the clinicopathologic features,prognosis and influencing factors of invasive cervical adenocarcinoma.Methods Clinical and pathologic data of 96 cases of invasive cervical adenocarcinoma and 100 cases of squamous carcinoma of the cervix treated in Shanxi Cancer Hospital from years of 1998 to 2007 were retrospectively analyzed.Results The morbidity of cervical adenocarcinoma was showing the arise and low-age trend.The overall 5-year survival rate was 60.4 %,The overall survival rates of patients with stage Ⅰ,Ⅱ,Ⅲ,and Ⅳ were 82.9 %,51.5 %,13.3 % and 0,respectively.Ocervical adenocarcinoma had many prognosis-related factors:clinical stage,lymmphatic spread,vascular invasion,volume of tumor,histologic grade.Multivariate analysis showed that clinical stage,lymph nodal metastasis and vascular invasion were independent prognosis-related factors.However,whether ovary was reserved,it was irrelative to prognosis.Conclusion The morbidity of cervical adenocarcinoma is showing the arise and low-age trend.Earlier diagnosis is extremely important for improving the survival rate of cervical adenocarcinoma,the reserved ovary doesn't influence the overall 5-year survival rate for young women with early cervical adenocarcinoma.

Objectives To investigate the inlfuence of polymorphisms of SLC19A1 80G>A, MDR1 exon26C>T and MDR1 exon21G>T/A on curative effect and adverse reaction of high-dose methotrexate in patients with acute lymphoblastic leukemia. Methods MALDI-TOF-MS technique was used to detect the polymorphisms of SLC19A1 80G>A, MDR1 exon 26C>T and MDR1 exon21G>T/A in 108 patients with acute lymphoblastic leukemia (ALL). The relationship of genetic polymorphism, survival rate and toxicity was analyzed. Results The 36-month event-free survival was not related to any polymorphisms of MDR1 and SLC19A1. Patients with mutant types of MDR1 exon26C>T and MDR1 exon21G>T/A showed a much higher MTX plasma levels at 24 hours and higher incidence of hepatic injury (PT, MDR1 exon21G>T/A has a large inlfuence on hepatic toxicity and plasma concentra-tions of MTX.

<p><b>OBJECTIVE</b>To compare the ettect and side-ettect of fluoxetine and combination of fluoxetine and Chinese or Tibetan medicine in treating senile depression in plateau district. Methods Ninety patients with diagnosis of senile depression conformed to CCMD-3 standard, in plateau district of 2260 - 3200 m altitude were randomly divided into three groups and treated with fluoxetine (group A), fluoxetine plus Sanpu Xinnao Xin granule (group B) and fluoxetine plus Xiaoyao pill (group C), respectively, 30 cases in each group. Therapeutic effects were evaluated with Hamilton' s depressive scale (HAMD) and treatment emergent symptom scale (TESS) after 6 weeks treatment.</p><p><b>RESULTS</b>There was no significant difference in the therapeutic effects between the three groups. The adverse reaction in Group B and C was less than that in Group A (P<0.01). Conclusion Sanpu Xinnao Xin granule and Xiaoyao pill can raise the tolerance of patients with senile depression in plateau area against the adverse reaction of fluoxetine.</p>

Objective: Effects of Shenmai injection and aminophylline alone or in combination on transdiaphragmatic pressure and electrical activity of diaphragm were observed, so as to evaluate their effecfs on diaphragmatic fatigue. Methods: The diaphramatic fatique (DiF) model was estab- lished by stimulating bilateral phrenic nerves for 40 minutes. The animals were allocated to three groups: Shenmai group (2mL/kg, N=7), aminophylline group (20mg/kg, N=7) and combi- nation group (Shenmai injection 2mL/kg+aminophylline 20mg/kg, N=7) The transdi- aphragmatic pressure (Pdi) and diaphragmatic electromyogram (EMGdi) were recorded before and after treatment. EMGdi was analysed by computer and then the high/low frequency ratio (H/L) and the central frequency (Fc) were caculated. The diaphragm evoked potential (DEP) was record- ed simutaneously. All data was analysed by analysis of variance and q test. RESUMO

Experiments were performed on spontaneously breathing rabbits anesthetized with urethan. The diaphragmatic fatigue (DiF) was reproduced by the phrenic nerve stimulation (30 Hz, 10 V,0. 2 ms). The transdiaphragmatic pressure (Pdi) and diaphragm evoked potential (DEP) were measured before injury and after injury 30, 60, 90, and 120 min. Pdimax and the amplitude of DEP obviously decreased and the latency of DEP obviously delayed after injury 30min. The amplitude of Pdi and DEP were quickly promoted by introvenous injection of neostig-mine after DiF. The latency of DEP was partially recovered by introvenous injection of neostig-mine. It is concluded that neostigmine can improve the generation of the fatigued diaphragm. and promote its recovery.
