RESUMO

【Objective】 To summarize the key points of robot assisted radical nephrectomy combined with resection of metastatic lymph nodes around inferior vena cava. 【Methods】 The patients undergoing the operation during Jan.2019 and Dec.2021 were analyzed and followed up. The surgical procedures and key points for right renal cancer with huge lymph node metastasis around inferior vena cava were illustrated. 【Results】 A total of 5 patients completed operation successfully, including 4 cases of clear cell carcinoma and 1 case of papillary carcinoma. The average operation time and estimated blood loss were 135 min and 300 mL, respectively, with no major complications. 【Conclusion】 It is feasible to perform robot assisted radical nephrectomy with resection of metastatic lymph nodes for selected patients of renal cancer especially with large lymph node metastasis around vena cava. The value of minimally invasive surgery in the comprehensive treatment of renal cancer deserves further attention and research.

RESUMO

Prostate cancer is the most common malignancy of male genitourinary system, with a high risk of invasion, recurrence and metastasis in its high risk subtype. Neoadjuvant therapy would provide improvement in therapy after radical prostatectomy for high risk prostate cancers. This review would discuss several hot issues of neoadjuvant therapy and radical prostatectomy for high risk prostate cancer.

RESUMO

Genome-wide association studies use high-throughput genotyping technologies to scan thousands of nucleotides of the entire human genome to investigate the relation of clinical conditions and measurable phenotypes.Bladder cancer is the result of the interaction between genetic variants and environmental factors.Many progresses have been acquired from genome-wide association studies.New genetic regions have been discovered to provide us new strategies for the etiology investigation of bladder cancer.

RESUMO

Objective To investigate the relationship between non muscle-invasive bladder can-cer and polymorphisms of DNA repair genes among Han nationality in Shanghai. Methods From January 2006 to June 2008, 94 patients with non muscle-invasive bladder cancer and 304 controls were enrolled. Known single nucleotide polymorphism(SNP) in the XPC, XPG, XRCC1 genes were detec-ted by TaqMan real-time PCR. After adjusted for age, sex, and smoking, interaction effects of the genotypes and non muscle-invasive bladder cancer risk, genotypes and clinical and pathological features of bladder cancer were analyzed using unconditional Logistic regression. Results After adjusted for age, sex, and smoking, the XPC 939 Lys/Gln, XPC 939Gin/Gin genotype and XPG 1104 Asp/His, XPG 1104 His/His genotype were more frequent in patients with non muscle-invasive bladder cancer, adjusted OR=1.89, 95%CI 1.14-3. 23 and OR=1.07,95%CI 0.86-1.87, respectively. The XRCC1 Arg399Gln polymorphisms were not significantly associated with non muscle-invasive bladder cancer, adjusted OR= 1.15, 95% CI 0.55-2.40. There were no significant associations between tumor clinical and pathological features in patients who possessed either the XPC or XPG polymor-phisms (P>0.05). Conclusion XPC Lys939Gln and XPG Asp1104His may modulate non muscle-invasive bladder cancer risk among Han nationality in Shanghai.

RESUMO

Objective To investigate essentiality of bone scintigraphy in patients with renal cellcarcinoma.Methods The clinical data of 152 patients with confirmed renal cell carcinoma from Jan uary 1999 to June 2007 were retrospectively analyzed.There were 106 men and 46 women with a meanage of 56 years (range 11-86 years).The tumor size was 1.5-20.0 cm (mean 6.0 cm).Accordingto the TNM and AJCC staging classification,88 patients had stage I,included of T1a in 47 cases,T1bin 41; 38 had stage Ⅱ; 16 had stage Ⅲ,included of T3a,in 10 cases,T3b in 6; and 10 had stage Ⅳ.Histological grade was well differentiated in 90 cases,moderately in 43,poor in 19.Ten clinical pa thology factors were evaluated by Logistic analysis to present the significant factors related to osseousmetastatic lesions.Results Overall osseous metastatic lesions were present in 22 of the 152 patients(14.5%).Of the 126 patients with clinically localized,11 cases (8.7%) had osseous metastatic le sions.Of the 16 patients with clinically advanced,5 eases (31.2%) had osseous metastatic lesions.Ofthe 10 patients with metastasis,6 cases(60%) had osseous metastatic lesions.Clinical stage was re lated to osseous metastatic lesions by Logistic analysis.The incidence of osseous metastatic lesionswas 40.9% in localized renal cell carcinoma patients with bone pain.Conclusions Bone seintigraphymay be omitted in patients with clinically localized renal ceil carcinoma(stage Ⅰ,Ⅱ) unless bone pain is pres ent.Bone scintigraphy should be performed in patients with stage Ⅲ or Ⅳ regardless of symptoms.

Objective To investigate the expression of Kruppel like factor 8(KLF8)in human renal cell carcinoma(RCC)specimens and clinical significance.Methods RT-PCR and immunohistochemistry were used to detect the KLF8 mRNA and protein expression respectively in RCC cell strain 786-0,OS-RC-2 with 37 RCC specimens and adjacent normal tissues of the same patients.Results The expression of KLF8 mRNA was high in RCC cell strain 786-0,OS-RC-2,was 0.80±0.28 and 0.41±0.11 in the RCC specimens and normal tissues respectively(P<0.01).The level of KLF8 protein expression of 37 RCC specimens(17 was expressed at++-+++)was significantly higher than that of the normal tissues(29 was expressed at--+)(P<0.01).KLF protein was high expressed in RCC cell strain 786-0,OS-RC-2.The levels of KLF8 mRNA and protein expression were positively correlated with the tumor size,cell differentiation,clinical stage,but not correlated with the patients sex and age.Conclusions The KLF8 mRNA and protein expressions are elevated in the human RCC.KLF8 may be involved in the development and progression of RCC.

RESUMO

Objective To evaluate the surgical technique,urinary continence and clinical efficacy of extraperitoneal laparoscopic radical prostatectomy for prostate cancer. Methods This series included 28 cases of prostate cancer aged 60 -75 years (mean,68 years). The PSA level ranged from 0. 7 ng/ml to 23. 6 ng/ml. TNM classification showed T1N0M0 tumor in 11 cases, T2N0M0 tumor in 15 and T3aN0M0 tumor in 2. All cases underwent extraperitoneal laparoscopic radical prostatectomy. The functional reconstructive technique consisted of preservation of the urethral and bladder outlet sphincter muscles, and reconstruction of the bladder neck. Results All the operations were successful. The operative time was 180 - 380 min( mean, 240 min) ,and the blood loss was 400 - 1200 ml( mean,800 ml) . All the catheter were removed 2 weeks after surgery. Mild urinary incontinence occurred in 3 cases, and it improved after supportive treatment. The PSA level was 0. 02 -0. 10 ng/ml 3 months after operation. No recurrence of prostate cancer was found during the follow-up of 1 month-2 years. Conclusions Extraperitoneal laparoscopic radical prostatectomy is safe and effiective for the treatment of prostate cancer. It can be widely applied in clinical practice.

RESUMO

Purpose To detect the prevention of EGF and GLN on the side effect of TPN. Methods We randomized SD rats into four groups:group A (control),B(STPN),C(TPN+GLN),D(TPN+GLN+EGF).All rats were maintained on their respective diets for 8 days,at which time they were killed and the proximal jejunum was incised for biopsy,the MLN and celiac vein blood were collected for bacterial culture(aerobic culture). Results Group B rat mucosal cell atrophied,DNA and RNA content and plasma cell decreased,the positive rat of MLN bacterial culture was 62.5%.Atrophy in group C alleviated and bacterial translocation rate declined(37.5%).Group D was similar with group A. Conclusions GLN combined with EGF is more effective in protecting gut barrier function and preventing bacterial translocation than GLN alone.

RESUMO

Objective To manufacture rapamycin (RPM)-a new type immunosuppressant made in our country and investigate its immunosuppressive mechanism.Methods The effect of RPM on mouse splenocyte (Tc) proliferation induced by ConA,splenocyte (Bc) proliferation induced by LPS,DTH induced by DNCB and rat GVHR were studied.Results RPM obviously restrained the proliferation of Tc with IC50=1 nmol/L,significantly lower than IC50(10 nmol/L) of CsA (P<0.05).RPM evidently repressed the proliferation of Bc with IC50=1 nmol/L significantly lower than IC50>10 nmol/L of CsA(P<0.05).RPM inhibited murine DTH to DNCB with ED50:1.8 mg/kg,significantly lower than ED50>30mg/kg of CsA(P<0.05).RPM controlled rat's GVHR with ED50=3 mg/kg,significantly lower than ED50≥30 mg/kg of CsA(P<0.05).Conclusions RPM can inhibit lymphocytic proliferation and DTH,GVHR more markedly than CsA and they have combined efforts.

RESUMO

AIM:To examine the role and dynamic changes of mast cell(MC) and its subpopulations in simple and irradiated-wound of rats.METHODS:MC and its subpopulations were estimated using alcian blue-safranin (ABS)double staining RESULTS:(1)The total number of MC in two groups decreased coincidently on day 2 after wounding, and the total MC increased rapidly and reached maximal gradually on day 5 and 7 after wounding, the increment of MC remained consistently 28 day after wounding (2)Both mucosal MC( MMC) and Mix MC decreased obviously on day 2 after wounding, hereafter,they remained the low level all the time However, the CTMC kept in the high level after wounding (3)The Mix MC on day 5 and the total MC during day 5-15 after wounding were lower in irradiated group than in simple wound group CONCLUSION: MC and its subpopulations could delay the healing process of simple and irradiated wound

To study the apoptotic regularity of peripheral lymphocytes in radiation compound wound healing and explore their relationship to radiation delayed wound healing, apoptosis and Bax and Bcl 2 proteins were estimated by in situ terminal labeling (TUNEL) and immunohistochemical method of alkaline phosphatase, respectively. White blood cells in radiation compound wound(RW) group decreased and reached the lowest 3 days after wound, it was still lower in RW group than in simple wound(SW) group 5 days after wound; apoptosis of peripheral lymphocytes of both groups increased, however, the lymphocyte apoptosis in RW group was higher than that in SW from beginning to end in the whole process of wound healing; the expression of Bax protein of lymphocyte increased at different times after wound, and the expression in RW group was obviously higher than that in SW group, while Bcl 2 showed an opposite tendency. It is suggested that reduction of white blood cells and increment of lymphocyte apoptosis are the major causes of radiation delayed wound healing; Bax and Bcl 2 proteins take part in the regulation of lymphocyte apoptosis in wound healing.
