RESUMO

Most mucosa-associated lymphoid tissue (MALT) lymphomas are found in the gastrointestinal tract. The most common site is the stomach, whereas colon MALT lymphomas are rare. There are a few reports of simultaneously diagnosed stomach and colon MALT lymphomas. However, diagnosis of primary colonic MALT lymphoma after complete remission of gastric MALT lymphoma is extremely rare. Although the treatment protocol for gastric MALT lymphoma is well established, there is no consensus protocol for colonic MALT lymphoma owing to its rarity. Herein, we report a case of colonic MALT lymphoma incidentally diagnosed 6 months after completion of radiation therapy for gastric MALT lymphoma and treated via polypectomy, with no recurrence for 3 years.

RESUMO

Intussusceptions are defined as the telescoping of one segment of the gastrointestinal tract into an adjacent distal segment. In the small bowel, intussusceptions are typically caused by benign processes, but can occasionally be caused by inflammatory fibroid polyps, which often present as intussusception and bowel obstruction. These polyps are rare, benign, tumorous lesions in the gastrointestinal tract and are typically observed in the stomach, but can occur anywhere in the gastrointestinal tract. Any case of a jejunojejunal intussusception caused by inflammatory fibroid polyps is considered rare, and we report the case of a 51-year-old woman with an inflammatory fibroid polyp of the jejunum presenting as an intussusception who was successfully treated with a resection.

RESUMO

We report herein a case of gastric undifferentiated carcinoma with focal yolk sac tumor and hepatoid carcinoma differentiation. A 57-year-old man was referred after a gastroscopy for anemia evaluation. Gastroscopy revealed an approximately 3 cm ulcerofungating mass occupying the greater curvature of stomach body. Biopsy results revealed a poorly differentiated adenocarcinoma. Left gastric lymph node was enlarged, but there were no evidence of distant metastasis on the abdominal CT. He underwent a radical subtotal gastrectomy and gastroduodenostomy with dissection of the enlarged lymph nodes. Postsurgical histological examination revealed an undifferentiated carcinoma with focal yolk sac tumor and hepatoid carcinoma differentiation. Immunohistochemical staining revealed that the tumor cells were focal positive for AFP, anti-hepatocyte antibody.

Epithelial gastric polyps can be categorized as being either hyperplastic or adenomatous. Hyperplastic polyps accounts for 75~90% of gastric polyps. In contrast to the adenomatous polyps, the potential of a malignant transformation of hyperplastic polyps was originally believed to be quite low (an incidence of 1.5~2.1%), which is considerably lower than that of an adenoma of the stomach. Most of reported cases of a hyperplastic gastric polyp with a transformation to adenocarcinoma were well differentiated histopathologically. There are few reports of a hyperplastic polyp in which malignant changes developed during the endoscopic surveillance. We report a rare case of a 1.2 cm sized moderately differentiated adenocarcinoma arising in a hyperplastic polyp on the cardia in the stomach with a brief review of the relevant literature.

BACKGROUND/AIMS: The aim of this study was to investigate the value of expression of COX-2 and p53 protein in colonic adenoma with or without malignant change. METHODS: We studied twenty-five cases of colonic adenoma with malignant change and twenty-five cases of colonic adenoma without malignant change treated by colonoscopic polypectomy in Chungnam National University Hospital from January, 1999 to July, 2002. We compared the expression of COX-2 and p53 protein by immunohistochemical stain in both group. RESULTS: In immunohistochemical staining, the expression of COX-2 was demonstrated 68.0% (17 of 25) of colonic adenoma with malignant change and not different with that of colonic adenoma without malignant change. The overexpression of p53 protein was detected immunohistochemically in 88.0% (22 of 25) of colonic adenoma with malignant change and 32% (8 of 25) of colonic adenoma only (p <0.05). CONCLUSIONS: The degree of COX-2 expression was similar in colonic adenoma and colonic adenoma with malignant change, but the expression of p53 in colonic adenoma with malignant change was significantly higher (p <0.05). These results suggest COX-2 might operate on early step of adenoma-carcinoma sequence. Also these results remind us that mutation of p53 gene operates on the late step of adenoma-carcinoma sequence.

RESUMO

Sacrococcygeal teratoma is the most common solid tumor to occur in neonates, in whom it mostly takes the form of an obvious exophytic mass. They are, however, rarely found in adults. We report the CT and histopathologic findings of an adult cystic teratoma arising from the lower sacrococcygeal region that was discovered incidentally. Unenhanced CT scans showed an oval-shaped, cystic lesion with hyperattenuating content and no contrast enhancement. After surgery, the gross specimen was seen to be a cystic lesion filled with mucin. Microscopy revealed three germ cell layers in the cystic wall and the lesion was confirmed as cystic teratoma.

RESUMO

Lipoblastoma is a benign tumor which frequently arises in the limbs but rarely in the mediastinum, and is common in children under the age of three years. We report a case of mediastinal lipoblastoma in a 27-month-old female child complaining of dyspnea. Plain radiography demonstrated a huge well-defined mass in the me-diastinum, while CT scanning revealed a soft tissue mass with an internal hypodense portion. When radi-ographic images in children under the age of three reveal a soft tissue mass with a fat component in the mediastinum, especially where there is no calcification or a cystic component, lipoblastoma should be included in the differential diagnosis.

BACKGROUND: The prognosis of patients with lung cancer is still poor. Lung cancer exhibits a variable clinical outcome, even in those patients with same stage Numerous reports suggest that oncogene expression night play a role in explaining the variability of response and survival But many of these reports are still under debete. So we studied the clinical relevance of oncogene expression in Korean lung cancer patients. lmmunohistochemistry of p53, erbB-2, CEA expression was performed. METHOD: From March, 1992 until March, 1997, 120 patients with lung cancer were reviewed. p53, erbB-2, and CEA expression were detected on paraffin-embedded tumor blocks with the use of monoclonal antibodies. The survival arid response has correlated with the expressibility of p53, erbE-2, arid CEA oncoprotein. RESULTS: Overall, the expression rates of p53 erbB-2, and CEA were 33.7%, 59.3%, and 32.6% respectively. Expression rates were not con-elated to cell type or stage. Compared with response to chemotherapy, no correlation was found. The expression of p53, erbB-2, or CEA was not correlated with 2-year survival. With simultaneous applications of p53. erbB-2, and CEA, patients with 2 or more expressions also did not show poor response to chemotherapy. CONCLUISON: We conclude the p53, erbB-2, and CEA expression are clinically less useful in predicting response to chemotherapy or survival.
