RESUMO

PURPOSE: Polydeoxyribonucleotide (PDRN), consisting of a mixture of deoxyribonucleotide polymers, has been suggested to have anti-inflammatory effects and enhance angiogenesis as an adenosine A(2A) receptor agonist. The aim of this study was to report the effectiveness of PDRN as an adjuvant therapy after surgical debridement in MRONJ (medication-related osteonecrosis of the jaw) patients. MATERIALS AND METHODS: Five patients (1 male, 4 females, age 65~79 years) who were diagnosed with MRONJ stage 2 or 3 underwent surgical debridement and PDRN mucosal injection. After surgical debridement, patients were subject to daily injection with 1 ml of PDRN around the surgical wound for 14 days. RESULT: The patients' symptoms gradually disappeared. The surgical wound uneventfully healed, and no recurrence was observed during the follow-up period. CONCLUSION: Although further studies are required, the present study first describes the possibility of PDRN as a useful option for MRONJ treatment.

RESUMO

The present study was performed to evaluate the effect of Tetracycline-HCl and Saline on the change of implant surface microstructure and surface roughness according to application time. Implants with resorbable blasting media surface were utilized. Before test all 13 implants were measured surface roughness. Among them, 6 implants were rubbed with 50mg/ml Tetracycline-HCl solution and other 6 implants with saline for 1/2min., 1min., 1 1/2min., 2min., 2 1/2min and 3min. Then, specimens were processed for scanning electron microscopic observation and surface roughness after test. The results of this study were as follows. 1. Control group showed a few irregular, rough, uneven surface with crater-like depression. 2. The test group with Tetracycline-HCl conditioning showed an altered surface when Tetracycline-HCl was applied for 30secs, and showed a various surface alteration as application times go on. 3. The test group with Saline conditioning showed no significant surface differences and surface roughness. 4. The significant increase of Ra value was showed when Tetracycline-HCl was applied for 30secs. In conclusion, the 50mg/ml Tetracycline-HCl must not be applied for the RBM surface implant for surface treatment.

RESUMO

We selected 123 cases of general anesthesia and 227 cases of local anesthesia in ophthalmic surgery, which were divided into three age groups, and checked pulse rate changes in various states: before anesthesia, during operation, and after operation. The results were as follows; 1) Marked decrease was seen in pulse rate during operation in all groups. 2) Pulse rate decrease was seen in 72,4 of all cases during operation, compared with before operation. 3) They revealed a marked pulse rate decrease in general and regional anesthesia, 874J and 68% respectively. 4) Children and the elderly group showed a more marked pulse rate decrease than the adult group during operation. 5) Extraocular operation showed pulse rate decrease more markedly than in ocular procedures.

RESUMO

We have experience with one case of peroneal nerve palsy following ureterolithotomy in a 63-year old male patient who has suffered from diabetes and hypertension since 5 years ago. He also has suffered from pulmonary tuberculosis for 30 years. On admission, he had pain, numbness, and burning sensations in the lower extrernities. Blood pressure was 200/100 mm Hg and fasting blood glucose was 165 mg/100 ml. Urinary glucose was ++ by Klini test. Premedication was done by chloropromazine(39mg) and atropine(0. 6mg) intramuscularly. General anesthesia was begun with thiopental, pancuronium and SCC, and maintained with halothane and N2O by semi-closed circle system. Ureterolithotomy was performed with the patient in right kidney position for 3 hours and 20 minutes, Two days post-operatively, we noticed right foot-drop and limitation of dorsiflexion of the great toe. The patient recovered after physiotheraphy for 3 months. The incidence of nerve palsy is very low in patients without complications but we note that diabetes can be a factor in nerve palsy.
