RESUMO

Multiple myeloma is a plasma cell disease with the second highest incidence among hematological malignancies. Its risk of concurrent thrombosis is high, which greatly affects the quality of life and even endangers life of multiple myeloma patients. The pathogenesis of thrombosis in patients with multiple myeloma is complex, which is still unclear. However many mechanisms are considered to be related to hypercoagulable state in patients with multiple myeloma. Currently, there is a lack of thrombosis risk assessment model for multiple myeloma. Therefore, this paper reviews the research and the latest progress of venous thrombosis in patients with multiple myeloma, and provides a theoretical basis for the prevention and treatment of thrombosis.

RESUMO

Objective To investigate the clinical value of the intervertebral disc morphous in patients with thoracolumbar vertebral compression fracture. Methods The MRI, X-ray and CT data of 75 patients with thoracolumbar vertebral compression fracture were retrospectively analyzed. The intervertebral disc damage degree was observed, and its correlation with vertebral fracture degree and endplate damage degree was studied. The vertebral body leading edge height, intervertebral space height and back convex Cobb angle in patients with different intervertebral disc damage degree were measured. Results The intervertebral disc damage degree height was positively correlated with endplate damage degree (P0.05). Conclusions It is more important to observe the morphological changes of the intervertebral disc in patients with thoracolumbar vertebral compression fracture, and the damage degree is closely related with the vertebral fracture degree and endplate damage degree.

RESUMO

Objective Detecting the influence of swainsonine bring in the expression of N?glycan of osteosarcoma tumor stem cells. Methods LM8 cells were cultured in the culture medium (free of serum), then selected the LM8 cells of both CD44 and CD133 positive by magnetic activated cell sorting technique. Detecting the levels ofβ1, 6 branch N?glycan expression in the selected LM8 cells (treating by different doses of N?glycosylation inhibitor swainsonine) surface through the lectin binding assay (L?PHA). Results (96.5 ± 1.2)%of LM8 cells were both CD133 and CD44 positive which selected from all the LM8 cells in culture medium (free of serum) by magnetic activated cell sorting technique. Lectin binding assays shown that the bound rare of osteosarcoma tumor stem cells and L?PHA was (90.3 ± 2.1)%, which was higher than normal LM8 cells and L?PHA by (54.3 ± 3.1)%(P<0.05). The positive rare ofβ1, 6 branch N?glycan on the selected LM8 cells(treat with 1mg/mL swainsonine) surface was 90%. The positive rare of β1, 6 branch N?glycan on the selected LM8 cells (treat with 5mg/mL swainsonine) surface was 21%. Conclusion N?glycosylation inhibitor swainsonine plays a role in inhibiting the expression of N?glycan on cell surface of osteosarcoma tumor stem cell.

BACKGROUND:Fragile bone in senile osteoporosis patients easily weakened pedicle screw fixation capacity. Therefore, bone cement leakage and screw removal cannot be solved during pedicle screw repair in osteoporotic patients with degenerative lumbar spine. OBJECTIVE:To investigate the biomechanical properties of novel pourable pedicle screws and bone cement application effect in osteoporotic patients with degenerative lumbar spine. METHODS:Six lumbar specimens (T11-L5) at the mean age of (72.9±4.2) years were selected, total y 42 vertebrae. The average bone density was 0.696 g/cm2. Any side of al vertebrae was inserted with pourable pedicle screws. 2 mL of bone cement was perfused under the X-ray fluoroscopy with cement push rod and fil ing cylinder. The same number of conventional screws was inserted into the opposite side of the samples. Three-point bending test was performed in two kinds of screws. The maximum vertebral axial pul out force test and the maximum spin torque test were conducted in vertebra to observe the destruction of the vertebral body and implanted effect. RESULTS AND CONCLUSION:(1) The ultimate strength and yield load of novel pourable cement pedicle screws were significantly more than conventional screws (P<0.05). Ultimate displacement and yield displacement were significantly less in novel pourable cement pedicle screws than in conventional screws (P<0.05). (2) The maximum vertebral axial pul out force and the maximum spin torque were significantly higher in novel pourable pedicle screws than in conventional screws (P<0.05). (3) In summary, novel pourable cement pedicle screw is simple to operate, can effectively control bone cement penetration, and improve screw stability in osteoporotic vertebrae. Moreover, after treatment, the removal of screws is convenient, cannot evidently destroy vertebral body or screw channel, and promote early recovery.

RESUMO

Osteosarcoma ( OS) is the most common primary malignant bone tumor in children and adolescents. Currently, the combination of surgical resection and chemotherapy is the main method of treatment for OS in the clinic.Al-though, the survival rate has been greatly improved in OS patients with localized disease, the 5-year survival rate has re-mained <20%.It is very important to understand the cause of disease, to develop novel drugs or therapeutics, and to learn the disease through animal models.This review will introduce the progress of animal models of osteosarcoma over the last years.

RESUMO

Objective To analyze the learning curve of microendoscopic lumbar discectomy (MED) and summarize the operative experience. Methods From July 2010 to March 2012, the first 100 patients with single level lumbar disc herniation managed with MED were divided into chronological groups (A, B, C and D), 30 patients in group A, B, C and 10 patients in group D. The comparsons were conducted in terms of operative time , estimated blood loss , number of cases with transition from MED to open surgey or for reoperation , rate of complications and postoperative efficacy. Results The mean operative time and the mean estimated blood loss in each group was valuated respectively. In group A , the operative time and estimated blood loss were significantly decreased as compared with those in group B, C and D respectively (P 0.05). There were 6 complications in group A, 1 complication in group B and C respectively and no complication in group D. The leg pain visual analogue scale and Oswestry disability index at the first month and the twelfth month postoperatively were all improved respectively as compared with those preoperatively (P < 0.01). Conclusion The learning curve of microendoscopic lumbar discectomy generally may reach the plateau when about 30 cases are operated on. The key procedure to pass over the learning curve includes puncture and location , flavectomy , anatomy mark identification, and precaution of damage in dural sac.
