RESUMO

【Objective】 To explore the rational management of contralateral patent processus vaginalis (CPPV) in laparoscopic high ligation of processus vaginalis. 【Methods】 A total of 300 children with unilateral oblique inguinal hernia/hydrocele who received laparoscopic high ligation of processus vaginalis in Baoding Children’s Hospital during Jun.2018 and Jun.2022 were selected and divided into two groups by random number table method, with 150 in either group. In the control group, 53 cases of CPPV were found intraoperatively, which were treated simultaneously. In the study group, 58 cases of CPPV were detected, among which 11 met the indications of high ligation and received simultaneous surgical treatment. The incidence of recurrence was compared between the two groups. 【Results】 After 1 year of follow-up, the recurrence rate was 8.62% in the study group and 1.88% in the control group (P>0.05). The detection rate of CPPV was 23.02% in children with unilateral inguinal hernia, significantly lower than that in children with unilateral hydrocele (49.07%, P<0.001). The detection rate of CPPV was 42.71% in children with left patent processus vaginalis and 32.95% in children with right patent processus vaginalis (P=0.19). The detection rate of CPPV was 62.93% in the age group of 1-2 years, significantly higher than that in other age groups (P<0.001). 【Conclusion】 The incidence of CPPV conversion into oblique inguinal hernia or hydrocele is low. Only children who meet the indications can be treated at the same time during surgery.

OBJECTIVE To compare the efficacy ,safety and immunogenicity of bevacizumab biosimilars and original drugs for non-small cell lung cancer (NSCLC),and to provide evidence-based reference for clinical use. METHODS PubMed,Embase, Web of Science ,Cochrane Library ,CBM,CNKI,VIP,Wanfang database ,ClinicalTrials.gov,and Clinical Trial Center of China were searched from the establishment of the database to September 25,2021,randomized controlled trials (RCTs)about bevacizumab biosimilars(trial group )versus bevacizumab original drugs (control group )for NSCLC were collected. After literature screening , data extraction and quality evaluation of included RCTs with bias risk assessment tool recommended by Cochrane Handbook 5.1.0, meta-analysis,sensitivity analysis and publication bias analysis were performed by using RevMan 5.3 software. RESULTS A total of 11 RCTs were included ,involving 6 596 patients in total. Meta-analysis showed that there was no statistical significance in the difference of overall response rate [RR=0.97,95%CI(0.92,1.02),P=0.22],the total incidence of adverse reaction [RR=1.00, 95%CI(0.99,1.01),P=0.79],the incidence of severe adverse reaction [RR=1.04,95%CI(0.96,1.13),P=0.38],positive rate of anti-drug antibody [RR =1.10,95%CI(0.88,1.36,P=0.41] and the incidence of common adverse reactions (except for vomiting)among 2 groups(P>0.05). The sensitivity analysis results showed that the obtained results were robust. The results of publication bias analysis showed that there was little possibility of publication bias. CONCLUSIONS The efficacy ,safety and immunogenicity of bevacizumab biosimilars used for NSCLC are equivalent to those of bevacizumab original drugs.

Objective:To compare the positioning errors of individual head-rest combined with thermoplastic fixation mask versus thermoplastic fixation mask alone in patients with head and neck tumors. Methods:Twenty-eight patients who received irradiation with helical tomotherapy in Cancer Hospital of the University of Chinese Academy of Sciences (Zhejiang Cancer Hospital) between October 2019 and April 2020 were included in this study. They were randomly assigned to receive position fixation with either individual head-rest combined with thermoplastic fixation mask (N1 group, n = 14) or thermoplastic fixation mask alone (N2 group, n = 14). Megavoltage computed tomography (MVCT) scanning registration was used to obtain the positioning errors in translation and rotation (ROLL) in the left-right (X), head-food (Y), and belly-back (Z) directions. There were a total of 841 CT scans, consisting of 425 scans in group N1 and 416 scans in group N2. Results:The positioning errors in X, Y, Z and ROLL directions in the N1 group were (1.37 ± 1.04) mm, (1.38 ± 1.12) mm, (1.47 ± 1.62) mm and (1.47 ± 1.62) ° respectively, and they were (1.57 ± 1.21) mm, (2.10 ± 1.51) mm, (1.61 ± 1.50) mm and (1.40 ± 1.30) ° respectively in the N2 group. There was significant difference in positioning errors in the Y direction between N1 and N2 groups ( P = 0.013). In the N1 group, the outward expansion boundaries in X, Y and Z directions was 4.15 mm, 4.23 mm and 4.81 mm respectively, and it was 4.77, 6.31 and 5.08 mm, respectively in the N2 group. In the X direction, there was significant difference in positioning errors taking 3 mm as the dividing point between N1 and N2 groups ( χ2 = 10.516, P < 0.001). In the Y direction, there was significant difference in positioning errors taking 1, 2 and 3 mm as the dividing points between N1 and N2 groups ( χ2 = 24.889, P < 0.001; χ2 = 42.202, P < 0.001; χ2 = 46.204, P < 0.001). In the Z direction, there was significant difference in positioning errors taking 2 mm as the dividing point between N1 and N2 groups ( χ2 = 7.335, P = 0.007). In the N1 group, the percentage of positioning errors < 3 mm in the X, Y and Z directions was 92%, 90% and 92%, respectively. Conclusion:Compared with thermoplastic fixation mask alone, individual head-rest combined with thermoplastic fixation mask can better effectively improve the positioning stability and reduce positioning errors in patients receiving irradiation with helical tomotherapy for head and neck tumors. The combined method is of certain innovation.

Objective:To explore the clinical effects of penile ventral Z-shaped cross flap and penile dorsal pedicled transfer flap in penoplasty for concealed penis.Methods:From January 2017 to June 2019, the data of 151 patients with concealed penis admitted to our hospital was retrospectively reviewed. The patients were divided into 2 groups according to the surgical method. 69 cases were treated with penis ventral Z-shaped cross flap to form penis and 82 cases were treated with dorsal penis pedicled flaps to form the penis. In Z-shaped flap group, the penis length of 33 patients with tight scrotum was (3.06±0.25)cm before surgery and the penis length of 36 patients with relaxed scrotum was (2.99±0.28) cm before surgery. In flap with transfer group, the penis length of 39 patients with tight scrotum was (3.04±0.30)cm before surgery and the penis of 43 patients with relaxed scrotum was (3.04±0.24)cm before surgery. The length of the penis after surgery and incidence of postoperative complications were compared between Z-shaped flap group and flap with transfer group. Common complications included penile body retraction, foreskin edema, foreskin stenosis and penile wound splitting.Results:151 patients were followed up for 6-12 months, and all patients were satisfied with penis appearance. There was no penile necrosis or urinary fistula. In Z-shaped flap cross group, the penis length of 33 patients with tight scrotum extended (2.47±0.22)cm after surgery.The penis length of 36 patients with scrotum relaxation extended (2.61±0.27)cm after surgery, 39 patients was adopted the penile dorsal pedicled transfer flap with scrotum tight had extended penis (2.90± 0.16)cm, which significantly different from the Z-shaped flap group( P<0.05). In flap with transfer group, 43 patients with relaxed scrotum extended (2.79±0.18)cm after surgery, which was significantly different from the Z-shaped flap group ( P<0.05). In Z-shaped flap group, 33 patients with scrotum tight, there were 2 cases of penile retraction, 1 case of stenosis of the foreskin, 2 cases of foreskin edema, 2 cases of penile wound rupture. In transfer flap group, of 39 patients with scrotum tight, there was 1 case of foreskin edema. The incidence of complications that adopted the penile dorsal pedicled transfer flap with scrotum tight was lower than those adopted penile ventral Z-shaped cross flap [2.56%(1/39) vs. 21.21%(7/33), P=0.033]. In transfer flap group, of the 43 patients with scrotum relaxation, there were 3 cases of penile retraction, 3 cases of foreskin stenosis, 2 cases of penile ventral foreskin edema, and 1 case of penile wound rupture. Z-shaped flap group: 36 patients was scrotum relaxation was 1 case of foreskin edema. The incidence of complications that adopted the penile dorsal pedicled transfer flap was higer than those adopted penile ventral Z-shaped flap [20.93%(9/43) vs. 2.78%(1/36), P =0.038]. Conclusions:In terms of children with tight scrotum or loose scrotum, the effect of the transfer flap method to extend the penis is better than that of the Z-shaped flap method. However, the transfer flap method has a low complication rate for children with tight scrotum, while the Z-shaped flap method has a low complication rate for children with loose scrotum.

RESUMO

Fibrous hamartoma of infancy (FHI) in the scrotum of children is a rare benign soft tissue tumor, which mostly occurs in children under 2 years old. It grows rapidly in the early stage and is easily misdiagnosed as a malignant tumor adjacent to the testis. A case of FHI in the scrotum was admitted in our hospital in recent years, a tumor resection with preservation of testicle was performed, the lesion was completely removed. Postoperative follow-up was 20 months, and there was no evidence revealing recurrence of the tumor after excision.

Objective@#To investigate the correlation between testicular torsion in adolescence and seasonal temperature changes.@*Methods@#The clinical data of 28 children with testicular torsion diagnosed surgically at Wuhan Children′s Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology from June 2009 to December 2017 were retrospectively analyzed.Seasons and temperatures at the onset of the disease in the area were recorded based on hospital information and China Meteorological Science Data Sharing Service Network.According to the seasons of onset, 28 children were divided into spring group, summer group, autumn group and winter group.@*Results@#The average age of 28 cases was 13.0 years (10.4-17.0 years). The duration of the disease was 1-4 days [(1.0±0.4) days]. Testicular torsion was found in 17 cases (60.7%) on the right side and 11 cases (39.3%) on the left side.Six patients (21.4%) underwent testicular reduction and bilateral testicular fixation, 22 patients (78.6%) underwent torsional orchiectomy plus contralateral testicular fixation.Testicular torsion was observed in 16 patients (57.1%) during the 3 months from December to February when the median temperature of onset was 1.3 ℃ (-2.1-5.6 ℃). Testicular torsion was observed in 6 patients (21.4%) during the 3 months of spring (March to May) when the median temperature was 9.4 ℃ (6.5-12.1 ℃). During the 3 months of summer (June to August), testicular torsion was observed in 2 patients (7.1%) when the median temperature was 21.8 ℃ (19.0-26.5 ℃). Testicular torsion was observed in 4 patients (14.4%) during the 3 months of autumn (September to November) when the median temperature was 11.1 ℃ (8.0-13.2 ℃). The incidence of testicular torsion in different seasons was significantly different (χ2=3.270, P=0.011). Pearson correlation analysis showed that the incidence of testicular torsion in adolescence was significantly negatively correlated with the season (winter) (r=-1.000, P=0.026).@*Conclusions@#Testicular torsion in children tends to be seasonal, and acute testicular torsion is more likely to occur in cold seasons.

Objective To investigate changes in nutritional status in senile dementia patients with malnutrition treated with enteral nutrition combined with rehabilitation therapy evaluated by using the Mini Nutritional Assessment-Short Form(MNA-SF).Methods Fifty hospitalized patients with senile dementia and malnutrition at the internal medicine department of our hospital from July 2014 to July 2017 were enrolled and further divided into an enteral nutrition group(a control group,n =25)and an enteral nutrition combined with rehabilitation therapy group(an observation group,n=25).Anthropometric parameters including body mass index,biceps circumference,calf circumference,triceps skin-fold thickness,and blood biochemical parameters including hemoglobin,total cholesterol,triacylglycerol,albumin,lymphocyte count before and after treatment were compared between the groups.Results Anthropometric parameters such as body mass index,biceps circumference,calf circumference and triceps skin-fold thickness had no significant difference between the two groups before treatment (P > 0.05),while after 30 days of treatment,anthropometric parameters saw improvement in both groups compared with those before treatment,and the improvement was greater in the observation group than in the control group(P <0.01).Blood biochemical parameters had no significant difference between the two groups before treatment(P>0.05).After 30 days of treatment,levels of total cholesterol and triacylglycerol had no significant difference compared with those before treatment or between the two groups(P >0.05),while levels of hemoglobin,albumin and lymphocyte count improved compared with those before treatment (t =2.645,2.843,2.967,respectively,P <0.01),and had significant differences between the groups (t =2.548,2.864,2.976,respectively,P<0.01).Conclusions Enteral nutrition combined with rehabilitation therapy has a measurable,positive influence on nutritional status in senile dementia patients with malnutrition.

BACKGROUND: Non-specific cyclooxygenase inhibitor and specific cyclooxygenase inhibitor are commonly used nonsteroidal anti-inflammatory and analgesic drugs, but there is a lack of experimental evidence concerning the effect of these drugs on the fracture healing and the administration time. OBJECTIVE: To investigate the effect of non-specific cyclooxygenase-2 inhibitor versus specific cyclooxygenase-2 inhibitor on the healing of tibial fractures in rabbits. METHODS: Thirty-six 3-5-month-old New Zealand rabbits either in sexes were selected (provided by Laboratory Animal Center of Guangxi Medical University). Rabbit models of unilateral tibial fractures (3 mm) were established and randomly divided into modeling group (same volume of distilled water via gavage), specific cyclooxygenase-2 inhibitor group (1.15 mg/kg?d etoricoxib via gavage), and non-specific cyclooxygenase-2 inhibitor group (7.6 mg/kg?d aspirin via gavage), with 12 rabbits in each group. At 2, 4 and 8 weeks postoperatively, the fracture healing was observed and assessed by gross observation and histopathological sections. The serum level of osteocalcin, and levels of transforming growth factor β1 and vascular endothelial growth factor were detected. RESULTS AND CONCLUSION: (1) The non-specific cyclooxygenase-2 inhibitor group was superior to the other two groups in the formation and remodeling of callus and healing. (2) The serum level of osteocalcin in the non-specific cyclooxygenase-2 inhibitor group was significantly higher than that in the specific cyclooxygenase-2 inhibitor group (P < 0.05). (3) Hematoxylin-eosin staining results: the non-specific cyclooxygenase-2 inhibitor group was earlier than the specific cyclooxygenase-2 inhibitor group in the formation of collagen fibers, cartilage tissue, trabecular bone and bone matrix. (4) The average absorbance value of transforming growth factor and vascular endothelial growth factor in the non-specific cyclooxygenase-2 inhibitor group was significantly higher than those in the specific cyclooxygenase-2 inhibitor group at 4 weeks postoperatively (P < 0.05). (5) These results suggest that specific cyclooxygenase-2 inhibitor for rabbit tibia fractures took longer time than non-specific cyclooxygenase-2 inhibitor, indicating that cyclooxygenase-2 specific inhibitor is less effective than non-specific cyclooxygenase-2 inhibitor in fracture healing.

@#By using(S)-MonoPHOS and [Rh(COD)2]BF4 as catalyst, the asymmetric hydrogenation reactions of methyl-2-acylamino-3-arylacrylate and the effect of different amino protective groups on hydrogenation efficacy were studied. The products resulting from asymmetric hydrogenation were hydrolyzed by hydrochloric acid, and the corresponding amino acids were obtained at yields of 63%- 92%.

Objective To compare the effect of fluoroscopic triggering method and empirical delay method on image quality in the liver Gd-EOB-DTPA dynamic enhanced MRI,and to investigate the value of fluoroscopic triggering method in Gd-EOB-DTPA dynamic enhanced MRI.Methods The patients underwent Gd-EOB-DTPA dynamic enhanced MRI were randomly divided into two groups according to the starting modes in the artery phase.Group A used fluoroscopic triggering method and group B used empirical delay method.Eliminating the images with severe respiratory motion artifacts,the quality of the remaining images in 78 cases of group A and 85 cases of group B were assessed in scores (excellent=5 scores;good=4 scores).Data was statistically analyzed with Mann-whitney tests,and P<0.05 was considered statistically significant.Results The excellent rate of the images in group A was 96.15% (75/78).The excellent rate of the images in group B was 67.06% (57/85).There were significant differences between the two groups in the excellent rate (χ2=27.889, P<0.001)and the image quality scores (Z=-4.747,P<0.001).Conclusion For the liver Gd-EOB-DTPA dynamic enhanced MRI, fluoroscopic triggering method is more likely to get better image quality and higher success rate in artery phase than empirical delay method,which indicates that fluoroscopic triggering method have obviously advantages in clinical applications.

Objective@#To assess the collateral grade based on 4-dimensional magnetic resonance angiography (4D-MRA) in predicting short-term outcome in patients with acute ischemic stroke (AIS) .@*Methods@#Forty-seven patients with unilateral MCA stroke were enrolled in this study. All patients underwent multi-modality MRI within 4.5 to 24.0 hours onset of stroke. The collateral grade was assessed through the dynamic MRI angiograms derived from perfusion raw data. The AIS patients were divided into favorable and unfavorable outcome group according to the improvement of national institutes of health stroke scale (NIHSS) score. The differences in baseline data, infarct volume, the ratio of volume of ischemic tissue on perfusion to infarct core on diffusion (rVPD) and collateral grade between the two groups were analyzed. Logistic regression analysis was used to identify variable influencing the short-term clinical outcomes. The Kappa coefficient was used to analyze the consistency of the collateral grade assessed between the two observers. Spearman rank-order correlation test was used to analyze the relationship between the collateral grade, infarct volume, hypoperfusion volume and rVPD.@*Results@#The collateral grade used ASITN/SIR based on 4D-MRA was performed in 47 patients. Grade 1 in 4 patients, grade 2 in 21paitients, grade 3 in 16 patients and grade 4 in 6 patients, respectively. The collateral grade had a high consistency among the observers (Kappa=0.806, P<0.01). Of the patients who enrolled the study, the median of infarct volume was 29.63 (4.92, 69.17) ml, the hypoperfusion volume was 73.76 (29.75, 178.42) ml, and the rVPD was 3.1 (1.5, 5.8). It was negatively correlated with initial infarct volume (r=-0.627, P<0.01) and hypoperfusion volume (r=-0.354, P<0.01) . There was a significant positive correlation between the collateral grade and rVPD (r=0.575, P<0.001). There was also significant differences in infarct volume, rVPD, and collateral grade between favorable and unfavorable outcome group (P<0.05). The collateral grade based on 4D-MRA was an independent predictor of favorable clinical outcome (OR=4.419, P<0.05) .@*Conclusions@#The collateral circulation classification based on 4D-MRA proved to be a significant predictor of clinical outcome, it can be considered as a reliable method for analyzing the cerebral hemodynamic changes and collateral grade in AIS patients.

Objective To evaluate the diagnostic ability of fusion images of SWI and DWI in ischemic penumbra(IP)of patients with acute ischemic stroke(AIS).Methods 47 AIS patients were retrospective analyzed.Two neuroradiologists analyzed the fusion images of SWI and DWI respectively.SDM was defined as that the hypo-intensity signals of intramedullary or sulcal veins were dilated or increased on fusion images compared with contralateral areas.The results compared with the PDM,which was considered as the“gold standard”.Areas under the receiver operating characteristic curve(AUC)were used to assess the efficacy of SDM which determined by fusion images and compared to PDM.Chi-square test was used to analyze the consistency between the two neuroradiologists and also the consistency between SDM and PDM in IP assessment Results The fusion images of SWI and DWI had a high diagnostic efficacy compared to PDM, AUC of the two radiologists were 0.885,0.877,the diagnostic sensitivity were 84.2%,78.9% and specificity were 92.9%,96.4%respectively.There was also a high consistency in SDM assessment through fusion images between the two neuroradiologists (Kappa=0.908,P >0.05). Conclusion Fusion images of SWI and DWI have a high diagnostic efficacy in IP assessment,which may be considered as a simple approach for IP assessment in patients with AIS.

Objective To investigate the value of early?phase enhancement ratio combined with peripheral vascular diameter in the differential diagnosis of benign and malignant breast lesions using 3.0 T dynamic contrast?enhanced magnetic resonance imaging (DCE?MRI). Methods Sixty seven cases of patients (35 with malignant lesions and 32 with benign lesions in the breasts) were retrospectively analyzed. Their diagnoses were confirmed by surgery and pathology and all the patients underwent breast MRI plain scan and DCE?MRI in the two weeks before surgery. Lesion ROIs were drawn and time?signal intensity curves in the DCE?MRI were generated. Early?phase enhancement rate, time to peak, early?phase enhancement ratio, numbers of tumor vessel within 3 cm of the lesion and diameter of the largest vessel were recorded. Mann?Whitney U test was used to compare the difference of DCE?MRI between benign and malignant lesions, and the ROC curve was used to evaluate the efficiency of early?phase enhancement rate, early?phase enhancement ratio and vascular diameter in differentiating benign and malignant lesions. Results With breast malignant lesions, the medians of time to peak, early?phase enhancement rate, early?phase enhancement ratio, numbers of tumor vessel and vascular diameter were 2.2 s, 176.0%, 100.0%, 4 and 2.96 mm respectively, while with benign lesions of these parameters were 4.7 s, 113.3%, 81.9%, 0 and 0.00 mm respectively, and the differences were statistically significant (all P<0.05).When early?phase enhancement rate was used for differential diagnosis of breast benign and malignant lesions, the area under the ROC curve was 0.702 and the sensitivity and specificity were 82.86%and 56.25%with a threshold of 120.0%. When early?phase enhancement ratio was used, the area under the ROC curve was 0.854 and the sensitivity and specificity were 94.29% and 68.75% with a threshold of 86.0%. When peripheral vascular diameter was used, the area under the ROC curve was 0.896 and the sensitivity and specificity were 74.29%and 84.38% with a threshold of 2.78 mm. When early?phase enhancement ratio was combined with peripheral vascular diameter, the area was 0.925 and the sensitivity and specificity were 97.14% and 62.50%. Conclusion In the differential diagnosis of benign and malignant breast lesions under DCE?MRI, early?phase enhancement ratio combining with peripheral vascular diameter has improved sensitivity.

Objective To investigate the effect of Bcl-2 gene expression on the proliferation and apoptosis of human pancreatic cancer SW1990 cells.Methods Bcl-2 short guide RNA (Bcl-2-sgRNA) was designed and synthesized,and it was combined with CRISPR-Cas 9.After confirmation by gene sequencing,it was transfected into human pancreatic cancer cell line SW1990,then the cells with stable Bcl-2 gene knock-out were selected,and wild type SW1990 cells were used as control.The cell growth curve was determined by CCK-8 method.The number of clone formation was measured.Flow cytometry was used to measure cell cycle and apoptosis.Results Human pancreatic cancer cell line SW1990 with Bcl-2 gene knock-out was successful constructed.Compared with wild type SW1990 cells,the growth of SW1990 cells with Bcl-2 gene knock-out was inhibited,the number of clone formation was significantly decreased [(160.7 ± 10.0) vs (285.3 ± 14.2)],the proportion of G1 cells was significandy increased [(84.51 ± 0.97) % vs (57.49 ± 1.08) %],the proportion of S phase cells significantly decreased [(12.82 ± 0.99) % vs (27.56 ± 1.65) %],and apoptosis rate was remarkably increased [(12.67 ± 0.59) % vs (0.37 ± 0.35) %],and the difference between the two groups was statistically significant (P < 0.01).Conclusions Knock-out of Bcl-2 gene can inhibit the growth of human pancreatic cancer cell line SW1990,decrease the ability of clone formation,block the cell in G1 phase and greatly increase cell apoptosis rate.

Objective To evaluate the diagnostic value of intra-arterial thrombi under went susceptibility weighted imaging(SWI) in patients with acute ischemic stroke(AIS).Methods The MRI images of 133 patients with AIS was analyzed,included time of flight MR angiography(TOF-MRA )and SWI.The patients were divided into two groups according to the time between onset of symptoms and MR imaging (group A,interval time<24 h;group B,interval time 24-72 h).Two neuroradiologists who were blin-ded to the patients clinical information,documented the number and location of susceptibility vessel sign (SVS)in SWI and embolic occlusion or stenosis on TOF-MRA in AIS patients.Results The thrombus detection rate in group A was higher than that in group B.There was no significant difference in the overall detection rate of thrombi between SWI and TOF-MRA in all AIS patients,but the thrombi detection rate in M3 segement of middle cerebral artery(MCA)with SWI was higher than that with TOF-MRA in group A(P <0.05).Conclusion SWI performs a high diagnostic value for thrombus detection in AIS patients,especially for the thrombus detection of M3 segement in early period of AIS.

@#In the investigation of asymmetric catalytic hydrogenation reaction, (Z)-methyl-2-acetamidocinnamate is often used as a standard substrate to assess the chiral catalysts. However, its previous syntheses suffered from low yield, relying on column chromatography for isolation. Herein, (Z)-methyl-2-acetamidocinnamate was obtained with 68. 5% isolation yield via simple operations such as wash and extraction using dicyclohexylcarbodiimide(DCC)as dehydrant and 4-dimethylaminopyridine(DMAP)as catalyst in the last synthetic step.

Objective To explore the clinical effect of percutaneous osteoplasty with SKy bone expander system in the treatment of calcaneal fracture.Methods 63 feet of 55 patients with calcaneal fractures were involved in this study.According to Sanders fracture classification including 38 feet of Sanders Ⅱ,18 feet of Sanders Ⅲ,7 feet of Sanders Ⅳ.There were 6 to 16 days interval between the injuries and the surgeries.The patients were treated by percutaneous osteoplasty with the SKy bone expander system.The standard of operation was the satisfaction of reduction and Bohler's and Gissane's angles under X-ray.Results All of 63 feet of 55 patients were followed up for average 22 months.According to the criterion of therapeutic effect,the results were as follows:excellent in 30 cases,good in 26 cases,fair in 7cases,and no poor case.The excellent and good rate was 88.9%.Conclusion Percutaneous osteoplasty with SKy bone expander system in the treatment of calcaneal fracture,especially in reduction and fixation of Sanders type Ⅱ and Sanders type Ⅲ,can recover Bohler's and Gissane's angles,significantly shorten the duration of illness,and has fast recovery and can possess satisfactory curative effect,and it is worth popularizing.

Tissue expression and blood level of YKL-40 protein are elevated ia patients with tumor.The protein may have a role in cancer cell proliferation,infiltration,metastasis and remodelling of tumor stroma.Elevated YKL-40 levels are highly associated with short survival and poor response to treatment for patients.Thus,YKL-40 may serve as a biomarker in diagnosis of disease,evaluation of severity and assessment of prognosis.It's more likely to be a new target for anti-tumor treatment.

RESUMO

The resistance of anti-VEGF therapy which casually occurs during the course of tumor therapy by using anti-VEGF drugs may be associated with alternative angiogenesis signaling activation, bone marrowderived stem cells recruitment, pericyte coverage, perivascular invasion and so on. Thus, to investigate the mechanisms of drug resistance will guide us to understand deeply the relationship among normal tissues, tumors and drugs and to find out new therapeutic targets for anti-tumor therapy.
