RESUMO

Objective:To study the effect of depression on the quality of life of infertile couples by using a binary analysis method, namely the Actor-Partner Interdependence Model (APIM).Methods:A total of 191 infertile couples admitted to the reproductive center of Peking union medical college hospital from May 2019 to August 2019 were selected for a cross-sectional survey using the simplified Chinese version of FertiQoL and PHQ-9, and the path relationship of the mutual influence of the quality of life of couples was analyzed through the Actor-Partner Interdependence Model.Results:Depression in both wives and husbands significantly predicted their own quality of life ( β=-0.569, P<0.01; β=-0.588, P<0.01), the main effect was significant. In terms of object effect, male depression had a significant effect on women's quality of life ( β=-0.253, P=0.032), while wife depression had no statistical significance on men's quality of life. Conclusions:The results confirm that the quality of life of infertile couples is affected by individual and object depression. Therefore, evaluating both partners from a systematic perspective can improve the complexity of psychological problems in infertile couples and help healthcare professionals develop interventions to help infertile couples manage psychological and social stress.

Objective To establish an "integrated management system" for pelvic floor dysfunction and explore its effects on clinical practice. Methods A pelvic floor nursing team was set up to connect the outpatient management system, the ward perioperative management system and the home follow-up management nursing system, and to build an integrated management system for pelvic floor dysfunction diseases. ResuLts After five years of practice, the qualified rate of residual urine in pelvic floor dysfunction patients increased from 58.2% (191/330) to 93.6% (309/328), the difference was statistically significant( χ2=113.008, P<0.01). The compliance of pelvic floor muscle exercise at home increased from 63.6%(210/330) to 83.8%(275/328). The difference was statistically significant( χ2=36.654, P<0.01). ConcLusions The three level prevention of pelvic floor dysfunction disease has been formed through "the integrated management system", which can play a significant role in the development of disease management and specialized nursing.

Objective To investigate the influencing factors for patient safety competencies of junior nurses, and provide theoretical basis for the cultivation of patient safety competencies of junior nurses. Methods A total of 247 junior nurses were investigated by using the general information questionnaire and the patient safety competency self-rating scale of nurses, PSCSSN questionnaire. Results The gross score in patient safety competencies of junior nurses was (126.74 ± 13.62) points, they scored lowest on the skills factors, the score was (25.23±3.96) points, and they scored highest on the attitude factors, the score was (32.28±2.40) points. The Single factor analysis revealed that the influence on patient safety competencies of age, work experience, work department, technical titles were statistically significant (P<0.05). The multivariate analysis revealed that the factors impacting patient safety competencies were work experience, work department. Conclusions The patient safety competencies of junior nurses remains to be improved, and the nursing administrators and educators should pay more attention to the one lacking in work experience, intervene the controllable factors, make up the training gap, improve patient safety competencies and the quality of nursing service.

Objective To explore the effect of group management mode on compliance of pelvic floor function exercise for patients with post partum urinary incontinence (PPUI). Methods A total of 80 cases of patients with PPUI were randomly divided into the observation group and the control group. Patients in the control group received one-to-one routine health guidance. Patients in the observation group received group management mode including special subject teaching guidance, induction and communication between patients. Three months after the intervention, two groups of patients were evaluated at six months after postpartum by the pelvic floor muscle strength, one-hour urine pad test and pelvic floor muscle function exercise compliance. Results Six months after postpartum, the cure rate of the pelvic floor muscle strength was 100%(40/40) for typeⅠmuscle, 100%(40/40) for typeⅡmuscle in the observation group. In the control group, the cure rate for typeⅠmuscle was 70%(28/40) and 65%(26/40) for type Ⅱmuscle. The difference between these two groups was statistically significant (χ2=14.118, 16.970, P=0.000). The total effective rate of urinary incontinence was 100.0%(40/40) in the observation group, 67.5% (16/40) in the control group, and there was statistical significance (χ2=25.232, P=0.000). Evaluation of the compliance of pelvic floor function exercise showed that the rates were 72.5% (29/40) for complete compliance, 27.5%(11/40) for incomplete compliance and 0 for total non-compliance in the observation group. In the control group, these rates were 2.5%(1/40), 55.0%(22/40), 42.5%(17/40). And there was statistical significance as well (χ2=54.847, P=0.000). Conclusions Group management mode can improve the compliance of pelvic floor muscle function exercise and the strength of pelvic floor muscle, and improve the degree of urinary incontinence in postpartum patients with urinary incontinence.

Objective To explore the sex experience of infertile women and provide feasible basis for clinical nursing measures. Methods Thirteen subjects from April to October 2016 were recruited and investigated by a qualitative research design.The data were analyzed,sorted and induced the theme. Results There were 5 categories: normal sexual needs and sexual repression, sexual behavior changes and reduced satisfaction; inferiority, self blame and guilt; uncertainty in marital relationship; desire for information and emotional support. Conclusions Nurses should focus on female infertility patients attitude to sex, to strengthen health education, to develop personalized nursing intervention measures, corrective psychological deviation,promote the improvement of patients'spouses sexual health education knowledge,providing effective family,social,emotional support,to promote patients to improve the quality of life of both partners.

Objective To evaluate the application effect of continuity care model for hospitalized infertile patients after discharging from hospital. Methods Select 108 cases of infertility patients were randomly divided into trial group and control group, each group of 54 cases, control group given routine nursing intervention, intervention group in conventional nursing care and treatment on the basis of implementing continuity nursing intervention, intervention 6 months after the evaluation of two groups of patients with anxiety, depression, quality of life, and pregnancies. Results The scores of SAS and SDS were (48.32 ± 11.28), (50.62 ± 11.20) points before in the intervention group, and after the intervention (34.63 ± 8.95), (37.68 ± 9.72) points, and there were significant difference(t=-5.478, P<0.05;t=-4.840, P<0.05);before in the intervention , the scores of SAS and SDS were (49.76±13.08), (48.64±12.23) points in the control group, and after the intervention (41.18 ± 12.84), (40.31 ± 10.12) points, and there were significant difference (t=-7.361, P<0.05;t=-4.840, P<0.05), and there were significant differences in two group after intervention (t=6.682, P<0.05;t=7.341, P<0.05). The scores of quality of life were significant differences after intervention in two group, and six months after the intervention, the experimental group intrauterine pregnancy rate is higher than the control group (χ2=46.790, P < 0.05). Conclusions Applying continuity care model e for hospitalized infertile patients can improve the psychological state, reduce depression and anxiety, increase the quality of life and rate of pregnancy.

RESUMO

Objective To understand the actual psychological conditions of pregnant patients with gynecologic malignancies during chemotherapy so as to instruct the nurses to provide individualized psychological care to the patients and their relatives. Method The in-depth interviews were conducted to 7 patients by implementing the phenomenological approach of qualitative study. The acquired data were analyzed. Result The patients experienced four psychological conditions: pessimism about prognosis, anxiety to know more about the chemotherapy and guilty to their family and hope for treatment effect. Conclusion Nurses should pay attention to the psychological conditions of pregnant patients with gynecologic malignancies and help them successfully live through the treatment.

Objective To explore psychological experience of nurses as the relatives of patients with malignant tumors in order to carry out mental care to malignant tumor patients as well as their family members. Method Nine nurses whose family members were malignant cancer patients were interviewed. Results As the relatives of malignant cancer patients, all the nine nurses presented with anxiety and fear. They were lack of ways to relieve negative emotions, desperate for more information and worried uncertainties about future and medical fees. Conclusion In order to offer better nursing experience, it is necessary for nurses to change ideas and improve recognition in order to establish a psychological supportive system for patients′ relatives in different phases and a social supportive system and fulfill their requirements for knowledge, which can thus help patients and their relatives out of the predicament.

ObjectiveTo investigate the nursing key points to 8 end-stage ovarian cancer patients with totally implantable venous access ports.Method The totally implantable venous access ports were used in the 8 patients and nursing was carried out to them.Result There were no occurrences of infection,leakage,seepage,pipe obstruction and the needlepoint shift.Conclusions The ovarian cancer patients in the end-stage had very low resistance and high risk of infection.Individualized nursing care to them may ensure the use of the ports in a safe way.

RESUMO

Objective To investigate the perioperative health care to patients with perineal endometriosis (PME) involving anal sphincter.Method The clinical data of 17 patients diagnosed with PEM involving anal sphincter at the Peking Union Medical College Hospital from January 2009 to June 2013 were analyzed retrospectively.Results The perineal incisions in all cases were satisfactorily healed,without any fatal incontinence or other complications.There was only one case of recurrence,accounting for 5.9%.Conclusion Preoperative psychological nursing,postoperative close observation on the incisions,diet instructions,management of defecation and targeted health education can all contribute to patients’ recovery.

Objective To explore the effect of pre-operation intestinal preparation by orally taking magnesium sulfate of different concentrations. Methods 50 patients were divided into the high-concentration group and the low - concentration group, according to the concentration of magnesium sulfate orally taken for pre-operation intestinal preparation. Some factors were compared between the two groups including: the acting amount of water, starting and lasting timing of magnesium sulfate, times of stool, the efficiency of the intestinal preparation, the tolerance, uncomfortable reaction and changes of serum electrolyte of the patients. Results There were no difference between the two groups on the acting amount of water, starting and lasting timing of magnesium sulfate, times of stool, the efficiency of the intestinal preparation, the tolerance of the patients. The rate of the stool without solid substance was higher in the high-concentration group than the low-concentration group(40% vs 8%). The uncomfortable reaction rate was also higher in the high-concentration group, the concentration of serum electrolyte decreased in both groups, among which serum Ca2+ decreased significantly in the low-concentration group. Conclusions Orally taking magnesium sulfate of different concentrations both has effect on bowel preparation, influence on patients' serum electrolyte is remarkable.
