RESUMO

Objective:To investigate the clinical value of endometrial receptivity determination by transvaginal three-dimensional power ultrasound in the prediction of pregnancy outcome after in vitro fertilization and embryo transfer. Methods:A total of 220 infertile patients who underwent in vitro fertilization and embryo transfer in Wenzhou Hospital of Integrated Traditional Chinese and Western Medicine from July 2018 to July 2021 were included in this study. Before in vitro fertilization and embryo transfer, all patients were tested for endometrial receptivity by transvaginal two-dimensional and three-dimensional power ultrasound. Pregnancy outcomes were followed up. Anatomical indexes (endometrial thickness, endometrial volume, and endometrial morphology) and endometrial blood flow indexes [endometrial blood flow type, pulsation index, resistance index (RI), ratio of peak systolic velocity to end-diastolic velocity (S/D), vascularization index (Vl), blood flow index (FI) and vascular blood flow index (VFI)] were compared between different pregnancy outcomes. Results:There were no significant differences in endometrial thickness, endometrial volume, and endometrial morphology between successful pregnancy and failed pregnancy groups ( t or χ2 = 0.23-0.54, P = 0.523-0.890). There was no significant difference in endometrial blood flow typing between successful pregnancy and failed pregnancy groups ( χ2 = 0.217, P = 0.897). PI, RI, and S/D in the successful pregnancy group were (2.46 ± 0.29), (1.07 ± 0.21), and (0.57 ± 0.10), respectively, which were significantly lower than (2.71 ± 0.34), (1.35 ± 0.24), and (0.66 ± 0.11) in the failed pregnancy group ( t = 4.51, 5.27, 3.43, all P < 0.001). VI, FI, and VFI values in the successful pregnancy group were (23.04 ± 2.95), (32.26 ± 3.17), and (6.59 ± 0.94) respectively, which were significantly higher than (16.85 ± 2.17), (28.42 ± 3.04), and (4.88 ± 0.86) in the failed pregnancy group ( t = 10.94, 6.25, 8.37; all P < 0.001). Conclusion:Endometrial blood flow parameters such as PI, RI, S/D, VI, FI, and VFI determined by transvaginal three-dimensional power ultrasound have a certain predictive value for pregnancy outcomes after in vitro fertilization and embryo transfer. They can be used as sensitive indexes for evaluating endometrial receptivity.

Objective:To investigate the effects of Qingfei Shenshi Decoction on the expressions of matrix metalloproteinase-2 (MMP-2) and MMP-9 and tissue inhibitor of metalloproteinase timps-1 (TIMP-1) in lung tissue of asthma mice.Methods:Totally 50 male BALB/C mice were divided into 5 groups: normal group, model group, dexamethasone group, Qingfei Shenshi Decoction low- and high-dosage groups (10 mice /group) according to random number table method. Asthma model mice were prepared by ovalbumin (OVA) challenge method. After successful modeling, the dexamethasone group was given dexamethasone for gavage at the rate of 1.56 mg/kg, while Qingfei Shenshi Decoction groups were given high and low doses of Qingfei Shenshi Decoction for gavage at the rate of 14.235 g/kg and 28.470 g/kg, respectively. Normal group and model group were given 0.9% sodium chloride solution by gavage. At the end of gavage administration for 4 weeks, the airway reactivity (Penh value) in each group was detected; HE staining was used to observe the pathological changes of lung tissue; the contents of interleukin-6 (IL-6), interleukin-1β (IL-1β) and tumor necrosis factor -α (TNF-α) in alveolar lavage fluid were determined by enzyme Linked immunosorbent assay (ELISA); the expressions of MMP-2, MMP-9 and TIMP-1 in lung tissue were detected by Western-blot.Results:Compared with model group, the damage of airway wall and alveolar wall of lung tissue in Qingfei Shenshi Decoction groups was significantly reduced. Compared with model group, the Penh value, IL-6, IL-1β and TNF-α levels in Qingfei Shenshi Decoction low- and high-dosage groups decreased ( P<0.05), and the expressions of MMP-9, MMP-2 and TIMP-1 in lung tissue decreased ( P<0.05), with a certain dose dependence. Conclusion:Qingfei Shenshi Decoction can effectively alleviate airway inflammation, reduce airway hyperresponsiveness, improve lung function and inhibit airway remodeling in asthmatic mice. Its mechanism may be related to down-regulating the expressions of MMP-2, MMP-9 and TIMP-1.

【Objective】 To investigate the effect of exosomes produced by hepatitis B virus (HBV)-infected cells on the phenotype and function of macrophages. 【Methods】 The exosomes secreted by HepAD38 cells, which were capable of producing HBV and HepG2 cells, were collected by ultracentrifugation combined with immunosorbent method.The quality and purity of the extracted exosomes were verified by nanoparticle tracking analysis (NTA), scanning electron microscope and Western blot.The M0 THP-1 macrophages differentiated by PMA were stimulated by HepAD38 derived- or HepG2 derived exosomes.Total RNA and protein samples were collected at different time points after stimulation.Real-time fluorescence quantitative PCR (qRT-PCR) and enzyme-linked immunosorbent assay (ELISA) were used to detect cytokine mRNA and protein expressions, respectively.Meanwhile, neutral red assay was performed to analyze macrophage pinocytosis activity, and a commercial kit was used to measure reactive oxygen species (ROS) in THP-1 macrophages.Human reverse transcription chip detection was performed to obtain the microRNAs profile of the exosomes.And the effect of selected miRNA on macrophages was further confirmed by qRT-PCR. 【Results】 Compared with HepG2-derived exosomes, HepAD38-derived exosomes increased the mRNA and protein expressions of IL-1β, MCP-1 and TNFα significantly.However, no difference of pinocytosis capacity or ROS production was found between the HepAD38-derived exosomes group and HepG2-derived exosomes group.Human reverse transcription chip detection results were verified by KEGG analysis and qRT-PCR, and it was found that miR-6824-3p could also significantly increase the expression levels of IL-1β, MCP-1 and TNFα after high expression. 【Conclusion】 This study found that exosomes produced by HepAD38 cells may stimulate macrophages to produce inflammatory factors such as IL-1β, MCP-1 and TNFα through miR-6824-3p, thereby playing a role in HBV infection.

Objective:To investigate the effects of ultrasound-guided adductor block with chloroprocaine combined with fentanyl on analgesia and early rehabilitation after total knee arthroplasty.Methods:Eighty-eight patients who underwent total knee arthroplasty in Wenzhou Hospital of Integrated Traditional Chinese and Western Medicine between June 2018 and June 2020 were included in this study. They were randomly divided into control and study groups, with 44 patients per group. The control group was subjected to distal adductor block with ropivacaine. The study group received distal adductor block with ropivacaine combined with fentanyl. Before and after distal adductor block, resting pain response was evaluated using the Visual Analogue Scale. The success rate, onset time of block, and duration of block effect were recorded. The quadriceps femoris muscle strength, knee joint range of motion, and Hospital for Special Surgery knee score before and after block were measured. The time spent in stand-to-walk test and 10-meter walking test, and incidence of falls during rehabilitation training as well as drug-related adverse reactions were recorded.Results:There were no significant differences in resting-state and task-state Visual Analogue Scale scores post-block between the two groups ( t = 0.43, 0.46, P = 0.689, 0.644). The onset time of block and duration of block effect in the study group were (5.02 ± 0.94) minutes and (2.64 ± 0.39) minutes, respectively, which were significantly shorter than those in the control group [(7.49 ± 1.12) minutes, (7.08 ± 0.92) minutes, t = 5.73, 13.02, both P < 0.001]. There was no significant difference in block success rate between study and control groups (100.0% vs. 100.0%, χ 2 = 0.00, P = 1.000). The quadriceps femoris muscle strength, knee range of motion, and Hospital for Special Surgery knee score in the study group were (4.68 ± 0.44), (112.57 ± 9.96) o and (70.56 ± 6.84) points, which were superior to those in the control group [(4.19 ± 0.42), (101.30 ± 9.67) o,(62.47 ± 6.16) points, t = 3.42, 4.64, 6.58, all P < 0.001). The time spent in stand-to-walk test and 10-meter walking test were (7.95 ± 1.48) minutes and (4.67 ± 0.63) minutes, respectively, which were significantly shorter than those in the control group [(13.41 ± 2.05) minutes, (6.24 ± 0.77) minutes, t = 8.23, 6.74, both P < 0.001). The incidence of falls in the study group was significantly lower than that in the control group (11.3% vs. 29.5%, χ 2 = 4.47, P = 0.034). There was no significant difference in total incidence of drug-related adverse reactions between the two groups (9.1% vs. 13.6%, χ 2 = 0.45, P = 0.502). Conclusion:Distal adductor block with cloprocaine combined with fentanyl meets the requirement of short-term intensive analgesia after total knee arthroplasty, and achieves sufficient analgesia, maximum retention of motor function, rapid onset, and repaid recovery.

RESUMO

Zika virus(ZIKV) is an arbovirus transmitted mainly by mosquitoes. ZIKV infection usually causes mild self-limiting disease, and most of the infected patients only developed relatively mild symptoms which were often easily overlooked. However, serious damage of the central nervous system is observed in a few of the infected patients, and microcephaly of embryos and fetuses in pregnant women infected by ZIKV has attracted wide attention in recent years. Although there is a high proportion of asymptomatic infection and a high incidence of viremia, ZIKV has not been included in the routine pathogen screening before blood donation in China yet, and there is a risk of transmission through blood transfusion. This article briefly introduces the biological characteristics, epidemic characteristics, and pathogenic mechanisms of ZIKV,, and discusses ZIKV infection and the safety of blood transfusion.

Objective:To investigate the clinical pathway operation of medical institutions in Nanjing from 2016 to 2018.Methods:Thirty-three medical institutions(15 tertiary hospitals and 18 secondary hospitals)in Nanjing were investigated for their clinical pathway operation. The relevant data such as the number of specialties, types of diseases, number of cases to be admitted, number of cases admitted, number of cases completed and number of cases of variation were counted.Results:From 2016 to 2018, the entry rate and completion rate of clinical pathway in Nanjing were relatively stable, maintaining at about 80% and 90%. The variation rate increased year by year from 7.06% to 16.50%, and the clinical pathway management ratio increased from 25.18% to 39.90%.Conclusions:The number of clinical pathway specialties and diseases in Nanjing medical institutions still need to be improved. The coverage of clinical pathway should be expanded from system design, path refinement, technology integration and other aspects, and improve the management ratio and operation efficiency of the pathway.

Objective To evaluate the predictive value of cervical size change rate and morphological distri-bution detected by ultrasound in the second trimester of pregnancy for preterm delivery .Methods From June 2016 to June 2018,300 pregnant women who underwent antenatal testing and gave birth in ,the Integrated Traditional Chinese and Western Medicine Hospital of Wenzhou were selected in this research.The length,width and shape of cervix were measured by transvaginal color doppler ultrasonography at 14 and 28 weeks respectively.According to whether or not premature delivery occurred , they were divided into premature delivery group and full-term delivery group.The differences of cervical length ,cervical inner mouth width ,cervical length shortening rate ,cervical inner mouth width increasing rate and cervical shape distribution between preterm and full-term pregnant women were compared ,and the correlation between the above indicators and the occurrence of premature delivery were analyzed .Results The length of cervix in the preterm delivery group [(21.41 ±6.28)mm] was significantly shorter than that in the full-term delivery group at 28 weeks of gestation [(34.17 ±5.76)mm](t=10.295,P=0.000),and the width of cervi-cal inner mouth in the preterm delivery group [(9.54 ±2.57) mm] was significantly longer than that in the full-term delivery group [( 4.06 ±0.91 ) mm] ( t =25.416, P =0.000 ).The shortening rate of cervical length [(39.28 ±8.65)%] and the increasing rate of cervical width [(149.74 ±15.09)%] in the preterm pregnant women during the second trimester were significantly higher than those in the full-term pregnant women [(10.15 ± 5.29)%and (16.33 ±3.84)%] (t=21.471,39.420;P=0.000,0.000).The proportion of T-type cervix in the preterm delivery group (77.17%) was significantly higher than that in the full-term delivery group (63.28%) (χ2 =10.935,P=0.001).According to the correlation analysis ,the length of cervix was negatively correlated with premature birth ( r=-0.612,P=0.035),and the width of cervical inner mouth ,the shortening rate of cervical length in the second trimester and the increasing rate of cervical inner mouth width in the second trimester were posi-tively correlated with premature birth (r=0.743,0.665,0.807;P=0.013,0.026,0.004).Conclusion Ultrasound monitoring of cervical size change rate and cervical morphology during the second trimester of pregnancy is helpful to the early prediction of preterm labor ,which should be paid attention to clinically.

Objective@#To understand the working experience of responsible nurses after the day surgery in the ward under the concept of accelerated rehabilitation surgery, in order to provide reference for the development of targeted interventions.@*Methods@#In this study, 10 primary nurses in 4 departments of day surgery in Shanghai Tenth People′s Hospital were interviewed by using the method of objective sampling in April-May, 2018. The semi-structured interview was conducted using the phenomenological research method in the qualitative study, and the Colaizzi phenomenological data was used. The analysis method analyzes the interview data.@*Results@#Through the repeated refinement, analysis and extraction of the interview data, the following two themes reflecting the work experience of the daytime surgical department responsible nurses: own growth and harvest, difficulties and challenges.@*Conclusion@#Based on the concept of accelerated rehabilitation surgery, daytime surgery is carried out to encourage responsible nurses to continuously improve their knowledge level and interpersonal communication skills. The nurses' professional identity is continuously improved. At the same time, due to the increase in nursing workload, the increase in nurse training needs and the incomplete workflow make nursing management Facing new problems and challenges.

ObjectiveTo investigate the role of lymphocyte apoptosis and endoplasmic reticulum stress (ERS) on the development of sepsis and their association with the prognosis of sepsis patients.Methods A prospective cohort study was conducted. Seventy septic patients admitted to intensive care unit (ICU) of Shanghai East Hospital of Tongji University were enrolled. Blood samples were collected on days 1, 3 and 7 to measure percentage of circulating apoptotic lymphocyte with flow cytometry analysis. The relative expressions of endoplasmic reticulum specific glucose regulated protein 78 (GRP78) mRNA and transcription factor CHOP mRNA were measured by real-time reverse transcription-polymerase chain reaction (RT-PCR). The correlation between CHOP mRNA expression and percentage of circulating apoptotic lymphocyte was analyzed by Spearman relative analysis. The patients were divided into death (n = 23) and survival subgroups (n = 47). Twenty healthy volunteers during the same period were selected as the healthy control group.Results① Rate of lymphocyte apoptosis: compared with healthy control group [(2.86±0.66)%], septic patients, either survival or death subgroup, exhibited higher rate of lymphocyte apoptosis on days 1, 3 and 7 [survival subgroup: (12.44±4.43)%, (8.57±3.38)%, (6.78±3.35)%; death subgroup: (14.42±2.01)%, (11.32±2.53)%, (8.87±3.62)%, allP 0.05], and GRP78 mRNA expression in the death subgroup was gradually lowered, but it was still higher than that in the healthy control group [3 days: (7.27± 3.64)×10-3, 7 days: (5.23±1.94)×10-3, bothP< 0.01].④ Spearman relative analysis showed that the expression of CHOP mRNA was positively correlated with the rate of lymphocyte apoptosis (r = 0.414,P = 0.000 ).Conclusion The increase in the rate of lymphocyte apoptosis and activation of ERS play an important role in the development of sepsis, and it is associated with worse outcome in the septic patients.

Objective To study the clinical characteristics,pathogens,drug sensitivity and prevention measures in malignant hematological disease combined with septicemia.Methods The clinical data of 95 malignant hematological disease combined with septicemia patients from January 2009 to June 2013 were retrospectively analyzed.Results One hundred and ten episodes of septicemia occurred in all the 95 patients,among which acute leukemia patients accounted for 69.1% (76/110),patients with neutrophil counts <0.5 × 109/L accounted for 82.7% (91/110),and patients with agranulocytosis time >7 days accounted for 66.4%(73/110).Major pathogenic bacteria was gram negative bacteria,including Escherichia coli 30.9%(34/110),Klebsiella pneumoniae (14.5%,16/110),Pseudomonas aeruginosa (12.7%,14/110).Carbapenems and amikacin was the most sensitive in susceptibility testing.Ninety-eight episodes of septicemia cured or improved,and 12 patients died,with a mortality of 12.6% (12/95).Fifteen patients had septic shock,of whom 7 patients were dead.Conclusions Septicemia is very serious for malignant hematological patients,which may lead to rapidly progress,high incidence to septic shock and high mortality.Early prevention and treatment is important for patients,and strong anti-infection and supportive treatment is needed.

<p><b>OBJECTIVE</b>To elucidate the role of let-7d in regulating the biological behavior of ovarian cancer cells and their expressions of HMGA2 and ras proteins.</p><p><b>METHODS</b>The pre-let-7d sequence was synthesized and inserted into pcDNA6.2GW/EmGFPmiR and transfected into ovarian cancer IGROV1 cells to cause pre-let-7d overexpression. Real-time quantitative RT-PCR was employed to examine the expression levels of let-7d miRNA and HMGA2 mRNA, and Western blotting was performed to detect the expressions of HMGA2 and ras protein in the transfected cells. The effect of pcDNA6.2GW-let-7d transfection on IGROV1 cell proliferation was determined using MTT assay and the cell apoptosis rate was measured using flow cytometry.</p><p><b>RESULTS</b>The eukaryotic expression vector containing the target gene let-7d was successfully constructed and transfected into IGROV1 cells. The transfected cells showed a marked reduction of HMGA2 expression but a less obvious down-regulation of ras expression. Transfection with pcDNA6.2GW-let-7d to suppress the expression of HMGA2 caused alterations of the phenotype of IGROV1 cells shown by a reduced proliferative activity and increased cell apoptosis.</p><p><b>CONCLUSION</b>Let-7d plays an important role in altering the malignant cell phenotype of ovarian cancer IGROV1 cells by regulating the expression of HMGA2.</p>

<p><b>OBJECTIVE</b>To characterize a new alternative splicing isoform of human vascular endothelial growth factor (VEGF) gene.</p><p><b>METHODS</b>The total RNA was extracted from the lung tissue of a legally aborted 4-month-old fetus and amplified by RT-PCR. The amplified product was cloned into the plasmid pMD18-T and plasmid pcDNA3.1- for sequence analysis.</p><p><b>RESULTS</b>Electrophoresis of the RT-PCR products displayed one short band for VEGF(121) (487 bp) and a long band. The latter was characterized to contain two fragments: one was normal VEGF(165) (619 bp), and the other (639 bp) had an identical nucleotide sequence to VEGF(165) with a 20 bp fragment inserted between exons 3 and 4. Sequence analysis showed that this 20-bp nucleotide was inserted from the 3' end of the third intron containing a splicing signal, thus causing shift mutation in the reading frame of VEGF gene and early appearance of the stop codon UAG in the middle of exon 4.</p><p><b>CONCLUSION</b>A new alternative splicing isoform of VEGF probably exists in the lung tissue of a legally aborted human fetus, and its biological significance remains to be further investigated.</p>

RESUMO

AIM To observe Arg-Glu*ss activity of antiplatelet and anticoagulation in rats and to further explore its mechanism. METHODS Twenty four male sprague dewley rats were divided into three groups randomly (eight rats in each group).Each group was orally given Arg-Glu (61 4 mg?kg -1 ?d -1 ), ASA (36 mg?kg -1 ?d -1 ) and equal placebo solution respectively.Eight days later all rats were given anesthesia and blood was taken from abdominal aorta with a plastic catheter, then the indexes related to blood hemodynamics,coagulation and blood fibrinolysis were measured. Twenty four SD rats were treated in the same way mentioned above. Platelet aggregation was measured by turbidimetric method. Plasma level of cyclic GMP and prostacyclin was evaluated by radioimmunoassay and nitric oxide by spectrophotometric determination. Common carotid artery-external jugular vein bypass was established in another 24 rats, and the weight of wet thrombus was measured with high sensitive electrical scale. RESULTS In group of Arg-Glu, statistically significance decrease versus placebo group was observed on blood apparent viscosity, reduced viscosity, hematocrit and plasma viscosity, increase versus placebo was also observed on blood reciprocal calcium time, activated partial thromboplastin time and prothrombin time ( P 0 05). Platelet aggregation activation decreased compared with placebo 〔(0 75%?3.62%) vs (32 8%?6 62%) P 0 05)〕. CONCLUSION Arg-Glu with its properties of antiplatelet、anticoagulation and improvement of blood rheology status in rats could be regarded as one antiplatelet drug and one NO precursor .Its mechanism of action was thought to be by Arg-NO-cGMP metabolism pathway while other mechanism might be involved.
