RESUMO

PURPOSE: This study was undertaken to understand the determinants of job satisfaction for hospital nurses in Korea. Organization culture is deemed as a strong factor which contributes to overall job satisfaction. METHODS: A systematic review was conducted using five electronic databases to identify Korean studies for the years 1998 to 2017. The Comprehensive Meta-Analysis Software Ver 2.0 was then utilized in data analysis. RESULTS: A meta-analysis of data from 36 studies indicated that the overall effect size of correlation between organizational culture and job satisfaction was moderate (ESr=.36), and publication year was negatively associated with these factors in the meta-regression model. In addition, the magnitude of the types of organizational culture and job satisfaction varied according to size of the hospitals: innovation-oriented culture for secondary hospitals (ESr=.49) and relation-oriented culture for tertiary hospitals (ESr=.46). Lastly, of four different organizational cultures, innovation-oriented culture showed the strongest correlation with job satisfaction (ESr=.50), followed by relation-oriented culture (ESr=.49), and task-oriented culture (ESr=.30). CONCLUSION: Results indicate that nursing organization culture plays a significant role in Korean nurses' job satisfaction. The implication of the study is that creating an innovation-oriented and relation-oriented culture in hospitals may effectively promote nurses' job satisfaction more than hierarchy-oriented culture.

PURPOSE: The purpose of this study was to examine the effects of a neonatal nursing practice program for nursing students on students' stress, self-efficacy, and confidence. METHODS: A 1-group pre- and post- study design was used. The participants consisted of 64 nursing students who were in a pediatric nursing clinical practicum at a nursing college in Seoul from September 2015 to May 2016. The program consisted of 3 stages-orientation, practice, and debriefing-and was conducted for 3 hours during a 2-week period of the pediatric nursing clinical practicum. The dependent variables were neonatal nursing practice stress, self-efficacy, and confidence. Data were analyzed using the paired t-test, the Pearson correlation coefficient, the x² test, and descriptive statistics with SPSS for Windows version 22.0. RESULTS: The neonatal nursing practice program was effective at decreasing clinical practice-related stress and increasing confidence and self-efficacy regarding neonatal nursing practice. CONCLUSION: The neonatal nursing practice program may effectively promote the integration of theoretical knowledge and practice. In the future, we propose to develop various educational programs that reinforce basic nursing skills for neonates as part of an effective pediatric nursing clinical practicum.

Leaf of Sasa borealis, a species of bamboo, has been reported to exhibit anti-hyperglycemic effect. However, its antidiabetic mechanism is not fully understood. In this study, we examined whether an extract of S. borealis activates AMP-activated protein kinase (AMPK) and exerts anti-hyperglycemic effects. Treatment with the S. borealis extract increased insulin signaling and phosphorylation of AMPK and stimulated the expression of its downstream targets, including PPARalpha, ACO, and CPT-1 in C2C12 cells and PPARalpha in HepG2 cells. However, inhibition of AMPK activation attenuated insulin signaling and prevented the stimulation of AMPK target genes. The S. borealis extract increased glucose uptake in C2C12 cells and suppressed expression of the gluconeogenic gene, PEPCK in HepG2 cells. The extract significantly reduced blood glucose and triglyceride levels in STZ-induced diabetic mice. The extract enhanced AMPK phosphorylation and increased Glut-4 expression in the skeletal muscle of the mice. These findings demonstrated that the S. borealis extract exerts its anti-hyperglycemic effect through activation of AMPK and enhancement of insulin signaling.

Nosocomial Legionnaires' disease has often been documented to occur in immunocompromised patients and to be severe, potentially fatal, pneumonia. We report a case of fatal nosocomial Legionnaires' disease developed shortly after steroid pulse therapy. A 39-year old woman with systemic lupus erythematosus was admitted via emergency room due to generalized edema and gross hematuria. Under the diagnosis of lupus nephritis, she was given intravenous steroid pulse therapy for 3 days and then maintained with oral prednisolone. On the 7th day of admission the patient's conditions got worse with progression to acute renal failure and respiratory difficulty. On the 10th day of admission when she was started on hemodialysis, chestradiograph showed newly developed multifocal mass-like consolidations on both lung fields. In spite of empirical therapy with roxithromycin and rifampin, the consolidations were aggravated and rapidly extended to both whole lung fields. On the 15th day of admission she was mechanically ventilated due to respiratory failure, but died of hypoxia and shock on the 19th day. Later, a legionella species was isolated from the tracheal aspirates and identified as L. pneumophila serogroup 1. We also detected L. pneumophila from the tracheal aspirates by duplex PCR which amplified both 5S rRNA and mip genes of L. pneumophila.

BACKGROUND: The age group of Rubella virus (RV) infection in most industrialized nations located in temperate climates, has in large part shifted from children to young adults since introducing of the RV vaccine in late 1960's. Interestingly, there were rubella outbreaks from 1995 through 1996 in Korea, and middle and high school students were mostly affected during that time. Although continued cycling of the rubella epidemics, there were no reports about the isolation or genetic information of rubella viruses circulated in Korea. METHOD: To isolate RV circulated in Korea, and determine the phylogenetic relationship between RV strains in Korea and RV isolates from other geographic regions including vaccine strains, we inoculated nasopharyngeal secretion samples from clinically diagnosed rubella patients to Vero E6 cells, and sequenced corresponding region of the 5' E1 encoding genomic regions of RV isolates. RESULT: Seven RV strains isolated from Korea showed 93.6 to 97.8% and 98.3 to 100% sequence homologies in nucleotide and amino acid levels, respectively, compared to RA27/3 vaccine strain. Phylogenetic tree based on 359bp of RV indicated that at least two different groups of RV circulated in Korea during 1995-1996 epidemics. CONCLUSION: Our data suggested that mutant RV strains were possibly not the cause of recent rubella epidemics in Korea.
