RESUMO

OBJECTIVE@#To explore the clinical application of lockedge suspension combined with three steel wires vertical fixation in comminuted fracture of inferior pole of patella.@*METHODS@#From August 2016 to May 2019, 23 patients with comminuted fracture of the lower pole of the patella, including 14 males and 9 females, were treated with lockedge suspension combined with three steel wires vertical fixation. The age ranged from 34 to 68 (55.0±1.2) years. One year after operation, the pain and function were evaluated by pain visual analogue scale(VAS) and knee flexion and extension range of motion, and the clinical efficacy was evaluated by Lysholm knee score standard.@*RESULTS@#All 23 patients were followed up for 12 to 14, with a mean of(13.0±0.5) months. One patient had skin irritation by the tail of the steel wire, and the rest had no postoperative complications such as incision infection, internal fixation loosening and fracture displacement. The fractures of 23 patients were healed, and the healing time was 10 to 14 weeks with a mean of(12.0±1.1) weeks. The VAS score decreased from 7.96±0.93 before operation to 0.83±0.65 one year after operation. The range of knee flexion and extension activities increased from(20.30±8.69) ° before operation to 1 year after operation(127.39±6.55) °. Lysholm knee score increased from 18.48±4.00 before operation to 96.09±4.91 one year after operation(P<0.05).@*CONCLUSION@#The treatment of comminuted fracture of the lower pole of patella by lockedge suspension combined with three steel wires vertical fixation has reliable fixation and high fracture healing rate. It can meet the requirements of rapid rehabilitation and functional exercise, and the early clinical effect is satisfactory.

<p><b>OBJECTIVE</b>To analyze the cause of in-hospital death among acute myocardial infarction patients undergoing primary percutaneous coronary intervention (PPCI) in Beijing area to evoke better individualized preventive approach.</p><p><b>METHODS</b>In-hospital mortality and causes were analyzed based on database from Beijing percutaneous coronary intervention registry study (BJPCI Registry) in 2010.</p><p><b>RESULTS</b>A total of 4660 PPCI patients from 48 hospitals were included. In-hospital mortality was 2.4% (n = 110). Cardiogenic shock (39.1%, 43/110), mechanical complications (28.2%, 31/110) and intervention-related complications [28.2%, 31/110: procedure related (n = 28), drug related (n = 3)] were the leading causes of in-hospital death. Five deaths was attributed to comorbidity related reason (4.5%, 5/110). The in-hospital mortality had no significant difference among hospitals of different grade or total annual PCI (all P > 0.05). In-hospital mortality was slightly higher in hospital with annual PPCI < 300 than in hospitals with annual PPCI ≥ 300 (2.9% vs. 1.8%, P < 0.05).</p><p><b>CONCLUSION</b>Cardiogenic shock, mechanical complications and intervention-related complications are the main causes of in-hospital death among acute myocardial infarction patients receiving PPCI.</p>

<p><b>OBJECTIVE</b>To compare craniofacial structural characteristic of syndromic and non-syndromic patients with cleft palate and to probe into the reason of their maxillary hypoplasia and to lay a foundation for better treatment protocol for syndromic patients with cleft palate.</p><p><b>METHODS</b>The sample consisted of 8 individuals with Treacher Collin syndrome, 9 patients with Pierre Robin syndrome, and 40 patients with non-syndromic cleft palate which met certain criterions. Lateral cephalometric radiographs were obtained from each study subject. A total of 18 variables, comprising 9 angular, 7 linear, and 2 ratio measurements, were studied. SPSS 12.0 software was used to describe the statistics data, consisting of means, SD, and t test.</p><p><b>RESULTS</b>The tendency for smaller value in SNB and larger value in ANB indicate a severer anteroposterior mandibular hypoplasia in the syndromic type cleft. There was no difference between syndromic and non-syndromic cleft palate patients with regards to maxillary anteroposterior development. No significant difference on the development in cranial base area was found either.</p><p><b>CONCLUSION</b>This interplay of cranio-facial bones is very important in the development of this complex, which will not only affect the growth but also the defect development. In this study, the patients with the two syndromes showed the similar growth potential of maxilla with the non-syndromic cleft patients.</p>

<p><b>OBJECTIVE</b>To explore the relation of body mass index (BMI) and waist circumference (WC) of the aged with clustering of other cardiovascular risk factors.</p><p><b>METHODS</b>Total 654 old people were checked up, and the systolic blood press (SBP), diastolic blood press (DBP), total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C) and fasting plasma glucose (FPG) of them were investigated according to different BMI and WC, then the other cardiovascular risk factors analysis was carried out on the detection rates of SBP, DBP, TC, HDL-C, FPG level and high blood pressure, high TC, high TG. The clustering of the other cardiovascular risk factors was analyzed.</p><p><b>RESULTS</b>The rising of BMI and WC was regular cardiovascular risk factor in the aged. The average level of SBP, DBP, TC, TG, and FPG of the aged was obviously rising along with BMI and WC increasing, meanwhile the HDL-C was obviously decreased, and the detection rates of high blood pressure, high TC, TG and diabetes mellitus were obviously increased. When the BMI was 24 kg/m(2)-27.9 kg/m2, the detection rates of high blood pressure, high TC, TG and diabetes mellitus were 59.74%, 3.89%, 28.57%, 10.06% respectively, and their risks of suffering high blood pressure, high TC and TG were 1.65, 1.88, 1.85 times of those with normal BMI. When the BMI > or = 28.0 kg/m2, the detection rates of high blood pressure, high TC, TG and diabetes mellitus were 83.05%, 5.08%, 35.59%, 15.25% respectively, and their risks of suffering high blood pressure, high TC and TG were 5.44, 2.60, 2.98 times of those with normal BMI. When the man WC > or = 85 cm or women WC > or = 80 cm, the detection rates of high blood pressure, high TC, TG and diabetes mellitus were 66.15.%, 4.47%, 29.57%, 10.12%, and their risks of suffering high blood pressure, high TC and TG were 3.52, 6.51, 1.68 times of those with normal BMI. The rate of the aged with several cardiovascular risk factors was significantly increased for those with BMI > or = 24.0 kg/m2 or man WC > or = 85 cm or women WC > or = 80 cm.</p><p><b>CONCLUSION</b>The rising of BMI and WC should be important factor for the clustering of cardiovascular risk factors in the aged.</p>

Objective To investigate the alterations and molecular mechanism of neural cell apoptosis after frontal lobe sharp injury in infant rats.Methods Brain damage was induced by frontal lobe sharp injury.dUTP nick end labeling(TUNEL) method was used to detect cell apoptosis after brain injury,and immunohistochemistry was used to detect the expression of Bax,Bcl-2 proteins in and around the area of injury,respectively.Results The apoptosis cells were found as early as 1 hour and reached a peak at 24 hours post-injury,then gradually diminished until 120 hours when only few apoptosis cells presented.The expression of Bax and Bcl-2 proteins increased significantly 1 hour after frontal lobe injury.Bax protein reached its peak at 12 hours while Bcl-2 protein reached its peak at 1 hour post-injury.The ratio of Bax to Bcl-2 increased after lesion with peak at 24 hours,and then decreased thereafter.At 120 hours,very weak expression of Bax and Bcl-2 protein detected.There were few Bax and Bcl-2 positive cells in frontal lobe cortex in blank control and sham operation group.Conclusions Cell apoptosis exists after frontal lobe sharp injury in infant rats and the change of the number of apoptotic cells is correlated with time course of post-injury.The ratio of Bax to Bcl-2 may play a vital role in cell apoptosis.

RESUMO

The optimum medium and fermentation conditions of the Xenorhabdus nematophilus from Steinernema carpocapsae BJ strain were studied. The relationship between antibiotic activity and pH, reducing sugar, total sugar, amino-nitrogen in process of fermentation was analyzed. The optimal medium contained tryptonl. 5 %, corn powderl o4, soybean flour 3 %, sucrose1 %KH2PO4 0.02 % ,MgSO4 0. 2% and activator 0. 1%, Stock cultured for 16h, inoculum size at 4%(V/V)and primary pH of medium ranged from 6.0 to 8.0, fermentation for 72h were of benefit to the yield of antibiotic. The pH, reducing sugar, total sugar and amino nitrogen in process of fermentation were related to the antibiotic activity. The yield of antibiotic increased by 56. 3 % comparison with nutrient broth.

The symbiotic bacterium exists in the intestines of entomopathogenic nematodes and is a potential biological agent.Systematic classification of these bacteria is scarce in China.In this paper,seven strains of symbiotic bacteria from local entomopathogenic nematodes were identified by both observation of mor-phology,physiological,biochemical characteristics and sequence analysis of 16S rDNA fragments.
