RESUMO

PURPOSE: We experienced a case of ocular motility disturbance with esotropia and hypotropia, following implantation of Molteno to the superotemporal quadrant. METHODS: The patient recovered to orthophoria after removal of implant and fibrotic scar tissue with recession and resection. RESULTS: Multiple complications have been reported in association with Molteno implantation, including hypotony, cataract, choroidal hemorrhage, retinal detachment, endophthalmitis, hyphema, phthisis. However ocular motility disturbances have been documented in only a few cases and never in Korea to our knowledge. Therefore, we report this case with the review of related literatures.

RESUMO

PURPOSE: Patients with canalicular or common canalicular obstruction have been treated by carrying out conjuctivodacryocystorhinostomy mainly through skin or the nasal cavity, but all sorts of defects attendant on an operation have been indicated. At this, the authors inquired into a result of a surgery and their satisfaction in a different viewpoint for silicone intubation. METHODS: We inquired into a result of a surgery and their satisfaction in a different viewpoint for 68 patients' 72 eyes who can be observed over 3 months among patients diagnosed as canalicular or common canalicular obstruction and operated on with silicone intubation as subject. RESULTS: After surgery, silicone tube was removed at average postoperative 5.8 months and we observed them for average 10.5 months. The result of operations showed that successful cases are 72.2% (52 eyes) and failing cases are 28.8% (20 eyes). CONCLUSION: For the cases with canalicular obstruction, especially common canalicular obstruction for short period, primary silicone intubation would be one of the ideal lacrimal surgery that should be explained the possible secondary conjuctivodacryocystorhinostomy later.

RESUMO

Lipoprotein(a), a complex formed by apolipoprotein(a), apo B-100 and lipids, is considered an independent, genetically determined, predictor of cardiovascular disease. It may have antifibrinolytic properties in view of its similarity to plasmmogen. To evaluate whether lipoprotein(a) may be a risk factors in patients with diabetic retinopathy or not, we measured the circulating serum level of lipoprotein(a) in each group classified by the severity of diabetic retinopathy and control group. The serum lipoprotein(a) level was higher in the diabetic patients than in the control group, and diabetic retinopathy, which was expressed by the grade of retinopathy in the worse eye, was correlated significantly with the duration of disease, and the serum level of lipoprotein(a) independently(P<0.05). There was no correlation between the diabetic retinopathy and the age at the time of diagnosis, systolic or diastolic blood pressure, or the serum levels of HbA(IC), cholesterol, triglyceride, and total lipid. We concluded that the lipoprotein(a) may play a role as one of the independent risk factors in diabetic retinopathy.

RESUMO

Osteoma of the paranasal sinus is benign tumor, and it is mostly located in frontal and ethmoid sinus, produces symptoms of headache, nasal obstruction and ocular complication of exophthalmos, ocular pain, restricted ocular motility, and diplopia. Recently the authors have experienced one case of osteoma of ethmoidal sinus which could be removed by external ethmoidectomy.

RESUMO

We did a follow-up study of 49 patients who had vitrectomy with removal of the macular epiretinal membrane. The patients were followed over 3 months (3-86 months). The patients had an average age of 39.3(13-72) years. Changes of postoperative visual acuity, complications, cause of poor visual outcome, and the prognostic factors were evaluated. 34 eyes(69.4%) showed improvement of visual acuity two lines or more compared to preoperative visual acuity and 25 eyes(51.0%) achieved a final visual acuity of 0.5 or better. Complications were noted in 13 eyes. They were consisted of cataract progression in 6 eyes, retinal break in 3 eyes, recurred membranes in 2 eyes, increased intraocular pressure and endophthalmitis in each eye. The causes of poor visual outcome related with the complications and the preoperative macular damages. Among the various prognostic factors, the preoperative visual acuity and the duration of disease show statistical significance in this study(p<0.001, p<0.002).
