RESUMO

ObjectiveTo investigate the association of lipid ratios with diabetes and pre-diabetes in residents aged 35-75 years in Changzhou City. MethodsA multistage whole-group random sampling method was used to survey permanent residents aged 35-75 years in Tianning and Wujin districts of Changzhou City, and the study data were obtained by questionnaires, physical examination and laboratory tests. The relationship between lipid ratios and diabetes and pre-diabetes was analyzed by dichotomous logistic regression method. ResultsThe prevalence of diabetes in the surveyed population in Changzhou was 18.69%, and the prevalence of pre-diabetes was 10.53%. In the total population, the risk of pre-diabetes was significantly increased in the highest TC/HDL-C, TG/HDL-C and LDL-C/HDL-C groups, by 68%, 93% and 38%, respectively; the risk of diabetes was also significantly increased in the highest TC/HDL-C, TG/HDL-C and LDL-C/HDL-C groups, by 105%, 149% and 78%, respectively. The risk of diabetes was also significantly increased in the highest TC/HDL-C, TG/HDL-C and LDL-C/HDL-C groups, by 105%, 149% and 78%, respectively. All three lipid ratios increased in women compared to men, leading to a stronger association with increased risk of diabetes and pre-diabetes. ConclusionLipid ratios TC/HDL-C, TG/HDL-C, and LDL-C/HDL-C were correlated with the risk of diabetes and pre-diabetes in people aged 35-75 years, with TG/HDL-C having the strongest association with diabetes and pre-diabetes, and is expected to be a key predictor for assessing the development of diabetes.

RESUMO

Objective To analyze the factors that influence the articular function after open reduction and internal fixation for ankle fractures. Methods From July 2014 to January 2016,111 ankle fractures received surgery in our hospital. There were 54 males and 57 fenales, with a mean age of 43.5 years (frown 18 to 75 years).By the Broos & Bisschop classification, there were 43 unimalleolar,38 bimalleolar and 30 trimalleolar fractures. The postoperative articular function was evaluated according to the American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot scoring at the final follow-up.Comparisons were made between the patients with excellent functional scores and those with lower than excellent functional scores. The potential influencing factors were analyzed, including age, gender, body mass index, smoking history, primary hypertension history, diabetes history, injured side, fracture cause,Broos & Bisschop classification, combined presence of obvious inferior tibiofibular separation, combined presence of ankle dislocation, combined presence of other injury, time from injury to surgery, physical status classification by the American Society of Anesthesiologists system, operation time, early functional exercise, removal of internal fixation and postoperative complications. The influencing factors were identified using univariate analysis and logistic regression analysis. Results The average follow-up period was 15.5 months (from 8 to 25 months). The clinical union time of the fractures averaged 3.2 months (from 3 to 5 months). The ankle function at the final follow-up was excellent in 58 cases, good in 48,fair in 5 and poor in 0,with an excellent and good rate of 95.5%.Removal of internal fixation (P =0.001),early functional exercise (P =0.002),and postoperative complications (P =0.049) were identified as the independent risk factors influencing the articular function after surgery for ankle fractures. Conclusion For patients with ankle fracture, enhancing intraoperative procedures, reducing postoperative complications, encouraging the patients to do early functional exercise, and removing internal fixation after firacture union can effectively improve their ankle function.

RESUMO

Fragility fracture caused by osteoporosis affects the health of the elderly. The International Osteoporosis Foundation (IOF) advocate that the fracture liaison service (FLS) can improve the detection rate, treatment rate and compliance of patients with fragility fractures, and hence to reduce the incidence of secondary fractures. Currently, there are different models of FLS available globally, appropriate management is the key to effective implementation of FLS projects.

RESUMO

Objective:To study the effects and mechanisms of Hongqi formular combined with exercise preconditioning against myocardial ischemic reperfusion injury in rats.Method:Fifty health male SD rats were randomly divided into sham,IR,EP,HP and EHP group. After establishing of EP animal model, the rats were ligated ramus descendens coronary artery to induce myocardial ischemic reperfusion model. The serum cardiac troponin Ⅰ (cTn Ⅰ),myocardium IL-6,TNF-α were detected by enzyme linked immunosorbent assay (ELISA).The myocardium MDA was detected by chemical colorimetry method. The myocardium SOD was detected by xanthine hydrocarbonylation. The serum ET,6-k-PGF1α,TXB2 density were detected by radioimmunoassay, and the myocardium Ca2+ density was detected by atom absorption spectrophotometry.Result:①As compared with the group IR,the serum cTn Ⅰ decreased significantly in groups EP,HP and EHP;As compared with the group EHP,the cTn Ⅰ level in serum was increased in groups EP and HP.②As compared with the group IR,the Ca2+ level in myocardial sarcoplasmic reticulum was decreased significantly in groups EP,HP and EHP;As compared with the group EHP,the Ca2+ level in myocardial sarcoplasmic reticulum was decreased significantly in group EP.③As compared with the group IR,the ET in serum were decreased significantly in groups EP,HP and EHP;As compared with the group EHP,the ET,TXB2 in serum were increased significantly in groups EP and the 6-k-PGF1αin serum were decreased significantly in groups EP.④As compared with the group IR,the IL-6 and TNF-α in cardiac muscle were decreased in groups EP,HP and EHP;As compared with the group EHP,the IL-6 and TNF-α in cardiac muscle were increased in groups EP and HP.⑤As compared with the group IR,the SOD in cardiac muscle was increased significantly in groups EP,HP and EHP and the MDA in cardiac muscle was decreased significantly in groups EP,HP and EHP;As compared with the group EHP,the SOD in cardiac muscle was decreased significantly in groups EP and the MDA in cardiac muscle was decreased significantly in group EP.Conclusion:Hongqi formular combined with exercise preconditioning produces cardio-protective effect during myocardial ischemic reperfusion, which may be related with the enhanced anti-oxidant ability, the decreased blood cell damage, the maintainace of vascular endothelium function, the improved microcirculation and the decreased calcium overloading.

RESUMO

ObjectiveTo analyze the clinical efficacy of open reduction and internal locking system (PHILOS) in management of three- and four-part proximal humeral fractures in the elderly patients.MethodsA retrospective study was performed on 18 elderly patients (age≥80 years) with proximal humeral fractures who were treated surgically from June 2008 to June 2010 and received complete follow-up.The fractures were Neer three- and four-part fractures and managed with PHILOS.Rehabilitation exercise was performed postoperatively and followed up regularly.ResultsThe patients were followed up for average 15.3 months, which showed sound healing of all fractures.Shoulder joint function was assessed by Constant-Murley score that was 55 to 83 points (mean 72.5 points) at 12 months, with an overall excellence rate of 67%.ConclusionOpen reduction and fixation with PHILOS can attain satisfactory clinical outcome for the elderly patients with three- and four-part proximal humeral fractures.

RESUMO

Purpose: To investigate the therapeutic effect of acupuncture and herbs in chloasma. Methods: Chloasma was divided into three patterns and treated by acupuncture plus oral administration and external application of Chinese herbs. Results: There was an obvious therapeutic effect in chloasma treated by combination of acupuncture and herbal drugs. After two courses of the treatments, the effective rate accounted for 93.3% in 45 cases. Conclusion: There was an obvious therapeutic effect in chloasma treated by combination of acupuncture and herbal drugs.
