RESUMO

Objective:To analyze the characteristics of elderly patients with moyamoya disease and clinical outcomes after combined revascularization, and to evaluate the safety and efficacy of this method in elderly patients.Methods:Moyamoya disease patients aged 60 years or older who had undergone combined revascularization at the Neurosurgery Center of Aeronautical General Hospital from January 2016 to September 2021 were selected as study subjects.Then patients' clinical data and postoperative complications within two weeks of surgery were collected and a followed-up was conducted.The results were compared with those of previous reports.Results:A total of 46 combined revascularization procedures were carried out in 30 patients.The ages at surgery ranged between 60-75 years, with a mean age of(66.1±4.4)years.The rate of complications within two weeks after surgery was 37%, and the most common complication was speech dysfunction, followed by epilepsy and hyperperfusion.No serious complications such as cerebral hemorrhage and death occurred.The follow-up varied between 3-67 months, with an average follow-up time of(26.8±18.4)months.The MRS score was <3 at 3 months after surgery, and there was no death or severe disability.Conclusions:Perioperative complications in elderly patients with moyamoya disease have a higher incidence rate than in other populations, but patients can usually recover after close perioperative management for 2 weeks.The follow-up has demonstrated that outcomes at 3 months and in longer terms after surgery are satisfactory and clinical symptoms will improve significantly.

RESUMO

The application and significance of learning method of self-evaluation and mutual-evaluation for practical teaching ofcurriculum were investigated. The practical teaching followed theory teaching in the curriculum plan, which applied grouping pattern to provide demonstration, practice and assessment. According to the technique points of practical teaching, the teachers established evaluation form and listed detailed points of self-evaluation and mutual evaluation. The focus was student assessment, including individual self-evaluation, subject-evaluation intra-group evaluation, external-group evaluation and teacher evaluation. 100 points were taken as maximum score, including 10 points in the self-evaluation from operator, 20 points in the evaluation from receiver, 20 points in the intra-group evaluation, 20 points in external-group evaluation and 30 points in teacher evaluation. Through practical application, it was found learning system of self-evaluation and mutual-evaluation had important significance in promoting mutual learning among students, improving students' learning enthusiasm and initiative, and helping teachers understand and improve their roles.

RESUMO

Purpose To describe clinicopathological features, diagnosis and differential diagnosis of Castleman′s disease. Methods Retrospective analyses of the clinical data, clinicopathology and immunohistochemistry were conducted in ten cases of Castleman dis-ease and reviewed of literature. Results There were 8 cases of unicenrtic Castleman′s disease and 2 cases of multicentric Castleman′s disease. Pathologically, there were 6 cases of hayline vascular types, one case of plasmatcyic type and 3 cases of mixed type in all Castleman′s disease. Immunohistochemically, all cases were negative for BCL-6 and CD10, and Ki-67 expression was less than or e-qual to 30%. There were 4 cases with complete follow-up data, including one case of intermediate type, 3 cases of hyaline vascular type which were healed by surgical resection without recurrence. Conclusions Castleman′s disease is a rare and lymphoproliferative disorders with unknown cause, it is not easy to diagnose before the operation. Whether immunohistochemical features reflect abnormal immune function or play unknown role in the pathogenesis of Castleman′s disease is also demanded further study.

RESUMO

To investige the relationship between expression levels of interferon regulatory factors [IRF] in patients with coronary artery disease. Prospective case-control study. Department of cardiology, Puren Hospital, Wuhan University of Science and Technology, Wuhan, China [PRC]. Between October 2010 and July 2011, 106 patients [aged 37-71 years] were enrolled. Patients were assigned to four groups: acute myocardial infarction [AMI] [n = 28], stable angina [SA] [n = 26], unstable angina [UA] [n= 26], and control group [without CAD, n=26]. Blood samples were drawn from all the patients via the antecubital vein following a 12 h fasting period [HDL-C], low density lipoprotein cholesterol [LDL-C] and high sensitivity C-reactive protein [hs-CRP] were measured. The mRNA and protein expression levels of IRF-1, IRF-2, IRF-4 and IRF-8 in each patient were measured using PCR and western blotting. Serum levels of IL-12 and IFN-gamma were detected by ELISA. Within both the AMI and UA groups, IL-12 and IFN-gamma were significantly increased compared with SA and control patients. Both the mRNA and protein expression levels of IRF-1, IRF-2 and IRF-8 in the AMI and UA groups were significantly higher than those in the SA and control groups. There was no significant difference between the SA and control groups. The increase in expression levels of IRF-1, IRF-2 and IRF-8 may be associated with atherosclerotic plaque formation and rupture, which are seen in AMI and UA

ObjectiveTo investigate the clinic and pathologic features of one patient diagnosed with neurocutaneous melanosis ( NCM ) by biopsy.MethodsA 21-year-old woman presented with a 2-month history of tinnitus,headache,vomiting and 1-month history of impaired vision.At birth,a massive nevus covering most of the posterior abdomen had been noted as well as the presence of multiple smaller lesions all over the body.Magnetic resonance imaging demonstrated a posterior fossa cyst compatible with the Dandy-Walker syndrome and extensive leptomeningeal enhancement. Surgery was performed to cystectomy and to obtain pathologic specimens from the leptomeninges. Biopsy and immunohistochemical study was performed.ResultsAt surgery,diffuse black pigmentation of the leptomeninges and the cyst was found.Under microscope,the cyst and leptomeninges were composed with melanocytes with variable pigmentation.Those cells positive for HMB45,MelanA,S100 and vimentin.Ki-67 positive cells < 1%.The pathologic diagnosis wasleptomeningeal diffusemelanocytosis. Thepatientdied 2months after thesurgery.ConclusionsNCM is characterized by a focal or diffuse proliferation of melanin-producing cells in both the skin and the leptomeninges.NCM could be compatible with the Dandy-Walker syndrome.Definite diagnosis relies upon the histological data obtained by mean of biopsy.

Objective To obtain genetic characterization of the HA1 of new isolates of influenza A (H1N1) virus. Methods Virus was amplified in embryonated chicken eggs, the virion DNA was transcribed into cDNA by reverse transcriptase, cDNA amplified by PCR, the products of PCR were purified. Afterward, DNA sequence analysis was performed by the dideoxy-mediated chain termination method, using synthetic oligo nucleotide primers. Results The HA1 domain of new isolates of influenza A (H1N1) virus showed that their HA1 genes were 981 nucleotide in length coding for a HA1 protein with 327 amino acids, deletion of a glycosylation site and an amino acid. The homology of amino acid sequences of protein molecules on HA1 domains of new influenza A virus when compared with A/Guifang/10/94(H1N1) and A/Bayern /07/95 (H1N1) viruses,was 92.8% and 91.3% respectively; The homology of amino acid sequences of protein molecules on HA1 domains of A/Jingjun/11/98(H1N1) when compared with A/Jingjun/13/98(H1N1) or A/Jingjun/807/97(H1N1) viruses,was as high as 98%; The homology of amino acid sequences of protein molecules on HA1 domains of A/Guifang/10/94(H1N1) when compared with A/Bayern/07/95(H1N1) viruses was as high as 96%. Conclusions The HA1 gene of new H1N1 virus strains is different from those of A/Guifang/10/94 (H1N1) and A/Bayern/07/95(H1N1), they will probably be new mutation strains.
