RESUMO

@#Objective    To explore the key points and difficulties of intraoperative frozen section diagnosis of pulmonary diseases. Methods    The intraoperative frozen section and postoperative paraffin section results of pulmonary nodule patients in Beijing Chaoyang Hospital, Capital Medical University from January 2021 to January 2022 were collected. The main causes of misdiagnosis in frozen section diagnosis were analyzed, and the main points of diagnosis and differential diagnosis were summarized. Results    According to the inclusion criteria, a total of 1 263 frozen section diagnosis results of 1 178 patients were included in the study, including 475 males and 703 females, with an average age of 58.7 (23-86) years. In 1 263 frozen section diagnosis results, the correct diagnosis rate was 95.65%, and the misdiagnosis rate was 4.35%. There were 55 misdiagnoses, including 18 (3.44%) invasive adenocarcinoma, 17 (5.82%) adenocarcinoma in situ, 7 (35.00%) mucinous adenocarcinoma, 4 (2.09%) minimally invasive adenocarcinoma, 3 (100.00%) IgG4 related diseases, 2 (66.67%) mucinous adenocarcinoma in situ, 1 (16.67%) atypical adenomatous hyperplasia, 1 (14.29%) sclerosing pulmonary cell tumor, 1 (33.33%) bronchiolar adenoma, and 1 (100.00%) papillary adenoma. Conclusion    Intraoperative frozen section diagnosis still has its limitations. Clinicians need to make a comprehensive judgment based on imaging examination and clinical experience.

RESUMO

Objective To predict the structure and antigenic epitope of the Strongyloides stercoralis serine protease inhibitor 1 (Ss-SRPN-1) protein using bioinformatics tools, and to construct prokaryotic expression plasmids for expression of recombinant Ss-SRPN-1 protein, so as to provide the basis for unraveling the function of the Ss-SRPN-1 protein. Methods The amino acid sequence of the Ss-SRPN-1 protein was downloaded from the NCBI database, and the physicochemical properties, structure and antigenic epitopes of the Ss-SRPN-1 protein were predicted using bioinformatics tools, including ExPASy, SWISS-MODEL and Protean. Primers were designed according to the nucleotide sequences of Ss-SRPN-1, and the Ss-SRPN-1 gene was amplified, cloned and sequenced with genomic DNA extracted from the infective third-stage larvae of S. stercoralis as a template. The Ss-SRPN-1 protein sequence was cloned into the pET28a (+) expression vector and transformed into Escherichia coli BL21 (DE) cells for induction of the recombinant Ss-SRPN-1 protein expression. The recombinant Ss-SRPN-1 protein was then purified and identified using Western blotting and mass spectrometry. Results Bioinformatics analysis showed that the Ss-SRPN-1 protein, which was composed of 372 amino acids and had a molecular formula of C1948H3046N488O575S16, was a stable hydrophilic protein, and the subcellular localization of the protein was predicted to be extracellular. The Ss-SRPN-1 protein was predicted to contain 11 dominant B-cell antigenic epitopes and 20 T-cell antigenic epitopes. The Ss-SRPN-1 gene with a length of 1 119 bp was successfully amplified, and the recombinant plasmid pET28a (+)/Ss-SRPN-1 was constructed and transformed into E. coli BL21(DE) cells. The expressed recombinant Ss-SRPN-1 protein had a molecular weight of approximately 43 kDa, and was characterized as a Ss-SRPN-1 protein. Conclusions The recombinant Ss-SRPN-1 protein has been expressed successfully, and this recombinant protein may be a potential vaccine candidate against strongyloidiasis.

Objective:To survey on the home blood pressure measurement and blood pressure control rate among patients with hypertension in Beijing Yuetan community.Methods:A questionnaire survey was conducted among 504 hypertensive patients, who were visiting Beijing Yuetan Community Health Service Center and selected by convenience-sampling method during June 2018 to January 2019. The status of home blood pressure measurement, the awareness of hypertension, the blood pressure control rate and factors related to not regularly measuring were surveyed.Results:Of the 504 patients, 93.7% (472/504) took regular medication, but only 61.5% (310/504) had regular blood pressure measurements; 92.5% (466/504) had sphygmomanometers at home with electronic type mainly; 89.9% (453/504) patients knew the method of blood pressure measurement, but only 11.5% (52/453) had their sphygmomanometer calibrated. Among those who measured blood pressure, 77.4% (240/310) measured per week. The main reason for not measuring blood pressure regularly was that there was no need to measure blood pressure without any discomfort, accounting for 50.0% (97/194). The office blood pressure control rate (with desktop mercury sphygmomanometer) was 62.3% (314/504); the blood pressure control rate both in the morning and at bed time was 61.3% (73/119), that in the morning was 68.1% (81/119) and at bed time was 75.6% (90/119). The control rates of systolic blood pressure and heart rate by home measurement at morning and bedtime were significantly higher than those by office measurement (χ 2=5.02, 9.97, 15.51, 13.72; all P<0.05). Conclusions:The home blood pressure monitoring rate and knowledge level of hypertension among patients with hypertension in Yuetan community are relatively low. There are some differences in blood pressure control rate between office measurement and home measurement. Comprehensive evaluation is needed in the management process, and the treatment plan should be adjusted according to the specific condition of patients.

Objective@#To evaluate the efficacy and safety of apatinib combined with chemotherapy in the first-line treatment of advanced non-small cell lung cancer (NSCLC) with negative driving genes.@*Methods@#From January 2016 to March 2018, 62 advanced NSCLC patients with negative driving genes diagnosed at Xuzhou Cancer Hospital were randomly divided into study group (30 cases) and control group (32 cases), respectively. The patients in the study group were treated with standard first-line chemotherapy combined with apatinib, while those in control group were treated with chemotherapy alone.@*Results@#The disease control rate (DCR) and objective remission rate (ORR) in the study group were 60.0% and 16.7%, respectively, higher than 46.9% and 9.3% in the control group, but without statistical difference (P>0.05). The median progression-free survival (PFS) of study group and control group were 6.4 months and 4.9 months, respectively (P=0.004), and the median overall survival (OS) were 11.3 months and 9.2 months, respectively (P=0.006). Multivariate survival analysis indicated that treatment regimen (P=0.001) was the independent prognostic factor of PFS, and PS score (P=0.002), clinical stage (P=0.02) and treatment regimen (P<0.001) were the independent prognostic factors of OS. After treatment, the incidence of hypertension and hand-foot syndrome in the study group were 46.7% and 53.3%, respectively, significantly higher than 3.3% and 0 in the control group, respectively (P<0.05). The incidence of grade 3-4 adverse drug reactions (ADRs) in the study group was 26.7% (8/30), mainly including hypertension, hand-foot syndrome and bone marrow suppression. The incidence of grade 3-4 ADRs in the control group was 15.6% (5/32), all of which were bone marrow suppression, without significant difference (P=0.286). There was no difference in serum levels of VEGF and CEA between the two groups before treatment. After treatment, the serum level of VEGF in the study group was (169.3±10.1) pg/ml, lower than (211.8±16.7) pg/ml of the control group (P<0.05).@*Conclusion@#Apatinib combined with first-line chemotherapy for advanced NSCLC patients with negative driving genes is safe and beneficial for survival. This therapeutic strategy can significantly prolong the PFS and OS, and further improvement and application can be considered as a choice in the clinical treatment.

Objective To evaluate the effectiveness of standardized management for patients with type 2 diabetes in Beijing Qinan community.Methods Total 484 patients with type 2 diabetes in Qinan community were under standardized management.A cross-sectional study on the effectiveness of diabetes management was conducted in December 2016 based on the health records and clinic medical records.Results The mean age was(71.2 ± 11.2) years,the duration of diabetes was (12.2 ± 7.8) years,the mean BMI was(24.76 ± 3.06) kg/m2 in 484 patients.The TC was(5.08 ± 3.27) mmol/L and LDL-C was(2.72 ±0.78)mmol in 383 patients.The rate of achieving the target level of fasting blood-glucose was 51.96% (199/383);the rate of achieving glycosylated hemoglobin target was 60.08% (155/258);and the rate of achieving quick fasting blood-glucose target was 51.49% (52/101).There were 80.37% (389/484) patients with clinic blood pressure < 140/90 mmHg(1 mmHg =0.133 kPa),36.57% patients (177/484) had normal BMI;and 17.83% patients (46/258) with controlled clinic blood pressure,fasting blood glucose,glycosaylated hemoglobin and LDL-C.There were 53.79% patients with LDL-C < 2.6 mmol/L and 23.24% with HDL-C ≥ 1.0 mmol/L.The patients with fasting glucose < 7.8 mmol/L had lower LDL-C than the patients with fasting glucose ≥ 7.8 mmol/L,(F =4.42,P =0.04),The patients with fasting glucose < 7.8mmol/L had higher HDL-C than the patients with fasting glucose ≥ 7.8 mmol/L (Z =-2.30,P =0.02).The rate of achieving glycosylated hemoglobin target was decreased with the increasing of disease duration (x2 =12.57,P =0.01).Conclusion The comprehensive target-reaching rate of type 2 diabetes patients with standardization management in Qinan community is relatively low.It is necessary to strengthen the control of blood lipid,body weight and blood pressure for improving the effectiveness of standardized management of diabetic patients.

Objective To investigate correlation between seizures and uric acid level in the hippocampus in a mouse model with acute limbic seizures by pharmacological and genetic method. Methods Normal male C57BL/6 and mutant mice were used in this study,including urate oxidase overexpression (UOx-OE) and urate oxidase knock-out (UOx-KO) ;These mice were divided into the following groups,including the control,KA, All ,OE ,KO group respectively;during the experiment ,behaviors ,latency and duration time were recorded;dialysates were collected by microdialysis technique and uric acid level was detected by FPLC;Uric acid in the hippocampus,seizures status,latency and duration were compared. Results Twenty-four mice in total were enrolled and only 1 death occurred until the end of the study. Seizures state appeared after KA treatment. Compared to the KA group,uric acid and generalized seizures declined by the treatment of KA and All (P 0.05). Conclusions Uric acid level in the hippocampus of mice may have effects on seizures ,in which it suggests that uric acid and its relevant signal pathway could be a potential therapy target in seizures clinically.

OBJECTIVE:To investigate the influential factors of adverse drug reaction(ADR)in elderly patients during medi-cation,and to provide reference for reducing the incidence of ADR in geriatrics department. METHODS:The clinical data of elder-ly patients admitted to our hospital from Jan. 2011 to Dec. 2015 were collected and analyzed by single factor and unconditional Loigstic multiple regression analysis. The risk influential factors of ADR were investigated. RESULTS:A total of 1 680 patients re-ceived drug therapy,and the incidence of ADR was 7.92%(133/1 680). Organs/systems involved in ADR were mainly allergic re-action(49 cases,36.84%)and digestive system(48 cases,36.09%). The results of single factor analysis showed that the age of patients,drug dosage,route of administration,treatment course,the type numbers of drug combination and others were the inde-pendent influential factors of ADR. By unconditional Loigstic multiple regression analysis,it was found that the risk of ADR in-creased with the drug dosage,treatment course,intraverous administration and various drugs use,etc. CONCLUSIONS:It is nec-essary to pay attention to ADR monitoring,particularly risk factors monitoring of ADR in the elderly patients,strengthen medica-tion guidance to improve medication compliance,take the necessary preventive measures to reduce the incidence of ADR.

Objective To investigate the correlation between the clinical pathological factors and the expression of EGFR, HER2 and HER3 in non-small cell lung cancer tissues. Methods The expression of EGFR, HER2 and HER3 in non-small cell lung cancer tissues was detected by MaxVision immunohistochemical. The relationship between the protein expression and the clinical pathologic characteristics was studied. The correlation between the protein expression and the survival was also retrospectively analyzed. Results Increased expression of EGFR, HER2 and HER3 was detected in the NSCLC tissues and their rates were 45.9 % (73/156), 30.8 % (49/156) and 21.4 % (37/156), respectively. There were significant associations between the expression of these proteins and lymph node metastasis, tumor differentiation degree and TNM staging. Conclusions Overexpression of EGFR, HER2, HER3 or all three proteins is a predictor of poor prognosis. Moreover, combined detection of all 3 markers may work better than the individual detection in the prediction of the prognosis to guide the clinical treatment.

OBJECTIVE@#To explore the expression of interleukin-25 (IL-25) in chronic rhinosinusitis with nasal polyps (CRSwNP) and its potential significance in pathogenesis.@*METHOD@#IL-25 expression in blood was detected by enzyme-linked immunosorbent assay (ELISA). IL-25 expression in tissue was detected by immunohistochemistry (LSAB method) from polyps (68 CRSwNP patients) and 55 inferior turbinate mucosa from patients with deviation of nasal septum served as control. Complete blood count and HE staining of blood and tissue eosinophil infiltration degree.@*RESULT@#IL-25 expression in CRSwNP group were significantly higher than the control group, the difference was statistically significant (P < 0.01). IL-25 expression in local organizations was positively correlated with the number of eosinophil infiltration in CRSwNP group (r = 0.679, P < 0.01).@*CONCLUSION@#The expression of IL-25 in CRSwNP patients mutually reinforcing and might increase eosinophil infiltration and play an important role in the development of CRSwNP.

@#Objective To observe the effect of meridian balancing on speech disorder in children with cerebral palsy. Methods 130 childrenaged 1.5~7 years old were divided into the control group (n=62) and treatment group (n=68). The control group received routine rehabilitationwhile the treatment group received meridian balancing in addition. Results The rate of improvement was 89.7% in treatmentgroup and 79% in control group (P<0.05). The rate was more for those with language retardation than with anarthria in the treatment group(P<0.01). Conclusion The meridian balancing is effective on speech disorder in children with cerebral palsy, especially for those with languageretardation.

RESUMO

The purpose of this study was to observe whether HBV can infect HSC in vitro. LX-2 cells, the human activated HSC cell line, were incubated with human serum containing HBVDNA at final concen-trations from 0.01 to 10 copies/cell and harvested after 24,48 and 72 hours. Hepatitis B surface antigen (HBsAg) and hepatitis B core antigen(HBcAg) were detected by lmmunohistochemistry dyed by DAB. By Immunohistochemistry, no positive particles in LX-2 cells, a few of dark brown particles (HBsAg and HB-cAg) were found in cytoplasm and nucleoli of HepG2.2. 15 cells,a lot of clark brown particles were found in liver biopsies of patients with chronic hepatitis B. In vitro, there was no evidence that HBV can infect and express antigens in LX-2 cells.

Objective To investigate the effect of mycophenolic acid on the proliferation of smooth muscle cells (SMCs) in rats' pulmonary arterial at cellular level. Methods Growth curve, MTT tests, and flow cytometry were used to detect the number of proliferative cells, the A-value of living cells and the DNA content of the control and drugs groups respectively and the number of cells in G1-phase, S-phase, G2M- phase and the proliferation index were calculated. Results Compared with the control group, the number of living cells in the mycopbenolic acid groups (with the concentration of 1, 10, 100 μmol/L) decreased, except the lowest dosage group (100 nmol/L). The difference was statistically significant (P<0.01). The living cells measured by MTT dose-dependently reduced in the mycophenolic acid groups. In the mycophenolic acid groups, the fraction of living cells in the S-phase and G2M-phase decreased respectively while that in G1- phase increased, and the proliferation index decreased. All these responses presented with a dose-dependent manner. Conclusion Mycophenolic acid can effectively inhibit the proliferation of rats' pulmonary arterial SMCs. This process happens mainly in ONA synthesis phase, and is dose dependent. In addition, these effective concentrations are all in the available range for clinical application.

Objective To assess myocardial contractility in essential hypertention(EH) of different remoldings by means of circumferential midwall fractional shortening (mFs), relation between mFs and circumferential end-systolic stress(mFs-cEss). Methods The eject fraction(EF), circumferential end-systolic stress(cEss), fractional shortening(Fs), mFs was calculated in 115 EH patients and 29 normal controls. Relations in mFs-cEss, Fs-cEss between the subjects were analysed with linear correlation and linear regression. Results In using EF,Fs,Fs-cEss to assess chamber function of the patients with hypertension of four left ventricular remoldings, it shows apart from eccentric hypertrophy, the rest three remoldings were overestimative. MFs assessing myocardial contractility in four remoldings was lower than that of the controls, especially obvious in concentric hypertrophy patients(P

To verify the role of introvascular ultrasound(IVUS) in diagnosis of atherosclerosis. MethodsINUS wasused to measure the position, quality and the thickness of the plaque in the elastic artery and biopsy materials. After using HE andHart stains, a micrometer was used to measuse the thickness of plaques and arterious walls. And electron microscopy was used to ob serve the pathological changs of the plaque. ResultsBy histological observation compared with IVUS, the conformable rate of plaque anmounted to 94%. The plaque could be seen in 12 sites of control group. Among 33 sites, 32 plaques were found pathologic exmination while IVUS showed 30 plaques in the experimental group. The two methods had a gcod correlation on the thick measurement of plaque and the media( r = 0.835, P<0.05 and r = 0.814, P< 0.05). But they had a poor correlation on the measurement of the intirna( r = 0.055). ConclusionsBy pathological examination and the quantitative analysis, IVUS can obtain the common purpose in diagnosing atherosclerosis and measuring the media thickness. But it can not be used in measurement of the intima.

Objective To choose a best cultivar by comparing the content of total flavonoid and naringin between different cultivars of Citrus grandis 'tomentosa'.Methods The content of total flavonoids was determined by spetral photometric analysis.The content of naringin was determined by HPLC.Results The content of total flavonoid was 19.24 % and 18.06 %,and naringin content was 11.72 % and 12.38 % in the cultivars from Dachaling and Fengwei respectively,which were much higher than the other cultivars.Conclusion The contents of total flavonoid and naringin were different in different cultivars of Citrus grandis 'tomentosa'.The cultivars of Citrus grandis 'tomentosa' from Dachaling and Fengwei have the best quality.
