RESUMO

Abstract Behçet's disease (BD) is a systemic vasculitis that can affect multiple systems, including the skin, mucous membranes, joints, eyes, gastrointestinal and nervous. However, the pathogenesis of BD remains unclear, and it is believed that immune-inflammatory reactions play a crucial role in its development. Immune cells are a critical component of this process and contribute to the onset and progression of BD. By regulating the function of these immune cells, effective control over the occurrence and development of BD can be achieved, particularly with regards to monocyte activation and aggregation, macrophage differentiation and polarization, as well as T cell subset differentiation. This review provides a brief overview of immune cells and their role in regulating BD progression, which may serve as a theoretical foundation for preventing and treating this disease.

Objective@#This study analyzes the development of school doctor personnel since the implementation of the national "School Health Work Regulations", from the salary of school doctors, followed by the management system of school doctors in primary and middle schools.@*Methods@#Representative provinces in the eastern, central and western regions of China were selected and surveyed through quantitative and qualitative method.@*Results@#The proportion of school doctors with medical background accounted for 33.64%, 31.6% school doctors had t o be established. A huge gap between income, bonus distribution and appraisal exsited between school doctors and school teachers. Wages of school doctors was much lower than that of education and social health sectors. The salary level showed regional imbalance, relatively higher in the eastern region and lower in the in the central and western regions with few exceptions. There were significant differences between medical background school doctors and non medical background school doctors in the highest educational background, income level, job nature, job placement, career development opportunity, income satisfaction, welfare and training satisfaction( χ 2=10.73,26.64,313.44,14.13,29.14,13.22,12.97,19.44, P < 0.05 ).@*Conclusion@#Government needs to pay attention to the school doctors in primary and secondary schools, integreate school health into public health management system. Relevant policies regarding school doctor management authority, school clinic and health center guideline, salary standards, as well as professional career development, are expected.

RESUMO

Rheumatoid arthritis (RA), the most common inflammatory arthropathy word wild, is a systemic autoimmune disease that mainly affects the synovium of joints with a high disability rate. Metabolic mis-regulation has emerged as a fundamental pathogenesis of RA linked to immune cell dysfunction, while targeting immunometabolism provides a new and effective approach to regulate the immune responses and thus alleviate the symptom of RA. Recently, natural active compounds from traditional Chinese medicines (TCMs) have potential therapeutic effects on RA and regulating immunometabolism. In this review, in addition to updating the connection between cellular metabolism and cell function in immune cells of RA, we summarized that the anti-inflammatory mechanisms of the potential natural compounds from TCM by targeting metabolic reprogramming of immune cells, and discusses them as a rich resource for providing the new potential paradigm for the treatment of RA.

Objective@#To analyse the construction and use of primary and secondary school clinics in China, and to provide the reference for the construction and management of infrastructure of the clinics of primary and secondary schools.@*Methods@#By using stratified cluster random sampling method, 873 primary and secondary schools were selected in east, middle and west of China as research objects to describe the construction and management of the clinics of primary and secondary schools. Regional differences were analyzed by statistical method.@*Results@#Among the 873 schools investigated, 294 (33.7%) schools had clinics, 177 (20.3%) schools had healthcare rooms, and 402 (46.0%) schools had no clinics or healthcare rooms. Among the 294 clinics, only 56 (19.0%) schools had medical institution licenses; 73 (24.8%) schools had a construction area of ≥40 square meters; 135 (45.9%) schools had legal clinics, and 38 (12.9%) schools was a disinfection supply room, 2 (<0.01%) schools had a laboratory, 165 (56.1%) schools was not independent between the departments; the overall standard school clinics construction rate was less than 5%. There were differences in the construction of clinics and healthcare rooms in the east, middle and west parts of China (χ2=237.33, P<0.01). Equipment allocation of the country's primary and secondary schools’ clinics were uneven among regions. The normal allocation rates of thermometer, visual acuity chart and lever weight scale were 96.6%, 83.0% and 75.9%, respectively, and the allocation rate of ultraviolet lamp was 66.7%. Less than 50% syringes and high-pressure sterilizers could be used normally. The reasons for the failure to provide health services in clinics was the absence of equipment and space, which account for 43%.@*Conclusion@#The construction of school clinics and healthcare rooms in China is insufficient, and regional differences are obvious, especially in the west. It is necessary to strengthen regional construction, allocate and use the equipment reasonably.

OBJECTIVE@#To observe the clinical effectiveness and safety of fire-needle therapy, an external approach of Chinese medicine in treating plaque psoriasis.@*METHODS@#This study was a two-parallel-arm randomized controlled trial. A total of 151 participants with plaque psoriasis were randomly assigned to the fire-needle therapy group (treatment group, 76 cases) or the control group (75 cases) at a 1:1 allocation ratio using SAS software. All participants received Oral Huoxue Jiedu Decoction (, HXJDD) and applied externally vaseline cream twice a day. Participants in the treatment group received fire-needle therapy once weekly for 4 weeks plus HXJDD and vaseline cream applied the same as the control group. The primary outcome measure was Psoriasis Area and Severity Index (PASI) score, and the secondary outcomes were Dermatology Life Quality Index (DLQL), and Hamilton Anxiety Rating Scale (HAMA), as well as Chinese medicine (CM) syndrome score and photos of target lesions. The indices were evaluated before and after treatment.@*RESULTS@#Sixty-eight patients in each group completed the study. The treatment group has not yet achieved significant improvement in PASI score (P>0.05) compared to the control group. However, significant differences were found between the two groups in relieving CM syndrome (P<0.05) and improving quality of life (P<0.05).@*CONCLUSION@#Fire-needle appears to be safe and may have benefit for psoriasis, the short-term treatment and small sample size limit the conclusions of this study. Further rigorous randomized controlled trials with longer treatment are recommended.

El síndrome de Brugada es una enfermedad hereditaria caracterizada por una anormalidad electrocardiográfica y un aumento del riesgo de muerte súbita cardiaca. El síndrome de Brugada puede ser causado por la presencia de mutaciones en el gen SCN5A en aproximadamente el 20% de los casos familiares. El gen SCN5A codifica la subunidad a del canal iónico de sodio en las células cardiacas. Estudios realizados durante la última década en genética molecular han permitido identificar 11 nuevos genes con susceptibilidad para síndrome de Brugada además del SCN5A, lo que lleva a pensar que es una enfermedad con heterogeneidad genética y compleja de identificar en la clínica y a nivel molecular en el laboratorio. Una manera de heredar el síndrome de Brugada es por medio de un patrón de transmisión hereditaria autosómica dominante. Esta breve revisión se enfoca a describir el proceso de diagnóstico de marcadores genéticos en un caso reportado de síndrome de Brugada guiando al lector a través del proceso de identificación de las variantes genéticas responsables del síndrome y a determinar la consecuencia funcional de las mutaciones del canal de sodio sobre la alteración electrocardiográfica.


Brugada syndrome is a genetic disease that is characterized by abnormal electrocardiogram findings and an increased risk of sudden cardiac death. This syndrome is linked to mutations in the SCN5A gene in approximately 20% of Brugada syndrome probands. SCN5A encodes the a subunit of the cardiac sodium channel. Studies conducted over the past decade have identified 11 other Brugada syndrome susceptibility genes besides to SCN5A, pointing to genetic heterogeneity of the syndrome. Transmission of the disease shows an autosomal dominant inheritance pattern. This brief review focuses on a reported case of sodium channel-mediated Brugada syndrome, guiding the reader through the process of identification of the genetic variants responsible for the clinically-diagnosed syndrome, mutagenesis to clone SCN5A with and without the 2 variants identified and transfection of the 2 variants into TSA201 cells to determine the functional consequence of these genetic variants on sodium channel expression and function.
