RESUMO

@#【Objective】To explore the image evaluation value of multi-model CT in the treatment of acute ischemic stroke with Solitaire stent embolectomy. 【Methods】 A total of 62 patients diagnosed with acute ischemic stroke from January 2015 to June 2016 in Guangdong Second Provincial General Hospital were included in this study. Multi- model CT inspection, including CT scan(NCCT),CT angiography(CTA)and CT perfusion imaging(CTP),was performed in all patients within 3~8 h. The improved vascular TICI classification standard(mTICI)was used to assess vascular embolization, and we evaluated the responsible vessels and blood perfusion state by CTA and CTP blood vessels ,to determine the feasibility of embolectomy with Solitaire stent preliminarily. The patients underwent multi-mode CT examination 24 h after stent embolization to evaluate the responsible vessels. NIHSS was used to assess the neurological function at admission and 72 h after stent embolization.【Results】A total of 34 patients with indication of stent thrombus removal were selected by multi-mode CT examination from 62 patients. Re-examination of multi-mode CT after stent thrombus removal showed that 30 of the 34 cases(30/34,the successful rate was 88.2%)gained success in vascular recanalization. Before the stent thrombus removal of the 34 patients,CTP imaging showed ischemic penumbra(IP),and there was significant decrease in cerebral blood flow(CBF)and slight decrease in cerebral blood volume(CBV),significantly prolonged peak time (TTP) and mean transit time (MTT) compared with the contralateral image area. The difference is statistically significant(P < 0.01). After the stent thrombus removal, the relative cerebral blood flow(rCBF)and relative cerebral blood volume(rCBV)were elevated, the relative peak time(rTTP)and relative mean transit time(rMTT)were shortened. The difference is statistically significant(P < 0.01). Compared with admission, there is significant statistical difference in the NIHSS score of patients 72 h after operation(P < 0.01).【Conclusion】Multi-model CT has guiding effect and important evaluation value in the treatment of acute ischemic stroke patients with Solitaire stent thrombolysis.

RESUMO

Objective: To investigate the relationship between the erythrocyte membrane protein gene mutations and the clinical severity of hereditary spherocytosis (HS). Methods: Targeted sequencings were performed on 25 HS patients, correlation between HS mutations and patients' clinical characteristics were evaluated. Results: A total of 25 HS patients were enrolled, including 13 males and 12 females with median age of 20 (4-55) years, including 9 compensatory hemolysis patients, 9 patients with mild anemia, 3 patients with moderate anemia and 4 patients with severe anemia. Of them, 18 patients (72%) harbored HS-related mutations, including ANK1 mutation in 6 cases, SLC4A1 mutation in 6 cases, SPTB mutation in 5 cases and 1 case with EPB41 mutation. Seven patients (28%) didn't carry common HS mutations. SPTB and SLC4A1 mutations mainly affected male patients. There was no significant difference between the age of diagnosis (P=0.130) and HGB level (P=0.585) in patients with HS mutation and those without mutation, however, the EMA binding fluorescence intensity (P=0.015), AGLT50 (P=0.032) and EOF minimal hemolytic concentration (P=0.027) were significantly different in these two groups of HS patients. Conclusion: To screen erythrocyte membrane protein coding gene mutations could favor the diagnosis of HS, and patients without mutations have mild clinical phenotype.

RESUMO

Purpose To investigate the relationship between M2 microphage infiltration and the pathological grading and staging of prostate cancer. Methods 58 patients who accepted the radical resection of prostate cancer were selected. After operation, pathologic specimens were hematoxylin eosin stained and immunohistochemical of SP stained to identify the pathological grading, staging, and M2 microphage infiltration.Then,Chi-squared test was used to analyze the relationship between M2 microphage infiltration and the pathological grading and staging of prostate cancer. Results There were 71.4% high stage (≥T2c) cases and 60% lymph node metastasis cases in M2 microphage infiltration group, which was significantly higher than those in non M2 microphage infiltration group. However, there was no relationship between M2 microphage infiltration and pathological grading. Conclusion M2 macrophage infiltration may participate in the prostate cancer cell growth and metastasis, whose pathophysiological mechanism needs further exploration.

RESUMO

<p><b>OBJECTIVE</b>To analyze the computed tomography (CT) and magnetic resonance imaging (MRI) findings of small hepatocellular carcinoma to improve the accuracy in the diagnosis.</p><p><b>METHODS</b>This retrospective analysis involved 41 patients with small hepatocellular carcinoma cases confirmed by pathological examination of the biopsy samples or follow-up. These patients were assessed for CT and MRI findings including lesion size, density or signal intensity, enhancement patterns, and presence of tumor capsules.</p><p><b>RESULTS</b>On unenhanced CT images, small hepatocellular carcinomas were displayed mainly as low-density masses, and the majority of tumors presented with low signal intensity on T1-weighted unenhanced MR images with increased signal intensity on T2-weighted images in comparison with the surrounding liver parenchyma. Most of tumors showed intense enhancement during the arterial phase (CT in 15 cases and MRI in 13 cases), but some appeared isointense to the liver parenchyma (CT in 4 cases and MRI in 4 cases). In portal and delayed phases, the tumors typically had lower signal intensity than that of the surrounding liver tissues (CT in 25 cases and MRI in 12 cases) with enhancement of the tumor capsules (13 cases).</p><p><b>CONCLUSION</b>Dynamic enhanced scanning can be more informative of the pathology and blood supply of small hepatocellular carcinoma. Early and late arterial phase imaging may help in detecting the small lesions and in making differential diagnosis.</p>
