RESUMO

<p><b>OBJECTIVE</b>To observe the whole process of cranio-maxillofacial development of an animal model with bilateral premaxillary clefts, after being exerted with premaxillary retro-pressing forces.</p><p><b>METHODS</b>Experimental rabbits with premaxillary clefts and without clefts were exerted with retro-pressing forces respectively. Roentgenographic cephalometric analysis and different investigations of premaxillary suture were performed during the whole craniofacial development.</p><p><b>RESULTS</b>The longitudinal cephalometric analysis showed that a temporary retarding effect of mid-facial growth existed. The hindrance of maxillary growth potentiality did not occur in spite of being exerted retro-pressing force as to this experimental extent.</p><p><b>CONCLUSIONS</b>(1) Infant rabbit combined with the designed retro-pressing appliance is an ideal experimental model for quantitative study of active orthopedic treatment of protruding premaxilla. (2) Using active preoperative orthopedics to reposit the protruding premaxilla of bilateral cleft palate is a reasonable and effective approach.</p>

RESUMO

Objective To perform a biomechanical study on and compare the stability of internal fixation with closed Kirschner wire in different spots from the epicondylus medialis to sulcus nervi ulnaris and epicondylus lateralis of humerus in the treatment of GartlandⅢsupracondylar fractures of humerus in children in order to provide basis for avoiding cubitus varus in clinic.Methods Twenty models of su- pracondylar fractures were made from ten humerus samples,then divided into two groups(group A and group B).They were fixed with closed Kirschner'wires.Ulnar Kirschner wire varied.In group A five spots were selected averagely from the epicondylus medialis to the sulcus nervi ulnaris.Kirschner wires entered from one of the five spots into another spot of humerus.In group B Kirschner wires entered defferent spots of humerus from the central spot of sulcus nervi ulnaris.Biomechanical tests were carried out,such as axial load,axial displacement,stress intensity,axial rigidity and anti-twisty ability.Forty-six children with Gart- landⅢsupracondylar fracture of humerus were treated and followed up for more than one year.Results In beth groups,with the angle of Kirschner wires and fracture side increasing,the strain and the vertical displacement decreased,the stress intensity and the axial rigidity increased,and the anti-twists ability was enhanced.There was statistic difference(P<0.05).The follow-up of the 46 patients showed the elbow joint function recovered well with no cubitus varus malformation.Conclusion Internal fixation with closed Kirschner wires in sulcus nervi ulnaris from the epicondylus medialis and the epicondylns lateralis of humerus for GartlandⅢsupracondylar fracture of humerus in children has the advantages of enhancing bio- mechanical stability,effective prevention of secondary replacement of outlying black of the fracture and a- voidance of cubitus varus.It can be widely used clinically.

<p><b>OBJECTIVE</b>To compare results of the growth and development of the upper dental arch and the velopharyngeal closure of the cleft patients treated by two methods.</p><p><b>METHODS</b>The dental cast of patient and X-ray films were measured and the statistical medical records were analyzed.</p><p><b>RESULTS</b>The transverse distance of upper dental arch was found to be wider in group A than in group B. The anterior-posterior distance of the dental arch in bilateral cleft group was shorter in group A than in group B. The difference of the two groups were gradually lessened as age increases. Bony bridge in alveolar gap was 63% and 83.3% in unilateral and bilateral cleft group respectively. 15% of cases in group A and 35.2% in group B needed pharyngeal flap.</p><p><b>CONCLUSIONS</b>The stable upper dental arch in group A can opposes the pressure from the lip muscles, this maintains the width of the arch. But A-P distance of upper dental arch in BCLP in group A should be followed up after the age of 9 years. Pharyngeal flap is needed less in group A than in group B.</p>

RESUMO

<p><b>OBJECTIVE</b>To gain a clear idea of the function of middle ear and hearing in cleft palate with secretory otitis media after pressure equalization tube insertion.</p><p><b>METHODS</b>53 cleft palate cases with secretory otitis media including 62 ears were divided into treatment and control groups. They were examined by tympanogram and hearing threshold.</p><p><b>RESULTS</b>Hearing threshold in treatment group after PE tube insertion respectively was 23.3 dB and 23.4 dB in the near future and at a specified future date. It was normal. But there were still 64% tympanogram B and C.</p><p><b>CONCLUSIONS</b>PE tube insertion could solve hearing problem, but could not improve the function of middle ear.</p>
