RESUMO

PURPOSE: Laparoscopic approaches to the anterior abdominal wall are difficult because of the inherent limitations of laparoscopic surgery. METHODS: A 25-year-old young female visited to our hospital with an incidentally-found palpable abdominal mass. Computed tomography revealed a 7-cm round enhancing mass arising from the left posterior rectus sheath. The patient underwent laparoscopic excision of the inner mass arising from the anterior abdominal wall. RESULTS: The operation lasted for 45 minutes and there was no measurable bleeding during the procedure. Pathological assessment revealed that the tumor was a DTF 7.0 cm in size. The patient was discharged on the second postoperative day in good condition, and is currently being followed on a routine basis for surveillance without adjuvant therapy. CONCLUSION: Laparoscopic approach for the anterior abdominal wall tumor could be feasible in select patients. This minimally-invasive approach helps to ensure good cosmetic outcomes and quality of life.

RESUMO

OBJECTIVES: The purpose of this study was to review an implementation of u-Severance information system with focus on electronic hospital records (EHR) and to suggest future improvements. METHODS: Clinical Data Repository (CDR) of u-Severance involved implementing electronic medical records (EMR) as the basis of EHR and the management of individual health records. EHR were implemented with service enhancements extending to the clinical decision support system (CDSS) and expanding the knowledge base for research with a repository for clinical data and medical care information. RESULTS: The EMR system of Yonsei University Health Systems (YUHS) consists of HP integrity superdome servers using MS SQL as a database management system and MS Windows as its operating system. CONCLUSIONS: YUHS is a high-performing medical institution with regards to efficient management and customer satisfaction; however, after 5 years of implementation of u-Severance system, several limitations with regards to expandability and security have been identified.

RESUMO

OBJECTIVE: The development of information communication technology (ICT) and the demand for managing the healthy lives of individuals are accelerating the informatization of the health and medical field. Considering this environment and the needs of the individual, this paper has designed and developed a web and mobile storage device-based personal health record (PHR) system that individuals can manage by themselves anywhere, anytime, whether on-line or off-line. Based on the experience of implementing the system, its development method, results, and relevant technical issues are described. SYSTEM DESIGN AND DESCRIPTION: This system is implemented to manage PHR, including vital signs and ingested/consumed calories for a lifetime by connecting a PHR-integrated web server to each hospital's information system, and the PHR programs installed in the individual's PC or USB flash memory drive. To achieve this, an interface module, web server system, and PHR viewer program for individuals are developed. RESULTS: When it is off-line, the PHR program is operated to inquire the data saved in the DB, and the self-measured information can be inputted. When it is on-line, it calls the web service function to inquire the medical information, including hospital visit history, prescription history, diagnosis result, image inspection result and medical treatment result. CONCLUSION: This system connects home and mobile healthcare to hospitals but minimizes information leakage because the data is not accumulated. By loading a plug & play, PHR viewer to an easy-to-carry mobile storage device, the systems supports a sustainable health management.

RESUMO

Endotracheal intubation has many advantage such as airway protection, maintenance of patent airway, pulmonary toilet, application of positive pressure ventilation, and maintenance of adequate oxygenation. But risks that develop complications related to endotracheal tube always present, when we insert a tube into the trachea. We have experienced an armored tube obstruction with bulged inner wall after EO gas sterilization. So, it is important that we have to pretest a tube and check pateney of the inside of a tube, before endotracheal intubation.
