RESUMO

OBJECTIVE: This study examined the effect of social support on metabolic syndrome (MetS) among elderly population in a city in Korea. METHODS: A total of 352 people all over 60 years of age were analyzed from the data of the Korean Longitudinal Study on Cognitive Aging and Dementia. This study was started in 2010 and is in progress till date. We compared the documented diagnoses of MetS and Medical Outcome Study Social Support Survey at the baseline of the study and two years later. RESULTS: 315 were normal, 37 were developed after two years among subjects without MetS at baseline. When the level of social support was compared at baseline and then at the time of the reassessment two years later, the level had increased in the normal group and the level had not significantly changed for the MetS group. The findings documented at the two year mark indicate that the higher and stronger the level of social support, the lower the risk of developing MetS (odds ratio=0.053, p=0.040). CONCLUSION: The elderly with good social support are noted to be at lower risk in developing MetS as opposed to those with poor social support. It is apparent that adequate social support shows reduced prevalence of MetS among the elderly. Methods to identify strategies in improving social support networks are needed to expand public health resources and to better accommodate the community-based care of the elderly.

OBJECTIVE: To develop a model of group cognitive-behavioral therapy (CBT) for Korean patients with early psychosis. METHODS: The group CBT utilized in the present study consisted of metacognitive training, cognitive restructuring, and lifestyle managements. The Subjective Well-being Under Neuroleptics (SWN-K), Ambiguous Intentions Hostility Questionnaire (AIHQ), Drug Attitude Inventory (DAI), Beck Depression Inventory (BDI), Perceived Stress Scale (PSS), and Clinical Global Impression (CGI) were administered prior to and after CBT sessions. The participants were categorized into two groups according to the median duration of untreated psychosis (DUP; 4 months). RESULTS: A total of 34 patients were included in this analysis. From pre- to post-therapy, there were significant increases in the SWN-K and DAI scores and significant decreases in the hostility subscale of the AIHQ, PSS, and CGI scores. Significant time × DUP interaction effects were observed for the SWN-K, DAI, and BDI scores, such that there were significant changes in patients with a short DUP but not in those with a long DUP. CONCLUSION: The group CBT program had a positive effect on subjective wellbeing, attitude toward treatment, perceived stress, and suspiciousness of young Korean patients with early psychosis. These effects were particularly significant in patients with a short DUP.

OBJECTIVES: This article aims to describe the development of smartphone application for the case management of patients with schizophrenia. METHODS: Gwangju Bukgu-Community Mental Health Center developed and launched a smartphone application (HYM) for cognitive-behavioral case management and symptom monitoring. The development of the application involved psychiatrists, nurses, social workers, psychologists, and software technicians from a software development company (Goosl Corp.). RESULTS: The HYM application for clients includes six main modules including Thought record, Symptom record, Daily life record, Official notices, Communication, and Scales. The key module is the 'Thought Record' for self-directed cognitive-behavioral treatment (CBT). When the client writes and sends the self-CBT sheet to the case manager, the latter receives a notification and can provide feedback in real time. 'Communication' and 'Official notices' are useful for promoting communication between case managers and clients with schizophrenia. Ratings in 'Symptom record', 'Daily life record', and 'Scales' modules are stored in graphic or table form representing changes in them and shared with case managers. CONCLUSION: The interactive function of this application is the key characteristics that distinguishes it from other mobile self-treatment tools. This smartphone application may contribute to the development of a youth- and customer-friendly case management system for individuals with early psychosis.

RESUMO

Intensive case management provided at the critical period following the onset of schizophrenia is crucial for early social and vocational recovery. We report the young schizophrenia patient who had been treated for 2 years but frequently relapsed and attempted suicide. After receiving cognitive behavioral case management from community mental health center, she has functionally recovered without recurrence for 4 years. Collaboration between psychiatrists of hospital and case manager of community mental health center was very important factor that contributed to her functional recovery. Cognitive behavioral case management promoting coping skills and reducing perceived stress was effective on successful social adaptation even during the stressful situations. In conclusion, psychosocial management including cognitive behavioral case management at the early stage of schizophrenia is crucial to prevent the relapse and to recover patients' function.
