RESUMO

We report a 62-year-old man with histologically confirmed olfactory neuroblastoma who also had the syndrome of inappropriate antidiuretic hormone secretion (SIADH). An olfactory neuroblastoma is a rare malignancy of the nasal cavity or paranasal sinuses and is rarely associated with excess hormone syndromes. The ectopic production of antidiuretic hormone (ADH) by a tumor is most often due to a small cell carcinoma of the lung, while less common causes include an olfactory neuroblastoma. To our knowledge, this is the first reported case of SIADH secondary to an olfactory neuroblastoma in Korea.

RESUMO

Percutaneous transplant kidney biopsy has become the most important tool for diagnosing allograft dysfunction. However, renal biopsy has various complications. Among them, hemoperitoneum is a rare complication and has not been reported in Korea. We experienced a case of hemoperitoneum after transplant kidney biopsy. A 43-year-old man, 3 years after renal transplantation, was presented with elevated serum creatinine. Percutaneous renal biopsy was executed by real-time ultrasound guidance and 14-gauged spring loaded automated biopsy gun at upper pole of transplanted kidney. Renal biopsy was completed after 5 trials due to poor visualization of biopsy needle tip. After 2 hours, the patient complained of acute right side abdominal pain and dizziness. Abdominal pelvis CT showed moderate amount of hemoperitoneum. The patient underwent emergency laparotomy. Hematoma was seen in the omentum with minor vessel bleeding. Ligation was done. The patient was discharged after 15 days.

RESUMO

Pentalogy of Cantrell was very rare congenital anomaly and represent a regional prevalence of 5.5/1 million live births. The syndrome, Cantrell's pentalogy is characterized by defects in the lower sternum with ectopia cordis, anterior diaphragm defects, midline supraumbilical abdominal wall defects, defects in the diaphragmatic pericardium, and various intracardiac defects. Anencephaly involves acrania with a degenerated mass of brain tissue. Combination of both conditions are extremely rare. We experienced a case of Cantrell's pentalogy with anencephaly detected by ultrasonography at 15 weeks 1 day menstural age and confirmed at autopsy. Thus we report a case with brief review of the literature.

RESUMO

Angiosarcoma is defined as a malignant tumor of the endothelial cells present in blood vessels and represents less than 1% of all soft tissue sarcomas. Primary uterine angiosarcomas are particularly rare, and tend to have poor prognosis mostly related to the aggressive nature and the metastatic potential of these tumors. They present most commonly with vaginal bleeding or with weight loss and a pelvic mass. Although postoperative radiation and combination chemotherapy are options being utilized, the results have only shown limited success. We report a case of primary uterine angiosarcoma with a brief review of literature.

RESUMO

A 67-year-old male patient had coronary artery bypass graft. Before the induction of general anesthesia, 16 gauge central venous catheter was inserted via left subclavian vein. Initial chest anteroposterior (AP) radiograph showed adequate position, and the central venous pressure measurement through it was satisfactory. This patient's progress was satisfactory, and he was extubated on the second postoperative day. Daily chest AP radiograph had been unremarkable until the third postoperative day, when a right-sided pleural effusion was seen. A day later, the patient complained severe chest pain and dyspnea. We suspected perforation of superior vena cava wall by tip of central venous catheter on the chest computerized tomograph (CT). The injection of radiopaque dye through the central venous catheter confirmed spillage into the mediastinum on the chest CT. Right chest tube was inserted. The patient's condition dramatically improved after chest tube insertion.

RESUMO

Mechanical valves have generally good hemodynamic function and indefinite durability, but they have a higher thromboembolic potential and thus a requirement for permanent anticoagulation, because thrombotic occlusion is a potentially fatal complication of heart valve replacement surgery. We had experienced mitral valve replacement because of thrombosis around the replaced prosthetic valve. The patient's mechanical prosthetic valve was acutely obstructed by thrombosis, and it was a life threatening condition. We performed partial bypass through femorofemoral bypass for life saving. Femorofemoral bypass improved oxygenation and cardiovascular stability, and mitral valve replacement was successfully performed without complication.
