RESUMO

Nine scaphoid non-unions, which were treated by through curettage of the scar tissue on pseudarthrosis, iliac bone grafts between the fragments and Herbert screws fixation, were analysed. The average follow up period from operation was 16.9 months. The interval between the fracture and the time of the operation ranged from 10 months to 48 months(average 21.6 months). Average union time was 14 weeks after operation. The initial radiologic cystic changes disappeared and overall clinical results were improved. Mean postoperative range of motion of the wrist was flexion 55 and extension 45°. Three of them had DISI deformity preoperatively which their scapholunate angles (mean 104°) has been corrected after reduction (mean 75°). In one case among three DISI deformities, partial radial styloidectomy and triscaphe fusion were added (to bone graft and Herbert screw fixation). A case of incorrect positioning of the screw and a case of neuroma were complicated. Treatment of scaphoid nonunion with curettage of the scar tissue and iliac bone graft and Herbert screw fixation seems to be encouraging to regain the normal anatomy of scaphoid and function of the wrist.

Supracondylar fracture of femur is not well suited to internal fixation. A wide canal, thin cortex, comminution and compound wound make open reduction difficult. And the associated injuries, such as popliteal artery rupture are not infrequent. Some authors reported the favorable results with open reduction and rigid internal fixation. But still we have had failure cases with loosening or breakage of plate and screws, infection, post-operative joint stiffness and non-union etc. The present paper reports the result of surgically treated supracondylar fracture. From March 1982 to February 1989 25 patients who were treaed at Department of Orthopedic Surgery, St. Vincent Hospital were analysed. The results obtained as follows:l. Age distribution was between 29 and 74 years of age, and the ratio between males and females was 1.8:1. The most common of injury was traffic accident. 2. According to Neer's classification, there were 5 patients of type I, 7 patients of type II

RESUMO

Sleeve type fracture of the patella in children is an extremely rare injury. Its avulsed fragment is so small that the diagnosis may be missed by radiogram and patella slta is complicated. A 14 year-old boy having the sleeve type fracture of the patella with patella alta was treated by open reduction with s screw fixation at St. Vincent Hospital in December 1985 and was followed up over two years. The final clinical result was good.

RESUMO

There have been many methods of manual reduction of shoulder dislocation. But each method has its unique complications such as fracture of'humeral neck or shaft as well as vascular and nerve injuries around the shoulder due to excessive pulling and rotation during the maneuver. The present authors devised a method in which the backrest of a chair is used as a fulcrum during manupulative reduction. The dislocated shoulder can be abducted by the backrest rim, when the armpit is placed over the top of the backrest. In this abducted position of the shoulder, minimal adduction force on distal humerus and gentle backward pressure on humeral head are very effective in reducing the anterioly disocated shoulder.

RESUMO

We performed intra-osseous venography in 16 patients with chondromalacia patellae or patellofemoral osteoarthritis. Of 16 patients, eight patients had chondromalacia patellae and 6 patellofemoral osteoarthritis. Two patients with normal patellae who underwent meniscectomy were used as controls. The results obtained were as follows: 1. Dye congested in the normal patella was disappeared within 2 minutes after injection and extra-osseous drainage pattern disappeared within 1 minute. 2. in chondromaiacia patellae, large amount of dye was spread throughout the entire patella and remained till 10 minutes, while extra-osseous drainage pattern disappeared at about 5 minutes. 3. In patellefemoral osteoarthritis, venous engorgment in patella was similar to that of chondromalacia patellae and extra-osseous drainage pattern disappeared at about 10 minutes. Conclusively, authors thought the patella I.O.V. was one of the significant clinical methods to define the etiological factors of the various patellofemoral disorders such as patellofemoral osteoarthritis and chondromalacia patellae.
