RESUMO

Eosinophilic colitis is a chronic inflammatory bowel condition of unknown etiology and a rare subtype of eosinophilic gastrointestinal diseases. It is characterized by gastrointestinal symptoms and increased eosinophil numbers in the intestinal mucosa and absence of other potential causes of gastrointestinal eosinophilia. The clinical presentation is varied and depends on the involved layer of the large intestine. There are no confirmatory laboratory tests, and the morphologic evaluation of biopsies or surgical specimens is required to confirm the diagnosis. A 65-year-old man presented with an 8 day duration of bloody diarrhea and lower abdominal pain. The patient was diagnosed with eosinophilic colitis by histopathological evaluation of biopsies and was further categorized as a nonatopic variant associated with non-IgE-mediated reaction. The patient was successfully treated with systemic corticosteroid for 2 weeks. On follow-up after 9 months, the patient remained well without relapse or new lesions.

RESUMO

Pulmonary paragonimiasis is a food-borne zoonosis, which is acquired from eating the raw or undercooked freshwater crabs or crayfish (crustaceans). It was highly endemic in Korea the late 1960's. However, human paragonimiasis is still sporadically occuring because of pupularization of Korean traditional crab dishes and is one of the lung disease which needs differential diagnosis. Recently, we experienced a case of pulmonary paragonimiasis in a 55 years man with dyspnea, dry cough and abdominal pain. The patient had hypereosinophilia and pulmonary infiltrates with pleural effusion, which did not improve on antibiotics. Diagnosis was confirmed by sputum with Paragonimus westermani ova, which was treated with praziquantel for two days at a daily dosage of 75 mg/kg. The patient was readmitted to the hospital 1 month following first treatment with recurrence of symptoms and he was cured by a second course of praziquantel.

RESUMO

Verruciform xanthoma is an uncommon benign lesion. The most common presentation occurs in the oral cavity; however, it has also been described in other sites, especially the penis. Herein is reported the first case of verruciform xanthoma of the penis in China, the fourteenth case in the literature. Clinically, genital verruciform xanthoma is often confuses with papillomas, verrucous carcinomas and squamous cell carcinomas, and therefore a histopathological diagnosis is necessary.

PURPOSE: Anatomical change of the prostatic stroma, that is benign prostatic hyperplasia, results in a functional change which manifests as lower urinary tract symptoms (LUTS). We observed/investigated whether there are structural changes in the three-dimensional arrangement of muscle, collagen and elastin fibers of the prostatic urethra which can be another possible mechanism of LUTS. MATERIALS AND METHODS: Hyperplastic nodules, which were surgically en bloc resected, and normal prostate from radical cystectomy specimens were examined. Surgical specimens were fixed in formalin and sectioned serially in a transverse plane along the lumen of the upper segment of the prostatic urethra. Additional sagittal sections were also made serially. All tissue blocks were 2mm thick. All slides were prepared with hematoxylin-eosin, Masson-trichrome and Verhoeff's elastin stain. RESULTS: Muscle and collagen fibers surrounding prostatic acini seemed poorly oriented and blended with the outermost fibers of the urethra. Muscle and stromal fibers surrounding the prostatic urethra seemed to be stretched and oriented mainly in the longitudinal direction with small fibroblastic nodules in the periurethral stroma. A periurethral gland structure was observed between the urethral wall in which the fiber was arranged longitudinally. Circular muscle fibers were not observed in the periurethral area, indicating that the surgical capsule resulted from urethral muscle fibers. CONCLUSIONS: Apparent changes to the 3-dimensional arrangement of collagen, elastin and muscle fibers surrounding the acini and prostatic urethra were not observed in the proximal prostatic urethra or the bladder neck, except that the fibers surrounding the urethra were stretched.

RESUMO

A 69-year-old male was admitted to the neurosurgery department for traumatic intracra-nial hemorrhage in both frontal lobes. After 2 months, he complained of epigastric dis-comfort, nausea, vomiting, and loose stools. The gastric endoscopic examination found acute hemorrhagic gastritis and there were rhabditoid nematode larvae in the gastric fluid and biopsy sections. The filariform larvae of Strongyloides sp. were discovered from a fecal culture. The patient was treated with albendazole (200 mg, po bid, for 4 weeks). The epigastric discomfort disappeared and endoscopic findings improved after treatment.

RESUMO

A 67-year-old male visited Pusan Veterans Hospital due to general weakness and weight loss for 6 months. Physical examination showed non-tender 4 finger breaths sized splenomegaly and both inguinal and cervical lymphadenopathy. The white blood cell count was 25,300/uL with 91% morphologically mature lymphocytes. Bone marrow aspirate revealed hypercellularity with 74.5% lymphocytes morphologically similar to peripheral lymphocytes. The immunophenotpying study of lymphocytes displayed the phenotype of CD19(+), CD20(+), HLA-DR(+), sIg(+) but CD5(-). We concluded that this patients's diagnosis is CD 5 negative B-cell chronic lymphocytic leukemia.

RESUMO

We report a case of cutaneous focal mucinosis in a 60 year-old male, who had asymptomatic localized small flesh colored papule and nodule on the left side of the neck. The histopathologic finding revealed fibroblastic proliferatior in the dermis and an increased prodact on of hyaluronic acid by these fibroblasts. The lesions were treated by surgical excision.
