Background: “Team-based learning (TBL)” is a special approach by using small groups that is student centered, teacher guided Teaching and Learning method. Medical Council of India (MCI) emphasizes small group teaching. With increasing student numbers and decreasing/no change in faculty strength, the need for an effective T-L method for incorporating small group teaching in a large group is essential. So, this study attempts to evaluate the effectiveness and student reaction for TBL. Methods: A cross sectional study was conducted among III-year MBBS students attending block posting in the Department of Community Medicine, KIMS and RC. Pre-test and post-test after Team based learning implementation by designated faculties was conducted, and scores were analysed. The feedback of TBL was collected by using a Likert scale from students and also from faculties. Results: The mean scores of pre- and post-test were analyzed by Paired T test which was statistically significant (p value <0.05). The student reaction evaluated by 7 items Likert scale questionnaire showed that most of the students either agreed or strongly agreed that TBL enhanced their learning experience and confidence. The faculty feedback about TBL was also positive. Conclusions: TBL can be effectively implemented in medical colleges for the better results and student reaction.

Considering the lack of consensus related to the impact of selective IL-6 receptor inhibition on bone remodeling and the scarcity of reports, especially on large bone defects, this study proposed to evaluate the biological impact of the selective inhibitor of interleukin-6 receptor (tocilizumab) in an experimental model of critical calvarial defect in rats. In this preclinical and in vivo study, 24 male Wistar rats were randomly divided into two groups (n=12/group): defect treated with collagen sponge (CG) and defect treated with collagen sponge associated with 2 mg/kg tocilizumab (TCZ). The defect in the parietal bone was created using an 8-mm diameter trephine drill. After 90 days, the animals were euthanized, and tissue samples (skull caps) were evaluated through micro-CT, histological, immunohistochemistry, cytokines, and RT-qPCR analyses. Tocilizumab reduced mononuclear inflammatory infiltration (P<0.05) and tumor necrosis factor (TNF)-α levels (P<0.01) and down-regulated tissue gene expression of BMP-2 (P<0.001), RUNX-2 (P<0.05), and interleukin (IL)-6 (P<0.05). Moreover, it promoted a stronger immunostaining of cathepsin and RANKL (P<0.05). Micro-CT and histological analyses revealed no impact on general bone formation (P>0.05). The bone cells (osteoblasts, osteoclasts, and osteocytes) in the defect area were similar in both groups (P>0.05). Tocilizumab reduced inflammatory cytokines, decreased osteogenic protein, and increased proteases in a critical bone defect in rats. Ninety days after the local application of tocilizumab in the cranial defect, we did not find a significant formation of bone tissue compared with a collagen sponge.

One of the main public health issues is tobacco use, which is also associated with considerate morbidity and mortality worldwide. Smoking cessation lowers mortality and risk of future cardiovascular events. Smoking is a true chronic condition that is defined by the emergence of a dependency state that is primarily caused by nicotine. Smokers are typically unable to quit smoking on their own. There are numerous approaches to treating smoking addiction, including both non-pharmacological such as behavioural counselling and pharmacological therapy. The effectiveness and safety of smoking cessation medications in everyday life are currently widely acknowledged. The primary pharmacological methods for quitting smoking include nicotine replacement therapy, varenicline, bupropion, and cytisine. Even in patients with chronic cardiovascular disease, their effectiveness and safety have been demonstrated. It has been demonstrated that nicotine replacement therapy, bupropion, varenicline, and cytisine increase the likelihood of cessation. Both varenicline and nicotine replacement therapy in combination are powerful tools for cessation. The likelihood of quitting is likewise increased by nortriptyline. Research suggests that none of the treatments seem to have a frequency of adverse reactions that would limit their usage. The clinician should tailor the smoking cessation strategy based on the most recent scientific data and the patient's preferences, giving particular attention to those patients who have certain cardiovascular and psychiatric comorbidities. The purpose of this research is to review the available information about pharmacological approach to smoking cessation and effective intervention models.

The strength-based approach challenges the traditional deficit model applied in psychosocial care practice, which takes an all-encompassing perspective. Creating a comprehensive portrait of each person's life and working in a multidisciplinary team collectively analyses the full capabilities and circumstances of individuals or families. This paper employs case illustrations to demonstrate how psychosocial care providers can do assessments and interventions based on a strength-based approach to improve the quality of life of patients and their families. The study adopted cases from the case file and derived how strength-based approaches can be utilized for the patients referred for psychosocial assessment and designing psychosocial care plan in the tertiary care hospital. The case illustrations depict practice through strength-based approach. The assessments were conducted, plans were made, and interventions focused on the strengths in various domains. The result portrayed strength of each case on innate, acquired/learned, supportive strengths in personal, family, social and health domains of life. The analysis of the strengths of the patient allowed to develop a tailor-made intervention plan, and these interventions have resulted in empowerment and recovery. The strength-based approach is one of the approaches which came differently from the deficit models of psychosocial practice. In India, the practice of a strength-based approach for interventions in various fields is in its infancy. The legislations in India have provisions that support a strength-based approach.

To update the accommodation mechanisms and propose a dual-wavelength, dual-function laser system for presbyopia and glaucoma treatments.Study Design: Laser sclera softening (LSS) for increased accommodation of presbyopic eyes.Place and Duration of Study: New Taipei City, Taiwan, between Jan., 2023 and Feb., 2023.Methodology: Accommodation gain (AG) can be improved by: (i) thermal shrinkage of the scleral stroma and ciliary body, or (ii) softening of the scleral stroma (with temperature range of 700C to 900C), such that the the lens front and back curvature change (or lens thickening), leading to the thickening of ciliary body and its apex, and the increase of the space of ciliary body and lens equation (SCL), and the length of the posterior vitreal zonules (PVZ) increases.Results: A novel dual-color laser system having wavelength A and B, acting on the front-zone and back-zone of the sclera, respectively, where laser-A has a deep thermal penetration the sclera and ciliary body (CB) (0.5 to 1.0 mm); and laser-B has a shallow penetration depth in the sclera (0.3 to 0.5 mm), based on the optical property of the sclera. Laser-A (having a wavelength about0.8 to 0.98 um) leads to thermal shrinkage of the ciliary body such that the CLS is increased for accommodation gain which is much more effective than the prior art.Conclusion: The increase of AG can be achieved by scleral softening and ciliary body shrinkage which increase the SCL. A proposed novel dual-color laser system acting on the front-zone and back-zone of the sclera, respectively, could provide higher AG than that of single wavelength, or prior arts using scleral ablation. However, further clinical studies are required to justified the proposed novel system with predicted advantages and efficacy based on the optical properties of sclera.

There is a high demand for stroke rehabilitation in the Brazilian public health system, but most studies that have addressed rehabilitation for unilateral spatial neglect (USN) after stroke have been performed in high-income countries. Therefore, the aim of this study was to analyze USN patient recruitment in a multicenter noninvasive brain stimulation clinical trial performed in Brazil and to provide study design recommendations for future studies. We evaluated the reasons for exclusion of patients from a multicenter, randomized, double-blinded clinical trial of rehabilitation of USN patients after stroke. Clinical and demographic variables were compared between the included and excluded patients. A descriptive statistical analysis was performed. Only 173 of the 1953 potential neglect patients (8.8%) passed the initial screening. After screening evaluation, 87/173 patients (50.3%) were excluded for clinical reasons. Cognitive impairment led to the exclusion of 21/87 patients (24.1%). Low socioeconomic status led to the exclusion of 37/173 patients (21.4%). Difficulty obtaining transportation to access treatment was the most common reason for their exclusion (16/37 patients, 43.3%). The analyzed Brazilian institutions have potential for conducting studies of USN. The recruitment of stroke survivors with USN was restricted by the study design and limited financial support. A history of cognitive impairment, intracranial stenting or craniectomy, and lack of transportation were the most common barriers to participating in a multicenter noninvasive brain stimulation trial among patients with USN after stroke.

Behavior is a useful trait for comparative studies that provide the comprehension of phylogenetic relationships among species. Here, we present a description of two spiny-rats species' behavioral repertoire, Clyomys laticeps and Trinomys setosus (Rodentia: Echimyidae). The affiliative and agonistic behavioral patterns were sampled during a three-year study of captive populations of wild animals. Observational data were collected in two phases under different arrangements of individuals in groups. We also compare the behavioral traits of T. setosus and C. laticeps with the known behavioral patterns of Trinomys yonenagae. We add categories to the previous descriptions of T. setosus and a standard ethogram for C. laticeps. Trinomys setosus showed a visual and vocal display we called foot-trembling, which was not described in this form and function for other species studied until now. We discuss the differences in their sociality levels and similarities and differences among behavior patterns and repertoires.
O comportamento é uma característica útil para estudos comparativos que fornecem a compreensão das relações filogenéticas entre as espécies. Apresentamos aqui uma descrição do repertório comportamental de duas espécies de ratos-de-espinho Clyomys laticeps and Trinomys setosus (Rodentia: Echimyidae). Os padrões comportamentais afiliativos e agonísticos foram amostrados durante um estudo de três anos em populações de animais silvestres em cativeiro. Mesenchymal stromal/stem cells stem (MSC) have been widely studied due to their great potential for application in tissue engineering and regenerative and translational medicine. In MSC-based therapy for human diseases, cell proliferation is required to obtain a large and adequate number of cells to ensure therapeutic efficacy. During in vitro culture, cells are under an artificial environment and manipulative stress that can affect genetic stability. Several regulatory agencies have established guidelines to ensure greater safety in cell-based regenerative and translational medicine, but there is no specific definition about the maximum number of passages that ensure the lowest possible risk in MSC-based regenerative medicine. In this context, the aim of this study was to analyze DNA damage and chromosome alterations in adipose-derived mesenchymal stromal cells (ADMSC) until the eleventh passage and to provide additional subsidies to regulatory agencies related to number of passages in these cells. Thus, two methods in genetic toxicology were adopted: comet assay and micronucleus test. The comet assay results showed an increase in DNA damage from the fifth passage onwards. The micronucleus test showed a statistically significant increase of micronucleus from the seventh passage onwards, indicating a possible mutagenic effect associated with the increase in the number of passages. Based on these results, it is important to emphasize the need to assess genetic toxicology and inclusion of new guidelines by regulatory agencies to guarantee the safety of MSC-based therapies for human diseases.

Background and Aims: Manotes expansa Sol. ex Planch. and Aframomum alboviolaceum (Ridl.) Schum. are two plants belonging respectively to the family Connaraceae R.Br. and Zingiberaceae Martino widely used in traditional medicine for the treatment of eye diseases, fever, headaches, gastritis as well as asthma. The aim of the present study is the valorization of these two plants collected in the Democratic Republic of Congo by a quantitative and qualitative analysis of secondary metabolites and mineral elements in their leaves. Materials and Methods: The determination of secondary metabolites in the leaves of Manotes expansa and Aframomum alboviolaceum was carried out by UV-Visible spectrophotometry and X-ray fluorescence spectrophotometry for the identification and quantitative analysis of mineral elements. Results: The results showed that the leaves of these two plants are rich in phenolic compounds, i.e. 442.2 mgEqAG/g for the leaves of A. alboviolaceum, 370.64 mgEqAG/g for the red leaves and 282.64 mgEqAG/g green leaves of M. expansa. Although being part of the same plant, the red and green leaves of M. expansa presented a totally different phytochemical profile. The contents of condensed tannins, anthocyanins and flavonoids are respectively 0.3%, 0.68% and 3.29% for the leaves of A. alboviolaceum; 0.58%, 0.36% and 6.89% for the red leaves, and 0.65%, 0.26% and 7.55% for the green leaves of M. expansa. The mineral content in the leaves of both plants remains dominated by the high concentration of potassium (K), calcium (Ca), Magnesium (Mg), Manganese (Mn) and Iron (Fe). Conclusion: The high content of phenolic compounds and essential trace elements makes the leaves of M. expansa and A. alboviolaceum potential candidates to alleviate several health problems in Africa in general and particularly in the Democratic Republic of Congo.

Purpose: To derive and provide, for the first time, comprehensive analytic formulas for scleral softening volume efficacy (SVE) for accommodative gain (AG) via the increased space between ciliary body and lens (SCL) and mobility of the posterior vitreous zonules (PVZ).Study Design: To increase the AG of presbyopic eye by a new procedure, laser scleral softening (LSS).Place and Duration of Study: New Taipei City, Taiwan, between June 2022 and July 2022.Methodology: The SVE is calculated based on the time and spatial integral of the scleral temperature profiles, T(z,t), solutions of a heat diffusion equation. Analytic formulas for SVE is derived based on the covered area given by a triangle area. The SVE of a 3-D model is governed by the "volume" covered by the laser beam, or its spot size area, the effective penetration depth (z"), which is an increasing function of laser dose, but a decreasing function of the absorption coefficient (A), due to the Beer's law of laser intensity, I(z)=I0exp(-Az). The efficacy depth-range (dZ) and time-ranges (dT) are defined for efficient softening with T(z,t)>T*, where T* is the scleral softening threshold temperature.Results: The accommodative gain is proportional to the 3-D SVE given by: SEV(3D) = SEV(1D) x laser beam spot (2-D area) x total number of spots (N) acting on the sclera, which is proportional to the efficacy ranges dZ and dT, in which dZ is an increasing of laser irradiation time, whereas dT is a decreasing function of depth. Softening of the scleral tissue after a thermal laser leading to the increase of PVZ mobility and SCL. However, the actual relation of SVE and the PVZ and SCL changes require measured data.Conclusion: Safety and efficacy of scleral softening for presbyopia treatment depend upon the laser parameters (intensity, dose, spot size, wavelength) and the effective depths. The SVE is proportional to the efficacy depth-range (dZ) and time-range (dT), in which dZ is an increasing of laser irradiation time and dT is a decreasing function of depth. The AG is proportional to the SVE(in 3-D).

Purpose: To derive and provide analytic formulas and proposed protocol for accommodative gain of presbyopia eyes via laser scleral softening, which causes increased space between ciliary body and lens (SCL) and mobility of the posterior vitreal zonules (PVZ).Study Design: To increase the accommodation of presbyopia by laser scleral heating/softening.Place and Duration of Study: New Taipei City, Taiwan, between April 2022 and June 2022.Purpose: To analyze the safety and efficacy of presbyopia treatment via scleral softening.Methodology: The scleral softening efficacy is calculated based on the rate equation of scleral tissue with a rate coefficient given by an Arrhenius formula, Temperature spatial and temporal profiles are given by the numerical solutions of a heat diffusion equation with a volume heating source. Various effective depths including tissue damage depth, temperature penetration depth and conversion depth, governed by tissue absorption coefficient, light intensity and dose (or irradiation time), and the related threshold values, are introduced in replacing the conventional penetration depth based on a Beer's law.Results: Given the the temperature spatial and temporal profiles, scleral softening efficacy can be calculated. Scleral surface damage can be prevented by cooling window. The suggested protocol for scleral softening treatments include: a diode laser at about 1.45 to 1.5 祄 or about 1.86 to1.9 祄, or about 2.0 to 2.15 祄, wavelength (with absorption coefficient about 20 to 100 cm-1); laser power about 0.2 to 0.8 W per spot, having a total of 4 to 16 spots; and irradiation time of 100 to 600 ms. Results of corneal thermal shrinkage are demonstrated by the topography changes of pig eyes, in which the scleral softening does not affect the corneal shapes. The accommodative gain is proportional to the softening efficacy (Seff) of the scleral tissue after a thermal laser leading to the increase of PVZ mobility and SCL. However, the actual relation of Seff and the PVZ and SCL changes require measured data.Conclusion: Safety and efficacy of scleral softening for presbyopia treatment depend upon the laser parameters (intensity, dose, spot size, wavelength) and the effective depths. By choosing the laser treated areas, a dual function treatment using scleral softening for presbyopia, and cornea stromal shrinkage for hyperopia is proposed and demonstrated by topography of pig eyes.

Aims: Intestinal parasitic infections are persistent in Africa, and we questioned here whether prevention measures imposed by the occurrence of the COVID-19 pandemic in 2020 could alleviate this threat in lowlands of the city of Yaoundé, Cameroon. Study Design: We monitored the trend of intestinal parasites in human and environmental samples from the seven subdivisions of the city of Yaounde, before (November-December 2019) and during (July-August 2020, November-December 2020) the COVID-19 pandemic. Methodology: Parasitological analysis were performed to check for the presence of helminths and protozoans in stools from inhabitants as well as in water, soil and fresh vegetables, using standard Kato Katz and Formol Ether methods.The minimum effective sample size considered for the estimation of parasite richness, parasite infection and contamination indexeswas 30 for human and 30 for environmental samples (i.e. soils, water and vegetables) per location. Results: Of the 19 parasite species identified in human and the environments, twelve were helminths and seven protozoans. The overall parasite species richness reached 16 in 2019 (12 helminths and 4 protozoans) and 19 in 2020 (12 helminths and 7 protozoans), with about 62.5-68.4% of species shared by human and environments. The parasite frequencies in human (21.03%) and water/soils (32.3%) in 2019 did not differ statistically with those of 2020 (14.6-20.3% and 10.8-35.4%, respectively). The contamination rate of vegetables (i.e. carrots, lettuce, basil, celery, etc.) has increased from 2019 (6.1-9.1%) to 2020 (9.1-24.2%), and was frequently due to roundworms, hookworms, Entamoeba and Cryptosporidium cysts. The findings suggest persistent risk associated with intestinal parasite irrespective to measures imposed by COVID-19 in study locations. Conclusion: The current control approaches may therefore integrateecological epidemiology of the intestinal parasite infections as complementary strategy in African cities.

Aim: The objective of this study was to determine the phytochemical profile of Senna alata LINN using chemical screening in solution and thin-layer chromatography, and to assess the antioxidant and anthelmintic activities of the plant抯 aqueous extracts. Methodology: All the analyses performed in this study were, respectively, done as described by the standard protocols. These were: the microscopic examination of the plant powders performed using a light microscope, the search for secondary metabolites carried out by chemical screening in solution and by thin-layer chromatography, the determination of the secondary metabolites, and the antioxidant activity carried out by UV-visible spectroscopy and the anthelmintic activity performed by dilution in decreasing order of concentration. Results: Micrographic analysis of the powder of Senna alata revealed the histological elements rich in unicellular covering hairs with a punctate surface and in fragments of palisade parenchyma, with elongated cells. The presence of polyphenols (flavonoids, anthocyanins, tannins, leuco anthocyanins, free quinones), steroids, terpenoids, and iridoids was detected by phytochemical screening in solution and confirmed by thin-layer chromatography. The determination of total phenolic compounds, flavonoids, total tannins, and anthocyanins showed that Senna alata contains 254.64 mg EQ/g, 12.3%, 9.5%, and 6.5%, respectively, of these metabolites. The aqueous extract of the leaves of Senna alata showed a good anthelmintic activity after 41 minutes of exposure to 5.00 mg/mL of the extract and the antioxidant activity was reported, of which the value of IC50 ( g/mL) of the extract for the DPPH tests is 91.42 15.56. Conclusion: Histological elements rich in unicellular covering hairs with a punctate surface and in fragments of palisade parenchyma, with elongated cells were revealed in the micrographic analysis of Senna alata. The plant抯 leaf methanol extract showed good antioxidant activity, while the anthelmintic activity was demonstrated in its aqueous extract.

Purpose: To analyze the safety and efficacy of corneal photovitrification (CPV) for improved visions of age-related macular degeneration (AMD) eyes.Study Design: Using CPV for improved visions of AMD eyes.Place and Duration of Study: New Taipei City, Taiwan, and Austin, TX, USA; between April, 2022 and June, 2022.Methodology: The CPV efficacy is calculated based on the rate equation given by dM/dt=-k(t) M(t), where M(t) is the PCV-treated corneal stroma; and k(t) is the rate coefficient given by an Arrhenius formula, k(t) = A0 exp[?Ea/(RT(t,z)], where t and z are the laser irradiation time and depth of the cornea stroma; Ea is the activation energy and R is the gas constant. The temperature spatial and temporal profiles are given by the numerical solutions of a heat diffusion equation with a volume heating source. Various effective depths including the tissue damage depth, temperature penetration depth and conversion depth, governed by the tissue absorption coefficient, light intensity and dose (or irradiation time), and the related threshold values, are introduced in replacing the conventional penetration depth based on a Beer's law.Results: The suggested protocol for CPV treatments include: a diode laser at about 2 祄 wavelength (with absorption coefficient about 100 cm-1). The laser dose is about 25 J/cm2/spot and irradiation time of 150 ms.Conclusion: The efficacy of CPV may be predicted/calculated by our modeling based on rate equation and the corneal stroma temperature rise due to laser heating. The preferred retinal locus (PRL) movement observed post-CPV is caused mainly by neuroadaptation.
RESUMO
Aims of the Study: To identify plant species traditionally used to treat anemia in Yakoma territory and to evaluate their chemical composition. Place and Duration: Yakoma Territory (survey) and University of Kinshasa (Phytochemical study), from August and October 2019. Methods: Ethnobotanical survey according to the "snowball" sampling technique among traditional healers (based on the free consent of the respondents), chemical analyses of plant materials (chemical screening, TLC, phytomarkers content, minerals composition) according to standard methods. ED-XRF was used for mineral analysis. Microsoft Excel version 2010, Origin version 8.5 Pro and IBM SPSS statistics version 20 software packages were used for data processing and analysis. Results and Discussion: The survey showed that 18 plant species are traditionally used by Ngbandi traditional healers to treat anemia in Yakoma territory. They belong to 16 families and 17 genera. The most used organs are the leaves (68.4%) and the roots (10.5%). Decoction and infusion are the most used mode of preparation (33.3% each), followed by cooking (22.2%) and maceration (11.1%). The oral route (77.8%) is the most used mode of administration followed by the enema (16.7%) and the anal route (5.6%). Morphological types consist of herbs and trees (33.3% each) and shrubs and lianas (16.7% each). The biotope types consist of forest plants (44.4%), cultivated plants/Crops (38.9%), marshy ground plants (11.1%) and ruderal plants (5.6%). These plants belonging to five biological types: erected therophytes (44.4%), mesophanerophytes (27.8%), microphanerophytes and climbing phanerophytes (11.1% each) and lianescent phanerophytes (5.56%). These anti-anemic plant species are mostly Pantropical and Afro-tropical (39% of species each) followed by Guineo-Congolese (17% of species) and American (5%). Chemical analysis revealed the presence of alkaloids, anthocyanins, flavonoids, anthraquinones and terpenoids and various minerals including iron, zinc, copper, calcium, magnesium and manganese. Conclusion: In the current state of knowledge, the survey of anemic plants from this part of the Democratic Republic of the Congo is reported for the first time.

Adipose tissue-derived mesenchymal stromal/stem cells (ASCs) are considered important tools in regenerative medicine and are being tested in several clinical studies. Porcine models are frequently used to obtain adipose tissue, due to the abundance of material and because they have immunological and physiological similarities with humans. However, it is essential to understand the effects and safe application of ASCs from pigs (pASCs) as an alternative therapy for diseases. Although minipigs are easy-to-handle animals that require less food and space, acquiring and maintaining them in a bioterium can be costly. Thus, we present a protocol for the isolation and proliferation of ASCs isolated from adipose tissue of farm pigs. Adipose tissue samples were extracted from the abdominal region of the animals. Because the pigs were not raised in a controlled environment, such as a bioterium, it was necessary to carry out rigorous procedures for disinfection. After this procedure, cells were isolated by mechanical dissociation and enzymatic digestion. A proliferation curve was performed and used to calculate the doubling time of the population. The characterization of pASCs was performed by immunophenotyping and cell differentiation in osteogenic and adipogenic lineages. The described method was efficient for the isolation and cultivation of pASCs, maintaining cellular attributes, such as surface antigens and multipotential differentiation during in vitro proliferation. This protocol presents the isolation and cultivation of ASCs from farm pig as an alternative for the isolation and cultivation of ASCs from minipigs, which require strictly controlled maintenance conditions and a more expensive process.
