RESUMO

Background:Phacoemulsification requires a smaller incision, few or no stitches are needed and the patient's recovery time is usually shorter when using a foldable IOL.Limbal Relaxing Incisions, or LRIs, are partial thickness corneal incisions strategically placed to reduce or eliminate pre-existing astigmatism duringcataractsurgeryorrefractivelensexchangesurgery. Present study was aimed to evaluate the visual and refractive outcome in patients undergoing limbal relaxing incision during clear corneal phacoemulsification surgery with foldable intra ocular lens.Material And Methods:Present study was single-center, prospective, observational study, conducted in patients of age more than 18-75 years, with clear cornea and regular astigmatism on keratometry, with preoperative astigmatism more than or equal to 0.5D on keratometry, fit for phacoemulsification cataract surgery with foldable IOL.Result:A total of 44 eyes of 44 patients who for cataract phacoemulsification surgery, mean age of the patient was 56.98 ± 05.98 years. Refractive acceptance preoperatively was 0.50 D to 2.0 D.Onslit lamp examination showed most common Cataract Nucleus grade is NS 3 with PSC preoperatively. We observed improved diopteric difference by automated keratometry on postoperative 90th day as compared to preoperative values. Majority of patients had Preop Keratometric Difference (K1-K2) of 1.5 D, all required LRI Incisions length of 4 clock hours (60°).The difference between the visual acuity preoperatively and post operatively was found to be statistically significant with a p<0.01 by employing the statistical test Analysis of Variance (ANOVA). Visual outcome a change after limb relaxing incisions statistically significant. Keratometry changes preop and postop is statistically significant with p value <0.001.Conclusion:Visual and refractive outcome changes as improved best corrected visual acuity (BCVA), decrease in Corneal Astigmatism was noted in patients undergoing limbal relaxing incision during clear corneal phacoemulsification surgery with foldable intra ocular lens

Objective: The objective of the present work was to establish a simple, precise, accurate and robust method for simultaneous estimation of gallic acid, curcumin and piperine from the marketed ayurvedic formulation by liquid chromatography. Methods: The separation was carried out on Hemochrom C18 Column (250 mm × 4.6 mm ID, 5 µm pore size) with a mobile phase methanol: acetonitrile: water (pH 3.2adjusted by using orthophosphate acid) in the ratio 70:20:10v/v by isocratic elution modeat 25 °C and the flow rate was setat0.8 ml/min. The analysis was carried out atisoabsorptive wavelength of 295 nm. Results: The retention time of gallic acid, curcumin and piperine was found to be 3.3(±0.2), 4.7 (±0.2) and 5.6 (±0.2) min, respectively. The linearity range for gallic acid, curcumin and piperine was found to be 10-70 μg/ml, 20-80 µg/ml and 2-14 µg/ml, respectively with the coefficient of linear regression greater than 0.99 for all markers. Mean percent recoveries for gallic acid, curcumin, and piperine were found within the limit of acceptance (99-100%). The percent relative standard deviation (%RSD) for precision and robustness was found less than 2%, which indicates the method is precise and robust. The developed method applied for quantification of these markers from the marketed ayurvedic formulation of Dekofcyn tablet. Conclusion: The developed method was found to be simple, rapid, precise and reproducible for standardization of Dekofcyn tablet and can be useful for other formulations containing these three markers.

Objectives: To study the drug utilization pattern in Medical Intensive Care Unit (MICU) in a tertiary care teaching hospital in rural area of Maharashtra. Materials and Methods: It was a retrospective and observational study, conducted during the period of March, 2013 to August 2013. Data was obtained through the prescription record of 123 patients admitted in the Medical ICU in the hospital from Medical Record Department (MRD) of the hospital. The study was conducted after obtaining permission from the Institutional Ethics Committee (IEC) of the college. Data was analyzed for- Most common causes for admission to MICU, male and female admission ratio, average number of drugs prescribed per prescription, Outcome of the patients, percentage usage of various antimicrobial groups. Results: Most common cause for admission in MICU was found to be Myocardial Infarction (MI) followed by angina. In all diseases number of male patients was found to be more than female patients. Average no of drugs per patients was found to be 7.82, Improvement was seen in 87.23% patients and mortality was seen in 2.23% patients. While condition remained same in 10.54% patients at the time of discharge. Cephalosporins were the most commonly prescribed antimicrobial group (65.33%) followed by aminoglycosides.

Background: Several studies have investigated the relationships of lipid levels with psychiatric patients and their results revealed an association between lipid derangement and psychiatric disorders. The aim of our study is to evaluate the lipid profile alteration in psychiatric disorder and compare with normal control. Materials and Methods: This study was conducted at People’s College of Medical Sciences and Research Centre, Bhopal. Sixty newly diagnosed psychiatric patients were included in this study and compare with 40 normal subjects. In both the groups we have measured lipid profile which includes serum total cholesterol (TC), triglycerides (TG), high density lipoprotein cholesterol (HDL), low density lipoprotein cholesterol (LDL), very low density lipoprotein cholesterol (VLDL), and cardiovascular risk factors (R-1and R2). Results: The levels of serum TC, TG, LDL-C and VLDL-C and risk factors in psychiatric patients was significantly increased as compared to control group (p<0.05). While serum HDL-C level was significantly decreased in test group (p > 0.05). Conclusion: In our study it is clearly evident that psychiatric disorders are associated with significantly higher levels of lipids (constituents of lipid profile) and risk factors for coronary heart disease.

RESUMO

Forty cases of Austin Moore Replacement done for transcervical fractures of the femur in patients were reviewed after a period of 12 to 48 months postoperatively (mean 26 mth). 30 cases (75%) had mild to severe pain of non-infective origin, starting as early as 6 months postoperatively. This was irrespective of the make, size or position (varus/valgus) of the prosthesis. Though the Aufranc and Sweet clinical scoring was satisfactory in 65% cases, radiological evidence of complications like sinking, protrusion, etc. were seen in majority of the cases. Calcar resorption was seen in 34 cases (85%) as early as 4 months postoperatively. Results of THR and bipolar replacement done for transcervical fractures in recent literature show 85% pain-free cases at 5 years. We feel that Austin Moore Replacement should be reserved for patients more than 65 years of age and those who are less active or debilitated because of other factors, because of increased acetabular wear with time in the younger individual. This is corroborated by unsatisfactory results in patients less than 65 years of age (p < 0.05).
