RESUMO

Congenital smooth muscle hamartoma usually presents as a well-circumscribed, hypertrichotic, hyperpigmented or skin colored patch or plaque on the trunk or an extremity at birth. Histologically, numerous thick, long, well defined bundles of smooth muscle fibers are scattered throughout the dermis in various directions. We report a case of congenital smooth muscle hamartoma in an 8-month-old female infant who showed hypertrichotic, skin colored plaque on the lumbosacral area since at birth. The pseudo-Darier sign was positive. Histopathologic findings were numerous, well-defined bundles of smooth muscle fibers in the dermis (H and E, Masson-trichrome stain).

RESUMO

Microsporum (M.) gypseum is geophilic and abundant in soil throughout the world. The source of human infection has been traced to soil or animals. But tinea corporis caused by M. gypseum is rare. We report 4 cases of tinea corporis caused by M. gypseum in a 38 year-old female, a 7 year-old girl, a 2 year-old boy, and a 18 year-old female. The lesions were manifested by localized pruritic fine scaly erythematous annular patches on the shin, forearm, neck, and antecubital fossa. Direct microscopic examination of scales on the potassium hydroxide preparation showed hyphae and spores in 3 cases. But cultures from the skin lesion of all patients on Sabouraud dextrose agar showed typical colonies of M. gypseum. The patients were treated with topical ketoconazole application and/or oral antifungal agents.

RESUMO

Although dermatophytes are still the main etiologic agents of onychomycosis, some species of nondermatophytic molds and yeasts are also capable of invading the nails. Scopulariopsis (S.) brevicaulis is a nondermatophytic mold which is saprophytic fungus in soil. We report four cases of toenail onychomycosis caused by S. brevicaulis in a 42-year-old male, a 46-year-old male, a 37-year-old male, and a 64-year-old male. Three patients presented with a typical distal subungual onychomycosis and one case was proximal subungual onychomycosis. Direct microscopic examination of scrapings on the potassium hydroxide preparation revealed fungal elements in all cases. Cultures from toenail lesions of the all patients on Sabouraud dextrose agar showed a typical brown colony with powdery surface of S. brevicaulis. Numerous branched conidiophores with chains of lemon-shaped conidia with rough walled were observed in slide culture and scanning electron microscopy (SEM). We confirmed S. brevicaulis by colony, light microscopic morphology and SEM.

RESUMO

Idiopathic hypereosinophilic syndrome(HES) is a multisystem disease characterized by unexplained prolonged eosinophilia and evidence of specific organ damage, including skin. Cutaneous involvement occurs in more than 50 percent of patients but cutaneous nodule as the only manifestation of HES is rare. We report a case of hypereosinophilic syndrome in a 39-year-old male, who showed a solitary tender coin-sized erythematous nodule on his right lower abdomen for 2.5 months without evidence of systemic involvement.

RESUMO

Pemphigus foliaceus is characterized by superficial blisters with acantholysis in the upper portion of the epidermis. Pemphigus foliaceus in pregnancy is rare. We report a case of pemphigus foliaceus in a 26-year-old pregnant woman who showed erythematous bullae, erosions and crusts on the whole body for 2 months. She was diagnosed by clinical and histopathological findings, direct immunofluorescent test and immunoblotting analysis. She was treated with prednisolone for 28 days and skin lesions were markedly improved.

RESUMO

Multiple cutaneous reticulohistiocytoma, a unique pattern in the spectrum of the reticulohistiocytoses, is a rare reactive granulomatous condition. We report a case of multiple cutaneous reticulohistiocytoma in a 36-year-old female, who showed multiple, pruritic, yellow-reddish brown papules on her posterior neck and right upper back for 1 year. Histologically, numerous multinucleated giant cells with a ground glass appearance observed in the dermis. Immunohistochemical stains of CD68, Vimetin were positive. The lesions were subsided after applying topical steroid for 1 month.

Kerion celsi presents as an inflammatory, boggy mass studded with broken hairs, oozing purulent material from follicular orifices. This infection is caused most commonly by zoophilic or geophilic dermatophytes and occurs almost exclusively in children. We report a case of kerion celsi caused by Trichophyton (T.) verrucosum in a 67-year-old female, who showed a tender, indurated, swollen, boggy mass exuding pus on the right parietal scalp for 20 days. Culture from a scalp lesion of the patient on Sabouraud dextrose agar showed T. verrucosum. Therapy was initiated with 250 mg of terbinafine daily with almost complete resolution of scalp lesion one month after treatment. No recurrence has been observed to date.

Perifollicular granuloma of the leg is a variant of Majocchi's granuloma. We report a case of perifollicular granuloma of the leg caused by Trichophyton(T.) rubrum in a 31-year-old female, who showed mild pruritic, erythematous indurated plaque with central ulcerations on the right shin for 8 months. Fungal culture of the biopsy specimen grew out typical white cottony colonies of T. rubrum. Histopathologic examination of the skin lesions showed chronic granulomatous inflammation and fungal elements in the dermis. After one month of terbinafine therapy (250 mg/day), lesions were completely cleared. No recurrence has been observed to date.

Hamster is mammal which belongs to the rodent family, usually small in size and frequently nocturnal in their habits and small pet kept in many countries today. We report a case of Trichophyton(T.) mentagrophytes infection probably transmitted from hamster. A 11-year-old girl was presented with a 1 month history of pruritic 3x2cm-sized erythematous scaly papular patch on the left posterior thigh. Culture from a thigh lesion of patient on Sabouraud dextrose agar showed T. mentagrophytes. One hundred mg of itraconazole per day for 15 days was tried and skin lesion was improved without recurrence.

BACKGROUND: Vitiligo was divided into two types, segmental and non-segmental vitiligo. OBJECTIVE: The purpose of our study is to investigate the clinical differences of segmental and non-segmental vitiligo. METHODS: A cross-sectional study, using standard questionnaire items on 1190 cases of vitiligo was performed. RESULTS: The results were summarized as follows 1. The non-segmental vitiligo was 2.4fold more common than segmental vitiligo. 2. The mean age of onset was older in non-segmental vitiligo than in segmental vitiligo. 3. The mean duration was longer in non-segmental vitiligo. 4. The depigmented area was larger in non-segmental vitiligo. 5. The incidences of Koebner phenomenon, progressiveness and mucosal involvement were more common in non-segmental vitiligo. 6. There were no differences about sex ratio, blood typing, family history, and associated disorders. CONCLUSION: Segmental and non-segmental vitiligo differ substantially in various clinical manifestations, which suggests that two types of vitiligo have different pathogenic mechanism.

RESUMO

Pityriasis versicolor is a benign chronic superficial fungal disease of young adults, but it is rarely observed in infancy. We report three cases of pityriasis versicolor in infancy in a 5-month-old boy, a 4.5-month-old boy, and a 7.5-month-old girl. The lesions were manifested by slightly scaly, hypopigmented macules on the face and scalp. Potassium hydroxide(KOH) preparation showed numerous short hyphae and spores, and Wood's lamp examination showed yellowish brown fluorescence of involved skins. Fungal culture on Leeming and Notman's media grew out the typical colonies of M. sympodialis and M. globosa after 2 weeks. The isolates grew well at 34degreeC. The patients were treated topically with ketoconazole twice a day for 4 weeks.

Trichophyton rubrum is an anthropophilic dermatophyte that is found all over the world, It has become one of the most important causative agents in tinea unguium and tinea pedis. But tinea capitis caused by T. rubrum is rare. Tinea capitis is uncommon in adult. We report a case of tinea capitis caused by T. rubrum in a 67-year-old female, who showed fine scaly erythematous pustules with alopecia on the frontal scalp for 2 months. Culture from a scalp lesion of patient on Sabouraud dextrose agar showed typical whitish cottony colonies of T. rubrum. The patient had been treated with terbinafine.

RESUMO

The skin lesion of herpes zoster is limited to one dermatomal area, but multidermatomal invasions have been occasionally reported. Most cases of multidermatomal herpes zoster have contiguous skin lesion. We report a case of herpes zoster duplex unilateralis with herpes zoster generalisatus that has characteristic clinical manifestation of noncontiguous skin involvement with multiple scattered vesicles. A 68-year-old male patient showed painful grouped erythematous vesicles on the left abdomen and the left face accompanyed with multiple scattered vesicles and pustules. Histopathologic findings were consistent with herpetic infection. We treated with an administration of acyclovir 1.5g a day for 7 days.

RESUMO

Pheniramine maleate(avil(R)) is a H1-antagonist that is derived from alkylamine. Skin reactions with parenteral administration of avil(R) are uncommon. A 29-year-old woman visited our department with a 3-year history of relapsing generalized multiple pruritic evanescent erythematous wheals after antihistamine and steroid injection. Intradermal skin test with Avil(R) was positive. We treated with 5 mg of mequitazine administration three times a day for 2 years.

RESUMO

We report a case of subcutaneous phaeohyphomycosis caused by Exophiala(E,) jeanselmei in a 66-year-old female, who showed a mild tender, 4.5x3.5cm sized, erythematous cystic mass with satellite lesions on the left forearm for 4 months. Histopathologically, suppurative granulomatous inflammation, brownish conidia in a chain and hyphae were observed. Fungal culture grew out the typical black-gray velvety colonies of E. jeanselmei after 2 weeks. The isolate grow well at 25 C, but very poorly at 37 C. No growth could be observed at 40 C. Sporulation adequate for evaluation was present on the malt extract agar. We confirmed E. jeanselmei by colony and microscopic morphology, temperature tolerance and sugar assimilation tests. The patient had been treated with itraconazole for 6 momths. Complete remission was observed.

RESUMO

We report a case of congenital self-healing reticulohistiocytosis in a newborn male, who showed numerous reddish brown papulonodules on the whole of his body and an extremely large tumor mass on the left sole. No extracutaneous involvement was found. All lesions involuted spontaneously within two months. Histopathologically, multiple histiocytes with abundant eosinophilic cytoplasm, multinucleated cells, and eosinophils were observed in the dermis. Immunohistochemical staining with S-100 protein showed positive.

RESUMO

We report two cases of cutaneous Mycobacterium(M.) marinum infections in a 43-year-old man and a 31-year-old woman who had multiple subcutaneous nodules on the left upper extremity and an erythematous ulcer on the right elbow, respectively. Skin biopsy specimens revealed chronic granulamatous inflammation and AFB positive bacilli in the dermis. The culture from tissue specimens on Lowenstein-Jensen media at 30'C for 10 days, yielded cream-colored colonies, which became yellow when exposed to light. The organism was identified as M. marinum by selected biochemical reactions. The patients were treated with surgical excision followed by administration of minocycline and administration of minocycline, respectively.

BACKGROUND: Clinical concern and incidence of mycotic keratitis in ophthalmic practice has been increasing. OBJECTIVE: The purpose of this study was to investigate clinical and mycological feature of mycotic keratitis. METHODS: We evaluated the clinical and mycological aspect of mycotic keratitis in 14 patients from October 1993 to March 1997 in Dongguk Unversity Hospital. RESULTS AND CONCLUSION: Mycotic keratitis showed high incidence in fifth (42.9%), sixth (35.7%), and fourth decade (14.3%). The ratio of male to female patient was 1:3.6. The seasonal prevalence was highest in autumn. A scratch or abrasion from vegetation was the most common type of the eye trauma in mycotic keratitis. The positive rate of KOH examination and culture was 92.9%, respectively. The common causative organisms of mycotic keratitis were Fusarium sp. (38.5%) and Alternaria sp. (38.5%), followed by Curvularia sp. (7.7%), Aspergillus flavus (7.7%) and Acremonium sp. (7.7%).
