RESUMO

Objective To compare the efficacy,safety and tolerability between bromfenac sodium ophthalmic solution 0.1% eye drop and loteprednol etabonate 0.5% eye drop after femtosecond laser in situ keratomileusis (FS-LASIK).Methods A prospective randomized controlled trail was performed.Seventy-six patients who had been undergone FS-LASIK in Daping Hospital were enrolled from December 2013 to December 2014.They were divided into two groups randomly by using the random number table method.The bromfenac sodium group included 38 patients administered 0.1% bromfenac sodium ophthalmic solution,and the loteprednol etabonate group comprised 38 patients treated with 0.5% loteprednol etabonate.Patients in both groups initially administered tobramycin-dexamethasone four times a day after FS-LASIK for 1 week.Then the patients in the bromfenac sodium group were treated with bromfenac sodium twice a day until 1 month postoperatively.While the patients from the loteprednol etabonate group were treated with loteprednol etabonate twice a day until 1 month after surgery.Visual acuity,refraction,intraocular pressure and ocular manifestation were examined preoperative and 1 day,1 week,1 month,3 months,6 months after FS-LASIK.This study was approved by the medical ethics Committee of Daping Hospital.Results No significant difference was found in the rate of uncorrected visual acuity (UCVA) which achieved to preoperative best corrected visual acuity (BCVA) between the two groups at each time point after operation (x2 =8.000,P =0.613).There was no difference in spherical equivalent (SE) between the two groups postoperatively (Fgroup =1.137,P =0.288),but a significant difference was found between the different time points (Ftime =29.771,P =0.000).Postoperative SE in the two groups tended to have mild hyperopia in the early days,and then gradually tended to " zero" or mild myopia.There was a significant difference with intraocular pressure among different time points (Ftime =87.519,P =0.000),but no significant difference of intraocular pressure was found between the groups (Fgroup =1.989,P =0.161).One month after surgery,the intraocular pressure were higher than 20 mmHg in two eyes in the loteprednol etabonate group,and two eyes were 5 mmHg higher than preoperative intraocular pressure.In bromfenac group,no one was higher than 5 mmHg compared with the preoperative intraocular pressure,and the pressures of all the eyes were lower than 18 mmHg.All the patients in bromfenac group were no drug withdrawal.But three patients were forced to guide drug withdrawal due to high intraocular pressure in loteprednol group.Conclusions Bromfenac sodium is safe,effective and well tolerated after FS-LASIK.

Objective To observe the effect of bandage contact lens on cornea epithelial healing after Epi-LASIK,and to evaluate the efficacy and safety.Methods Prospective case-control study.The 92 cases(181 eyes) performing Epi-LASIK were devided into observation group and control group.The 46 cases(91 eyes) of the control group were covered with routine nursing and the other 46 cases (90 eyes) of the observation group were served with bandage contact lens after Epi-LASIK.The symptoms and signs,time of corneal epithelial recovery,visual acuity and pain of postoperation of the two groups were examined within one week.Results After Epi-LASIK treatment,there was no significant difference in visual acuity between the two groups(P > 0.05).At the 1 st,3rd,and 7th day after Epi-LASIK treatment,the ache grading results of the observation group were (2.53 ± 0.62),(1.39 ± 0.71),(0.07-± 0.25);and the ache grading results of control group were (3.22 ± 0.79),(1.74 ± 0.49),(0.26 ± 0.49).Pain scores of observation group were significantly lighter than that of the control group (P < 0.05).After Epi-LASIK treatment,the average time of corneal wound healing in the observation group and control group were (2.80 ±0.86) days and (4.07 ±0.68) days respectively.The average time of postoperative corneal wound healing in the observation group was shorter than control group(P < 0.05).Conclusion The application of bandage contact lens on cornea epithelial healing after Epi-LASIK treatment can reduce the pain of patients and promote the recovery of cornea in myopia patients.

Background Corneal biomechanical properties is important in the safety assessment of corneal refractive surgery.Corvis is a new device for measuring corneal biomechanics properties.Objective This study was to observe the correlation among corneal thickness, Corvis intraocular pressure and corneal biomechanical properties with Corvis.Methods A prospective observational study was performed.One hundred and fifty eyes of 75 patients with corneal thickness from 501 μm to 590 μm were divided into three groups according to the corneal thickness:low corneal thickness group (corneal thickness range from 501 μm to 530 μm), middle corneal thickness group (corneal thickness range from 531 μm to 560 μm) ,and high corneal thickness group (corneal thickness range from 561 μm to 590 μm);and 50 eyes of 25 patients for each group.The difference of intraocular pressure, corneal thickness and deformation amplitude (DA) among the three groups were analyzed by one-way ANOVA and the correlation among the groups were analyzed by liner regression.Results The DA in the low corneal thickness group and middle corneal thickness group were significantly higher than that in the high corneal thickness group (P < 0.05).The intraocular pressure was statistically different among the 3 groups (F =9.98, P<0.05).DA was negatively correlated with intraocular pressure and corneal thickness (r=-0.84,-0.33;both at P<0.01), with the linear regression DA =1.69-0.04×IOP (F=366.19, t=-19.14,P<0.01).Conclusions Corneal thickness cannot simply represent the corneal biomechanical properties in the safety assessment of corneal refractive surgery,IOP should be considered.

Corneal refractive surgery continues to develop accompanied by a rapid advance in scientific knowledge and technology.Improvement in visual quality after corneal refractive surgery is an issue of increasing concerns.Furthermore,corneal biomechanical alteration after corneal refractive surgery is a new problem.Corneal biomechanical parameters vary upon not only the quality of refractive surgery,but also the human physiological cycles and application of some postoperative drugs.These effects are sometimes very important factors for 'unstable'outcomes.At present,much attention has been focused on the change of corneal biomechanics after operation,but the event of postoperative application of anti-inflammatory eye drops leading to ocular hypertension,refractive regression,even visual quality decline is still not well understood.Herein,the therapeutic effects and adverse responses of topical anti-inflammatory eye drops,especially the change in characteristics of corneal biomechanics after corneal refractive surgery,as well as the application of topical anti-inflammatory eye drops were discussed.It is essential that corneal refractive surgeons should fully understand the potential impact of anti-inflammatory eye drops on corneal biomechanics after corneal refractive surgery.

Objective To observe and compare the accurate dynamic changes of types Ⅰ, Ⅲ, Ⅴ, and Ⅵ collagens in the wound healing processes of the rabbit cornea undergoing laser epithelial keratomileusis (LASEK) or photorefractive keratectomy (PRK) and to investigate the possible mechanisms of corneal haze and myopic regression. Methods New Zealand White rabbits were divided into 8 groups: normal control group (n=6), 1, 7 d, and 1, 3, 4, 5, and 6 month groups (n=14). Every rabbit underwent LASEK in one eye while the other one with PRK. We used Western blot analysis to compare the wound healing process of dynamic changes of types Ⅰ, Ⅲ, Ⅴ, and Ⅵ collagens in the rabbit cornea at every time point. The results were analyzed with data analysis software. Results Western blot analysis showed that after LASEK, the cornea wound healing with types Ⅰand Ⅲ collagens were much faster than that of PRK, and the wound response was also much weaker. Whereas for types Ⅴ and Ⅵ collagens, their dynamic changes were similar between LASEK and PRK, and they both reached the peak value at 3 month after the surgery, but LASEK group returned to normal earlier than PRK group. The value of these two types of collagens after PRK was higher than that by LASEK. The changes of these four types of collagens may offer us at least partial explanation to the difference of formation of corneal haze and refractive regression. Conclusion There are significant differences between LASEK and PRK in types Ⅰ, Ⅲ, Ⅴ, and Ⅵ collagens or the time of reacting, reaching apex, and returning to normal. LASEK has slighter intensity of reaction. The results indicate that there is excessive aggradation of collagens after PRK, which may be the histological foundation of obvious haze and myopia regression.

RESUMO

The authors analyse the ultrasonographic findings of 10 cases of appendiceal mucocele retrospectively. In atotal of 10 cases, 6 cases showed only cystic masses with posterior reinforcement. The remaining 4 cases showedinternal echoes in the cystic masses. Differential diagnosis of mucocele from periappendiceal abscess is notpossible in ultrasound. The possibility of mucocele shold be considered if cystic mass in right lower quadrantabdomen is observed in ultrasound.

RESUMO

Stomach carcinoma is more common disease in korea than western contries. The reported ultrasonographicfindings of gastric carcinoma were thickening of gastric wall and “pseudokidney” sign. The auther analizedultrasonographic findings of 101 cases with gastric carcinoma who were performed ultrasonography and gastroscopyat Kyung Hee Universtiy Hospital from Oct. 1982 to Oct. 1985. The results were as followings; 1. Types of gastriccarcinoma were consisted with infiltrative type 68 cases, infiltrative type with ulceration 16 cases, polypoidtype with ulceration 1 cases, infiltrative and polypoid type 4 cases, linities plastica type 3 cases, lcerativetype 1 case and polypoid type 1 case. 2. Extent of the lesions were in body and antrum 45 cases, entire stomach 18cases, antrum 18 cases, body 12 cases, body and fundus 6 cases. 3. Ultrasonography was useful in demonstrating theextent of the tumor and the presence of materials elsewhere in abdoment.

RESUMO

Authors analyzed CT findings of 8 patients with tuberculous lymphadenitis and one case of tuberculous abscessdiagnosed surgically or clinically. The results were as follows; 1. Soft tissue density masses were noted in 8patients in paraaortic, mesenteric, peripancreatic, celiac, porta hepatis, and esophagogastric junction areas inorder of frequency, and these correspond to lylmph node groups of the same name. On contrast enhanced CT, rimenhancement with multilocular low dinsties indicating caseous necrosis were noted in 3 patients, ill defined lowdensities were seen in 3 patients, and no definite changes were noted in 2 patients. 2. Two or more lymph nodegroups were involved in 6 patients, and one lymph node group was involved in two patients. 3. A huge cystic masswith relatively irregular rim enhancement and small anount of solid component occupied nearly entire upper abdomenin 1 patient and this was confiremd as tuberculous abscess in peritoneum. 4. In 2 cases, bowel wall thickening wassuggested in cecum, ascending colon, and terminal ileum.
