RESUMO

Objective:To evaluate incision scars of transaxillary breast augmentation by different methods of scar assessment.Methods:A retrospective study was carried out on 30 patients (age range 20 to 50 years, with mean age of 32 years) who underwent endoscopic assisted transaxillary type Ⅰ dual plane breast implant augmentation by a same surgeon from August 2014 to November 2016. Scars were estimated by 3 methods which included Vancouver Scar Scale (VSS), Visual Assessment Scale (VAS) and patients' questionnaire.Results:VAS score for left side scars ranged from 0 to 8 and the median was 1. VAS score for right side scars ranged from 0 to 8.3 and the median was 1. A total of 48 scars were scored in the 0-2 point range, representing 80% of the 60 total. VSS score for left side scars ranged from 0 to 11.6 and the median was 0.8. VSS score for right side scars ranged from 0 to 11.3 and the median was 1.2. A total of 46 scars were scored in the 0-2 point range, representing 76.7% of the total 60 breats. The scores between VSS and VAS had significant statistical differences ( P<0.001). 80.0% of our patients regarded scars as unconspicuous or basically invisible in our questionnaire. Conclusions:The majority of transaxillary incision scars recover in favorable status with high patients satisfactory rate. VAS is a practical tool for evaluating transaxillary incision scars. The VSS score is not equivalent to the VAS score when grading scars only by photos.

RESUMO

Interleukin-33 (IL-33) is a new member of the IL-1 cytokine family which plays roles in the nucleus as a nuclear factor and is released by damaged or necrotic cells to act as a cytokine. It can be released via damaged or necrotic cells and functions as a cytokine. The released IL-33 activates the downstream NF-κB and MAPKs signaling pathways through the isomers of the specific receptor ST2 and the interleukin-1 receptor accessory protein (IL-1RAcP), resulting in danger signals and the activated multiple immune responses. IL-33 is abnormally expressed in various tumors and involves in tumorigenesis, development, and metastasis. Moreover, IL-33 can play both pro-tumor and anti-tumor roles in the same type of tumor.

Objective:To explore the effect of decision-making assistance during the implantation selection of breast augmentation patients to provide a reference for improving decision-making aids.Methods:From June 2018 to June 2020, the decision-making assistance programs were used in the Department of Plastic and Reconstructive Surgery, Shanghai Ninth People's Hospital for prosthesis selection in 51 breast augmentation female patients, aged 23 to 42 years, with an average of 31 years old. The BREAST-Q scale was applied to assess the postoperative results of augmentation patients. Postoperative breast satisfaction was also evaluated by the physicians.Results:With decision-making assistance, breast augmentation patients' satisfaction with breasts was (80.27±11.45) points, satisfaction with surgical results (83.41±12.29) points, social and psychological status scores (87.24±7.62) points, and sexual life status scores (85.49±7.90) points, physical condition score (73.94±8.98) points. There was no statistical difference in the scores of breast size between physicians and patients ( P>0.05). In the satisfaction score and total score of breast shape and feeling, physicians' scores were higher than patients' self-report scores, and the difference was statistically significant ( P<0.05). Conclusions:The patient self-reported postoperative outcomes are at a high level under the application of decision aid program. We can further improve the decision aid program for breast augmentation patients, adjust patient's surgical expectation, and realize shared decision making.

Objective@#To provide reference for clinical screening of individualized therapy by detecting the drug susceptibility of primary tumor cells derived from malignant pleural effusion (MPE) of the patients with non-small cell lung cancer (NSCLC). @*Methods@#MPE specimens were collected from 20 patients diagnosed as NSCLC by histopathology. They were separated by density gradient centrifugation cultured in vitro to remove non-tumor cells, and then primary cell lines were established. The half-inhibitory concentrations (IC 50 ) of conventional anti-tumor chemotherapy drugs on primary tumor cells were determined by the MTT method, and an absolute predictive value (R) was obtained by comparing the IC 50 with the theoretically calculated value of maximum plasma concentration (IC 50m ). Last, the R value was compared with the actual clinical efficacy of the NSCLC patients. @*Results@#After 20 MPE samples were pretreated and cultured for 4 generations, the primary tumor cell lines passaged stably in vitro were established successfully. Papanicolaou staining confirmed that these tumor cell lines had the characteristics of cancer cells, and their purity was nearly 100% under the microscope. The MTT results showed that the invalid IC 50 values beyond the upper limit of test concentration could be further included in the following evaluation on the therapeutic effect of patients when R values were used. When R values between 0.5 and 2.0 and more than 2.0 were used for predicting the actual therapeutic effect as disease stability and disease progression, respectively, the overall consistency between R value and actual therapeutic effect was 77% (10/13) in six patients with complete history of chemotherapy. @*Conclusion@#The primary culture of tumor cells in MPF and the detection of drug sensitivity have the clinical application value in predicting the actual therapeutic effect of NSCLC patients.

OBJECTIVE: To design an electronically controlled hydraulic damper cylinder as a key part of the computer-controlled intelligent knee joint.METHODS: According to the characteristics of hydraulic fluids and processing technology, new type of micro-structure of the digital throttle and unique structure of the two-cylinder piston had been applied to the damper. It could control extension and flexion of the knee joint in two-way automatically, which improved the gait symmetry of artificial limb. Finally, a special self-made testing device was used to evaluate the performance of the hydraulic damper. The damping force adjustment was simulated in test by controlling the two needle valves with micro-computer. The kinematics speed was detected under different state of simulating the driving moment of hip joint and the damping moment of knee joint. The testing results indirectly built the dynamics relationship among the inertial force, damping force and the swing speed of the knee joint. RESULTS: When the position value X of the needle valve was reduced by the step motor driving, the resistance of the hydraulic cylinder was increased which resulted in faster speed of the cylinder rod. The sensitivity of the speed to the driving force decreased with the opening of needle valve increasing. When the stepping motor controlled the needle valve closer, the damping moment increased which led to a faster swing speed.CONCLUSION: The design of hydraulic damper cylinder meets the requirements of prosthetic knee dynamics control.

RESUMO

Objective To evaluate the accuracy of RUT for the diagnosis of H. pylori infection before and after eradication therapy using bacterial culture and histological examination as gold standard. Methods A total of 250 patients undergoing gastroendoscopy, including 127 and 123 patients with or without previous H. pylori eradication therapy respectively were randomly selected. Each of the biopsy specimens was taken from the antrum and body of stomach for RUT, culture, and histological examination. The data of positive culture and/or positive histology were defined as H. pylori infection. It was judged H. pylori negative or eradication of the bacteria only when the results of culture and histological examination were both negative. Results In cases before eradication therapy, RUT correctly diagnosed in 84 of 86 H. Pylori-positive cases, and 34 of 37 H. Pylori-negative cases. The overall accuracy was 95.9%. The sensitivity and specificity of RUT before eradication were 97. 7% and 91. 9% respectively. In cases after eradication therapy, RUT gave correct diagnoses in 18 of 28 H. pylori - positive cases and in 98 of 99 H. pylori-negative cases with sensitivity 64. 3% and specificity 99. 0% . However, when RUT was used more than 6 months after the end of eradication therapy, both the sensitivity and specificity increased to 100%. Conclusions RUT is highly accurate for the diagnosis of H. pylori infection in untreated and treated patients who were examined more than 6 months after the completion of therapy.
