RESUMO

Objective:To compare the detection success rates (DSRs) of different kinds of near-infrared spectrum non-invasive hemoglobin monitors in high-altitude environments.Methods:One hundred and forty-four healthy volunteers of either sex, aged 18-50 yr, were assigned to one of 3 groups using a random number table method: simulated high-altitude 3 500 meter group ( n=35), 4 000 meter group ( n=55) and 4 500 meter group ( n=54). Hemoglobin was detected by Radical-7, NW-9002SHM, A5 and TensorTip MTX type hemoglobin monitors in plain environment and simulated environment at different altitudes, and the DSRs were compared.Logistic regression analysis was conducted to identify the risk factors affecting the success rate of instrument detection, and the cut-off value was determined by ROC curve and the Youden index. Results:In the simulated high-altitude environment of 3500, 4000 and 4500 m, the DSR of TensorTip MTX was significantly higher than that of Radical-7, NW-9002SHM and A5 ( P<0.001), and there was no significant difference in the DSR among Radical-7, NW-9002SHM and A5 ( P>0.05). Low SpO 2 was the main factor affecting the DSRs of the Radical-7, NW-9002SHM and A5 type hemoglobin monitor in high-altitude environment ( P<0.001), and the cut-off value of SpO 2 in determining the success of detection was 88.5%, 87.5% and 89.5%, respectively.The DSR of TensorTip MTX was not affected by low SpO 2. Conclusions:The DSR of TensorTip MTX hemoglobin monitor is minimally affected by the high-altitude environment and can be preferred in the absence of oxygen supply; when Radical-7, NW-9002SHM or A5 hemoglobin monitor applied in high-altitude environments, oxygen saturation needs to be increased to ensure a high DSR.

AIM To investigate the protective effects of polysaccharides from Dendrobium nobile Lindl.on cerebra ischemia/reperfusion injury in rats and its possible mechanism of action.METHODS One hundred and five SD rats were divided into sham group,model group,Nimodipine group (10 mg/kg) and treatment groups with low-,medium-,and high dose (50,100,and 200 mg/kg) of polysaccharides from D.nobile.The right middle cerebral artery of rats was occluded by inserting a.thread through internal carotid artery for 2 h,and was sampled after reperfusion for 24 h.The rats received gavage once a day for 7 d before operation.Neurological deficits score,brain index,brain water content and infarct size in rats were conducted at the end of reperfusion;the activity of superoxide dismutase (SOD),glutathione peroxidase (GSH-Px),myeloperoxidase (MPO) and the content of malondialdehyde (MDA) of brain tissue and blood serum were measured by chemical colorimetry;the infiltration of neutrophile granulocyte in rat ischemic cortex were detected with immune-fluorescence staining.RESULTS Sham group had no neurological deficit,but the model group showed severe neurological deficits,meanwhile,the infarct size,brain index and brain water content rose markedly,the content of MDA and the activity of MPO of the brain tissue and the blood serum increased remarkably,while the activity of SOD and GSH-Px apparently decreased;compared with the model group,neurological deficits of rats were improved significantly in polysaccharides from D.nobile dose groups,moreover,the infarct size,brain index and brain water content markedly declined,the MDA level and the activity of MPO significantly decreased,but the activity of SOD,GSH-Px increased remarkably;the infiltration of neutrophile granulocyte in the high dose group significantly decreased compared with the model group.CONCLUSION Polysaccharides from D.nobile has some neuroprotective effects on local cerebral ischemia/reperfusion injury,and that may be related to the remove of oxygen free radicals and the decrease in inflammation reaction.

RESUMO

Objective To investigate the regulation mechanism of dendrobium nobile polysaccharides (DNP) for nuclear factor kappa B (NF-κB) signal pathway in focal cerebral ischemia/reperfusion rats.Methods According to the digital random method,90 SD rats were divided into 6 groups:sham operation,model,DNP low-dose (DL,50 mg/kg),moderate-dose (DM,100 mg/kg) and high-dose (DH,200 mg/kg),and nimodipine (10 mg/kg) groups (n =15 in each group),and according to the random number method,selecting 5 in each group for the corresponding index detection.A focal cerebral ischemia/reperfusion model in SD rats was induced by middle cerebral artery occlusion.The improvement effect of DNP on rat neurological deficit (Bederson behavioral score) and brain water content,and infarct volume were observed.Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of proinflammatory cytokine tumor necrosis factor-α (TNF-α) and interleukin-β (IL-1 β) in brain tissue.Real-time fluorescence quantitative polymerase chain reaction (RT-qPCR) was used to detect microglial cell marker BCL-2-related protein A1 α (A1) and astrocyte marker glial fibrillary acidic protein (GFAP) mRNA transcription levels.Western blot was used to detect the NF-κB signaling pathway phosphorylated IκBα protein and nuclear p65 protein expression levels.Single factor variance analysis was used to compare the measurement data among the groups.Results (1) There were significant differences in the neurological deficit score,brain water content,cerebral infarction volume,brain tissue TNF-α and IL-1β levels,A 1 and GFAP mRNA transcription levels and phosphorylated IκBα protein and nuclear p65 protein levels among the 6 groups (F =22.24,8.699,33.89,19.26,27.53,109.5,15.28,66.86,and 41.63,respectively (all P < 0.01).(2) The neurological deficit score,cerebral water content,and cerebral infarction volume in the model group were 2.8 ± 0.3,86.1 ±3.8%,and 31.0 ±4.5%,respectively.The TNF-α and IL-1β levels,A1 and GFAP m RNA transcription levels in brain tissue,and phosphorylated IκBα protein and nuclear p65 protein levels were increased significantly compared with those in the sham operation group.There were significant differences (all P <0.01).(3)The above indices in the DH group were 1.5 ± 0.5,72.9 ±5.4%,and 17.5 ±4.1%,respectively.Compared with the model group (including TNF-α and IL-1 β levels,A1 and GFAP mRNA transcription levels and phosphorylation of IκBα protein and nuclear p65 protein levels in brain tissue).There were significant differences (all P < 0.05).Compared with the nimodipine group,there were no significant differences (all P > 0.05).Compared with the sham operation group,except for there were no significant difference in brain water content,phosphorylated IκBα protein and nuclear p65 protein (P >0.05),there were significant differences in other indices (all P < 0.05).(4) Compared with the model group,only IL-1 β and phosphorylated IκBα protein levels were decreased significantly in the DM group,there were no significant differences in other indices (all P > 0.05).Compared with the sham operation group,there were significant differences in other indices (all P < 0.05).(5) Compared with the model group,there were no significant differences in other indices in the DL group (P > 0.05).Compared with the sham operation group,there were significant differences in all indices (all P < 0.05).Conclusion Highdose DNP may reduce focal cerebral ischemia/reperfusion injury in rats.The mechanism of may be associated with the inhibition of activation of early NF-κB signaling pathway.The effects of low-and moderate-dose DNP on reducing inflammatory brain damage of focal cerebral ischemia/reperfusion may be not obvious.

BACKGROUND: Compared with single proprioceptive training and simple neuromuscular training in the trunk and hip, the integrative neuromuscular training possesses comprehensive effect, which not only prevents sport-induced knee injury, but also improves the athletes' sport ability.OBJECTIVE: To validate whether the integrative neuromuscular training can enhance sport quality of juvenile female athletes and improve biomechanical pattern of the lower limb when jumping.METHODS: Eighteen juvenile female tennis athletes were selected and randomized into experimental and control groups. The athletes in the experimental group received 8-week integrative neuromuscular training and technical training involving the strength of the lower limb, core stability, plyometrice training, dynamic equilibrium, agility and speed. Those controls were subjected to 8-week traditional physical training and technical training. The vertical jump dynamic equilibrium, agility and biomechanics of the lower limb were measured at 1 week before intervention and 7 weeks after intervention to quantize the effect of integrative neuromuscular training on the sport ability and prevention of knee injury.RESULTS AND CONCLUSION: The vertical jump, agility, overall stability index, the knee extension angle as well as knee adduction and abduction moment in the experimental group were significantly improved (P < 0.05). Integrative neuromuscular training cannot only enhance the explosive strength of the lower limb, dynamic equilibrium, and agility when juvenile female athletes exercising, but also improve the biomechanics of the lower limb when jumping, increase the knee extension angle, and reduce knee adduction and abduction moment

Objective To observe the effect of electroacupuncture(EA)at Baihui(GV20)and Siguan(Hegu/LI4 and Taichong/LR3)of affected side on expression of Tax1-binding protein 1(TAX1BP1)in cerebral cortex in focal cerebral ischemia-reperfusion rats,so as to in-vestigate its protective mechanism in inhibiting nuclear factor kappa B(NF-κB)signaling pathway and promoting neurobehavioral recovery. Methods A total of 105 Sprague-Dawley rats were randomly assigned into sham group,model group and EA group.Each group was ran-domly assigned into reperfusion six hours,twelve hours,24 hours,48 hours,72 hours groups after two hours of ischemia.The model was es-tablished by right middle cerebral artery occlusion and reperfusion.EA group received electroacupuncture at Baihui and left Siguan(Hegu and Taichong)acupoints.Neurobehavioral evaluation,TAX1BP1 protein expression,TAX1BP1 positive cell count,zink finger protein A20 expression, and nuclear NF-κB p65 protein expression were tested in each group. Results There was no neurological deficit in the sham group. Compared with the model group, the neurological scores at 48 hours, 72 hours after reperfusion decreased in EA group (P<0.05). Compared with the control group,the TAX1BP1 expression at twelve hours,24 hours and 48 hours after reperfusion increased in the model group(P<0.05),and further increased at twelve hours,24 hours,48 hours and 72 hours after reperfusion in EA group(P<0.05),and peaked at 24 hours after reperfusion.Compared with the sham group,the expression of A20 and NF-κB p65,and the number of TAX1BP1 positive cells increased in the model group(P<0.05),and the expression of A20 and the number of TAX1BP1 positive cells further increased,(P<0.05)and the expression of NF-κB p65 decreased in EA group(P<0.05)at 24 hours after reperfusion.Immunofluorescence labeling indicat-ed that TAX1BP1 protein primarily expressed in the cytoplasm,TAX1BP1 protein and A20 protein co-expressed in the cytoplasm.Immuno-histochemistry showed indicated that NF-κB p65 mainly expressed in the nucleus in the model groupr,and mainly expressed in the cyto-plasm in the EA group.Conclusion Electroacupuncture could significantly inhibit neuronal NF-κB signaling pathway and promote neurobe-havioral recovery in focal cerebral ischemia-reperfusion,which may related with up-regulating TAX1BP1 protein expression.

Objective To investigate compliance and influencing factors of hand hygiene (HH)among health care workers(HCWs).Methods From November 2014 to February 2015,implementation of HH among HCWs in 15 clinical departments in a hospital was investigated through questionnaires,the effect of HH purpose on HH behav-ior of HCWs was analyzed with multivariate logistic regression method.Results A total of 364 HCWs were investi-gated,245 (67.31% )of whom implemented HH well. Univariate analysis revealed that implementation of HH a-mong HCWs of different occupations,education levels,and HH purpose were all significantly different (all P<0. 05). Multivariate logistic regression analysis showed that HH purpose as well as HH compliance were both signifi-cantly different (P= 0.042). Compared with HH purpose of self-protection among HCWs,HCWs with HH pur-pose of cross-infection prevention had better HH behavior (OR,2.17[95% CI,1.29-3.65]).The top five objective factors influencing HCWs'HH implementation were as follows:lack of warm water in winter (70.73% ),heavy workload,had no time (61 .38% ),skin irritation caused by disinfectant or hand sanitizer (54.88% ),without hand dryer (47.56% ),and use disposable gloves (45.12% ).Conclusion On the basis of improving HH facilities,chan-ging HCWs'awareness of HH only for self-protection may be an another way to improve HH compliance among HCWs.

<p><b>OBJECTIVE</b>To understand the characteristics of embB gene mutation of Mycobacterium tuberculosis (MTB) isolates from tuberculosis patients in Chongqing, and the value of embB306 as a molecular marker used to diagnose ethambutol (EMB)-resistant MTB strains.</p><p><b>METHODS</b>Direct sequencing was used to analyze the polymorphism of embB mutation in 51 EMB-resistant MTB strains and 50 EMB-sensitive MTB strains. And diagnostic testing was used to evaluate the value of embB306 as a molecular marker of EMB -resistant MTB strains as compared with the traditional sensitivity test.</p><p><b>RESULTS</b>All 34 of 51 EMB-resistant strains (66.7%) and 3 of 51 EMB-sensitive strains (6%) had had embB306 mutation. The embB306 mutation rate in EMB-resistant strains coming from previously treated case was 87.5%, showing significantly higher than that from new cases (48.1%, P < 0.01); embB306 mutation rate was increased with the number of the resistant drugs; embB306 mutation serving as a marker to diagnose EMB-resistant MTB strains comparing with the traditional sensitivity test, had the rate of sensitivity = 66.7%, specificity = 94.0%, accuracy = 80.2% and Youden index = 60.7%.</p><p><b>CONCLUSION</b>embB306 mutation should be the main mechanism of MTB resistance to EMB in Chongqing, showing an association with the history of the treated and numbers of the resistant drugs. embB306 mutation should be a good marker to diagnose EMB-resistant MTB strains.</p>
