RESUMO

Objective:To explore the effect of suspension exercise training (SET) on the lumbar function and balance ability of patients with thoracolumbar fracture and spinal cord injury.Methods:Totally 64 patients with thoracolumbar fracture and spinalcord injury were randomly divided into a control group and an observation group, each of 32. Both groups received pedicle screw reduction with internal fixation and spinal dome decompression, followed by routine post-operative rehabilitation. In addition, the observation group performed double bridge and single leg suspension training and pelvic lift training. Everyone′s walking ability, spinal nerve functionality and balance were evaluated 3 days after the operation and 2 months after the training. Stride frequency, stride length and walking speed were observed. American Spinal Injury Association (ASIA) nerve function classifications were assigned. The trajectory length, trajectory included area and movement deviation of each subject′s center of gravity (CG) was recorded.Results:The average stride frequency, step length and walking speed of the observation group had all improved significantly after the intervention, and were significantly better than the control group′s averages at that point. This was also true of the proportion in ASIA neurological function grade D (65.63%), CG trajectory length and area, and the Romberg rate of deviation along the X-axis and Y-axis of the CG.Conclusion:SET training after pedicle screw reduction and internal fixation and spinal dome decompression can significantly improve the spinal nerve functioning of persons after a thoracolumbar fracture with incomplete spinal cord injury. It enhances their balance and walking ability. Such therapy is worthy of clinical promotion and application.

Cardiovascular diseases have become the most common and most prevalent chronic diseases in the elderly.Smoking,hyperlipidemia,diabetes and other high -risk factors participate in the occurrence and development of atherosclerosis by impairing the structure and function of the vascular wall ,resulting in the occurrence of arterial stiffness.Pulse wave conduction velocity (PWV) is the rate at which a pulse wave travels from one specific location of the artery to another specific location along the wall of the artery.It is a simple,effective and repeatable noninvasive index for evaluating arterial elasticity and vascular function.PWV is closely related to cardiovascular disease.In recent years,the relationship between PWV and cardiovascular disease has been deeply studied.The relationship between PWV and coronary heart disease , hypertension, cardiac function, atrial fibrillation and pulmonary hypertension is described in this paper.

Objective To improve the diagnosis and treatment of infective endocardits(IE)by exploring the causes,pathogenic microorganism,clinical characteristics and therapeutic results of patients with IE.Methods From January 2006 to January 2016,89 cases with IE in Ganzhou People's Hospital were collected,and the clinical charac-teristics were analyzed retrospectively.Results Among the IE patients,40.5%had congenital heart diseases,33.7%had rheumatic heart diseases,18.0% were post-PCI or operative endocarditis,and patients without basic cardiac diseases accounted for 7.9%.The most common clinical manifestations were fever (86.5%),followed by anemia (85.4%).The blood culture positive rate was 63.4%.Streptococci viridians were the most common causative organ-isms(44.2%),followed by coagulase -negative staphylococci(21.2%).Eighty-six patients received echocardio-graphy examination,vegetation was observed in 61cases (68.5%).The hospital mortality rate was 10.1%,mostly due to refractory congestive heart failure and sepsis.Conclusion Congenital heart disease showed an upward trend, rheumatic heart disease decreased significantly.Intermittent fever,anemia are the main clinical manifestations.Blood culture positive rate is not high,Streptococcus viridians is the most common causative organism.Echocardiography is the main basis for the diagnosis.

RESUMO

The real-time training and mutual network system of stomatology is particularly suited for evaluating the clinical practice training process.This mutual system introduced the closed loop type mutual mode,which ensured the reliability of the evaluation in the practice teaching process by the mutual feedback between students and students,students and teachers.The system solved the deficiency of the previous open loop mutual systemand made the evaluation results more objective.The system realized the process control in practice teaching,and made progress in many ways such as improving the evaluation system of students' abilities,and promoting independent learning and feedback of the teaching.Through this system platform,the students make homework as a link to discover knowledge and further solve the problem.This system guides students to develop the abilities of independent learning and mutual learning,which combines students' mutual evaluation and teachers' evaluation,and shows teaching points comprehensively.The realtime training and mutual network system can explore an effective mean in the transformation education of stomatology,and further meet the new requirements of the social development.

This paper aims to discuss the initial clinical effect of treating bone defect of long bone osteomyelitis with Masquelet technology. A retrospective analysis was made among the recruited 25 patients with long bone osteomyelitis patients from April, 2013 to February 2014 from Henan Provincial People's Hospital. Clinical effect of treating long bone osteomyelitis with Masquelet technology and the possible effects of vancomycin in different doses on knitting were also evaluated in follow-up visit lasting for 6 tilde 18 months, and 22 cases' bone was healed on imaging at the last follow up. According to Samantha X ray score criteria, Samantha score of regular dose group and high dose group in 4 months after operation was 4.16 and 3.09, respectively. There were 12 cases in regular-dose antibiotic group, among which, 1 case had delayed wound healing, and 3 cases reoccurred; there were 10 cases in high-dose antibiotic group, among which, no cases reoccurred after operation. Masquelet technology is a reliable and easy osteomyelitis treatment. Notably, it shows significant advantages for long bone reconstruction induced by infection and trauma. Bone cement with high dose of vancomycin has better effect on controlling osteomyelitis than bone cement with regular dose of vancomycin

<p><b>OBJECTIVE</b>To observe the inhibition of NK4 protein in the proliferation of human Raji lymphoma xenografts in nude mice, and to explore its molecular mechanism.</p><p><b>METHODS</b>Models of human Raji lymphoma xenograft transfected with HGF gene were established by subcutaneous inoculation in nude mice. After establishment of the models, the mice received continuous NK4 protein via tail vein for 4 weeks, and the weight and tumor growth were monitored every week. After 8 weeks, the expression of HGF mRNA and c-Met mRNA of tumor tissues was measured by real-time fluorescent quantitation PCR. The apoptotic index (AI) and microvessel density (MVD) were evaluated by terminal deoxynucleotidyl transferase-mediated nick end labeling (TUNEL) and immunohistochemistry, respectively.</p><p><b>RESULTS</b>The models of human Raji lymphoma xenograft were successfully established. Although the animal weights of all groups declined, especially in the groups with NK4 protein injection, there was no statistical significance (P > 0.05). The tumor volume in HGF gene transfected group was larger than those of the control groups (P < 0.01), and there was no statistical significance among the control groups (P > 0.05). However, the tumor volume of the NK4 protein injection group decreased significantly (P < 0.01). Expression of HGF mRNA and c-Met mRNA in HGF gene transfected group increased significantly after injection of NK4 protein (P < 0.01). AI in HGF gene transfected group (33.5% ± 12.3%) was significantly lower than that of control groups (89.1% ± 22.3% vs. 81.9% ± 27.0%, P < 0.05), but became significantly higher (119.1% ± 18.9%) after NK4 protein injection (P < 0.01). MVD in HGF gene transfected group (28.5 ± 2.0) was higher than that of control groups (12.2 ± 1.4, 13.8 ± 1.3, P < 0.01), although declined (15.5 ± 2.5) after NK4 protein injection (P < 0.01).</p><p><b>CONCLUSIONS</b>NK4 protein suppresses significantly the growth of human Raji lymphoma xenografts transfected with HGF gene. The pathogenesis may be involved in promoting tumor cell apoptosis and restraining tumor angiogenesis through competitive interrupting HGF/Met signal pathway.</p>

Objective To compare radiation doses to patients between rotational angiography and standard angiography.Methods According to different modes,our experiment was divided into RA (Dual-axis Rotational Angiography) group and SA (Standard Angiography) group.The latter was divided into 3 subgroups (SA1,SA2 and SA3) in terms of different time of cine.The procedure time and the position were controlled by the order preset into the machine.Each group consisted of 10 repeated operations with a unique phantom to simulate the true patients.The radiation doses were measured by using dose meter provided by the machine and TLD system; the means of the data were compared.Results The mean of total accumulated DAP and AK in group RA were 3 061.6 mGy·cm2 and 64.2 mGy,respectively,compared with group SA1 (4 213.9 mGy·cm2 and 85.5 mGy),SA2 (6 436.0 mGy·cm2 and 112.2 mGy),SA3 (12 810.4 mGy·cm2 and 243.7 mGy).The mean of accumulated dose in group RA was 37.70 mGy,higher than group SA1 (26.56 mGy) but lower than group SA2 (46.86 mGy) and SA3 (77.79 mGy).The gap between highest dose and lowest dose in RA group was insignificant whereas significant in the subgroups of SA.Conclusions In comparison with conventional angiography,the dual-axis rotational angiography has the potential to reduce radiation exposure of patients significantly so as to prevent skin radiation injures.

Objective: To observe curative effect of radiofrequency catheter ablation (RFCA) and changes of cardiac function and left atrial diameter after operation in patients with atrial fibrillation (AF). Methods: A total of 28 AF patients with obvious clinical symptoms and without effective for drug therapy received RFCA in our hospital, their data were retrospectively analyzed. They received RFCA under guidance of three dimensional electro-anatomic mapping (EAM) system. Changes of cardiac function and left atrial diameter were evaluated by echocardiography and curative effect was evaluated by ambulary blood pressure monitoring before, three and six months after operation. Results: Pulmonary vein isolation rate was 100% in all patients. No severe complication occurred during or after operation. After six-month follow-up, 27 cases(96.4%)did not recur AF among the 28 patients; Compared with before operation, there were significant decrease in left atrial diameter [(37.3±4.8) mm vs. (33.6±4.5) mm] and significant increase in left ventricular ejection fraction [(59.8±8.7) % vs. (64.2±6.8) %] by echocardiography, P<0.05 both. Conclusion: Radiofrequency catheter ablation is safe and effective in treatment of atrial fibrillation, and there are significant improvements in cardiac function and left atrial diameter.

The diabetes-induced cognitive impairment complications have serious effects on the patients' lives, and there is an enormous and financial burden on patients, their families and society as a whole. This review investigates the current research status of diabetes-induced cognitive impairment from different view points including molecular, models, clinics and electrophysiology. The relationship between diabetes and cognitive function and developments of research are hereby summarized. And finally, future issues of diabetes-induced cognitive impairments are pointed out, and the effective rehabilitation methods should be considered.

RESUMO

Due to the high variation in test results of indirect enzyme-linked immuno sorbent assay (ELISA) and complicated steps involved in the process of standardization, a platform used for standardizing the test results from indirect ELISA was developed. The platform was designed based on 'Improved Standardization Method for Optical Density' (I-STOD). Gauss-Newton iteration was applied to estimate parameters in a standard formula. Programming Language VB was used for developing interface of platform. The results indicated that the validity of experiment could be verified through platform. A well determined scope of standardization could be generated. The sample with concentration within the scope was standardized and the degree of dilution was calculated for those outside the scope. The platform was successfully developed which normalized the process of standardization. The function provides the researchers with an effective and convenient tool for quickly achieving standardization of ELISA test results.

Objective To prepare hepatocyte growth factor(HGF) recombinant protein and confirm its activity preliminarily according to building HGF gene prokaryotic expression vector and transforming into E.coli.Methods Clone HGF inserted into the vector pET-26b(+) to construct prokaryotic expression vector pET-26b(+)-HGF and transform into E.coli Rosseta(DE3).The transformed bacteria induced by IPTG was purified through Ni-NTA resin affinity chromatography frozen-drying after renaturation.Results HGF gene recombinant prokaryotic expression vector pET-26b(+)-HGF was constructed successfully.E.coli Rosseta(DE3) which was transformed into pET-26b(+)-HGF expresses the target protein as the form of inclusion bodies,accounting for 38% of the total bacterial proteins,and confirmed by Western blot.HGF protein which was purified by Ni-NTA resin affinity chromatography,has a purity of about 95%,and can promote proliferation,migration,and inhibition of apoptosis for human non-small cell lung cancer cell line A549 cells after interaction.Conclusion HGF gene recombinant prokaryotic expression vector pET-26b (+)-HGF was constructed and expressed in transformed E.coli Rosseta(DE3) successfully.They resumed their recombinant HGF protein structure after purification and renaturation,and had biological activity confirmed by in vitro studies.

Objective To observe the clinical safety of dual axis rotational coronary angiography (DARCA) in the diagnosis of coronary artery disease in Chinese population.Methods From March to December in 2010,74 patients undergoing diagnostic DARCA were enrolled.The improved isocentering technique was adopted in 34 of the patients at the end of the study during DARCA.Blood pressure,heart rate and symptoms were recorded immediately before-and-after contrast injections.Contrast dose,radiation exposure and procedure time for DARCA were recorded.Continuous variable data were analyzed using Student's t test,if normality assumption was violated,rank sum test would be used.Categorical variables were analyzed using x2 test.Results ( 1 ) Clinical safety:There was no chest pain documented during or immediately post-injection for all patients.Only 1 patient ( 1 % ) had an attack of ventricular tachycardia immediately after the contrast injection and then relieved automatically.Pre and post-injection systolic blood pressure values of left coronary artery were statistically different [ ( 116 ± 20 ) mm Hg vs.( 111 + 18) mm Hg( 1 mm Hg =0.133 kPa),t =3.303,P =0.001 ],and heart rates differed,too [ 73 ( 65- 84)bpm vs.71(64-78) bpm,Z =-4.789,P =0.001 ],but that imposed no clinical significance.(2)Contrast dose,radiation dose and procedure time:The mean contrast utilization,radiation dose and procedure time for DARCA were 28 (25-34) ml,8979 ( 6733-12 363 ) mGycm2 and 200 (164-270) s.Compared with conventional DARCA,improved isocentering technique during DARCA had less radiation exposure and procedure time in left coronary artery angiography and the whole coronary artery angiography [ left coronary artery angiographic radiation exposure:4004 (2932-5772) mGycm2 vs.5808 (4798- 8838) mGycm2,Z =-3.471,P =0.001 ;total radiation exposure:(8116 +2493) mGycm2 vs.( 11 371 ±4122) mGycm2,t =-4.176,P =0.001 ; left coronary artery angiographic procedure time:120 ( 80-180)s vs.150(126-214) s,Z =- 2.836,P =0.005; total procedure time:180 (139-240) s vs.220( 186-308 )s,Z =-3.004,P =0.003 ],but there was no statistically difference in contrast utilization [30(25-35) ml vs.27(25-34)ml,Z=-0.906,P=0.365].Conclusion This study demonstrates clinical safety of DARCA in the diagnosis of coronary artery disease in Chinese population.Compared with conventional isocentering technique of DARCA,improved isocentering technique can significantly reduce radiation exposure and procedure time on the basis of simplified operation,and replace the conventional isocentering technique,but randomized double-blind controlled studies should be conducted.

Polysaccharides from Dendrobium huoshanense possess immunostimulating activity, antioxidant activity and anticataract activity. In order to produce the active polysaccharides from Dendrobium huoshanense through cell culture, we investigated the effects of salicylic acid on cell growth, accumulation of polysaccharides and utilization of carbon source in suspension cultures of protocorm-like bodies from Dendrobium huoshanense. Although salicylic acid slightly inhibited the cell growth, it was beneficial to the utilization of carbon source, thus leading to significant increase in the contents of polysaccharides. The highest polysaccharide production occurred on the medium supplied with 100 micromoI/L salicylic acid. After 18 days of culture the production of polysaccharides reached 3.129 g/L, which was 1.63 times that of the control. Further, we established the kinetic models describing cell growth, polysaccharide production and carbon source utilization based on Logistic equation, Luedeking-Piret equation and Luedeking-Piret-Like equation. The calculated values from the kinetic models showed a good fit to the experimental values, suggesting that salicylic acid could be an effective compound to enhance the production of active polysaccharides from protcorm-like bodies from Dendrobium huoshanense.

@#Objective To explore the relationship between obstructive sleep apnea syndrome(OSAS)and the left ventricular diastolic dysfunction.Methods 20 consecutive newly diagnosed middle-aged OSAS patients(OSAS group)with neither controllable factors nor conditions affecting left ventricular diastolic function and 15 healthy control subjects(control group)were examined with echocardiography.Results The OSAS group exhibited left ventricular diastolic dysfunction compared with the control group,including increase of isovolumic relaxation time(P<0.05)and mitral deceleration time(P<0.05),decrease of E/A.There was no difference between tow groups in interventricular systolic thickness,posterior wall thickness,left ventricular systolic diameter,left ventricular diastolic diameter and others.Conclusion OSAS may be related with the left ventricular diastolic dysfunction.

Objective To compare the therapeutic efficacy and safety of ultrasound-guided thrombin injection (UGTI) combined with saline injection and ultrasound-guided compression repair (UGCR) for upper limb pseudoaneurysm (PSA). Methods The clinical features and therapeutic efficacy of UGCR and UGTI for PSA of the upper extremities were retrospectively assessed after catheterization through radial artery. When UGCR was conducted, PSA, its communicating emissaries with other arteries were first identified with color Doppler, and then direct manual compression over the aneurysm for 30 minutes until complete thrombosis was observed. When UGCR failed, a 20-22G needle was inserted into PSA and saline was injected with continuous color Doppler guidance to confirm that the tip of the needle was in the aneurysm. Then thrombin was injected into the aneurysmal sac. All the patients were followed-up clinically and with ultrasound 24h or 14-30 days after the treatments. Results For the 14 patients with upper limb PSA who received UGCR for the first time, 5 were cured with a success rate of 35.7%. Nine cases underwent repeated UGCR treatment, and it was successful in four patients. The overall success rate for UGCR was 64.3% (9/14). All the five patients in whom UGCR failed were successfully cured when UGTI was conducted. In four cases it was successful after the first UGTI, and only one patient received repeated UGTI treatment. The total success rate for UGTI was 100% (5/5). No obvious adverse effect and recurrence were found after follow-up for 14-30 days in all patients. Conclusions Both UGCR and UGTI are safe, effective, and expedient treatment for patients with PSA in the upper extremity. UGTI is the preferred method for its simplicity, high success rate and avoidance of possible distal arterial embolism.

Object To study the effects of different growth regulators and organic additives on rooting of regenerated shoots from Dendrobium huoshanense C Z Tang et S J Cheng protocorm like bodies Methods In order to induce root development, the regenerated shoots were cultured on MS medium containing 3% sucrose and 0 8% agar and supplemented with NAA, IBA, BA, KT, banana mud or amino acids at different concentrations During rooting induction, the development of roots was observed and the frequency of rooted shoots was scored Results The frequency and the process of root initiation were significantly influenced by different growth regulators or organic additives For initiation and development of roots, the suitable concentration of NAA, IBA, BA, KT, banana mud or amino acids was 0 2 , 0 1, 0 8, 0 8 mg/L, 20% and 0 1%, respectively Conclusion In all media tested, NAA, IBA or banana mud display greater effects on rooting of regenerated shoots of D huoshanense than others and KT enables shoots to keep green
