RESUMO

Serum amyloid A(SAA) is a novel marker widely used in the acute infection disease, especially viral infection. SAA has shown a cerntain value in assisting the clinical diagnosis, discrimination of severity and monitoring of progress and outcome of COVID-19. This paper introduces the application of SAA structural, function andits dynamic detection in the diagnosis of COVID-19, and the significance of combined detection with COVID-19 antibodies, nucleic acid and other diagnostic indicators.

Objective@#To identify the type and origin of ATP7B gene mutation in a family affected with Wilson disease by combined use of multiple methods.@*Methods@#Peripheral blood samples were collected from the proband, her parents and her brother. Sanger sequencing were used to detect point mutation and small deletion/insertion of the 21 exons and flanking sequences of the ATP7B gene in all family members. Array-based comparative genomic hybridization (aCGH) was performed to identify copy number variations (CNVs) of the ATP7B gene in the proband. The result was validated by quantitative PCR (qPCR) in other 3 members.@*Results@#Sanger sequencing indicated that the proband carried a heterozygous variation c. 2668G>A (p.V890M) derived from her mother. In addition, 5 common SNPs were detected in her mother, three of which were also identified in her father and brother. The 5 SNPs in the proband were of the wide type. aCGH analysis demonstrated that the proband was heterozygous for a 4 kb deletion, which encompassed exons 2 and 3 of the ATP7B gene and 2 SNPs. qPCR showed that the copy number in her father and brother was about half of the control, indicating heterozygous loss of exons 2 and 3.@*Conclusion@#The combined Sanger sequencing, array CGH and qPCR has identified a novel CNV involving the ATP7B gene. The strategy can improve the diagnostic rate for hereditary or rare diseases.

RESUMO

A new concept and solution of architecture of the intelligent POCT network, based on Internet of Things and intelligent POCT devices, is proposed. This network's topology structure and components with basic requirements are introduced. Through the experience of clinical application scenario, the main characteristics of the network and superiority over traditional POCT device have been analyzed.

RESUMO

Objective To discuss the clinical application value of 256-slice helical CT 3D-imaging in guiding interventional occlusion therapy for patent ductus arteriosus (PDA).Methods A total of 40 patients with sonography-proved PDA were randomly divided into group A (angiography group) and group B (CT-guided group) with 20 patients in each group.For the patients of group A,occlusion of PDA was performed based on the intraoperative angiography findings;and for the patients of group B,occlusion of PDA was carried out according to CT examination results.Intraoperative cardiac ultrasound monitoring was adopted and the curative effect was evaluated.Results The morphology of PDA demonstrated on CT 3Dimaging in group B was highly consistent with the configuration of PDA displayed on intraoperative angiography in group A.The most narrow diameters of PDA in group B and group A were (3.88±1.59) mm and (3.63±1.41) mm respectively,and the lengths of PDA in group B and group A were (6.1±1.06) mm and (6.82±0.74) mm respectively;the differences between the two groups were not statistically significant (P>0.05).The time spent for surgery in group B and group A was (34.3±9.11) min and (17.33±5.81) min respectively,and the intraoperative X-ray radiation doses in group B and group A were (33.93±11.0) mGy and (66.48±9.77) mGy respectively;the differences between the two groups were statistically significant (P<0.001).In group B,the preoperative X-ray radiation dose from CT examination was (119.79±29.45) mGy,when it was added to the intraoperative X-ray radiation dose the total cumulative radiation dose of group B was strikingly higher than that of group A.Conclusion Contrast-enhanced 256-slice helical CT scan and 3D-imaging technique can replace intraoperative angiography to get accurate anatomical imaging information of PDA,which are very helpful for the performance of interventional occlusion of PDA,meanwhile,it can effectively reduce the damage to the punctured artery and shorten the operation time.However,the radiation dose is a factor that should be taken into consideration.(J Intervent Radiol,2017,26:206-209)

Objective To investigate clinical value of the level of arterial plasma colloid osmotic pressure (COP) in predicting the prognosis of severe patients.Methods According to the prognosis of patients,the patients were divided into survival group and death group,12 h arterial plasma COP change level in the two groups were compared.According to APACHE Ⅱ score,the patients were divided into≤25 and >25 group,the COP into the ICU,12 h plasma COP change were compared.The correlation of APACHE Ⅱ score and arterial lactate into ICU,12 h arterial plasma COP were analyzed by Spearman correlation analysis.Results The level of plasma COP into the ICU and 12 h levels of survival were significantly lower than those of the death group; in the APACHE Ⅱ score≤25 group,plasma COP into the ICU levels and 12 h plasma COP levels were significantly higher than those of the APACHE Ⅱ score > 25 group,and 12 h COP levels were significantly higher than those of behind group (t =8.34,P < 0.01),Further analysis showed that the COP level into the ICU and APACHE Ⅱ score was significantly negative correlated; 12 h COP levels and APACHE Ⅱ score was significantly negative correlated.Conclusions Dynamic monitoring of COP levels is a good indicator to determine the prognosis of patients with the danger patients,the lower plasma COP,the more severe disease and poor prognosis.

RESUMO

Objective To summarize preliminary experiences on complete video-assisted thoracoscopes lobectomy for patients with non-small cell lung cancer.Methods From October 2009 to March 2012,42 patients with non-small cell lung cancer were treated with complete video-assisted thoracoscopes lobectomy.Tumors located in the left upper lobectomy in 7 cases,10 cases of left lower lobectomy,9 cases of upper right lung,4 cases of right lung,12 cases of lower right lobectomy.Preoperative cTNM stage was Ⅰ-Ⅱ (T1N0M0-T2N1M0),the size of tumor was < 5 cm,no obvious lung and mediastinal lymph node enlargement or pleura hypertrophy.Results All operations were successful.There were (9.5 ± 3.2) pieces of lymph nodes removed.Nz lymph node cleaning was more than 3 groups,the average was 3.3 groups.The operation time was 100-400 (220 ± 37) min,blood loss was 120-700 (150 ± 63) ml,duration of drainage was 3-12(4.5 ± 2.1) d.The postoperative hospital stay was 9-31 (12.2 ± 5.0) d.Conclusion Video-assisted thoracoscopes lobectomy is technically feasible and safe,but the operation indications should be paid special attention.

With wide application and exact effect, "Jajie" plays an important role in preventing and treating diseases, especially liver disease, digestive system diseases, metabolic diseases, skin diseases and so on. Therefore, systematic collection, analysis and studies on Dai ethnic medicine "Jajie" is an important content in the development of the theory of Dai ethnic medicine as well as new drugs. This essay briefs the general situation of "Jajie" and the advance of its studies, and gives comments on Jajie's development.
