RESUMO

Objective To determine the content of five alkaloids from extracts of Piper longum and test the pharmacodynamic effect of them on rats with experimental myocardial ischemia induced by injection of pituitrin. Methods The content of five alkaloids was determined simultaneously by HPLC. The experimental myocardial ischemia in rats was induced by injection of pituitrin, and the absolute value of T wave change and change of heart rate before and after model establishment were chosen to be the observation index. The effects of large, medium and small dose groups were evaluated. Results Three batches of samples were analyzed, with the contents of piperine for 56.1%, 49.7%, 51.6%; N-isobutyl-(2E,4E)-octadecatrienamide for 4.5%, 4.2%, 4.3%; guineensine for 0.46%, 0.38%, 0.40%; piplartine for 1.73%, 1.67%, 1.70% and piperamide for 0.55%, 0.46%, 0.49%, respectively. All dose groups from extracts of piper longum had significantly reduced the absolute value of T wave and almost have no effect on the change of heart rate, except the high dose group showed the effect of reducing heart rate at some time . Conclusion The HPLC method was suitable for the simultaneous determination of five alkaloids from extracts of Piper longum. It was shown that extracts of Piper longum had good bioactivity in anti-myocardial ischemia.

RESUMO

The identification and diagnosis of coronavirus disease 2019 (COVID-19) is of great significance in the treatment of COVID-19. Based on Diagnosis and Treatment Protocol for COVID-19 (Trial Version 5) issued on February 8, 2020 by the National Health Commission, there is no fungal pneumonia among the disease listed in the identification and diagnosis item. Thus, we reported a case of patient with AIDS associated with pneumocystis carinii severe pneumonia admitted to Shanghai Fourth People′s Hospital affiliated to Tongji University School of Medicine in January, 2020, which was helpful in improving the identification and diagnosis of COVID-19.

RESUMO

Objective To explore the effect and side effects of Group A Streptococcus Preparation(Sapylin)com-bined with mitomycin and lentinan combined with mitomycin in treating malignant pleural effusion through intrapleural catheter.Methods 70 malignant pleural effusion patients were selected in this study.All the patients were randomly divided into Sapylin group and lentinan group.One case with penicillin positive of the Sapylin group was adjusted to enter the lentinan group.After drainage of pleural effusion,the patients were divided into Sapylin group (34 patients) and lentinan group (36 patients).For the Sapylin group,Sapylin was prior injected and mitomycin was posterior injected.For the lentinan group,lentinan was prior injected and mitomycin was posterior injected.The side effects and clinical effect were observed and reported.Results The effective rate of Sapylin group was 85.3%,whereas 83.3%of lentinan group.There was no significant difference between the two groups(χ2 =0.051,P =0.822).Both groups had the following minor side effects:chest pain,fever,vomit and decreased white blood cells.No liver or renal injuries were reported.Conclusion The clinical outcomes of Sapylin combined with mitomycin and lentinan combined with mitomycin are both positive,in addition,the side effects are minor.

Objective To explore the characteristics of oxygen uptake efficiency (OUES) in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) and analyze the relationship between OUE and severity of disease.Methods Pulmonary function test,polysomnogram and cardiopulmonary exercise testing were performed in 35 patients with OSAHS and 25 age-matched healthy volunteers.Their successive breathing respiratory exchange parameters were collected and analyzed.And t and x2 tests were used for 2 sample comparison.Correlation analysis was performed by Pearson correlation test.Results Significant differences in peak VO2 and peak VO2 % pred existed between OSAHS and normal control groups [(18±4) vs.(28 ±6) L/min,P<0.01;(68±14) vs.(84±16) %,P<0.01].Compared with normal control group [(2.3 ±0.5) L · min-1 · lg-1 ; (36 ±4) ml/L; (36 ±4) ml/L],OUES,OUEP and OUE@AT of OSAHS group [(1.8 ± 0.4) L · min-1 · lg-1 ; (31 ± 5) ml/L; (30 ± 5) ml/L] were significantly lower (t =3.78-4.49,all P <0.01).And OUES,OUEP and OUE@AT in OSAHS patients were correlated (r =0.53-0.67,all P <0.01) positively with exercise tolerance (peak VO2% pred) while negatively with apnea hypopnea index (AHI) (r=-0.67--0.54,all P <0.01).Conclusion The oxygen uptake efficiency of patients with OSAHS is significantly reduced compared to that of normal subjects.And it is correlated negatively with severity of disease.

This study was aimed to investigate the dose-response relations and synergy effects of bioactive components in Ginkgo bilobaon antiplatelet aggregation and DPPH free radical scavenging.The antiplatelet aggregation experiment and DPPH free radical scavenging experiment were conducted respectively to investigate the dose-response relations and synergy effects.Firstly,the effect of inhibiting platelet aggregation induced by PAF of ginkgolides in rabbits was investigated.And then,the effect of DPPH free radical scavenging by ginkgo flavonoids was also investigated.Finally,the synergy effects among effective components were studied.The results showed that ginkgolide A,ginkgolide B,ginkgolide C and bilobalide had dose-response relations on antiplatelet aggregation.Quercetin,kaempferol and isorhamnetin had DPPH free radical scavenging effects with dose-response relation.It was concluded that ginkgolide had the dose-response relation on antiplatelet aggregation.Ginkgolide A and ginkgolide B had synergy effect.Ginkgo flavonoids had DPPH free radical scavenging effect.Quercetin and isorhamnetin had synergy effect.

RESUMO

Liver-Softening Anti-Fibrotic Decoction", a preparation consisted mainly of medicinal qerbs with touifying "Qi", flourishing"Yin", activating blood and eliminating stasis, was given by gavage to Dimethyl nitrosamine (DMN) induced fibrotic rat to assess its effect on ative and inactive liver fibrosis. It was found that this preparation, besides its promoting growth aud protein metabolism activeties, can also lower serum alkaline phosphatase, decrease serum hyaluronate and liver hydroxy proline, with a better curative effect on experimental liver fibrosis in rats than its prophylactic effect
