RESUMO

Objective:To analyze the therapeutic effect of adding interleukin (IL)-4Rα monoclonal antibody (duplizumab) to patients with multiple types of inflammatory diseases.Methods:Six patients with multiple types of inflammatory diseases who started to use duplizumab from August 2020 were enrolled. The changes of airway inflammation, lung function, sinuses computed tomography (CT), Asthma Control Questionnaire-5 (ACQ-5), Sino-Nasal Outcome Test-22 (SNOT-22), Pennsylvania Smell Identification Test (UPSIT) and Numerical Rating Scale (NRS) score before and after duplizumab treatment were retrospectively analyzed. All patients were treated with doprilumab for more than 5 months.Results:4 of the 6 patients had both severe asthma and chronic sinusitis, and 2 patients had both severe asthma, chronic sinusitis and atopic dermatitis. After treatment, the pulmonary function of 6 patients was improved, the levels of sputum eosinophils, exhaled nitric oxide and total IgE were decreased (all P<0.05), and the scores of paranasal sinus CT, ACQ-5, SNOT-22 and UPSIT were significantly improved (all P<0.05). However, 3 patients had increased peripheral blood eosinophils during the treatment. Conclusions:Duplizumab is effective for a variety of type 2 inflammatory diseases without obvious adverse reactions, and the peripheral eosinophils may increase during the treatment.

Objective To make rehabilitation nursing signs and apply them in positioning of unaffected extremities among patients with hemiplegia.Methods Totally 49 stroke patients with hemiplegia were recruited in the control group from July 2013 to November 2014,and 52 stroke patients with hemiplegia were recruited in the experimental group from January 2015 to September 2015.Same treatment and rehabilitation nursing were provided to patients in two groups.For the experimental group,rehabilitation nursing signs were applied together with regular guidance and demonstration of positioning of unaffected extremities.For the control group,only regular guidance and demonstration of positioning of unaffected extremities were provided.Results For the experimental group,the compliance was 86.03%(4 026 of 4 680 times),the accuracy was 72.73%(2 928 of 4 026 times);for the control group,the compliance and accuracy were 58.10% (2 562 of 4 410 times)and 50.94% (1 305 of 2 562 times),respectively.In the experimental group,no new complication occurred,no existing complication deteriorated,10 cases of complications were alleviated.However,in the control group,two cases developed complications,two cases of existing complication deteriorated,and only four cases of complications were alleviated.Subjective attention from patients and their family members in the experimental group was higher than that in the control group,and the difference was statistically significant.Conclusion For patients with hemiplegia,the rehabilitation nursing signs can significantly increase the compliance and accuracy of unaffected extremities positioning,and increase the attention from patients and their family members regarding unaffected extremities positioning in daily life.

@#Objective To explore the effect of community comprehensive rehabilitation on quality of life in patients with stroke sequelae under the family medical service mode. Methods From October, 2013 to October, 2014, 51 patients with stroke sequelae received compre-hensive rehabilitation intervention for three months, including rehabilitation training, rehabilitation guidance, health education and psycho-logical counseling. They were assessed with modified Barthel index (MBI), WHO-Disability Assessment Schedule (WHO-DASⅡ), Short Form 36 Health Survey Questionnaire (SF-36), Brain Injury Community Rehabilitation Outcome Scales (BICRO-39), Self-rating Anxiety Scale (SAS), Self-rating Depression Scale (SDS) before and after intervention. Results The scores of physical functionning, role physical, general health, mental health, social functioning and the total score in SF-36 increased after intervention (t>2.072, P<0.05). The scores of so-cial contact, locomotivity, self handling transaction ability and the total score in BICRO-39 decreased after intervention (t>2.434, P<0.05), as well as the scores of getting along with people, housework and society and participation in WHO-DAS II (t>2.507, P<0.05). Conclusion In the community health service, comprehensive rehabilitation guided by multidisciplinary rehabilitation team can facilitate to improve the quality of life in patients with stroke sequela.

Objective To analyze the effect of PDCA cycle quality management model on multi-drug resistant organism (MDROs) infec-tion control in a tertiary rehabilitation hospital. Methods From March, 2013 to December, 2015, targeted surveillance of MDROs infection control was performed with PDCA cycle quality control. The MDROs detection rate, the awareness rate of MDROs prevention and control, the rate of doctor issuing isolation orders and execution rate of medical sanitation disinfection and isolation were analyzed. Results The de-tection rates of methicillin-resistant staphylococcus aureus and Carbapenem-resistant Pseudomonas aeruginosa decreased (χ2>3.922, P399.17, P<0.001). Conclusion PDCA cycle quality management model played an impor-tant role in the prevention and control of MDROs in rehabilitation hospital.

Objective To explore the effects of collaboration intervention on self-efficacy and self-perceived burden in stroke patients. Methods 92 stroke patients hospitalized in the neurology department from January to June, 2014 were divided into control group (n=46) and intervention group (n=46). The control group received routine nursing and the intervention group received collaboration intervention in addi-tion. They were assessed with Self-Efficacy for Managing Chronic Diseases and Self-perceived Burden Scale before and after intervention. Results There was no difference in the scores of self-efficacy and self-perceived burden between groups before intervention (t0.05). The score of self-efficacy was more in the intervention group than in the control group (t=3.483, P<0.001) after intervention, and was less of self-perceived burden (t=-5.698, P<0.001). Conclusion The collaboration intervention can improve self-efficacy and relieve self-per-ceived burden in stroke patients.

@#Objective To investigate the factors related with swallowing function after stroke. Methods Stroke patients with dysphagia were reviewed, 296 cases with pseudobulbar paralysis, and 82 cases with bulbar paralysis. Swallowing function was evaluated with videoflu-oroscopic swallowing study (VFSS). The variable of age, sex, Body Mass Index (BMI), scores of National Institutes of Health Stroke Scale (NIHSS), Simplified Fug-Meyer Assessment (FMA) and modified Barthel Index (MBI), intubation, aspiration, length of hospitalization (LOH), and initiation of rehabilitation intervention were analyzed with Logistic regression. Results Age, NIHSS score, intubation, aspira-tion, initiation of rehabilitation intervention were the risk factors for poor outcome of swallowing function in all the patients, while the score of MBI was the protective factor for the pseudobulbar paralysis ones. Conclusion The younger the patients, the less the NIHSS score, no in-tubation, no aspiration, and the earlier the rehabilitation intervened, the more independent the pseudobulbar paralysis patients, the better swallowing function recovered.

PURPOSE: Extensive data support the influence of the upper airway on lower airway inflammation and pathophysiology in allergic disease. However, few studies have focused on allergic inflammation in the nose after an isolated lower airway allergen challenge, a situation that can exist clinically when human subjects breathe primarily through the mouth, as occurs when nasally congested. This study used a mouse model to investigate whether upper airway inflammation and hyperresponsiveness were induced by an isolated lower airway allergen challenge. METHODS: BALB/c mice were sensitized by systemic intraperitoneal injection of ovalbumin/saline and challenged with intratracheal ovalbumin/saline. Inflammation in the nose and lungs was assessed by cytology and histology of nasal tissues and bronchoalveolar lavage fluid (BALF), while nasal airway resistance and response were measured over 3 days post-challenge. RESULTS: Intratracheal application of an allergen in anaesthetized mice resulted in exclusive deposition in the lower airway. Compared to control animals, ovalbumin-sensitized mice after challenge showed bronchial hyperreactivity and increased IL-5 in the serum BALF, as well as eosinophil infiltration in the lungs. However, nasal histology of the ovalbumin-sensitized mice showed no increase in eosinophil infiltration. The nasal lavage fluid revealed no increase in eosinophils or IL-5, and the nasal airway resistance did not increase after challenge either. CONCLUSIONS: In a mouse allergy model, exclusive allergen challenge of the lower airway can elicit a pulmonary and systemic allergic response, but does not induce upper airway inflammatory or physiological responses.

@#Objective To explore the effect of quality control circle (QCC) on health education in stroke rehabilitation nursing. Methods Such activities were conducted in the VIP ward of neurological rehabilitation in our hospital as: theme selecting, activity planning, status mastering, cause analyzing, countermeasures formulating and implementing, etc., in accordance with QCC theory from January to April, 2014. Results The satisfaction of stroke patients to health education rose from 80.1% to 91.1% (P<0.001), and the dissatisfaction to guidance of activities of daily living and prevention of complications decreased significantly (P<0.05) after the implementation of QCC program. All the circle members benefited from the improvement of QCC technique, team work, professional knowledge, communication and coordination, activity confidence, duty and honor. Conclusion Carrying out the QCC activities can play a positive role on health education in stroke rehabilitation nursing.

@#Objective To study the effect of the scene teaching on the needle stick injury in nursing students. Methods 43 nursing students of grade 2011 were as control group, 53 ones of grade 2012 as intervention group. The intervention group was introduced the definition,the harm of needle stick injury and the basic protection knowledge, shown the scene of needle stick injuries often occurred, and trained to prevent the needle stick injury a day before internship. The control group was not given any intervention. Results 48.84% and 20.75% of students of the control and the intervention groups occurred needle stick injury during the internship, and the frequencies were 1.36 and 0.42 per person-year, respectively (P<0. 05). Conclusion Scene simulation teaching can effectively reduce the incidence of needle stick injuries of nursing students, that is valuable for the occupational protective education of nursing students.

Objective To investigate the effect of intensive education among the families on nutritional status of the stroke patients with dysphagia. Methods 42 stroke patients with dysphagia diagnosed with Standard Swallowing Assessment were enrolled in the study. The pa-tients and their families were randomly divided into intervention group (21 pairs) and control group (21 pairs). The families of the interven-tion group were given the intensive health education, 5~10 minutes each time, once a week for 1 month. The families of the control group were only given routine health education. The two groups of patients received conventional drug and swallowing function training. Hemo-globin (Hb), serum albumin (ALB), contralateral brachial triceps skin fold thickness (TSF) and arm muscle circumference (AMC) were mea-sured 1 month latter. Results Hb, ALB, TSF and AMC in the intervention group were higher than in the control group (P<0.05). Conclusion Intensive education among the families may improve the nutritional status of stroke patients with dysphagia.

Objectiev To explore the use of different nebulizer to establish mice model that have airway eosinophilic inflam -mation without airway hyperresponsiveness .Methods Female BALB/c mice were obtained and divided randomly into 3 groups:eo-sinophilic airway inflammation group ( experimental group ) , asthma group, and control group .Mice were immunized with ovalbumin ( OVA) .The experiment group and asthma group were challenged with an aerosol of 1% w/v OVA using a PARI TIA and PARI LC STAR nebulizer on day 28, 29, 30, respectively.The control mice were received saline sensitization and challenge .Airway respon-siveness was measured .Cell different counts in bronchial alveolus lavage fluid ( BALF) were performed and a pathologist performed histopathological evaluation of the trachea and lung .Results Airway responsiveness in the experimental group was not significantly different compared with the normal saline ( NS) group but was significantly different compared with the asthma group .Eosinophils in BALF were increased significantly in experimental group compared with the NS group , and significant difference was observed between experimental group and asthma group .The intensity of airway inflammation in experimental group was milder than that in the asthma model .Conclusions We established an eosinophilic bronchitis mice model without hyperresponsiveness successfully .Our model es-tablished a foundation for the further research in airway hyperresponsiveness .

Objective To investigate the factors of families related to the outcome of rehabilitation for cerebral palsy children. Methods Families of 153 cerebral palsy children were investigated with questionnaires of family condition, knowledge of cerebral palsy, compliance, and the Eysenck Personality Questionnaire, Marital Quality Questionnaire, Social Support Scale, Symptom Check List, Happiness Index. The children were assessed with Comprehensive Assessment for Disable Children before and after rehabilitation. Results There were signifi-cant differences of compliance, knowledge of cerebral palsy, being of other healthy children, personality of extraversion/introversion and ly-ing, and mental symptom between the families of the cerebral palsy children improved or not after rehabilitation (P<0.05). Multivariate Lo-gistic regression analysis showed that, being of other healthy children, personality of extraversion/introversion and mental symptom were the independent factors. Conclusion It may relate to the outcome of rehabilitation for cerebral palsy children of family condition, personali-ty and mental health of the families, which need further intervention.

@#Objective To explore the causes of pulmonary infection of stroke patient during recovery period. Methods 122 stroke patients combined with pulmonary infection in neural rehabilitation department from January 2008 to July 2011 were analyzed for the relative causes according to clinical nursing. Results Repeated pulmonary infection was correlated with dysphagia and tracheotomy of acute stage (P<0.05); Bilateral pulmonary infection was correlated with dysphagia, tracheotomy of acute stage and age (P<0.05). Conclusion Pulmonary infection is correlated with dysphagia, tracheotomy of acute stage and age in sequence.

@#目的了解偏瘫患者家属对康复教育需求内容及方式。方法采用自行设计的调查问卷,对在我院偏瘫康复治疗中心住院65例偏瘫患者家属进行康复教育需求方式问卷调查。结果家属对康复内容的需求主要为肢体功能训练方法、患者发生意外指导、给予心理支持方法、日常生活活动能力指导。对教育方式的需求住院期间主要选择专题讲座、个别指导、护理示范指导,出院后主要选择电话咨询。结论应根据需求选择合适的康复教育内容和教育方式,有针对性地进行指导。

Aim To investigate the effects of glycyrrhizin on the proliferation of ASMC stimulated by fetal calf or histamine in rats. Methods Cell culture of rat ASMC, MTT assay, flow cytometry and cell growth counts were used in this study. Results ①In the culture medium containing 100g?L -1 fetal calf serum, Glycyrrhizin at low concentration(6?10 -5 mol?L -1 ) stimulated the increase of A 570 in ASMC. This effect descended with increasing glycyrrhizin concentration and changed to be inhibitory at high concentration (from 384?10 -5 mol?L -1 to 1 536 ?10 -5 mol?L -1 ). In the culture medium containing 10 -2 mol?L -1 histamine, Glycyrrhizin at both low and high concentration inhibited the increase of A 570 in ASMC. ②In the culture medium containing 100 g?L -1 fetal calf, Glycyrrhizin at low concentration(6?10 -5 mol?L -1 ) stimulated the proliferation of ASMC. This effect descended with increasing glycyrrhizin concentration and changed to be inhibitory at high concentration (from 384?10 -5 mol?L -1 to 1 536 ?10 -5 mol?L -1 ). In the culture medium containing 1 g?L -1 histamine, Glycyrrhizin at both low and high concentration inhibited the proliferation of ASMC. ③In the culture medium containing 100 g?L -1 fetal calf or 10 -2 mol?L -1 histamine, with increasing glycyrrhizin concentration, the cell count in G 1 phase increase, the cell count in G 2 and M phase decrease. Conclusion ①Glycyrrhizin accelerated the proliferation of ASMC stimulated by fetal calf at low concentration, inhibited the proliferation at high concentration. ②Glycyrrhizin inhibited the proliferation of ASMC stimulated by histamine at both low concentration and high concentration.
