RESUMO

Objective To explore the clinical significance of monitoring drainage fluid parathyroid hormone (dPTH) for estimating the in situ reserves and function of the parathyroid by analyzing the change of serum calcium,serum parathyroid hormone(sPTH) and dPTH after thyroid surgery.Methods According to the operative method,the total of 144 patients with thyroid disease were divided into five groups:unilateral lobectomy,unilateral lobectomy plus isthmectomy with unilateral lymph node dissection,total thyroidectomy,total thyroidectomy with unilateral lymph node dissection,and total thyroidectomy with bilateral lymph node dissection group.The blood calcium,sPTH and dPTH level of patients were tested before operation and on the 1st,2nd,3rd and 4th day after operation.The depression of serum calcium,hypocalcemia and hypoparathyroidism were observed after operation.The serum calcium,serum PTH and dPTH level were summarized and analyzed statistically in order to evaluate the in situ reserves and postoperative function of the parathyroid.Results Among the 114 cases,the decline of serum calcium level mostly happened on the 2nd day after operation(70 cases,61.4%).There were 36 patients with hypocalcemia (31.58%) and 34 patients with hypoparathyroidism (29.82%).Serum calcium level increased gradually in all of the patients.Although sPTH level swung,it had a rising trend on the whole.The level of serum calcium and sPTH was positively correlated.The level of dPTH was discrete and decreased along with time.The decline level of dPTH among different groups had statistical difference.Conclusions It is a promising method to evaluate the in situ reserves and function of the parathyroid by monitoring the level and changes of dPTH after thyroid surgery,and it is of value for preventive calcium supplementation after thyroid surgery.

RESUMO

In recent years, though more and more ulcerations of oral mucosa caused by allergy to drug occurred clinically, allergy to sodium fluoride glycerin is extremely rare. A case of allergy to sodium fluoride glycerin occurred in Qianfoshan Campus Hospital of Shandong University. After treatment by sodium fluoride glycerin, there was mucosal edema, a large number of red miliary granules in buccal and palatal mucosa. After 3 hours, there were swallowing difficulties, but no breathing difficulties. Next day large ulcers of oral mucosa developed. The patient was cured 7 days after treatment. Fluoride-sensitive test result was positive.

Objective To investigate the effects of the serum from rats with hemorrhagic shock and Shenfu injection, on the expression of endothelial cell protein C receptor (EPCR) in human umbilical vein endothelial cells (HUVECs) cultured with rat serum. Method The soluble endothelial protein C receptor (sEPCR) in supernatant, the expression of EPCR mRNA and protein level of EPCR in HUVECs were detected by using enzyme linked immunosorbent assay ( ELISA), reverse transcription polymerase chain reaction (RT-PCR), and western bloting (WB) in normal control group, hemorrhagic shock serum (3 h, 12 h, 24h, 72 h) group, and Shenfu-treated (3 h, 12 h, 24 h, 72 h) group, respectively. Results The mean levels of sEPCR and the expression of EPCR mRNA were significantly higher in hemorrhagic shock serum (12 h, 24 h) group, and Shenfu -treated(24 h)group than those in normal control group (all P <0.01 ),the mean levels of sEPCR and the expressions of EPCR mRNA were significantly higher in Shenfu-treated ( 12 h) group than those in normal control group ( all P <0. 05 ), while the levels of protein were lower in hemorrhagic shock serum ( 12 h, 24 h) group and in Shenfu-treated(24 h)group than those in normal control group ( both P <0.01 ), and the level of EPCR protein was lower in Shenfu-treated( 12 h) group than that in normal control group ( P < 0. 05) . The mean levels of sEPCR and the expressions of EPCR mRNA were significantly lower in Shenfu-treated ( 12 h, 24 h) group than those in hemorrhagic shock serum ( 12 h,24 h) group (all P <0.05), while the levels of EPCR protein were higher in Shenfu-treated ( 12 h, 24 h)group than those in hemorrhagic shock serum ( 12 h, 24 h) group ( P < 0.05 ). Conclusions These data suggest that Shenfu injectio could affect the expression of EPCR mRNA and the level of EPCR protein, thereby it might be effective in prevention of development of hemorrhagic shock.

Cooperation networks of nursing techniques were constructde by way of perspective practice so as to enhance the quality of nursing critical cases, the success rate of emergency treatments and patients' life quality. The measures included training in techniques for nmsing critical cases, making the rounds of wards and consultations, and enlarging the scope ofthe cooperation networks. Quantification methods were introduced into management, networks of critical case assessment were used in managing patients, and the system for assessing training in techniques for nursing critical cases was used in managing nursing personnel, thus improving the quality of the nursing pensonnel, ensuring the rational allocation of human resources and promoting the enhancement of the hospital's medical quality and its social and economic benefits. It is therefore not difficu1t to see that the cooperation networks are of application and dissemination value.

Aim In order to understand infective conditions of Clonorchis sinensis in different populations in Nantong.Methods The antibodies of Clonorchis sinensis in fishermen area and cooks and shopkeepers et al. were investigated with PVC-Fast-Dot-ELISA. Results The average detective rate of Clonorchis sinensis antibodies was 3.92% (17/434),the detective rate of Clonorchis sinensis in residents of fishermen area, cooks and shopkeepers (6.45% ,2/31,5.59% ,9/161、4.29% ,3/70)was obviously higher than that of in the normal control group(0. 86% ,1/115). There was a significant difference in the statistics (X2 = 3.52,X2 = 4.01,X2 = 2.28,P<0. 01 ). Conclusions ①There is human infection of Clonorchis sinensis in Nantong area. ②The data emphasizes that the infection of Clonorchis sinensis has a strong association to do with occupation and contacting with raw fishes and opportunity of eating raw fishes and shrimps. ③It is recommended that the people of common eating raw fishes and shrimps must be examined and treated regularly.

RESUMO

The paper gives an account of the basic methods for assessing the intrinsic quality of patient nursing by nursing procedures. These include: ①monitoring the accuracy of nursing assessment via evaluating the integrity of patients health; ②evaluating the soundness of nursing diagnosis via analyzing and judging the individuality of patients; ③evaluating the rationality of nursing goals and the appropriateness and operability of nursing measures through patients changes in behavior, function, cognition and emotion; and ④evaluating the timeliness and cyclicity of nursing result assessment through the improvement of patients overall conditions. The paper also analyses the problems involved in employing nursing procedures and countermeasures, pointing out that timely transformation of the nursing management model can markedly enhance the intrinsic quality of nursing.
