RESUMO

Objective:To investigate whether rumination and family functioning can predict the level of depression after 1 year of follow-up in patients with first-episode depression, and whether family functioning plays a mediating role between rumination and depression level.Methods:Sixty-five patients with first-episode depression who met the enrollment requirements were included, and all subjects were assessed the 17-item Hamilton depression scale(HAMD-17), rumination response scale(RRS) and family assessment device(FAD). All subjects were followed up for 1 year, and the predictive effects of rumination and family functioning at baseline on the level of depression after 1 year of follow-up were investigated by hierarchical linear regression analysis and mediation analysis. Results:At the baseline stage, rumination, role, affective involvement (AI) and general functioning (GF) were significantly positively associated with depression level after 1 year of follow-up in patients with first-episode depression ( r=0.49, P<0.01; r=0.30, P=0.02; r=0.43, P<0.01; r=0.50, P<0.01; respectively). Rumination, AI and GF at the baseline stage predicted depression level after 1 year of follow-up ( β=0.315, t=2.954, P=0.005; β=0.261, t=2.550, P=0.013; β=0.323, t=2.952, P=0.005). Mediation analysis showed that AI and GF partially mediated the relationship between rumination at baseline and depression level at 1 year follow-up (point estimate value for AI=0.040, 95% CI=0.012-0.090); point estimate value for GF=0.066, 95% CI=0.017-0.143). Conclusions:Rumination and family functioning at baseline in first-episode depressed patients can predict the depression level at 1 year follow-up.Family functioning partly mediates the relationship between the baseline rumination and the depression level at 1 year follow-up.

RESUMO

Objective:To measure the space of motion of index finger with and without injury, to apply it in rehabilitation. Methods:Thirty healthy undergraduate volunteers (controls) and three undergraduate volunteers with recent index finger injury (patients) were asked to flex, extend, adduct, abduct and ring the index finger independently and combined with middle finger, twice for a motion with an interval of 30 seconds. The index finger movement trajectory was recorded with a high-definition camera, and analyzed the range of flexion, extension, adduction, abduction and ring rotation motion with MATLAB. The movement space and defect of the injured fingers were drawn. Results:The ranges of independent flexion-extension and adduction-abduction reduced 26.5% and 24.6% in the patients compared with those of the controls, respectively. The defects of ring rotation space were very different with the degrees of recovery, and the maximum loss happened on 160°, 70° and 170°. Conclusion:Loss in index fingers motion space in range and direction may help for rehabilitation assessment.

RESUMO

Objective To analyze the incidence and death of stroke and its influencing factors, to explore the relationship between a stroke and its influencing factors, and to provide a scientific basis for the formulation and evaluation of health policies for the prevention and treatment of stroke. Methods Using a multi-stage stratified random cluster sampling method, a cross-sectional survey was conducted among 18 942 permanent residents over 40 years old . A unified questionnaire was used to analyze and understand the demographic characteristics of the research subjects. The statistical analysis was performed to compare the prevalence of stroke by gender, age, urban and rural areas, education, marital status and occupation. According to the results of single factor analysis, the Logistic model and artificial neural network model of influencing factors of stroke in the city were established. Results There were 522 stroke cases with a prevalence rate of 2.76%. There were 7 cases of death, accounting for 1.34%. The prevalence of stroke increased with the increase of age, and the trend test showed a statistical significance (P<0.01). The prevalence of stroke in primary school was higher than that in junior high school and above (P<0.01). The prevalence in rural areas was higher than that in urban areas (P<0.01). The prevalence in smokers and drinkers was higher than that in non-smokers and non-drinkers (P<0.01). Physical examination and laboratory examination showed that the waist circumference, BM, SBP, DBP, FPG, TC, TG and HDL of stroke patients were higher than those of non-stroke patients (P<0.01). The ankle brachial index and LDL value were significantly lower than those of non-stroke patients (P<0.01). Multivariate analysis showed that after adjusting gender, age, and urban and rural areas, smoking, drinking, waist length, high blood pressure measurement and high-density lipoprotein abnormality were positively correlated to stroke. Conclusion Smoking, drinking, waist length, high blood pressure and HDL were positive influencing factors of stroke. It is suggested that the intervention measures should be formulated according to the characteristics of different populations to effectively prevent and control strokes.

RESUMO

Objective:To observe the effect of early weight-bearing on the appropriate population with intertrochanteric fracture after surgery. Methods:From April, 2017 to December, 2018, a total of 45 patients with Evans-Jensen type II intertrochanteric fracture and fracture reduction as positive medial cortex support (PMCS) after proximal femoral nail anti-rotation (PFNA) fixation were randomly divided into control group (n = 22) and experimental group (n = 23). Weight-bearing as tolerated (WBAT) was initiated from six weeks after surgery in the control group, and within 48 h after surgery in the experimental group. The frequency of WBAT in two groups increased gradually from three times a day for ten minutes a time to five times a day for 20 minutes a time until clinical healing of fracture. The length of stay, hospital cost, the fracture healing time and the complication incidence were compared between two groups, as well as the scores of Visual Analogue Scale (VAS) and Harris Hip Score at six weeks, three months and six months after surgery. Results:Compared with the control group, the length of stay was shorter (t = 3.716, P < 0.01), the hospital cost was lower, but no significant difference was found (t = 1.540, P > 0.05), and the fracture healing time was shorter (t = 6.248, P < 0.001) in the experimental group. The complication incidence was lower in the experimental group, but there was no significant difference (χ2= 2.198, P > 0.05). Six weeks, three months and six months after surgery, there was no significant difference in the score of VAS between two groups (t < 1.330, P > 0.05). The score of Harris Hip Score was significantly higher in the experimental group than in the control group six weeks after surgery (t = -5.115, P < 0.001), however, no significant difference was found in other time points (|t| < 1.799, P > 0.05). Conclusion:Early weight-bearing within 48 h after PFNA fixation for Evans-Jensen type II intertrochanteric fractures and reduction with PMCS could shorten the length of stay, shorten the bony healing time and promote early recovery of hip function.

RESUMO

Objective • To analyze the value of multi-parametric magnetic resonance imaging (MRI) of knee cartilage in evaluating the efficacy of allogeneic human adipose-derived mesenchymal progenitor cells (haMPCs) on the treatment of osteoarthritis. Methods • Twenty-two patients with knee osteoarthritis were treated with haMPCs and randomly divided into three dose groups, low, medium and high, according to the medication dose. Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score and carotid multi-parametric MRI scans which include 3D-SPGR, T2-mapping, T2*-mapping, T1ρ-mapping and DWI were performed before and after 48 weeks' treatment. The WOMAC score, cartilage volume, T2 value, T2* value, T1ρ value and apparent diffusion coefficient (ADC) value were recorded. The index changes and the inter-group differences were compared. Results • A total of 18 candidates completed the WOMAC score evaluations and MRI examinations twice. After treatment, except for cartilage volume, WOMAC score, T2 value, T2* value, T1ρ value and ADC value of all subjects were significantly different from those before treatment (P0.05). The inter-group comparisons showed that there was no significant difference before treatment (P>0.05), but there was a significant difference of T1ρ in cartilage between the high-dose group and the low-dose group (P=0.012) after treatment. Conclusion • Multi-parametric cartilage MRI can be used to evaluate the efficacy of haMPCs treatment on osteoarthritis, which may be more sensitive than cartilage volume assessment.

RESUMO

BACKGROUND@#Cervical cancer has the fourth highest incidence and mortality rate of all cancers in women worldwide; it seriously harms their physical and mental health. The aim of this study was to observe the roles and preliminary mechanism of Taurine (Tau)-induced apoptosis in cervical cancer cells.@*METHODS@#Cells from the human cervical cancer cell line SiHa were transfected with the recombinant plasmid pEGFP-N1-MST1 (mammalian sterile 20-like kinase 1); then, the cell proliferation activity was analyzed by the MTT assay, cell apoptosis by flow cytometry, and the related protein levels by Western blotting.@*RESULTS@#Tau inhibited the proliferation of SiHa cells and induced apoptosis in these cells (the apoptotic rate was 21.95% in the Tau 160 mmol/L group and 30% in the Tau 320 mmol/L group), upregulated the expression of the MST1 (control, 0.53; Tau 40-320 mmol/L groups, 0.84-1.45) and Bax (control, 0.45; Tau 40-320 mmol/L groups, 0.64-1.51) proteins (P < 0.01), and downregulated the expression of Bcl-2 (control, 1.28, Tau 40-320 mmol/L groups, 0.93-0.47) (P < 0.01). The overexpression of MST1 promoted the apoptosis of SiHa cells, enhanced the apoptosis-inductive effects of Tau (P < 0.01), upregulated the expression of the proapoptotic proteins p73, p53, PUMA (p53 upregulated modulator of apoptosis), and caspase-3, and promoted the phosphorylation of YAP (Yes-associated protein).@*CONCLUSIONS@#Tau inhibited the proliferation and induced the apoptosis of cervical cancer SiHa cells. The MST1 protein plays an important role in the Tau-induced apoptosis of cervical cancer cells.

RESUMO

To evaluate the effect of Alpinia officinarum Hance (A. officinarum) extract on lung injury caused by particulate matter (PM). Methods: The Kunming mice were intranasally instilled with PM and treated with A. officinarum extract for 3 weeks. Bronchoalveolar lavage fluid, blood and lung samples were collected for biochemical, serological and histopathological studies. Results: Serological analysis showed that albumin levels, lactate dehydrogenase and alkaline phosphatase activities in bronchoalveolar lavage fluid were significantly reduced after administrations of 50, 100 and 200 mg/kg of A. officinarum extracts to the PM injured mice. Markers of oxidative stress, nitric oxide, malondialdehyde levels and nitric oxide synthase activities, were significantly decreased. Correspondingly, total superoxide dismutase activity was improved dramatically. The expressions of interleukin-6 and tumor necrosis factor alpha were also down-regulated obviously. In addition, pathological sections of lung tissue showed that A. officinarum could reduce the infiltration of inflammatory cells, pulmonary edema and pulmonary fibrosis. These results showed that A. officinarum extract could alleviate PM-induced lung injury via reducing the permeability of cell membranes in lung tissue, eliminating oxidative stress and relieving inflammatory response. Conclusions: A. officinarum extract was an efficient treatment for PM-induced lung injury ir mice, and it may be a promising therapeutic agent in future.

RESUMO

Objective To propose a novel stereo-electroencephalography(SEEG) quantitative measure analyzing ictal high frequency (60-90 Hz) and calculating high frequency epileptogenicity index (HFEI) to localize epileptogenic zone and evaluate epileptogenic network. Methods The clinical presurgical evaluation and SEEG data of 15 patients who were performed SEEG electrodes implantation from April 2015 to March 2016 were analyzed retrospectively. Post-implantation head CT images and 3D MRI data were fused for accurately identifying and locating each electrode contact. Ictal SEEG quantitative measure HFEI was calculated and threshold was set. The epileptogenic network was divided into focal, regional, multiple regional and bilateral ones and the results were compared with the pathological results. Results The epileptogenic network was focal for four patients, regional for four patients, multiple regional for six patients and bilateral for one patient (7/15). In terms of the pathology, two cases with hippocampal sclerosis both showed regional network. In four cases with cerebral malacia, two cases showed multiple regional network and the other two cases showed focal network. In six cases with cortical malformation, three cases showed multiple regional network, the other three cases showed focal, regional and bilateral networks respectively. Conclusions We explored a novel SEEG quantitative measure based on the high frequency power analysis, which is objective and could localize epileptogenic zone and evaluate the epileptic network.

RESUMO

BACKGROUND:There have been a large number of reports on establishing induced pluripotent stem celllines, but studies concerning large-scale in vitro induced differentiation of induced pluripotent stem cels into hematopoietic progenitor cels stil have a lack of in-depth discussion. OBJECTIVE:To develop methods to induce differentiation of induced pluripotent stem cels into hematopoietic progenitor cels in vitro. METHODS: Using the method of infection with lentivirus particles containing four transcriptionfactor genes, which are Oct4, Sox2, Nanog and Lin28, human skin fibroblasts are transduced into induced pluripotent stem cels. In the induced differentiation system, Y-27632, a kind of tyrosine kinase inhibitor-ROCK (p160-Rho-associated coiled-coil kinase), was added, which obviously suppressed apoptosis of cels. Based on conditioned medium with OP9 cels, a differentiation system of inducing induced pluripotent stem cels differentiating into hematopoietic progenitor cels was established. RESULTS AND CONCLUSION:(1) Apoptosis of induced pluripotent stem cels at the first three passages was very obvious, and the cels were difficult in a large-scale expansion. After Y-27632 was added, the apoptosis of embryonic stem cels was obviously inhibited. (2) During embryoid body differentiation, induced pluripotent stem cels cultured in OP9 conditional growth medium differentiated into hematopoietic progenitor celsin vitro that were positive for CD34.

RESUMO

Objective To explore the heredity susceptibility of children to Kawasaki disease (KD) through studying expression and genomic density polymorphism of peripheral erythrocyte complement receptor-1 (ECRI). Methods Thirty cases of KD patients and 28 cases of healthy children were included in this study. The rates of red blood cell (RBC)-C3bRR and RBC-ICR were detected by method described elsewhere. The ECR1 activity and genomic density polymorphism were detected by Hind Ⅲ restriction enzyme digestion polymerase chain reaction-restriction fragment length polymorphism. Results Rates of RBCoC3bRR of KD patients during the acute phase was significantly lower than that of the control group (P < 0.01), and remained lower than the control group during the recovering phase (P < 0.05). The rates of RBC-ICR were significantly higher in KD patients than that of the control group (P < 0.05). Frequencies of HL and LL genotypes of KD patients were more than those of the control group (P < 0.01). A significant difference was found in the frequency distribution of ECR1 genotype between the two groups (P < 0.01). L allele frequency in the patient group was higher than that in the control group. Conclusions Depressed RBC immune function in KD patients may be linked to the high frequency of L allele, which implies the genomic density polymorphism of ECR1 play an important role in determining susceptibility to Kawasaki disease. RESUMO

<p><b>OBJECTIVE</b>To investigate the instigating effect of "shock heart" on injury to liver, kidney and intestine at early stage of severe burn in rat.</p><p><b>METHODS</b>Fifty-six healthy male Wistar rats were enrolled in the study and randomly divided into normal control (n=8, without treatment, NC) and burn (n=48, inflicted with 30% TBSA full-thickness scald, B) groups. The rats in B group were intraperitoneally injected with Ringer's lactate solution (4 ml x kg(-1) x 1% TBSA(-1) 30 minutes after burn following the Parkland formula, and they were observed at 0.5, 1.0, 3.0, 6.0, 12.0, 24.0 post-burn hour (PBH), with 8 rats at each time point. The parameters concerning myocardial mechanics, including systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), left ventricular systolic pressure (LVSP), left ventricular end diastolic pressure( LVEDP), +/-dp/dt max were recorded. The volume of blood flow in liver, kidney and intestine were detected. The serum contents of cTnI, TBA, 32-MG, DAO were determined.</p><p><b>RESULTS</b>In B group, LVSP and +/- dp/dt max decreased at 1.0 PBH, SBP, DBP and MAP decreased at 3.0 PBH ,all parameters of myocardial mechanics, decreased at 6.0 PBH and still lower than those in NC group at 24.0 PBH (P < 0.01). The volume of blood flow in liver, kidney and intestine in B group were markedly decreased at 1.0 PBH, and gradually decreased during 1.0-12.0 PBH, which were still lower than those in NC group at 24.0 PBH (P < 0.05 or P < 0.01). Compared with that in NC group (1.71 +/- 0.07 microg/L), the serum content of cTnI in B group were increased at 0.5 PBH (2.22 +/- 0.08 microg/ L, P < 0.01), and peaked at 12.0 PBH (7.07 +/- 0.44 microg/L), and persisted at high level (4.57 +/- 0.30 microg/L) at 24.0 PBH. The serum contents of TBA at 3.0 PBH, beta2-MG at 1.0 PBH, DAO at 1.0 PBH was obviously higher than those in NC group (P < 0.05 or P < 0.01), which all showed ascending tendency during 1.0-12.0 PBH.</p><p><b>CONCLUSION</b>Myocardial damage is earlier than other organs after severe burn, which is significantly correlated with the parameters of other organs damage and their blood flow volume. Shock heart may be one initiate factor to induce the damage of liver, kidney and intestine and decrease of their blood flow volume after severe burn.</p>

RESUMO

<p><b>OBJECTIVE</b>To investigate the protective effect of oral fluid resuscitation on cardiac function in severe burn rabbits.</p><p><b>METHODS</b>One hundred and fifty rabbits were randomly divided into normal control group (NC group, n = 6, without treatment), burn group (B group, n = 42, without fluid therapy), immediate oral fluid resuscitation group (C group, n = 42), delayed oral fluid resuscitation group (D group, n = 30) and delayed and rapid oral fluid resuscitation group (E group, n = 30). The rabbits in B, C, D, E groups were subjected to 40% TBSA full-thickness burn, then were treated with fluid therapy immediately after burn (C group), at 6 hour after burn (D, E groups). The myocardial mechanics parameters including mean arterial pressure (MAP), left ventricular systolic pressure (LVSP), left ventricular end diastolic pressure (LVEDP), LV +/- dp/dt max were observed at 2, 6, 8, 12, 24, 36 and 48 post burn hour (PBH). Urine output was also examined.</p><p><b>RESULTS</b>The level of LVSP, LV +/- dp/dt max in B roup were significantly lower than those in NC group. The level of LVSP, LV +/- dp/dt max in the C and E group were singnificantly increased during 24 hour after burn. The level of LV + dp/dt max and LV-dp/dt max in C group peaked at 8 PBH (892 +/- 116 kPa/s) and at 6PBH (724 +/- 149 kPa/s) respectively. The levels of LV +/- dp/dt max, LVSP in D group at each time point were similar to B group (P > 0.05). Both the levels of LV +/- dp/dt max in E group peaked at 8 PBH. The level of LVEDP was no obvious difference between B and other groups at each time point (P > 0.05). The changes of MAP and urine output on 24 PBH in each group were similar to above indices.</p><p><b>CONCLUSION</b>Effective oral fluid therapy in severe burn rabbits during 24 hours after burn can ameliorate myocardial mechanics parameters. The amount of fluid resuscitation can be estimated according to relevant formula for delayed fluid resuscitation in burn rabbits.</p>
