RESUMO

Objective:To explore the block-based charging method for centralized dispensing of neonatal drugs in pharmacy intravenous admixture service (PIVAS), analyze its effect on drug savings and inpatient drug cost, so as to provide the reference for the appropriate charging method of neonatal drugs. Methods:According to the balance quantity and amount of neonatal intravenous drugs that were centrally allocated by the PIVAS of our hospital, refer to the doctor′s orders, the dosage per dose as well as the number of patients per dose were analyzed, then the drug types and plans for block-based charging were formulated. Before and after the implementation of the plan, the monthly average drug balance quantity and amount, the average number of drug charges for the neonates, the average daily drug cost, and the adverse events of related drugs were used as the indicators to be investigated to clarify the implementation effect of the block-based charging mothod. Results:Fourteen medicines were charged by block-based, including 4 antibiotics, 2 ordinary infusion preparations, and 8 parenteral nutrition solution preparations. The monthly average drug balance quantity was reduced from 5 047±541 to 1 856±225, and the monthly average balance amount was reduced from 65 811±10 265 yuan to 20 659±6 002 yuan. The average drug dosage for children in the trial drug was significantly reduced with a decrease range of 39.2% to 90.1%. Both the inpatient daily drug cost of neonatus and the daily average antibacterial drug cost was decreased. During the centralized dispensing of neonatal drugs, no related adverse drug events occurred. Conclusions:The block-based charging method of centralized drug distribution can improve the utilization rate of drugs, reduce drug waste, reduce the cost of inpatient medicines the financial burden on children′s families, which is worthy of further promotion and implementation.

RESUMO

Objective@#To analyze myopia related factors among students in Shanghai, to explore associated risky behaviors associated with mild, moderate, high myopia,so as to provide the evidence for the implementation of home and school combination intervention measures.@*Methods@#Six primary and secondary schools (2 primary schools, 2 middle schools and 2 high schools) were selected from each district of Shanghai. The primary schools began to investigate from the fourth grade. All the students in the selected classes participated in the vision test and questionnaire survey. Chi square test was used for categorical data analysis. The relationship between myopia and related behaviors was analyzed by multivariate Logistic analysis.@*Results@#The prevalence of visual-related risky behaviors such as short outdoor time during the day, lack of sleep, long after-school reading and writing time, poor reading and writing posture, and longtime-using mobile electronic screen was higher in girls than in boys (P<0.05), boys were more likely than girls to use computer for long time and read books/electronic screen in sunlight (P<0.05). The group with 3 hours or more than of reading and writing compared with the less than 2 hours, the OR value of mild myopia model was 1.31(1.20-1.44), moderate myopia model was 1.78(1.62-1.96), severe myopia model was 2.37(2.07-2.71). In the model of moderate and high myopia, reading and writing posture, frequency of eye relax, outdoor activity time and watching TV time were also included.@*Conclusion@#The prevalence of myopia related behaviors among primary and secondary school students is high, and there are significant gender differences among different behaviors. Students reading and writing time should be strictly controlled after school. Intervention strategies and measures should be carried out according to the characteristics of different ages and genders.

RESUMO

Objective:To compare the difference of clinical effects between multidisciplinary treatment (MDT) mode and traditional consultation mode in diagnosis and treatment of patients with dangerous upper gastrointestinal hemorrhage. Methods:The clinical data of patients with dangerous upper gastrointestinal hemorrhage admitted to the department of emergency and critical care medicine of Yichang Central People's Hospital from January 2018 to December 2019 were analyzed retrospectively, taking 30 patients in traditional consultation mode from January 2018 to December 2018 as traditional consultation group, and 26 patients in MDT mode from January 2019 to December 2019 as MDT group. The recovery time of vital signs, length of intensive care unit (ICU) stay, length of hospital stay, medical expenses, rebleeding rate, surgical operation rate, amount of blood transfusion, hospital mortality, and clinical effect of the two groups were observed. Results:The recovery time of vital signs, length of ICU stay and length of hospital stay in the MDT group were significantly shorter than those in the traditional consultation group (days: 1.62±1.30 vs. 2.20±0.93, 10.04±2.82 vs. 13.27±2.86, 3.50±1.63 vs. 4.70±2.17, both P < 0.05), and the medical expenses, rebleeding rate and blood transfusion volume were lower than those in the traditional consultation group [medical expenses (ten thousands Yuan): 6.40±3.07 vs. 8.56±4.07, rebleeding rate: 15.38% (4/26) vs. 43.33% (13/30), blood transfusion volume (U): 5.54±3.20 vs. 7.71±3.83, plasma volume (mL): 330.77±258.87 vs. 458.33±322.73, cold precipitation volume (U): 4.52±4.15 vs. 6.67±4.85, platelet volume (U): 0.40±0.19 vs. 0.77±0.60, all P < 0.05]. The clinical total efficiency in the MDT group was significantly higher than that in the traditional consultation group (88.5% vs. 80.0%, P < 0.05). There was no significant difference in the surgical operation rate and hospital mortality between the two groups. Conclusions:Compared with the traditional consultation mode, MDT mode can significantly shorten the recovery time of vital signs, the length of ICU stay, the length of hospital stay, reduce blood transfusion volume, medical expense and rebleeding risk of patients with upper gastrointestinal hemorrhage, and has better clinical effect. However, there is no significant difference in surgical operation rate and hospital mortality between the two modes.

RESUMO

CD8+T cells are critical immune cells protecting the body against infection and cancer. Long-lived memory CD8+T cells formed in a prior infection can reproduce to mount a faster and stronger im-mune response at a second encounter with the cognate antigen. The activation, clonal expansion and re-sponse of T cells are energetically demanding processes tightly coupled in cellular metabolism. Meanwhile, changes in cellular metabolism could also affect the development of memory T cells following acute infection. In this review, we discussed the current understanding of the mechanism by which glycometabolic pathways manipulate the differentiation of memory CD8+T cells in order to provide reference for improving vaccine de-velopment and cancer treatment.

RESUMO

Objective To develop an effective process for isolating and purifying haptoglobin ( Hp) from Cohn fractionⅣby a new ion exchange chromatography and to preliminarily identify and analyze the product of each purification step . Methods The fraction was first diluted and impurities were adsorbed with Rivanol .Then, the supernatant was treated with 50%ammonium sulfate. Finally, the precipitate was redissolved , and Hp was purified further with Q Sepharose Fast Flow chromatography .Native-PAGE was used to measure the activity of the haptoglobin-bound hemoglobin , while SDS-PAGE analysis and immunoblot were used for identification of the target protein .Results After pretreatment , some of the impuri-ties were removed from the Cohn fraction Ⅳ, and the target protein was enriched .In our case, the target protein was Hp and Hp2-2 was the main phenotype in the human plasma fraction Ⅳ.Target protein band and high purity were identified by SDS-PAGE.Immunoblot analysis further proved that this method could successfully isolate the target protein Hp , and the activity of 2.8 U/ml was measured by Native-PAGE method. Conclusion Haptoglobin is successfully isolated from human Cohn fractionⅣwith this method. The purification process is simple and suitable for scale-up production with a good prospect.
