【Objective】 To analyze the difference of Ct value of HBsAg-/HBV DNA + in blood samples from different types of voluntary blood donors by double ELISA and HBV DNA (MP6) detection, and to investigate the correlation between Ct value and the frequency of repeated blood donation, the first nucleic acid reactivity and the interval time of previous blood donation, so as to provide reference for laboratory evaluation of the effectiveness of nucleic acid testing(NAT) strategy for repeated blood donors occult hepatitis B virus infection(OBI). 【Methods】 The Ct value and information of blood donors from February 2019 to January 2022 in our laboratory were collected. According to the cumulative number of blood donations, they were divided into two groups:first-time blood donor group (Group A) and repeated blood donor group (Group B). Group B was subdivided into Group C 1( twice of blood donation) and group C 2(three or more times of blood donation) according to the cumulative times of blood donation, and Group D 1(< 1 year), Group D 2(1-3 years), Group D 3(3 years or more) according to the first NAT reactivity and the time of previous blood donation, the difference of Ct value and resolution yeild of HBV DNA in each group was compared. The yeild of HBV DNA in two groups was compared by chi-square test, and the difference of Ct values were compared by Nonparametric test. 【Results】 From February 2019 to January 2022, a total of 270 283 blood donors were tested, including 135 695 in Group A and 134 588 in Group B. The yeild of HBV DNA in Group A was 0.150% (203/135 695), which was higher than that in Group B [0.083% (111/134 588)] (P <0.05).All Ct values were non-normal distribution by normal distribution test, and were expressed as median (quartile), the median values of MP6 and resolution Ct were 37.0(35.9,38.2) and 35.5(33.7,36.9) in Group A, 37.2(36.4,38.1) and 36.5(35.5,37.6) in Group B, respectively. Ct values of MP detection and resolution in Group A, of MP detection and resolution in group B, and of resolution in group A and B were all significant (P<0.05) From the cumulative number of blood donations to compare, the median values of MP detection and resolution Ct were 37.5(36.6,38.3) and 36.5(35.4,37.6) in Group C1,37.1(36.4, 37.9) and 36.6(35.6,37.8) in Group C2, respectively. Significant difference in resolution Ct value between Group A and Group C1, Group A and C2 was noticed(P<0.05), the median values of MP detection Ct in D1, D2 and D3 groups were 37.2(36.3,38), 37.1(36.5,37.9), 37.8(36.6.38.9),respectively, with median resolution CT values at 37.0(35.7,37.8), 35.9(34.8,36.9), 36.9(36.1,37.7), respectively. There was a significant difference in the resolution Ct values between between Group A and D1 and D3 groups (P<0.05), and there was a significant difference between the MP detection and resolution Ct values in D2 Group (P<0.05). The resolution Ct values in D2 and D3 Group were lower than those in D1 Group (P<0.05).The interquartile distribution of Ct values in Group A was wider than that in other groups, and the interquartile distribution of Ct values in Group B was more concentrated. Conclusion The Ct value of HBV DNA detected by nucleic acid in blood donors was correlated with different times of blood donation and different intervals of blood donation. The laboratories of blood station should pay attention to the nucleic acid test results of different types of blood donors to ensure blood safety.

The parabrachial nucleus (PBN) integrates interoceptive and exteroceptive information to control various behavioral and physiological processes including breathing, emotion, and sleep/wake regulation through the neural circuits that connect to the forebrain and the brainstem. However, the precise identity and function of distinct PBN subpopulations are still largely unknown. Here, we leveraged molecular characterization, retrograde tracing, optogenetics, chemogenetics, and electrocortical recording approaches to identify a small subpopulation of neurotensin-expressing neurons in the PBN that largely project to the emotional control regions in the forebrain, rather than the medulla. Their activation induces freezing and anxiety-like behaviors, which in turn result in tachypnea. In addition, optogenetic and chemogenetic manipulations of these neurons revealed their function in promoting wakefulness and maintaining sleep architecture. We propose that these neurons comprise a PBN subpopulation with specific gene expression, connectivity, and function, which play essential roles in behavioral and physiological regulation.

Objective:To evaluate the value of CT in preoperative assessment of tricuspid valve replacement with LuX-Valve.Methods:145 consecutive patients with severe tricuspid regurgitation from October 2020 to April 2021 were selected. The multi-slice spiral CT (256 slice and above) scans were used to measure related indicators. Then analyses were made to determine whether the patients were suitable for valves replacement and to select the appropriate valve model.Results:49 cases were excluded after CT measurements, including 8 cases because of the values of the valve annulus and the remaining 41 due to comprehensive indicators includes: valve annulus size and shape, fixation method and operational safety. A total of 96 TTVR operations were performed, of which 6 were treated with thoracic surgery, 1 had moderate valve regurgitation, and the rest had no or mild regurgitation or paravalvular leaks. The operations showed a high success rate of 92.7%. After comprehensive analysis, it was found that among the 89 successful tricuspid valve replacements with LuX-Valve, 26 cases had annulus models consistent with the measurements of the diameter from tricuspid annular circumferences, while the remaining 63 had valve models larger than the diameter measurements. The results indicate that the size of right atrium was statistically significant to the enlargement of the valve size.Conclusion:CT has important application value in the preoperative screening of TTVR. For non-radial force LuX-Valve, the annulus size is not the only decisive factor, and the valve model should be comprehensively judged considering all the measurement indicators.

【Objective】 To verify the detection performance of a newly introduced individual donation(ID) nucleic acid detection(NAT) system, and to confirm whether its main test parameters meet the expected requirements of blood screening. 【Methods】 Standard serum and plasma negative for HBV DNA, HCV RNA, and HIV RNA were diluted to different multiples samples (0.5~3 times) of the detection system′s limit of detection (LoD). These samples were conducted NAT test for HBV DNA, HCV RNA and HIV-1 RNA, to verify the testing sensitivity, stability, accuracy and anti-interference ability. 【Results】 The sensitivity of 3× LoD concentration of HBV DNA, HCV RNA and HIV-1 RNA was tested by the system, and the yielding rates were all 100%, with 1 ×LoD at 95.0%~100% and 0.5×LoD at 70.0%~90.0%. The intra-assay and inter-assay precision variation coefficient was 2.07%~2.62% and 2.33%~2.88%, respectively. The accuracy of 10 external quality assessment samples of National Center for Clinical Laboratories was 100%. Severe hemolysis and fatty blood had no effect on the detection of samples with 3×LoD concentration of HBV DNA, HCV RNA and HIV-1 RNA. Any combinations by samples with 2 × LoD concentration of HBV DNA, HCV RNA, and HIV-1 RNA were not inhibited by high concentrations of other viruses. Among 2 041 sero-negative samples from blood donor, the NAT yield of this system was 1.67% (34/2 041), which was a little bit higher than that of a imported minipool system (1.66%, 33/2 041) (P>0.05). 【Conclusion】 The ID-NAT system can meet the requirements in terms of sensitivity, stability, accuracy and anti-interference ability, and can be used for blood screening of blood donors.

Objective To explore the effects of triclosan (TCS) on the immune function of mice. Methods Forty male and female Kunming mice (25±2 g) were selected. The animals were divided into 5 groups according to body weight ratio, including a blank control (saline solution) group, dimethyl sulfoxide (DMSO) group, and three triclosan solution groups (59.375 mg/kg, 118.75 mg/kg, and 237.5 mg/kg, respectively). There were 8 mice in each group, half male and half female. Animals were treated with TCS by intragastric administration once a day for two weeks. Upon the completion of the treatment, animals were sacrificed, the spleen, thymus and other tissues were collected, and the ratios of their weight to body weigh were calculated. The peripheral blood was taken by eye-ball removal method, and the half hemolysis value was determined. Results Compared with the positive control group, the spleen index of male mice in the medium dose group and high dose group increased significantly (P < 0.05), and the spleen index of female mice in the high dose group showed significant difference (P < 0.05). Compared with the positive control group, the thymus index of male high dose group was significantly different (P < 0.05). The thymus indexes of female high, medium, and low dose groups all were significantly different compared to the control group (P < 0.05). HC50 results showed that the HC50 of both male and female mice decreased (P < 0.05). Conclusion High concentration of triclosan can inhibit the immune function of Kunming mice.

@#Carbon nanomaterials are one of the hotspots in the research of drug delivery systems for cancer. Compared with the traditional drug delivery systems for cancer treatment, carbon nanomaterials can be used as drug carriers after modification based on their advantages of large specific surface area and unique optical properties. They have the characteristics of high drug loading, good biocompatibility, tumor targeting and lasting action time, indicating great potential and development space. In this paper, the properties and functions of quantum dots, carbon nanotubes, graphene oxide and mesoporous carbon nanospheres are introduced in the order of dimensional quantum properties. The current research focus and existing problems of carbon nanomaterials are discussed in order to provide a reference for the safe and effective application of carbon nanomaterials in tumor therapy.

Objective@#To explore the role of mitochondrial biogenesis and the neuroprotective mechanism of resveratrol in fluoride neurotoxicity.@*Methods@#SH-SY5Y cells in exponential phase were treated with different concentrations (20, 40, 60 mg/L) of sodium fluoride (NaF) for 24 h. Co-treatment with 60 mg/L NaF, 20 μmol/L resveratrol (RSV) was administrated in the resveratrol intervene trial. Western blotting was used to determine the expression levels of mitochondrial biogenesis key regulating factor of peroxisome proliferator-activated receptor gamma coactivator 1α (PGC-1α) , nuclear respiratory factor 1 (NRF1) and mitochondrial transcription factor A (TFAM) in SH-SY5Y cells. The mRNA levels of PGC-1α, NRF1 and TFAM were determined by Quantitative Real-time PCR in SH-SY5Y cells, as well as the relative mitochondrial DNA (mtDNA) contents and mRNA expression of mitochondrial respiratory chain complexes subunit CO1 and ATP8. Flow cytometry was used to determine mitochondrial membrane potential in SH-SY5Y cells.@*Results@#Both the protein and mRNA levels of PGC-1α, NRF1 and TFAM were decresed after 60 mg/L NaF treatment in SH-SY5Y cells (P<0.05) . The relative mtDNA contents and mRNA expression of complexes subunit CO1 and ATP8 were also significantly decreased compared with control (P<0.05) . Mitochondrial membrane potential were also significantly decreased after 60 mg/L NaF treatment in SH-SY5Y cells (P<0.05) . Compared with 60 mg/L NaF group, the protein and mRNA levels of PGC-1α, NRF1 and TFAM in 20 μmol/L RSV+60 mg/L NaF group were significantly increased (P<0.05) . The relative mtDNA contents, mitochondrial membrane potential and mRNA levels of complexes subunit CO1 and ATP8 in 20 μmol/L RSV+60 mg/L NaF group were also significantly higher than that in 60 mg/L NaF group (P<0.05) .@*Conclusion@#Resveratrol may alleviate the fluoride-induced mitochondrial biogenesis dysfunction in SH-SY5Y cells.

Objective: To evaluate the medium and long term outcomes of tension-free mid-urethral sling in the treatment of female patients with mixed urinary incontinence (MUI).Methods: Twenty-six patients who underwent the tension-free mid-urethral sling procedure for MUI from April, 2010 to September, 2016, were followed up.Four of the 26 patients underwent retropubic tension free mid-urethral sling (TVT), and 22 of them underwent transurethral middle obturator sling (TOT).Scales were used in the follow-up, such as urinary incontinence severity score (UISS), detrusor instability score (DIS), incontinence quality of life scale evaluation (I-QOL), Urogenital Distress Inventory short form (UDI-6), and the outcomes before and after the procedure were compared.Results: The mean age was 62 years, with a range of 42-80 years.The mean body mass index (BMI) was 26.82 kg/m2, with a range of 21.48-31.14 kg/m2.The mean follow-up time was 26 months, with a range of 8-69 months.Twelve patients never took M-blockers and the rest 14 patients took M-blockers within two weeks.None of the pa-tients had complications, including dysuria, injury of bladder, urethra, obturator vessel or nerve during the surgery.After pulling out the catheter, no one suffered moderate or severe pain or difficulty of urination.The overall cure rate for stress urinary incontinence (SUI) was 96.15% with 25 patients cured, and for urge urinary incontinence (UUI) was 76.92% with 20 patients cured.The patients'' life quality also improved significantly (P<0.05).Conclusion: Ten of the 26 patients showed an overactive bladder according to urodynamic study, from whom all of the six failed patient were.And 16 patients didn''t show an overactive bladder, which may due to two reasons.One is that their sense of urge is not so serious, the other one is that their sense of urge is from urethra.Proximal urethra is full of nerve, which plays a role in sense and urine control.The sense of urge may come from urethra instead of bladder.Tension-free mid-urethral sling procedure is an effective treatment for women with mixed urinary incontinence.Even without taking the M-blockers, the cure rate for urge incontinence reached 76.92%.The efficacy of surgery remained stable in medium and long term, and the patients'' quality of life improved significantly.

Objective To discuss the relationship between cardio-vocal syndrome and pulmonary arterial hypertension in infants with congenital heart disease(CHD).Methods Based on the electronic bronchoscopy results,257 less than one-year-old infants with congenital heart disease were selected and divided into vocal cord paralysis positive group (31 cases) and negative group(226 cases).Factors including age,sex,body weight,pulmonary arterial hypertension degree and PA/AO value were compared between the two groups.The risk factors of left vocal cord paralysis were analyzed using logistic regression method.Results There were no significant differences in age and weight between left vocal cord paralysis positive group and negative group(P >0.05).While incidence of pulmonary arterial hypertension and PA/AO value of the positive group were significantly higher than those of the negative group (P < 0.01).The incidence of left vocal cord paralysis in no,mild-moderate and severe pulmonary arterial hypertension groups were 1.39%,10.6% and 22.0% (P < 0.05) respectively.Left vocal cord paralysis was positively correlated with the degree of pulmonary arterial hypertension (r =0.179,P < 0.01) and PA/AO value (r =0.169,P < 0.01).Pulmonary arterial hypertension was an independent risk factor of left vocal cord paralysis (P =0.005,OR =1.689).Conclusion Left vocal cord paralysis was significantly correlated with pulmonary arterial hypertension and pulmonary artery dilatation.So pulmonary arterial hypertension should be one of the possible causes of cardio-vocal syndrome in infants with CHD.

Objective:To investigate age related changes in urodynamic parameters of women with uri-nary incontinence.Methods:From May 2008 to October 2015,a total of 214 patients diagnosed with urinary incontinence in Peking University People’s Hospital was involved in this study.Average age was (56.97 ±10.68)years,ranging from 30 to 82 years,and average history was (8.44 ±8.85)years, ranging from one month to 50 years.Urodynamic examinations of each patient were taken before operation routinely in Department of Urology,Peking University People’s Hospital.The urodynamic study was composed of non-invasive and invasive procedures.Analysis included maximal flow rate (Qmax ),average flow rate,time to Qmax ,voiding time,detrusor pressure at Qmax ,maximal detrusor pressure,voided volume,post-void residual urine volume (PVR),the total capacity of bladder,first-,strong-,and urge-desire to void,cough leak point pressure (CLPP),and Valsalva leak point pressure (VLPP).Patients were divided into four groups according to age,Kolmogorov-Smirnov test and one-way ANOVA were used for data analysis.Results:A total of 214 patients were enrolled in this study.The data of Qmax ,average flow rate,voided volume,and total capacity of bladder decreased with statistical significance.The value of residual urine volume and voiding time increased without statistical significance,while the value of maximal detrusor pressure decreased.Conclusion:Urodynamic examination data of females with urinary incontinence changes along with the elapse of age,which was mainly observed as age ascends,and the changes in urodynamic parameters of women with urinary incontinence suggest that the value of Qmax , average flow rate,voided volume,and total capacity of bladder decreased significantly,while the value of PVR and the voiding time increased and the value of maximal detrusor pressure decreased.

Objective:To investigate the impact of preoperative urodynamic study on the diagnosis and treatment for female patients with clinical diagnosis of stress urinary incontinence by studying their diag-nosis and treatment database,and to assess its clinical significance of urodynamic study.Methods:From April 2011 to December 2015,196 female patients diagnosed clinically with stress urinary incontinence underwent preoperative urodynamics study,after excluding pelvic organ prolapse.The preoperative uro-dynamic data of these 196 cases were analyzed and the clinical significance of urodynamics on differential diagnosis and treatment for the female patients with stress urinary incontinence was evaluated.Results:In this study,23 cases (11.73%)changed or amended their diagnoses by the urodynamic study,which were inconsistent with the previous clinical diagnoses concluded by the symptoms,physical examinations, and lab tests.A total of 10 cases underwent a different surgery or conservative treatment instead of the original treatment according to urodynamic study.Of them,3 were diagnosed as detrusor overactive and undertook conservative treatment;3 were diagnosed as bladder outlet obstruction plus stress urinary in-continence and were undertaken the transurethral resection of the bladder neck (TURBN)plus tension free vaginal tape (TVT);2 were diagnosed as bladder outlet obstruction plus stress urinary incontinence and were undertaken TURBN alone;1 was diagnosed as bladder outlet obstruction plus stress urinary in-continence and was undertaken TURBN plus tension free vaginal tape obturator (TOT);1 was found no abnormal bladder function turned out to be interstitial cystitis and went for a bladder instillation of drug. The changed treatments avoided the risk of dysuria or residual urine increased after operation due to inap-propriate surgical methods.Conclusion:In order to make a correct diagnosis and suitable treatment for female patients with stress urinary incontinences,the preoperative urodynamic study is necessary besides detailed medical history,physical examination,and laboratory tests.With the help of the urodynamic study,the concomitant diseases of patients with stress urinary incontinence may be detected,the individualized treatment regimen can be developed,and more importantly,the inappropriate surgical de-cision can be avoided.

Objective Experience with high-frequency oscillatory ventilation (HFOV) after congenital cardiac surgery is limited.The aim of the present study was to investigate the effect and safety of high frequency oscillation ventilation for patients with serious pulmonary hemorrhaege after surgical repair of congenital heart disease (CHD).Methods The patients with serious pulmonary hemorrhaege after surgical repair of CHD using conventional mechanical ventilation or high frequency oscillation ventilation were retrospectively analyzed.From January 2010 to July 2012,there were fourteen patients suffered from serious pulmonary hemorrhaege after surgical repair of congenital heart disease in our hospital and all involved in this study.The mean age was (6.5 ± 5.9) months(ranged from 1 to 24 months) and the mean body weight was (5.8 ± 1.7) kg(ranged from 3.7 to 10 kg).Before May 2011 patients with serious pulmonary hemorrhaege after surgical repair of congenital heart disease were treated with conventional mechanical ventilation(CMV group,n =6),and after May 2011 high frequency oscillation ventilation (HFOV group,n =8) instead.The diagnoses were pulmonary atresia with ventricular septal defect and major aortopulmonary collateral arteries 7 cases,tetralogy of Fallot with imbalance pulmonary arterial development 5 cases,and total anomalous pulmonary venous connection 2 cases.Heart rate,arterial blood pressure,central venous pressure(CVP),inostmpic score,the blood gas analysis,the oxygenation index(OI),the outcomes and complications during both mechanical ventilations were all recorded.Results The PaO2,OI and systolic blood pressure of patients in HFOV group were significant higher than those in CMV group after 2 hours ventilation.There was no statistical difference in PCO2,diastolic blood pressure and inostropic score after 1,2,4,8,24 hours ventilation.All cases had no significant changes in hemodynamics.CVP of patients in HFOV group increased slightly(P <0.05),but there was no statistical difference in arterial blood pressure.Four patients died in CMV group.The mortality of CMV group and HFOV group was 66.7% (4/6) and 37.5% (3/8,one with pulmonary venous obstrution,one with tracheobronchial blocked,the other with re-hemorrhaege) respectively.The mortality of HFOV group was no significantly different from that of CMV group.Conclusion Compared to CMV,using HFOV achieved greater oxygenation function in patients with serious pulmonary hemorrhaege after surgical repair of CHD rapidly,had no significant changes in hemodynamics and rare serious complications.HFOV was effective and safe for patients with serious pulmonary hemorrhaege after surgical repair of congenital heart disease.

RESUMO

The da Vinci robotic surgical system assisted colorectal radical resection has been widely applied in many countries and regions.The advantages of the da Vinci robotic surgical system include stable and distinct stereoscopic vision,short learning curve,convenient operation and feasibility of remote manipulation,while its disadvantages are expensive cost,lack of tremor and prolonged operation time.The advantage of da Vinci robotic surgical system is not distinct when compared with traditional laparoscope in radical resection of colorectal cancer.With the accumulation of clinical data and the update of the system,the prospect of da Vinci robotic surgical system assisted colorectal radical resection is promising.

BACKGROUND: Earthworms are one of the most important constituents in the ecosystem and become an environmental information carrier between terrestrial organisms and soil ecosystem. They were known as important non-target terrestrial soil organisms for assessing the general impact of pollution on the soil community. Protein content and growth rate in response to organic and heavy metal contaminants in soil are often used to assess the soil ecotoxicity.OBJECTIVES: To investigate the effects of 1, 8-dihydroxyanthraquinone,one representative of hydroxyanthraquinones, on the earthworm eisenia foetide under laboratory conditions of the growth rate and protein content.DESIGN: Experimental study based on the experimental animals.SETTING: Eco-toxicological laboratory and genetic laboratory in a university.MATERIALS: The experiment was conducted in the Eco-toxicological Laboratory and the Genetic Laboratory, Dalian University of Technology from February to July 2004. Eisenia foetide, a kind of international standard earthworm, were maintained in a soil mixture before experiments. Earthworms used in this study were healthy adults with 1 -2 month old and 200 - 300 mg mass and the number of the earthworms in each analytical experiment was 10.METHODS: The earthworms were removed from the soil 12 hours before use and stored in Petri dishes on damp filter paper to void gut contents.Then, these earthworms were exposed for various durations to soils contaminated. The earthworm weight and protein content were determined respectively at every 7 days interval.MAIN OUTCOME MEASURES: Effects of 1, 8-dihydroxyanthraquinone on growth rate and protein content of earthworms.RESULTS: No lethal effect of 1, 8-dihydroxyanthraquinone was observed even at the highest concentration(1.0 g/kg soil) of exposure. The sub-lethal effect, however, was evident at all the concentration scale. 1,8-dihydroxyanthraquinone caused a significant reduction in the growth rate (maximum -22. 5% ) at the dosage of 1.0 g/kg and 28 days contact time.Additionally a reduction in total soluble protein was observed in all treated worms(maximum- 39.6% ) at the dosage of 0. 8 g/kg and 7 days contact time.CONCLUSION: 1, 8-dihydroxyanthraquinone was potentially dangerous to the soil ecosystem and more ecological risk assessment of this chemical material should be thoroughly carried out.
