RESUMO

Metabolic reprogramming refers to the phenomenon that tumor cells, in order to meet their own growth and energy needs, regulate their biological functions by changing their metabolic mode, help themselves resist external stresses, and thus enable cells to adapt to hypoxia, acid, nutrient deficiency and other microenvironments and rapidly proliferate. It was found that metabolic reprogramming could contribute to radiation resistance and it also could be induced in bystander cells which may result in radiation resistance and the cancellation. Investigation the mechanism of radiation-induced metabolic reprogramming may provide new ideas and a theoretical framework for radiation protection, radiotherapy, and radio-diagnosis. This article reviewed the research progress on the mechanism of metabolic reprogramming in the direct and bystander effects of radiation.

RESUMO

Objective@#To investigate the nursing effect of clinical treatment combined with early rehabilitation nursing intervention in patients with heart failure.@*Methods@#A total of 60 cases of patients with heart failure from January 2018 to May 2018 in Beijing Anzhen Hosipital, Capital Medical University were admitted. They were divided into control group (30 cases) and observation group (30 cases) by random digits table method. All patients in 2 groups were treated with clinical treatment, while those in control group were treated with conventional nursing and observation group combined with early rehabilitation nursing intervention. Echocardiogram, 6 min walking test (6MWT), body composition analyzer (measuring extracellular moisture ratio) and body mass index (BMI) was used to evaluate the changes of all indexes before and after nursing care in 2 groups.@*Results@#Using the receiver operating characteristic curve(ROC) of extracellular moisture ratio to predict heart failure, the area under the curve (AUC) of patients with heart failure was predicted to be 0.737(P<0.01). Analysis of the correlation between the decrease of extracellular moisture ratio after treatment and the increase of walking distance after 6 minutes of treatment showed a positive correlation. With the decrease of extracellular water ratio after treatment, the 6MWT distance increased continuously after treatment (r=0.164, P=0.032). In the observation group, the 6 minute walking distance, extracellular water ratio, the added value of BMI, added value of 6 minute walking distance after treatment, after treatment of extracellular water reducing ratio, and BMI lower values were (438.76±89.16) m, (0.380±0.021)%, (24.74±3.13) kg/m2, (129.20±18.62) m, 0.010±0.006, (2.71±0.89) kg/m2, respectively. The control group were (356.40±85.28) m, (0.384±0.014)%, (25.93±2.68) kg/m2, (60.25±8.43) m, 0.009±0.008, (1.37±0.85) kg/m2. The differences between the two groups were statistically significant (t=1.726-15.739, P<0.05).@*Conclusions@#The extracellular moisture ratio indicates the degree of heart failure. Clinical treatment combined with early rehabilitation nursing intervention is superior to conventional nursing in patients with heart failure, which can significantly increase the patient′s 6MWT and reduce the patient′s BMI, which is worth popularizing and applying.

RESUMO

Objective To investigate the relationship between body mass index (BMI) and Gensini score,to assess the predictive value of myocardial infarction. Methods Totally 348 patients with myocardial infarction and 1054 patients with non- myocardial infarction from January 2014 to August 2015.To analyze the difference of BMI and Gensini scores between the two groups. To analyze the relationship between BMI and Gensini scores in each group, and the value of BMI used to predict the risk of myocardial infarction. Results The BMI and Gensini scores of the myocardial infarction group were significantly higher than those of the non- myocardial infarction group. There was a positive correlation between BMI and Gensini score in the MI group (r=0.304, P=0.000). The BMI of the non-myocardial infarction group was positively correlated with Gensini's score (r=0.253, P=0.000). AUC was 0.647, P=0.000, 95% CI (0.617, 0.678), the risk of myocardial infarction was measured by ROC analysis. Conclusions The level of BMI in patients with coronary heart disease is associated with Gensini scores. BMI can be used as an effective indicator of myocardial infarction in the patients of coronary heart disease

RESUMO

PURPOSE: To produce competent undergraduate-level medical doctors for rural township health centers (THCs), the Chinese government mandated that medical colleges in Central and Western China recruit rural-oriented, tuition-waived medical students (RTMSs) starting in 2010. This study aimed to identify and assess factors that influence the choice to work in rural township health centers among both RTMSs and other students from five medical universities in Guangxi, China. METHODS: An internet-based self-administered questionnaire survey was conducted with medical students in Guangxi province. Multinomial logistic regression was used to identify factors related to the attitudes toward work in a rural township health center. RESULTS: Among 4,669 medical students, 1,523 (33%) had a positive attitude and 2,574 (55%) had a neutral attitude toward working in THCs. Demographic characteristics, personal job concerns, and knowledge of THCs were associated with the choice of a career in THCs. The factors related to a positive attitude included the following: three-year program, a rural-oriented medical program, being male, an expectation of working in a county or township, a focus on medical career development, some perceived difficulty of getting a job, having family support, sufficient knowledge of THCs, optimism toward THC development, seeking lower working pressure, and a lower expected monthly salary. CONCLUSION: Male students in a three-year program or a rural-oriented tuition-waived medical education program were more likely to work in THCs. Selecting medical students through interviews to identify their family support and intentions to work in THCs would increase recruitment and retention. Establishing favorable policies and financial incentives to improve living conditions and the social status of rural physicians is necessary.
