RESUMO

Medical physicists play an important role in the delivery of radiotherapy. Compared with China′s mainland, Hong Kong has established a more mature training mode and a more complete management system for medical physics talents. In this article, the authors introduced the current state of medical physics talent training, as well as the recruitment, certification and promotion of medical physicist in Hong Kong by querying the official websites of medical physics organizations, reviewing related literature and interviewing senior medical physicists in Hong Kong. The authors also analyzed the shortcomings in the construction of medical physics talent system in China′s mainland and made valuable suggestions.

ObjectiveTo observe the regulatory effect of Yuyetang on Ghrelin level in rats with type 2 diabetes mellitus (T2DM)-induced cognitive impairment (DCI) and explore the pathway in the prevention and treatment of DCI. MethodThe T2DM model was induced in rats by intraperitoneal injection of streptozotocin (STZ) combined with the high-fat and high-sugar diet (STZ). The model rats were divided into model group, metformin group (200 mg·kg-1), and low-,medium-, and high-dose Yuyetang groups(4.575,9.15, 18.3 g·kg-1)according to the blood glucose, with 10 rats in each group. A normal group was also set up. The rats were administered with corresponding drugs by gavage for 30 days, and the body weight and blood glucose of the rats in each group were observed and recorded. After drug intervention, the learning and memory abilities of rats were tested by the Morris water maze. After the test, the whole brains of rats were sampled for hematoxylin-eosin (HE) staining to observe the pathological changes in the hippocampal CA1 region, and the expression of Ghrelin in gastric tissues and hippocampal CA1 region was detected by immunohistochemistry. ResultCompared with the normal group , the model group showed increased blood glucose(P<0.01),reduced body weight(P<0.01),prolonged escape latency(P<0.05,P<0.01), shortened retention time and movement distance in the target area,decreased number of platform crossings(P<0.01), abnormal morphology and structure of cells with disordered arrangement and reduced number in the hippocampal CA1 region, and decreased expression of Ghrelin in the serum,hippocampal CA1 region, and gastric tissues(P<0.05, P<0.01). Compared with the model group, the medium- and high-dose Yuyetang groups showed increased body weight, while all Yuyetang groups showed reduced blood glucose(P<0.01), shortened escape latency (P<0.05), prolonged retention time and movement distance in the target area,increased platform crossings (P<0.05, P<0.01), improved morphology and structure of cells, increased number of normal cell in the hippocampal CA1 region, and elevated Ghrelin levels in the serum, gastric tissues, and hippocampal CA1 region(P<0.05, P<0.01). ConclusionYuyetang can effectively improve the cognitive ability of DCI rats, and its mechanism may be related to the regulation of Ghrelin levels in the serum, hippocampal CA1 region, and gastric tissues.

Objective:To compare the positioning errors of individual head-rest combined with thermoplastic fixation mask versus thermoplastic fixation mask alone in patients with head and neck tumors. Methods:Twenty-eight patients who received irradiation with helical tomotherapy in Cancer Hospital of the University of Chinese Academy of Sciences (Zhejiang Cancer Hospital) between October 2019 and April 2020 were included in this study. They were randomly assigned to receive position fixation with either individual head-rest combined with thermoplastic fixation mask (N1 group, n = 14) or thermoplastic fixation mask alone (N2 group, n = 14). Megavoltage computed tomography (MVCT) scanning registration was used to obtain the positioning errors in translation and rotation (ROLL) in the left-right (X), head-food (Y), and belly-back (Z) directions. There were a total of 841 CT scans, consisting of 425 scans in group N1 and 416 scans in group N2. Results:The positioning errors in X, Y, Z and ROLL directions in the N1 group were (1.37 ± 1.04) mm, (1.38 ± 1.12) mm, (1.47 ± 1.62) mm and (1.47 ± 1.62) ° respectively, and they were (1.57 ± 1.21) mm, (2.10 ± 1.51) mm, (1.61 ± 1.50) mm and (1.40 ± 1.30) ° respectively in the N2 group. There was significant difference in positioning errors in the Y direction between N1 and N2 groups ( P = 0.013). In the N1 group, the outward expansion boundaries in X, Y and Z directions was 4.15 mm, 4.23 mm and 4.81 mm respectively, and it was 4.77, 6.31 and 5.08 mm, respectively in the N2 group. In the X direction, there was significant difference in positioning errors taking 3 mm as the dividing point between N1 and N2 groups ( χ2 = 10.516, P < 0.001). In the Y direction, there was significant difference in positioning errors taking 1, 2 and 3 mm as the dividing points between N1 and N2 groups ( χ2 = 24.889, P < 0.001; χ2 = 42.202, P < 0.001; χ2 = 46.204, P < 0.001). In the Z direction, there was significant difference in positioning errors taking 2 mm as the dividing point between N1 and N2 groups ( χ2 = 7.335, P = 0.007). In the N1 group, the percentage of positioning errors < 3 mm in the X, Y and Z directions was 92%, 90% and 92%, respectively. Conclusion:Compared with thermoplastic fixation mask alone, individual head-rest combined with thermoplastic fixation mask can better effectively improve the positioning stability and reduce positioning errors in patients receiving irradiation with helical tomotherapy for head and neck tumors. The combined method is of certain innovation.

The training program of radiation therapists in the United States has been established early, and the mode of training, qualification and continuing education are relatively complete. Literature review was conducted at home and abroad and United States Department of Labor, American Registry of Radiologic Technologists, American Society of Radiologic Technologists as well as Joint Review Committee on Education in Radiologic Technology websites were reviewed. The training mode, qualification, work content, continuing education and employment situation of American radiotherapists were analyzed, aiming to provide some reference and enlightenment for the establishment of a new model for the training of professional radiologists suitable for the national conditions of China Mainland.

Objective:To introduce the workflow of individualized 3D-printed intracavitary/interstitial vaginal template design. Dosimetric parameters and operation safety were investigated to evaluate the performance of 3D-printed template and freehand implantation.Methods:Forty patients previously treated with intracavitary/interstitial Ir-192 HDR brachytherapy were enrolled in this study. All patients were randomly divided into the treatment ( n=20) and control groups ( n=20). In the treatment group, twenty patients were treated with individualized 3D-printed template. CT-based preplan was carried out to determine the needle implantation cannels. Template with customized shape and implantation cannels was then produced by a 3D printer. Finally, the template was inserted under CT guidance. In the control group, twenty patients received freehand implantation. Needle insertion was decided empirically without the preplan process. Results:The difference of D 90 for high risk CTV was found to be minor, while the D 2cm 3 in the rectum, bladder and sigmoid was significantly improved in the treatment group. Meanwhile, the high dose region and conformal index were also improved in the treatment group. A total of 273 needles were inserted and one (0.3%) not-used needle was found. No normal tissues were penetrated during needle insertion in the treatment group. In the control group, a total of 203 needles were inserted and 4(2.0%) not-used needles were observed, and normal tissue penetration occurred in 3(1.5%) needle insertion. Conclusions:The individualized 3D-printed template implantation approach has advantages in terms of dosimetry and safety compared with freehand implantation. The actual treatment can achieve the dosimetric design requirements of the preplan.

Objective To evaluate the optimum compatibility of nabufine mixed with flurbiprofen for patient-controlled intravenous analgesia (PCIA) after gynecological laparoscopic surgery.Methods A total of 210 patients,aged 18-64 yr,with body mass index of 18-30 kg/m2,of American Society of Anesthesiologist physical status Ⅰ or Ⅱ,scheduled for gynecological laparoscopic surgery under general anesthesia,were divided into 4 groups using a random number table method:sufentanil 2.0 μg/kg+flurbiprofen axetil 2.0 mg/kg group (SF group,n =55),nalbuphine 1.5 mg/kg+flurbiprofen axetil 2.0 mg/kg group (N1 F group,n=49),nalbuphine 2.0 mg/kg+flurbiprofen axetil 2.0 mg/kg group (N2F group,n =55) and nalbuphine 3.0 mg/kg +flurbiprofen axetil 2.0 mg/kg group (N3F group,n=51).PCIA solution was prepared correspondingly after surgery in each group.The PCA pump was set up to deliver a 1 ml bolus dose with a 15-min lockout interval and background infusion at 2.0 ml/h.Nalbuphine 5 mg or sufentanil 5 μg was intravenously injected as a rescue analgesic to maintain visual analogue scale score at rest <4 at 48 h after surgery in SF and N1 F-N3F groups.Ramsay sedation scores were recorded on admission to post-anesthesia care unit (T1),at the time of post-anesthesia care unit discharge (T2) and at 6,24 and 48 h after surgery (T3-5).The total pressing times of PCIA in 0-6 h,6-24 h and 24-48 h periods after surgery and requirement for rescue analgesics were recorded.The incidence of adverse reactions such as nausea and vomiting,drowsiness and shivering within 48 h after surgery was also recorded.Results Compared with group SF,the incidence of nausea and vomiting was significantly decreased in N1 F and N2F groups,the requirement for rescue analgesics was significantly decreased,and the total pressing times of PCIA was reduced in N2F and N3 F groups,and Ramsay sedation scores at T3,4 were significantly increased in group N3F (P<0.05).Compared with group N1 F,the requirement for rescue analgesics was significantly decreased,and the total pressing times of PCIA was reduced in N2F and N3F groups,and the incidence of nausea and vomiting and Ramsay sedation scores at T3,4 were significantly increased in group N3F (P<0.05).Compared with group N2F,the incidence of nausea and vomiting was significantly increased (P< 0.05),and no significant change was found in the requirement for rescue analgesics,total pressing times of PCIA or Ramsay sedation scores in group N3F (P>0.05).Conclusion Nabufine 2.0 mg/kg mixed with flurbiprofen 2.0 mg/kg is the optimum compatibility when used for PCIA after gynecological laparoscopic surgery.

Aging associated cognitive decline has been linked to dampened neural stem/progenitor cells (NSC/NPCs) activities manifested by decreased proliferation, reduced propensity to produce neurons, and increased differentiation into astrocytes. While gene transcription changes objectively reveal molecular alterations of cells undergoing various biological processes, the search for molecular mechanisms underlying aging of NSC/NPCs has been confronted by the enormous heterogeneity in cellular compositions of the brain and the complex cellular microenvironment where NSC/NPCs reside. Moreover, brain NSC/NPCs themselves are not a homogenous population, making it even more difficult to uncover NSC/NPC sub-type specific aging mechanisms. Here, using both population-based and single cell transcriptome analyses of young and aged mouse forebrain ependymal and subependymal regions and comprehensive "big-data" processing, we report that NSC/NPCs reside in a rather inflammatory environment in aged brain, which likely contributes to the differentiation bias towards astrocytes versus neurons. Moreover, single cell transcriptome analyses revealed that different aged NSC/NPC subpopulations, while all have reduced cell proliferation, use different gene transcription programs to regulate age-dependent decline in cell cycle. Interestingly, changes in cell proliferation capacity are not influenced by inflammatory cytokines, but likely result from cell intrinsic mechanisms. The Erk/Mapk pathway appears to be critically involved in regulating age-dependent changes in the capacity for NSC/NPCs to undergo clonal expansion. Together this study is the first example of using population and single cell based transcriptome analyses to unveil the molecular interplay between different NSC/NPCs and their microenvironment in the context of the aging brain.

The blood-brain barrier (BBB) is a tight boundary formed between endothelial cells and astrocytes, which separates and protects brain from most pathogens as well as neural toxins in circulation. However, detailed molecular players involved in formation of BBB are not completely known. Dentin matrix protein 1 (DMP1)-proteoglycan (PG), which is known to be involved in mineralization of bones and dentin, is also expressed in soft tissues including brain with unknown functions. In the present study, we reported that DMP1-PG was expressed in brain astrocytes and enriched in BBB units. The only glycosylation site of DMP1 is serine89 (S89) in the N-terminal domain of the protein in mouse. Mutant mice with DMP1 point mutations changing S89 to glycine (S89G), which completely eradicated glycosylation of the protein, demonstrated severe BBB disruption. Another breed of DMP1 mutant mice, which lacked the C-terminal domain of DMP1, manifested normal BBB function. The polarity of S89G-DMP1 astrocytes was disrupted and cell-cell adhesion was loosened. Through a battery of analyses, we found that DMP1 glycosylation was critically required for astrocyte maturation both in vitro and in vivo. S89G-DMP1 mutant astrocytes failed to express aquaporin 4 and had reduced laminin and ZO1 expression, which resulted in disruption of BBB. Interestingly, overexpression of wild-type DMP1-PG in mouse brain driven by the nestin promoter elevated laminin and ZO1 expression beyond wild type levels and could effectively resisted intravenous mannitol-induced BBB reversible opening. Taken together, our study not only revealed a novel element, i.e., DMP1-PG, that regulated BBB formation, but also assigned a new function to DMP1-PG.

Objective To investigate the effects of Schisandrae Sphenantherae Fructus and processed with vinegar on lipid metabolism of rats with type 2 diabetes mellitus (T2DM). Methods The rat model of T2DM was induced by high fat diet plus STZ. The rats were randomly divided into blank group, model group, Schisandrae Sphenantherae Fructus high-dose group and low-dose group, and vinegar Schisandrae Sphenantherae Fructus high-dose group and low- dose group. The rats in each group were fed with the corresponding medicine for gavage for 30 d. FINS, FFA, TC, TG, HDL-C, LDL-C, and MDH, total protein content of liver tissue were detected. HE staining was used to observe the histomorphological changes of liver and pancreas in rats. Results Compared with the model group, Schisandrae Sphenantherae Fructus groups and vinegar Schisandrae Sphenantherae Fructus groups did not show obvious effects on improving FBG and FINS, but it could raise varying degrees of HDL-C and MDH, and reduce FFA, LDL-C, TC, and TG, among which vinegar Schisandrae Sphenantherae Fructus could significantly regulate metabolism in T2DM rats. Conclusion Vinegar Schisandrae Sphenantherae Fructus can enhance the lipid metabolism regulatory function of T2DM rats.

Objective: To determine the antibody titer in the serum of allergic rabbits after the injection of 1, 3-di-caffeoylquinic acid contained in Shuanghuanglian injection.Methods: The complete antigen was prepared by incubating the suspected small molecular hapten 1, 3-di-caffeoylquinic acid contained in Shuanghuanglian injection with the serum from the normal rabbits.The specific antibody was obtained in the immunized rabbits.The antibody titers of antiserum were measured by ELISA kits.Indirect competitive ELISA was used to determine serological specificity, and the obtained data was used to plot the inhibition curves.The content of IgE antibody in the antiserum of rabbits was detected by rabbit immunoglobulin E (IgE) ELISA kits.Results: The antibody titer (A) of 1,3-di-caffeoylquinic acid was 2 times higher than that of the negative control, which indicated its potential allergenicity.The regression equation was I=0.170 6 lg C + 0.317 5 , which was with the correlation coefficient of r=0.985 4 , the detection limit of IC 10 =57.40 μg·ml-1 and the half inhibitory concentration of IC 50 =8.732 0 mg·ml-1.Furthermore, the exogenous IgE antibody was produced in the rabbits.Conclusion: The results indicated that the hapten substance 1,3-di-caffeoylquinic acid in Shuanghuanglian injection was allergenic under the present experimental conditions.

Objective To compare the efficacy of three solutions used in the patients who suffered chronic obstructive pulmonary diseases (COPD ) when combined with oral fungal infection,and to find the most suitable one. Methods Using randomized controlled trial method, 98 patients who suffered COPD complicated with fungal infection were randomly divided into three groups. Method 1, using 0.5% povidone for the oral care. Method 2, making 200 million U of nystatin powder dissolved in 2.5% NaHCO3 for the oral care.Method 3, gave oral care with 2.5% NaHCO3 first, then put 200 million U of nystatin powder into 20 ml glycerol and covered the oral spots with them. Three groups were observed in the effect of oral fungal infection and the feedback from patients and nurses. Results In the analysis of the white spots sharply disappeared or pharyngeal swab smear turned negative, method 1 and method 3 had no statistical difference, P>0.05, but method 2 and method 3 had statistical significance, t=3.892, P 0.05. The time made solution of method 1 occupied 41.7% of method 3, and the time made solution of method 2 occupied 54.4%of method 3, while there was no statistical significance in total time of three groups (P>0.05). Conclusions The oral care with 0.5% povidone had good effect on the patients who suffered COPD complicated with oral fungal infection,with good comfort,and most nurses saving time. Nystatin powder dissolved in 2.5% NaHCO3 worked later than nystatin powder dissolved in glycerol.

Objective To evaluate the effects of laparoscopic filling with pedicled greater retina for hepatic cysts. Methods Clinical data were reviewed on 36 patients with hepatic cysts undergoing laparoscopic filling with pedicled greater retina from January 2010 to May 2014.The diameters of cysts ranged from 10 to 26 cm, with a diameter smaller than 20 cm in 29 patients and bigger than 20 cm in 7 patients.Laparoscopic fenestration of hepatic cysts was performed to destroy cyst cells with iodine and absolute alcohol. Pedicled greater retina was used for filling the cavity of cysts. Results The procedures were completed smoothly.The operative time was 45-115 min, with an average of (80.4 ±13.1) min.No abdominal cavity infection happened.Primary healing of abdominal incision was achieved in all the patients.The postoperative hospital stay was 5-10 d, with an average of (6.4 ±1.2) d.All the 36 patients were followed up for 6-12 months with a mean of (9.3 ±1.5) months.None of the patients showed recurrence under liver B-ultrasonography or CT scanning. Conclusion Laparoscopic filling with pedicled greater retina for hepatic cysts has advantages of minimal invasion, quick postoperative recovery, short hospital stay, and good results.

OBJECTIVE@#To evaluate the efficacy of sublingual immunotherapy (SLIT) with Dermatophagoides farinae drops for allergic rhinitis (AR) of different symptom severity.@*METHOD@#This retrospective analysis to receive SLIT treatment of 143 cases of patients with allergic rhinitis, according to the severity of disease symptoms divid- ed into two groups, moderate group (62 patients) and severe group (81 patients). Before SLIT and after SLIT for half year, 1 year and 1. 5-2.0 years, the TNSS, TMS and sign scores of patients with allergic rhinitis were evaluated.@*RESULT@#The TNSS, TMS and sign scores had continuously improved significantly after SLIT for half year, 1 year and 1.5-2.0 years in two groups as compared with baseline (P 0.05). Half year after SLIT treatment, in two groups for sign scores, there were significant differences (Z = 3.32, P 0.05) and TMS (Z = 0.37, P > 0.05). 1 and 1.5-2.0 years after SLIT, there were no significant differences in two groups for TNSS, TMS and sign scores (P > 0.05).@*CONCLUSION@#SLIT with Dermatophagoides farinae drops for 1.5-2.0 years is effective in the patients with allergic rhinitis of different symptom severity. And equivalent efficacy could be achieved for different symptom severity.

OBJECTIVE@#To evaluate the efficacy of personal sublingual immunotherapy (SLIT) with dermatophagoides to study the efficacy of dermatophagoides farinae drops for allergic rhinitis (AR) of different age groups.@*METHOD@#The current study had analyzed the efficacy of SLIT in 150 patients with AR who were sensitized to house dust mites. All patients were treated with dermatophagoides farinae drops and combined with symptomatic treatment. The patients were divided into groups 1-5, group 1:17 patients (4-7 years old), group 2: 38 patients (> 7-12 years old), group 3:31 patients (> 12-18 years old), group 4: 38 patients (> 18 - 40 years old), group 5: 26 patients (> 40-63 years old). The total nasal symptom scores (TNSS) and total medicine scores (TMS) were recorded at each visit. Before and after SLIT for 0.5 year, 1 year and 1.5 to 2.0 years, the TNSS and TMS of each patient were evaluated. The dosage adjustment of immunotherapy according to the patient's symptoms were performed.@*RESULT@#The TNSS and TMS had continuously improved significantly after SLIT for half year, 1 year and 1.5 to 2.0 years in all groups as compared with baseline (P < 0.05). There were no significant differences in the different age groups for TNSS and TMS during all time points.@*CONCLUSION@#Individualized SLIT with dermatophagoides farinae drops for 1.5-2.0 years is the most effective in the patients with allergic rhinitis of different age groups. And equivalent efficacy could be achieved for different age groups.

<p><b>OBJECTIVE</b>For studying the pharmacokinetic of Yanshu injections in Beagel dogs, a sensitive and reproducible LC-MS method for quantitative determination of matrine, oxymatrine, sophocarpine and oxysophocarpine in dog's plasma were developed and validated using monocrotaline as an internal standard after iv of Yanshu injections (Sophorae Flavescentis Radix and Heterosmilacis Japonicae Rhizoma).</p><p><b>METHOD</b>The separation of plasma samples was performed on a CN column by isocratic elution with methanol-10 mmol x L(-1) NH4Ac-0.02% HCOOH-H2O 90:10 as the mobile phase. The plasma concentration of four kinds of alkaloids were calculated in dog plasta by detection of healthy dogs given Yanshu injection fluid after in twelve hours of plasma samples, All data of concentration-time of four kinds of alkaloids were treated with pharmacokinetics program DAS 2. 0.</p><p><b>RESULT</b>MT, OMT, SP and OSP have a good linear relationship in 0.01-16.0, 0.02-60.0, 0.01-4.0, 0.02-16.0 mg x L(-1), respectively. The average recoveries were more than 90% and the RSD of precision and stability of the test were less than 6.4% iv 1.2 g x kg(-1) Yanshu injection, four kinds of alkaloids in rats meet the two-compartment open pharmacokinetic model, Cmax and the concentration of the original liquid in the proportion of the basic line, the AUC(0-infinity) of matrine and oxymatrine, sophocarpine and oxysophocarpine compared to the original both in the proportion of liquid increases, the MRT(0-infinity) and t(1/2z) of matrine and sophocarpine were less than oxymatrine and oxysophocarpine; four kinds of alkaloids apparent volume of distribution matrine > oxymatrine, sophocarpine > oxysophocarpine.</p><p><b>CONCLUSION</b>A method with high recovery rate and good stabilitywas established to determine the blood concentration of MT, OMT, SP, OSP in Yanshu injection and applied in its pharmacokinetics successfully.</p>

RESUMO

Objective: To establish the quality standards for Lingyangqingfei capsules. Methods: Microscopic identification of products, TLC identifications of Fructus Gardeniae and Cortex Moutan were carried out.Baicalin was determined by HPLC. Results: Baicalin in Lingyangqingfei capsules was about 1.5mg/capsule. The average recovery was 99.82% and RSD was 0.96%. Conclusion: The result is accurate and the reproducibility is good. This method can be used as the quality control of this preparation.

RESUMO

Objective To study the effects of sibutramine on the human-ether-a-go-go-related gene (hERG) current in HEK293 cell line that overexpresses hERG potassium channels. Methods HEK293 cell line that overexpresses hERG potassium channels was constructed. Whole cell patch clamp techniques were employed to investigate the effects of sibutratime on Iherg in the cell line. Results Sibutramine reduced hERG current in a concentration-dependent manner:the inhibition rate was (8.82?2.32)%,(26.7?5.78)%,(44.93?4.60)% and (85.22?3.37)% at the dose of 3,10,30 and 100 ?mol/L. IC50 value was 32.66 ?mol/L (n=8). Ⅰ-Ⅴ relationship was inhibited by sibutramine (3,10,30 and 100 ?mol/L) at membrane potentials from -10 mV to +60 mV (step) and from 0 mV to +60 mV (tail) (n=8). Conclusion Sibutramine significantly inhibited Iherg in HEK293 cell line that overexpresses hERG potassium channels in a concentration-dependent manner,which may be a major mechanism of causing QT interval prolongation.

The explosive terror attack is the most frequent in all terror activities in the world. The authors reviewed its current situation and the challenges we are confronted with in China and elsewhere. In the present paper, the types, characteristics and emergency medical support (particularly classifying and emergency treatment, triage, transportation and evacuation, seeking and unearthing for buried victims) of explosive injuries in explosive attack site were systematically expounded.
