RESUMO

Safety and effect of intraocular lens implantation in pediatric cataract patients were studied. And properness of intraocular lens along postoperative refractive changes was evaluated. In the department of ophthalmology, Yeung-Nam University college of medicine, twenty-eight patients were performed lens aspiration with intraocular lens implantation for pediatric cataract from May 1992 to October 1996. Preoperative and postoperative visual acuity, refractive power and postoperative complication along operative methods were evaluated, retrospectively. After follow up period of 6-30 months(mean 17.7 months), visual improvement more than 2 lines was noted 28 eyes(77.8%) by Hahns visual acuity chart. The trends of myopic shift had no significance in statistically, it is proper that we selected intraocular lens power adjust to emmetropia in over 3 years old patients. The difference between preoperative and postoperative refractive power measured within 0 and -2 Diopters in 30 eyes(83%). Aimed intraocular lens power was calculated by using SRK/T fomula for emmetropia and adjustment for refractive error of fellow eye. Best corrected vision was obtained after medical treatment of amblyopia.

Among the postoperative complications of ptosis surgery, lagophthalmos and lid lag have been significant and untolerable problems. In this study, progressive change of lagophthalmos and lid lag following external levator resection was examined in 109 eyes of 74 patients from November 1988 through March 1996. Lagophthalmos and lid lag were measured at postoperative 1 month, 3 to 9 months and 12 to 18 months. The relations of lagophthalmos and lid lag according to preoperative MRD, levator function, resection amount of the levator muscle were analyzed. Lagophthalmos measured average 1.11mm and lid lag revealed average 4. 73mm at postoperative 11 to 84 months (mean 22.2 months). At the postoperative 1 year, lagophthalmos and lid lag were significantly decreased and decreasing amount measured 1.17mm and 0.90mm, respectively. Seventy eyes showed lagophthalmos less than 1mm at mean 13 months postoperatively. The postoperative change of lagophthalmos and lid lag had relation to the preoperative levator function.

RESUMO

Reis-Bucklers' dystrophy is an autosomal dominant disorder involving Bowman's membrane. It usually appears as a recurrent corneal erosion in early childhood and is characterized by an irregular, semi-opaque accumulation of fibrous connective tissue in the area of Bowman's membrane and symmetrical progressive opacity of superficial central cornea. Eventually, it causes visual disturbance due to re-ticulated gray-white corneal opacity and corneal surface irregularity. The ophthalmic surgeons have met difficulties in the management of Reis-Bucklers' corneal dystrophy because of poor long-term results following conventional penetrating and lamellar keratoplasties. We experienced a case of Reis-Bucklers' dystrophy in 25 years old man which was confirmed by clinical and pathological evidences and successfully treated by excimer laser phototherapeutic keratectomy(PTK) with VISX 20/20 B(R). In conclusion, we would suggest that PTK could be another modality of management in this anterior corneal disease when surgical intervention is required.
