RESUMO

Adrenocortical adenomas are relatively rare tumor in retroperitoneum, and most cases are nonfunctioning tumors. Recently, we experienced two cases of functioning Adrenocortical adenomas giving rise to adrenogenital syndrome and Cushing's syndrome.

RESUMO

Retroperitoneal teratoma is a rare congenital neoplasm derived from three germ layers and generally recognized as a disease of childhood. Those who were not operated on did not fare any better. The mortality from the disease was virtually 100%. Recently, we experienced a case of retroperitoneal teratoma.

RESUMO

Augmentation cystoplasty is effective in the rehabilitation of the contracted bladder of small capacity caused by tuberculosis or interstitial cystitis refractory to more conservative methods of management. Genitourinary tuberculosis is the most common cause of contracted bladder in Korea and it may lead to considerable vesicoureteral reflux and progressive deterioration of renal function. The use of hemi-Kock ileal pouch with an afferent intussuscepted nipple is a suitable solution in patient in whom the bladder outflow tract provides adequate resistance and supratrigonal cystectomy is to be performed with construction of an antireflux mechanism. Herein, we report 4 cases of hemi-Kock ileocystoplasty.

RESUMO

We report a case of vesical endometriosis in a 34 year old female patient and review literature. Managements include partial cystectomy, hormonal treatment and radiation. In this case, we select the hormonal treatrnent because the vesical endometriosis was in the fixed portion of the bladder.

RESUMO

Cystic lesions of the kidney were experimentally induced by a single injection of methylprednisolone acetate(MPA), 20mg/kg, and morphometric analysis and ultrastructural observations by means of trans. mission and scanning electron microscopes were carried out after 1,2,3 and 4 weeks to clarify the nature of the cysts and their Histogenesis. l. The cystic lesions developed by administration of MPA were categorized into tubular and glomerular cysts ; the former underwent gradual regression of its number after the first week together with reduction of its ratio to the renal volume, whereas the latter progressed. 2. The subcapsular cystic structures(average diameter of ll0um) was originated from the dilated collecting tubules by 180 degree rotation in regard with abnormality in maturation process of nephorgenic activity. 3. Abnormalities of both proximal and distal convoluted tubules were reflected by increases of tubular diameter(l.54~3.55 times) and nuclear density(1.77~l.72 times), representing a compensatory dilatation of tubular lumen in association with shortening of tubular length. 4.Convolution of tubules was reduced and parallel to the shortening of tubular length. The above morphometric and ultrastructural features support that cystic lesion in the subcapsular portion induced by administration of MPA arises from the subcapsular collecting tubules by maturation arrest of nephorgenic activity.

RESUMO

We present a case with bilateral synchronous seminoma in abdominal cryptorchid testes in a 35 year-old male patient and a review of the literature. Diagnosis was made by abdominal C.T. scan and exploration. The tumor stage was IIa. Radiation therapy and re-exploration of abdomen were done. In six months of follow-up, no evidence of recurrence was recognized.

RESUMO

A clinical observation was made on 54 hydronephrotic kidneys due to UPJ obstruction during 4 years from January 1979 to April 1983. Operative procedures consisted of 18 primary nephrectomies, 28 dismembered pyeloplasties, 4 Foley Y-V pyeloplasties, and 4 lyses for fibrous band. The diuretic renal scan was very helpful in evaluating an equivocal UPJ obstruction as well as in predicting prognosis after the relief of obstruction. In pyeloplasties, the nephrostomy tube and ureteral stent were applied in the case of infants, solitary kidney, reopration and the presence of infection. The early success rate of dismembered pyeloplasties were 100% in the groups using nephrostomy tube and ureteral stent together and using neither of them, 78% in the group of using nephrostomy tube alone. Delayed opening which occurred in 9 cases out of 32 pyeloplasties (28%) had no relation with the use of nephrostomy tube or ureteral stent. The secondary operations for failed primary pyeloplasties were performed in 3 cases, all of which had urinary tract infection, suggesting the importance of the presence of preoperative UTI. Follow-up varied from 6 months to 4 years, and the appearance of postoperative excretory urogram was disappointing when compared to the clinical result's in regard to symptomatology and renal function. The late result showed success rate of 90% in dismembered pyeloplasty, 67% in non-dismembered pyeloplasty and 33% in band lysis.

RESUMO

Records of 15 patients of primary ureteral tumor which were managed in Seoul National University Hospital during 13.5 years period from January 1971 to June 1983. 1. Primary ureteral tumor comprised of 2% of all urothelial tumors and the ratio of bladder:pelvis:ureter was 40:2:1. 2. There were 11men and 4 women (ratio of 3:1) with a mean age of 62.7 years. 3. The most frequent site of ureteral tumor was the lower one third of the ureter (53%). 4. Most frequent presentation was gross hematuria. usually intermittent and painless (100%). 5. Non-visualization of the kidney was the most prevalent finding (73%) on excretory urogram. Retrograde pyelogram was necessary in 11 patients whose findings of IVP was insufficient for diagnosis, and these procedures provided definite diagnosis in 9 of 11 patients. 6. Urine cytology was positive in 4 of 13 patients. 7. Cystoscopy was performed in all patients and which revealed concomitant bladder tumor in3 patients and tumor in ureteral orifice in 2 patients. 8. There was close correlation between the grade of cellular anaplasia and the degree of invasion. 9. There were 3 recurrences (50%) in the ureteral stump in 6 patients treated by nephrectomy and partial ureterectomy.

RESUMO

We report on a patient with matrix stone. The presenting symptoms were right flank pain and oliguria. The patient was in severe uremic condition secondary to the obstruction of the solitary kidney. A definite diagnosis was made by an exploration. However the surgical removal of stones resulted in only temporary improvement due to residual and recurrent stones. The literature is discussed.

RESUMO

A clinical observation was made on 112 cases of hypospadias which were treated at the Department of Urology, Seoul National University Hospital during the period June 1977 through May 1982. The results were as follows: 1. The ages were distributed from 7 months to 31 years and most of the cases (64.3%) was treated in the age group of less than 7 years. 2. Most of the types of hypospadias was penoscrotal type (43.8%). 3. Cryptorchism was the most frequent associated anomalies with 21 cases (18.8%). 4. The complication rate of other anomalies was greatest in the perineal type (81.8%). 5. Success rate of one-stage urethroplasty was 52.9%. 6. In two-stage operation, the first stage operation of 97 cases was successful except one of incomplete chordee release and the success rate of Thiersch-Duplay and Johanson second stage urethroplasty was 44.4% and 66.7% respectively. 7. As the urethral stent was used in the urethroplasty, the success rate of use of silastic tube was better than red rubber and feeding tube.
