RESUMO

PURPOSE: To evaluate the clinical results of modified tension band wire technique using cortical screw for treatment of displaced medial malleolar fractures of the ankle. MATERIALS AND METHODS: From January 2001 to January 2003, 24 patients were treated by modified tension band wiring using cortical screw for medial malleolar fracture. The follow-up period was 12~35 months (average 18 months). There were 13 males and 11 females, and the mean age was 46 years. Fractures were classified by Lauge-Hansen's classification. The results were analyzed by Meyer and Kumler's criteria. RESULTS: There were 13 cases (54%) of excellent, 9 cases (38%) of good, and one case of fair because of limitation of motion of the ankle joint and one case of poor which showed post-traumatic arthritis of the ankle. CONCLUSION: Modified tension band wire technique using cortical screw can be an effective operative method for the treatment of displaced medial malleolar fractures of the ankle.

RESUMO

Dupuytren's contracture is characterized by a proliferative fibroplasia and contracture of the palmar fascia with resultant deformity of hand and finger. It is most common in those of Scandinavian and Celtic origin but it is very rare in the orientals. We experienced 30 hands of 21 patients from January 1985 to December 1995 in Soonchunhyang University Hospital. Among 21 patients, operative treatment was done in 28 hands of 19 patients. Partial fasciectomy was done in 24 hands and complete fasciectomy was done in 4 hands. The longitudinal straight skin incision was done in 14 cases and the transverse or Zig-Zag incision was done in 12 cases. Average follow-up period was 24 months. The results of the operative treatment were classified by Honner's classification. Excellent result was obtained in 16 cases (57%), good in 8 (29%), fair in 3 (11%), poor in 1 (3%). The complications were 5 cases as follows; recurrence 2, skin defect 1, hypoesthesia 2. In conclusion, partial fasciectomy utilizing longitudinal straight incisions overlying diseased area is suggested as one of the good method for the treatment of Dupuytren s contracture.

RESUMO

Intraosseous lipoma is one of the rarest primary benign tumors of bone and it is characterised by a neoplastic overgrowth of univacuolar fat cells. The incidence is less than one per 1,000 bone tumors. Most commonly, long bones are affected, with a tendency for metaphyseal involvement. Because of its rarity and nonspecific clinicopathologic findings, biopsy is needed for accurate diagnosis. Currettage and bone-grafting is the treatment of choice if the lesion is painful or if structural stability is threatened. We report 3 cases of intraosseous lipoma in long tubular bones with review of literatures. Two cases were treated by surgical intervention and the other case by conservative management.

Segmental Spinal Instrumentation (S.S.I.) is considered to the effective operative procedure in unstable fracture and fracture-dislocation of the thoracolumbar spine, providing improved correction effect, high rate of fusion and immediate rigid fixation which obviates the need for postoperative immobilization. Retrospective study was carried out of 24 cases of fracture or fracture-dislication of the thoracolumbar spine, There were treated with Harrington rod instrumentation and sublaminar wiring (8 cases) and Luque rod instrumentatiom and sublaminar wiring (16 cases) in Soonchnhyang University, from January 1986 to June 1988. We have analyzed the results of treatment, which were as follows ; 1. Thoracolumbar junction (T12 Ll) was most commomly involved segment (63%) and falling from a height was most common cause of injuries. 2. The most common type by Denis classification was burst fracture (38%). 3. About improvement of neurologic status by injury site and type of instrumentation, thoracolumbar junction (38.5%) and lumbar spine (35%) were better prognosis than thoracic spine (8.75%). 4. In commparison with kyphotic deformity and displacement, Harrington rod (66.4%) was better than Luque rod (58.9%) in postoperative correction. Also in total correction, Harrington rod was better than Luque rod in spite of more or less large amount of loss of correction. words : Thoracolumbar spine, Unstable fracture-dislocation, Segmental spinal instrumentation (S.S.I.)

The ankle is a complex, weight bearing joint consisting of tibia, fibula, talus, and many soft tissue structures, which play an important role biomechanically. The injuries to the ankle joint include not only fractures of bone but significant injuries of ligaments and other soft tissues. It is very important to understand the mechanisms of trauma in order to make definite diagnosis and proper treatment. The author' analysed 127 eases(126 patients) of the ankle fractures which were admitted and treated in Orthopaedic Department,Hanyang University Hospital from January 1978 to December 1985. The results obtained from this study were as follows: 1. Of the 127 cases, male was 90 cases(70.87%), female was 37 cases(29.13%), and average age of the patients was 37.6 years. 2. Open fractures were 12 cases(9.4%) and closed fractures were 115 cases(90.6%). 3. According to the classification of Lauge-Hansen, the most common type of the ankle fracture was supination-external rotation type (35.43%). 4. Of the 127 cases, 63 cases were treated by closed reduction and 64 cases by open reduction. 5. The average period of the cast immobilization after open reduction was 7.8 weeks, where as that of closed reduction was 10.5 weeks. 6. According to clinical and radiological assessment of the result of the treatment, open reduction was better than closed reduction. 7. The best result of the treatment was noticed in supination-adduction type, according to the classification of Lauge-Hansen. 8. Accurate reduction and rigid internal fixation of the fractured lateral malleolus and distal fibuIa was important in treatment of the ankle fracture.
