RESUMO

A total of 133 smokers visiting our smoking cessation clinic from February 2009 to September 2010 were studied.The data were collected during initial assessments and over a follow-up period of 24 months.Long-term abstinence rate was observed.And logistic regression was employed to identify the independent predictors of successful quitting.There were 132 males and 1 female with a mean age of 47 years.They smoked an average of 26 cigarettes per day.The mean duration of smoking was 25 years.Among them,57 smokers (42.9%) were severely nicotine dependent and 76 smokers (57.1%) were followed up for 24 months.And 20 smokers (26.3%) maintained 24 month abstinence.The predictor of avoiding relapse was immediate family members being nonsmokers.

RESUMO

To analyze the data of 284 smoking patients visiting the clinic during January 2009 to December 2011.There were 273 males and 11 females with a mean age of 49 years and an average length of 26 years of smoking 24 cigarettes per day.The average volume fraction of exhaled carbon monoxide was 16 × 10-6.The average score of Fagerstrom test of nicotine dependence (FTND) was 5.The importance,confidence and preparation of quitting smoking were graded at an average of 8.5,6.7 and 7.6 respectively.Abstinence rate was 66.1% at 1 month and 50.9% at 3 months.A particular focus was placed upon those smokers with more serious nicotine dependence and longer smoking years.They knew much about the importance of smoking cessation,but there was a lack of confidence.Smoking cessation improves abstinence rate via behavioral counseling,psychotherapy and medication.We should strengthen publicity of smoking cessation and smoking cessation clinic to attract more patients with heightened confidence through treatment.

ObjectiveTo evaluate the effect of psychological and behavioral intervention combined with varenicline smoking cessation clinics and to analyze predictors of successful quitting.Methods Subjects were collected from quitters who went to receive consultation and intervention in “smoking and related diseases” clinic at Zhongshan Hospital,Fudan University from March 2009 to September 2010.Eligible subjects were screened and divided into strengthen follow-up group and control group.The 4 weeks continuous abstinence rate from week 9 through week 12 were observed logistic regression model and used to analyze the predictors of successful quitting.Results Subjects who are addicted to nicotine received strengthening psychological and behavioral intervention combined with varenicline in smoking cessation clinics.The total continuous cessation rate during the 9th-12th week was 52.3%,with 60.9% (28/46) and 46.2% (30/65) of strengthen follow-up group and control group respectively.The most frequent adverse effects were nausea 39.6% (44/111),insomnia and abnormal dreams 17.1% ( 19/111 ).Adverse effects were tolerable and withdraw symptoms were few.Preparation and medication time can be used as predictors of successful quitting.Conclusion The quit rate of varenicline therapy combining with strengthen intervention is high and strengthening psychological and behavioral intervention could increase the success rate more obviously,which is a good choice for cessation therapy in smoking cessation clinics.Better preparation and regular adequate treatment can improve quit rate.

Objective To further elucidate the CT characteristics and diagnostic approaches to non-acquired immune deficiency syndrome patients with pulmonary cryptococcosis. Methods The histories of forty-two pulmonary cryptococcosis (PC) patients diagnosed in Zhongshan Hospital from 2003 -2008 were collected and analyzed for demography data, underlying conditions, clinical symptoms, chest CT and diagnostic studies. Results None of the 42 PC patients had avian or its feces contacting history, and 71.4% (30/42) of them were immunocompetent. The most frequent CT lesions were multiple nodules (67. 9% ) with peripheral predominance (67. 9% ), and cavitations (50%) often presented within them. Masses/consolidation (31.4%) and patching lesions (2. 9% ) could exist occassionally. Positive detection rates of non-aggressive examinations including sputum, bronchoalveolar lavage fluid and bronchofibroscope aspiration were 4. 3%, 8. 3% and 6. 3% respectively, while those of aggressive approaches including transbrunchial lung biopsy (TBLB), thin needle aspiration biopsy (TNAB) and pneumonectomy by surgery were 64. 7%, 64. 3% and 100% respectively. Non-aggressive serum cryptococcus antigen test was performed in 14 patients who had been diagnosed by histopathology or pathogen culture, and all of them were positive. Conclusion Our study suggests that PC is common in immunocompetent population. Avian or its feces contacting is not so important as used opinion to PC differential diagnosis. CT characteristics of PC are diversiform and always change very slowly. Besides the most frequent multiple nodules with subpleural predominance, pulmonary lesions can present as masses, consolidation or patchings. Aggressive techniques such as TBLB and TNAB are benefit to clinical diagnosis of PC, and non-aggressive serum cryptococcus antigen test may be promising for its early diagnosis as well as clinical course follow-up and therapeutic effect evaluation.

RESUMO

<p><b>BACKGROUND</b>The down-regulation of constitutive nitric oxide synthase (cNOS) and up-regulation of inducible nitric oxide synthase (iNOS) are associated with the allergen-provocated airway hyperresponsiveness (AHR). This study aimed to determine whether their alteration also plays an important role in the AHR induced by lipopolysaccharide (LPS).</p><p><b>METHODS</b>Hartley male guinea pigs, weighing between 250 g and 350 g, were injected with LPS at a dose of 1 mg/kg every 24 hours for three days. A non-selective NOS inhibitor, N(omega)-nitro-L-arginine methyl ester (L-NAME), or a selective inducible NOS inhibitor, aminoguanidine (AG), were used thirty minutes before each injection of LPS. Airway reactions, nitric oxide (NO) production and inflammatory changes were detected 24 hours after the last dose of LPS.</p><p><b>RESULTS</b>AG significantly decreased the NO production in the bronchoalveolar lavage fluid (BALF) and sharply reduced the intensity of bronchoconstriction to histamine challenge. L-NAME also significantly decreased the NO production in the BALF, but had no effect on airway reactions or, perhaps, a tendency to enhance the intensity of AHR.</p><p><b>CONCLUSIONS</b>The data suggest that inducible NOS contributes to the AHR induced by repetitive intraperitoneal LPS, and constitutive NOS was also involved.</p>
