RESUMO

Eccrine hidrocystomas are benign tumors of the sweat glands that arise from cystic dilatation of the excretory sweat ducts. They may be solitary or multiple. Multiple lesions are observed as small papulonodules having prominent surfaces with a translucent intense bluish dome, localized predominantly to periorbital and malar areas. The diagnosis of multiple eccrine hidrocystomas is clinical, and surgery is the treatment of choice in the case of a single lesion. However, the treatment of multiple eccrine hidrocystomas is challenging and the outcome of conventional therapies varies. Botulinum toxin type-A (BTA) causes chemodenervation at the cholinergic nerve terminal of eccrine glands; accordingly our patient was given intradermal injections of BTA and the result was successful. Herein, we report a case of multiple eccrine hidrocystomas treated successfully with BTA; we believe that BTA can be a novel treatment option for this condition.

RESUMO

A 68-year-old woman presented with erythematous indurated plaques on both her eyebrows. The skin lesions appeared 3 months after she was tattooed on both eyebrows. On histopathologic examination, a granulomatous infiltration was observed in the dermis with no evidence of any fungal, mycobacterial organisms. She was treated with intralesional injection of triamcinolon acetonide, but there was no improvement. After she started to take allopurinol for 5 months, the skin lesion improved without side effects. To the best of our knowledge, this is the first case of foreign body granuloma that was successfully treated with allopurinol in the Korean dermatologic literature.

RESUMO

Calciphylaxis is a rare, but serious disorder that is usually observed in patients with renal disease and secondary hyperparathyroidism. It is characterized by the ischemic necrosis of the skin, the underlying tissue and other organs, as well as rapid vascular calcification. Prompt diagnosis and management is required because the intractable skin necrosis sometimes causes lethal sepsis. Sevelamer hydrochloride is an ion-exchanging resin that binds phosphates in the gut without increasing the calcium load. Treatment with it may lead to less vascular calcification and better survival for patients with chronic renal disease. Herein we present a case of calciphylaxis with renal failure and the patient improved after treatment with sevelamer hydrochloride therapy.

BACKGROUND: Actinic keratoses (AKs) are precancerous lesions that are treated with local liquid nitrogen cryotherapy, topical fluorouracil or topical chemotherapeutic agents. Photodynamic therapy (PDT) with various kinds of light sources has recently been tried for treating actinic keratoses. There have been no studies in the Korean literature about the effectiveness of aminolevulinic acid (ALA)-PDT combined with blue light for treating actinic keratoses. OBJECTIVE: We examined the safety and efficacy of PDT (using topical 20% ALA solution and blue light) for the treatment of actinic keratoses. METHODS: Twenty five AKs lesions of 17 patients were enrolled in the study. They were prepared with the application of topical 20% ALA solution for 2 hours and this was followed by exposure to 417+/-5 nm blue light at 10 J/cm2. The lesions were assessed at 48 hours and 2 and 4 weeks following PDT. Those target lesions remaining at week 4 were re-treated. Skin biopsies were done at 8 weeks to evaluate the response to ALA-PDT. RESULTS: Complete remissions were achieved for 64.0% of the AKs lesions after 2 treatment sessions. Only two patients suffered from a stinging sensation when exposed to the light therapy. CONCLUSION: Topical ALA-PDT with using a blue light source could be an effective treatment modality for actinic keratoses

RESUMO

Benign symmetric lipomatosis is a very rare disease that's characterized by a symmetric accumulation of excessive amounts of adipocytes. The lesions are located mainly on the neck, trunk and proximal extremities. Although its etiology is unknown, it has been described to be associated with a heavy alcohol intake. A 51-year-old woman presented with a two year history of ill-defined soft masses on both mandibular areas. We made the diagnosis of benign symmetric lipomatosis according to the histopathologic examination. As a difference from other reports of benign symmetric lipomatosis, our patient was non-drinker for her past history. We present here a case of benign symmetric lipomatosis at an unusual predilection site and this developed in a non-drinker.

RESUMO

Digital mucous cysts are observed frequently on the lateral or dorsal aspect of the digital fingers. Digital mucous cysts are prone to recur after simple excision, electrocoagulation, intralesional injection of corticosteroid and a variety of other therapeutic maneuvers. In this report, we describe one patient with digital mucous cyst who was successfully treated with intralesional injection of 1 mg/ml of minocycline hydrochloride without any sequelae.

RESUMO

Pleomorphic adenomas are the most common neoplasms of the salivary glands. A long-lasting pleomorphic adenoma can develop into a malignant tumor. Malignant mixed tumor of the salivary gland includes carcinoma ex pleomorphic adenoma, carcinosarcoma, and metastasizing pleomorphic adenoma. The metastasizing pleomorphic adenoma is to be distinct from the true malignant mixed tumor, because it remains histologically benign and can metastasize. We report the case of a 64-year-old female with a long-standing goose egg-sized neck mass and multiple small firm nodules on the trunk and extremities, which showed histological features metastasizing pleomorphic adenoma of the salivary gland.
