RESUMO

Tinea barbae is a dermatophyte infection of the bearded areas of the face and neck. The incidence of tinea barbae has decreased as improved sanitation. We report a case of tinea barbae in a 58-year-old-man, who presented with relatively well-defined erythematous pustular plaque on the upper lip. A fungal culture from tissue of the lesions was grown on Sabouraud dextrose agar and showed typical Trichophyton verrucosum. The nucleotide sequence of internal transcribed spacer for clinical isolate was identical to that of Trichophyton verrucosum strain ATCC 10695. The patient was treated with oral itraconazole for 3 months. The skin lesions improved 3 months after treatment, and recurrence has not been observed.

RESUMO

Kerion celsi is a severe inflammatory type of tinea capitis that presents as an inflammatory, boggy mass with broken hairs and hair loss. It is usually occurred in children between the age of 4 and 14 years that caused by zoophilic or geophilic pathogens such as Microsporum(M.) canis, Trichophyton(T.) mentagrophytes, T. verrucosum, M. gypseum, T. verrucosum was chiefly found from cattle which infect the human through direct contact. We report a case of kerion celsi caused by T. verrucosum probably transmitted from cattle in a 3-year-old boy. The patient had a solitary, tender, 6.0 x 5.5 cm sized, erythematous boggy plaque and pustules with hair loss on the right side of occipital scalp for 2 weeks. Chains of chlamydoconidia were observed in KOH mount and slide culture by light microscopy. The nucleotide sequence of internal transcribed spacer (ITS) region for clinical isolate was identical to that of T. verrucosum strain IFM 57570. He was treated with 125 mg of terbinafine daily for 12 weeks and short term therapy of low dose of prednisolone. Skin lesion was cured without recurrence.

RESUMO

Tinea incognito is the dermatophytosis of atypical clinical presentation that induced by topical and systemic corticosteroid treatment or new class of topical non-steroidal agent such as tacrolimus or pimecrolimus. We report a case of eczema-like tinea incognito in a 15-year-old girl. The lesions were manifested by pruritic to tender, scaly well-demarcated erythematous macules and patches with pustules on the lateral side of right shin. She had been treated with topical corticosteroid and pimecrolimus for 6 months. Direct microscopic examination of the lesion was positive for hyphae and fungal culture revealed colonies of Trichophyton(T.) rubrum. Histopathologic examination of the skin lesion showed mild perivascular inflammation with presence of fungal hyphae in the horney layer of the epidermis. The patient was treated with 250 mg of terbinafine daily for 1 month and topical lanoconazole application. Skin lesions improved one month after the treatment, and no recurrence was observed.

BACKGROUND: The host response to natural fibers results in granuloma formation in an effort to limit tissue destruction. Matrix metalloproteinases (MMPs) are important molecules in the inflammatory granulomatous or reparative reaction. Here, we studied the foreign body reaction that occurs following natural fibers implantation by investigating MMPs and tissue Inhibitor of MMPs (TIMPs) in an in vivo model. METHODS: Female Sprague-Dawley rats were treated with crocidolite fiber or fibrous talc via subcutaneous and intraperitoneal injections and immunohistochemistry was conducted to confirm the expression of MMPs and TIMP-2 in tissue sections. RESULTS: We identified that mineral fibers elicited granulomas. Fibrous talc or intraperitoneal injection resulted in larger granulomas and severe tissue destruction compared with the lesions induced by crocidolite or subcutaneous injection. The expression of MMPs was elevated while granulomatous lesions were formed. The relative levels of MMPs were lower in the talc injected or intraperitoneal route models than those of crocidolite injected or subcutaneous injection models during the entire experiment. CONCLUSIONS: These findings demonstrate that specific expression of MMPs/TIMP is inversely related to the grade of tissue destruction and suggest that expression of MMPs is required for promoting granuloma formation and limiting tissue destruction.

Tinea corporis is dermatophytosis of trunk and extremities except terminal hair present region. We report a case of psoriasiform tinea corporis in a 11-year-old girl. The lesions were showed multiple, pruritic, thick scaly erythematous macules and patches on the left forearm and trunk. She had been treated with topical and oral corticosteroid for 5 months. Direct microscopic examination and fungal culture were positive for hyphae and colonies of Microsporum canis. Histopathologic examination of the skin lesion revealed parakeratosis, psoriasiform epidermal hyperplasia and perivascular inflammatory infiltration in upper dermis with presence of fungal hyphae in stratum corneum. She was treated with 250mg of terbinafine daily and topical lanoconazole for 1 month. Skin lesions cured without recurrence.

RESUMO

The authors report a fatal case of disseminated tuberculosis in a 14-yr-old girl, which developed immediately after a measles-rubella (MR) vaccination. Despite a markedly accelerated clinical course which led to death within two weeks, the authors could not identify any possible cause of the tuberculosis aggravation in this case, with the exception of the MR vaccination. The possible role that MR vaccination had on the clinical course of tuberculosis in this case is discussed.

Kerion celsi is an inflammatory type of tinea capitis. It is usually seen with zoophilic pathogens, with common examples being Microsporum(M.) canis, Trichophyton(T.) mentagrophytes and T. verrucosum. Hamster is the mammal which belongs to the rodent family, usually small in size and frequently nocturnal in their habits and small pet kept in many countries today. We report a case of kerion celsi caused by M. canis probably transmitted from hamster in a 8-year-old boy. The patient had various sized, erythematous boggy nodules and pustules with irregular hair loss on the scalp for 2 months. Culture from a scalp lesion of patient on Sabouraud's dextrose agar showed M. canis. He was treated with 125 mg of terbinafine daily for 12 weeks and short term therapy of low dose of prednisolone. Skin lesions were cured without recurrence.

Spontaneous pneumothorax occurs in a variety of lung diseases. Primary or metastatic lung cancers are rare, but important causes of pneumothorax. We report here on a case of pulmonary metastasis of thyroid papillary carcinoma that initially presented as spontaneous pneumothorax. A 32-year-old male with no history of thyroid disease underwent wedge resection of the lung due to recurrent pneumothorax. Histologically, the pleura and subpleural lung parenchyma revealed several micronodular lesions and randomly scattered glandular structures with mild cytological atypia. The cells were characterized by their overlapped hypochromatic nuclei with longitudinal grooves and inconspicuous nucleoli. On the additional sections, we found a few intranuclear inclusions and colloid-like material in the follicular structures. The glandular cells were immunoreactive for thyroglobulin, TTF-1, CK 7 and HMW-CK and they were focally immunoreactive for EMA, but they were negative for S100, chromogranin and CEA. The Ki-67 labelling index was 4%. Neck ultrasonography revealed an ill-defined hypoechoic nodule in the left lobe of the thyroid. The diagnosis of conventional, classical papillary carcinoma was made by both the cytology and the total thyroidectomy specimen. Pathologists should conduct a through histologic examination for the patients with recurrent spontaneous pneumothorax to search for the underlying causes.

RESUMO

NK/T-cell lymphoma (NKTCL) is characterized by the expression of the NK-cell antigen CD56. Non-nasal NK/T-cell lymphomas are subdivided into primary cutaneous and 4 subtypes of secondary cutaneous lymphomas; nasal type, aggressive, blastic (blastoid), and other specific NK-like cell lymphoma. Aggressive NK/T-cell lymphoma/leukemia is a rare leukemic variant of nasal type NKTCL. We herein report a rare case of aggressive NK/T-cell lymphoma/leukemia with cutaneous involvement in adolescence.

We report a case of tinea barbae in a 51-year-old man, who presented with erythematous pustules on the upper lip, and a relatively well-defined pustular patch on his left cheek. Histopathologically, fungal hyphae were found in the hair shaft upon PAS staining. A fungal culture from scales and tissue of the lesions was grown on Sabouraud's dextrose agar and showed typical Trichophyton mentagrophytes. The patient was treated with 250 mg of terbinafine, 10 mg of prednisolone and 300 mg of roxithromycin daily for the first week, then 250 mg of terbinafine combined with topical lanoconazole for 4 weeks. The skin lesions improved 5 weeks after treatment, and recurrence has not been observed.

RESUMO

An adenocarcinoid is a mixed tumor comprised of both carcinoid and adenocarcinomatous components that is commonly found in the esophagus, stomach, duodenum, small bowel and appendix, and is also rarely found in the large bowel. A 61 year old male visited our institution because of a change in bowel habitus 3 months prior to the time of his visit. Colonoscopic findings revealed a 2 x 2 cm round smooth surface mass 7 cm from the anal verge, and an endoscopic ultrasonograph revealed a round hypoechoic mass in the submucosal layer. The mass was resected by endoscopic mucosal resection using a cap- fitted endoscope. Pathologic analysis of the mass confirmed that it was a composite tumor comprised of carcinoid and adenocarcinomatous components. To the best of our knowledge, this paper is the first case of rectal adenocarcinoid reported in Korea.

Paecilomyces (P.) lilacinus is a saprophytic filamentous fungus, mostly found in soil and very rarely pathogenic to humans. Although it has been reported to cause endophthalmitis, endocarditis and pulmonary infection, localized skin infection due to P. lilacinus has been rarely reported. We report a case of localized skin infection by P. lilacinus in an 83-year-old man. The lesion was manifested by a 7.0x5.0 cm-sized erythematous plaque on the left wrist. The fungal culture from the biopsy specimen on Sabouraud's dextrose agar showed velvety whitish pink colonies of P. lilacinus. The patient was treated with 200 mg of itraconazole daily for 3 months. The skin lesion was completely cured and recurrence has not been observed to date.

We report a case of kerion celsi caused by Trichophyton(T.) tonsurans in a 14-year-old student of a middle school wrestling team. The lesion was on the scalp for 1 month and 6.5X5.0X1.0 cm sized, markedly inflammatory, swollen, boggy pus exuding plaque. Culture from the scalp lesion of the patient on Sabouraud dextrose agar showed T. tonsurans. The patient was treated with 250 mg of terbinafine daily for 12 weeks and short term therapy of low dose of prednisolone. Skin lesions were completely cured and recurrence is not observed to date.

Scedosporium(S.) apiospermum is the asexual stage of Pseudallescheria boydii. It has been isolated from soil, plant debris, polluted water and sewage. It is an opportunistic organism with low virulence. Infection may occur via direct inoculation. This ubiquitous fungus causes not only mycetoma, but also infections of a variety of body sites including the skin. Localized skin infection without granule or grain production due to this organism is much rarer than mycetoma. We report a case of cutaneous S. apiospermum infection which occurred in a 69-year-old woman. The lesion was manifested by a 5.0 x 4.0 cm-sized, scaly erythematous plaque on the dorsum of the right hand. The fungal culture from the biopsy specimen on Sabouraud's dextrose agar showed white to gray colored floccose colonies of S. apiospermum. The patient was treated with 200 mg of itraconazole daily for 3 months. Skin lesions were completely cured and recurrence has not been observed to date.

RESUMO

Ganglion is a relatively-common, benign, soft tissue tumor that is almost always located near a joint capsule or tendon sheath. The common location is around the wrist, ankle or foot dorsum. We report a case of ganglion in a 53-year-old woman who presented with a solitary, tender, 1.0x1.0 cm sized, normal skin-colored, deep-seated nodule on the left sole. She had been treated by total excision of the tumor and recurrence has not been observed to date.

Sporotrichosis is a chronic infectious disease caused by Sporothrix(S.) schenckii. Regional lymphatic sporotrichosis is common type, accounting for 75% of the cases. But fixed cutaneous sporotrichosis is seen in 20% of cases. We report two cases of fixed cutaneous sporotrichosis in a 48-year-old man and 8-year-old boy. The lesions were manifested by erythematous plaques on right arm and left cheek, respectively. The fungal culture from biopsy specimen on Sabouraud's dextrose agar showed dark brown to black, moist and wrinkled colonies of S. schenckii. Histopathologically, pseudoepitheliomatous hyperplasia and chronic granulomatous inflammation were observed on H & E stain. The patients were treated with surgical excision of lesion & oral administration of 30 drops of saturated solution of potassium iodide three times a day and 100 mg of itraconazole daily for 3 months, respectively. Skin lesions were completely cured and recurrence is not observed to date.
