RESUMO

Objective:To study the protective effects and mechanisms of melatonin (MTn) on lipopolysaccharide (LPS) and hypoxic-ischemic(HI) induced white matter damage (WMD) in neonatal rats.Methods:Seventy-two 3-day-old newborn Sprague-Dawley (SD) rats were randomly assigned into sham operation group (the sham group), model group (the HI group) and MTn intervention group (the HI+MTn group) ( n=24 for each group). For the sham group, only dissection of the right common carotid artery was performed without ligation. Animal models of WMD were established using LPS pretreatment and HI method in both the HI group and HI+MTn group. The HI+MTn group received MTn intraperitoneal injection (15 mg/kg, 1 h before LPS injection and then once daily). The HI group and the sham group received equal volume of normal saline containing 1% ethanol intraperitoneal injection. The rats were sacrificed on d7 of experiment and periventricular white matter (PVWM) was collected for hematoxylin-eosin (HE) and TUNEL staining to determine WMD and apoptosis. The distribution and morphology of microglial cells in the PVWM were studied using IBA1 immunofluorescence staining. Reactive oxygen species (ROS) kit was used to detect ROS. The expression of nucleotide-binding domain-like receptor protein 3 (NLRP3) inflammasomes, interleukin (IL)-1β, IL-18 and mitochondrial autophagy markers (pink1 and parkin) were determined using real-time quantitative PCR. Results:Compared with the sham group, the HI group showed WMD, cell degeneration and necrosis,increased cell apoptosis and increased expressions of NLRP3 inflammasomes and downstream inflammatory factors (IL-1β and IL-18) in PVWM. Compared with the HI group,the HI+MTn group showed reduced WMD, cell apoptosis, microglia infiltration and inflammatory factors expression. MTn increased pink1 and parkin expression and reduced ROS production in PVWM.Conclusions:MTn reduces ROS production by enhancing mitochondrial autophagy and inhibits NLRP3 inflammasomes hyperactivation to alleviate endotoxin- and HI-induced WMD in neonatal rats.

RESUMO

Objective:To investigate the clinical manifestations and prognosis of severe coronal virus disease 2019(COVID-19)pneumonia combined with coronary artery disease(CAD)in the elderly.Methods:A total of 351 old patients(≥60 years)with severe COVID-19 pneumonia combined with CAD admitted to Tongji Hospital of Wuhan from February 2020 to March 2020 were enrolled in this retrospective study.The patients were divided into CAD group(n=52)and non-CAD group(n=299). The clinical data, the changes of blood tests and cardiovascular complications were observed.Results:Compared with non-CAD group, CAD group showed a longer time of stay in hospital[(12.3±2.6)d and(9.3±2.1)d, t=3.24, P=0.002], and higher incidence of cardiovascular complications, such as myocardial infarction(7.7% and 0.0%, χ2=21.6, P<0.001), heart failure(15.4% and 0.3%, χ2=34.7, P<0.001), arrhythmia(50.0% and 12.4%, χ2=24.45, P<0.001)and all-cause mortality(15.4% and 4.3%, χ2=7.94, P=0.005). The serum levels of interleukin-2 receptor, interleukin 6, CK-MB, cTnI, NT-proBNP, D-dimer and fibrinogen were higher in CAD group than in non-CAD group.The oxygenation index was significantly lower and the blood lactic acid level was higher in CAD group than in non-CAD group. Conclusions:The aged patients with severe COVID-19 pneumonia combined with CAD show obvious inflammatory reaction, high incidence of cardiovascular complications and high mortality.

Objective:To compare the effectiveness of venous flap and the flap based on superficial palmar branch of radial artery (SPBRA) in the reconstruction of the defect of finger with segmental defect of proper digital artery.Methods:From July, 2014 to March, 2018, 40 patients with soft tissue defects of finger and the proper arteries were treated by the venous flaps (18 cases, group A) and the flaps based on SPBRA (22 cases, group B). The operative time, postoperative 2-PD of the flaps, appearance, patient satisfaction, flaps donor site complications, flaps survival rate and the motion of digital joints were observed and compared by software SPSS 17.0. P<0.05 was considered statistically significant. Results:Forty flaps all survived, and the wounds healed primarily. The operative time of group A was (78.89±7.36) min, which was shorter than that of group B (97.27±9.26) min, with statistically significant difference ( P<0.05) between the two groups. At end of followed-up, there was a statistically significant difference in the 2-PD of the flaps between the 2 groups [(7.94±1.55) mm in group A, (6.59±1.65) mm in group B, P<0.05)]. The overall active motion of the digital joints: 13 cases were excellent, and 5 cases were good in group A. And 17 cases were excellent, 5 cases were good in group B. The satisfactory was 100% in both groups. Of patient satisfactory, group A: 4 cases were great satisfactory, 13 were satisfactory, and 1 was fair; Group B: 17 cases were great satisfactory and 5 were satisfactory. The appearance of flaps: in group A, 16 cases were good and 1 was bad, while in group B, 21 cases were good and 1 was bad. And there was no statistically significant difference between the 2 groups ( P>0.05). Conclusion:A higher survival rate and satisfactory clinical efficacy can be achieved by the venous flaps and the flap based on SPBRA while the area of defect in hand soft tissue, where there exists a segmental defect of proper arteries, is small. In terms of the recovery of sensing, the first choice is the flap based on SPBRA. In considering of the difficulty in surgical operation, the venous flap is a relatively simple procedure.

Colorectal cancer is one of the most common malignant tumors of the digestive system. The incidence of colorectal cancer in China is increasing year by year. Screening for adenomas, precancerous lesions and early cancers can effectively reduce the incidence and mortality of colorectal cancer. Therefore, colorectal cancer screening is particularly important. At present, the colorectal cancer examination is mainly divided into fecal examination and colon structural examination. The fecal examination includes fecal occult blood test, fecal exfoliated cell test and fecal genetic analysis. The colon structural examination includes barium enema, colonoscopy and computed tomography colonography. Colonoscopy is considered as the gold standard for the diagnosis of colorectal cancer. It has the advantage of high accuracy, and can be used to find precancerous lesions and remove them at the same time. However, the screening should be greatly restricted because of its invasive examination, the need for intestinal preparation, obvious discomfort and the high cost of medical treatment. Comparatively, fecal occult blood test is simple, easy, non-invasive, saving manpower and material resources, and is suitable for large population screening. Some scholars in China have begun to pay attention to colorectal cancer screening, and fecal occult blood test is widely used in these screenings.

Objective:To investigate the relationship of polymorphism of leptin receptor gene Gln223Arg with asthma and metabolic syndrome.Methods: Collected 120 asthma patients,92 metabolic syndrome patients,54 asthma combined metabolic syndrome patients and 81 normal controls.According to the severity,the asthma patients were divided into mild-medium group and severe group.The serum leptin level was measured by ELISA,the genotypes of leptin receptor were analyzed by the method of polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP),and statistcs of each subject′s MBI,blood pressure,lung function and fasting blood glucose were collected.Results: ①There were significant differences in genotype and allele frequency in leptin receptor gene Gln223Arg between the metabolic syndrome group,asthma combined metabolic syndrome group and normal control group(P<0.05).②The allele frequency and genotype in leptin receptor gene Gln223Arg were significant different between the severe asthma group and normal control group(P<0.05).③The serum leptin level,BMI and systolic blood pressure of AA+AG genotype group were significiant higher than GG genotype group(P<0.05),while the value of FEV1% and FEV1/FVC of lung function were lower than GG genotype group(P<0.05).Conclusion: Leptin receptor gene Gln223Arg polymorphism is correlated with asthma and metabolic syndrome,and by causing leptin resistance,the A allele might be the genetic factor that contribute to individual susceptibility for asthma and metabolic syndrome.

Objective:To investigate the relationship between polymorphism of leptin gene -2548A/G and leptin receptor gene ( LEPR) Gln223Arg and asthma and metabolic syndrome.Methods: 82 asthma-combined metabolic syndrome patients,114 asthma patients,100 metabolic syndrome patients and 96 normal controls were conducted.The polymerase chain reaction and restriction fragment length polymorphism ( PCR-RFLP) analysis were performed to investigate the polymorphism of leptin gene -2548A/G and LEPR gene Gln223Arg site.In addition, asthma was graded into mild AS and mod-severe AS according to lung function.Then the associations between the polymorphism of leptin gene -2548A/G and LEPR gene Gln223Arg and different grades of asthma were performed.Results:①The biochemical indicators were different compared between each group.②The genotype and allele frequencies in leptin gene polymorphism -2548A/G were significantly difference between metabolic syndrome patients(P=0.047 and 0.046), asthma-combined metabolic syndrome patients( P=0.038 and 0.044) ,mod-severe asthma patients( P=0.019 and 0.028) and control group.③There was a significant difference of genotype and allele frequencies in LEPR gene Gln223Arg between metabolic syndrome patients and controls(P=0.037 and 0.023);between metabolic syndrome patients and asthma patients(P=0.000 and 0.000).There was a significant difference of allele frequencies in LEPR gene Gln223Arg between asthma-combined metabolic syndrome patients and asthma(P=0.032) .Conclusion: Polymorphisms of the leptin gene -2548A/G site may be associated with metabolic syndrome and mod-severe asthma.Polymorphisms of the LEPR gene Gln223Arg site may be only associated with metabolic syndrome.The two genes would be the candidate genes in early prevention and control.

<p><b>OBJECTIVE</b>To establish the method for determining simultaneously eight kinds of ginsenoside Rg1, Re, Rf, Rg2, Rb1, Rc, Rb3 and Rd and their contents in Shizhu ginseng.</p><p><b>METHOD</b>HPLC was carried out on Agilent TC-C18 (4.6 mm x 150 mm, 5 microm) with acetonitrile -0.05% phosphoric acid, gradient elution, the volume of flow 1 mL x min(-1), column temperature 30 degrees C and detection wavelength 203 nm.</p><p><b>RESULT</b>The contents of eight kinds of ginsenosides in Shizhu ginseng were 1.32, 2.08, 0.72, 0. 24, 2.12, 1.06, 0.35, 0.55 mg x g(-1), respectively.</p><p><b>CONCLUSION</b>The contents of ginsenoside ingredients in Shizhu ginseng were determined in the first time. The established method can be used in determining the contents of multiple gensenosides in herb ginseng simultaneously.</p>

@#ObjectiveTo explore the pathological effect of Anti-stenosis No.1 which had effect of vivid Qi and promoting blood flow,eliminate sputum and circulating collaterals on restenosis after carotid artery angioplasty in experimental rats.Methods48 rats were randomly divided into the blank group,the hyperlipid group,the small-dose group and the large-dose group.Restenosis model in carotid artery after angioplasty was built by high-dose cholesterol diet and three times balloon injury.Chinese herbs were administered for three months.Lumen area,intimal area,media area and intimal area/media area in the restenosis region were calculated.ResultsLumen area and intimal area were very more significantly changed in the hyperlipid group,the small-dose group,and the large-dose group compared to the blank group(P<0.01).Media area and intimal area/media area were very more significantly changed in the hyperlipid group and the small-dose group compared to the blank group(P<0.01).There was no significant difference in the large-dose group.Compared to hyperlipid group,lumen area significantly increased in the large-dose group(P<0.01)),intimal area significantly decreased in the small-and large-dose groups(P<0.05),and media area significantly decreased in the large-dose group(P<0.01).There was a significant difference in media area between the small-and large-dose groups(P<0.05).ConclusionAnti-stenosis No.1 can significantly inhibit restenosis by inhibiting intimal and media hyperplasty,and the effect is dose dependent.
