RESUMO

Atrophic rhinitis is a chronic nasal disease of unknown etiology, characterized by nasal mucosal atrophy, enlargement of nasal cavity, formation of thick foul smelling, greenish nasal crusts, hyposmia or anosmia. This clinical trial conducted between 1996-2005. 30 patients with primary atrophic rhinitis were included in the study. The diagnosis was based on the typical signs of nasal obstruction, enlargement of nasal cavity and greenish nasal crusts in the clinical examination. Patients were treated surgically by a new and simple technique of partial closure of the nasal cavity with the use of intranasal semicircular flap. Degree of patient satisfaction and clinical findings before and after the operation were analyzed statistically. Excellent results were obtained in all patients with complete resolution or improvement of symptoms such as foul nasal crusts smelling, epistaxis, headache, hoarsness and olfaction except for the symptom of nasal obstruction. This simple surgical technique can be performed under local anesthesia with low casts and without any complications such as nasal deformity and flap failure, in contrast to other surgical techniques

RESUMO

CT scan is the standard method for purposes of diagnosis, disease monitoring, and surgical planning of paranasal sinuses. Yet there are varying reports of both positive and negative correlation between sino - nasal symptoms and CT evidence of sinusitis. We designed a study for survey of relative frequency of signs and symptoms of chronic sinusitis that correlate in clinical diagnosis with computed tomography scan results. Signs and symptoms of 200 patients with clinical diagnosis of chronic sinusitis after coronal CT scan results were evaluated and analyzed for sensitivity, specificity, positive and negative values. 111 patients [55.5%] had chronic sinusitis with respect to CT scan findings, but 89 patients [44.5%] didn't have any evidence of chronic sinusitis on CT scan. Age, sex and duration of disease didn't have significant statistical difference between these groups. The most sensitive symptoms were nasal stuffiness and facial or forehead pain and least sensitive but most specific was facial pressure. Least specificity was related to facial or forehead pain. The greatest positive predictive value was facial pressure and the least, was facial or forehead pain. The most valuable predictor symptom in diagnosis of chronic sinusitis is facial pressure, and least reliable of them is facial or forehead pain. Generally clinical symptoms have a little value in diagnosis of chronic sinusitis and must be checked with CT scan findings

RESUMO

Adenotonsillar hypertrophy is the most common cause of upper airway obstruction and sleep apnea syndrome in children, may be an important factor for failure to thrive by decreasing the secretion of serum growth hormone [GH] and insulin-like growth factor-1 [IGF-1]. This is an experimental interventional study. Fasting serum GH, IGF-1 levels, growth and body mass indexes were measured in 30 children with 3-13 years old at the operation's day and 110 days post-operation. There was no statistical correlation between serum IGF-1 levels before and after adenotonsillectomy in total group patients, but there was a significant statistical correlation between IGF-1 levels with grade III and IV of tonsillar hypertrophy. Grade III and IV of adenotonsillar hypertrophy is an important factor for serum GH and IGF-1 decreasing and failure to thrive [FTT]

RESUMO

The aim of this study was the evaluation of hearing threshold in patients with beta thalasemia major. For this purpose, 178 patients who were under regular chelation therapy with desferrioxamine [DFO] were selected and underwent ear, nose, throat examinations and audiological evaluations. Patients were between 4 to 25 years old and 54.9% were male and 45.1% were female. Three patients were excluded from the study due to middle ear disease and tympanic membrane perforation. Mean hearing threshold was 12.46db. The incidence of hearing loss in speech frequencies was 1.8% and there was a correlation between hearing threshold increase and the duration of disease and desferrioxamine consumption. But no correlation was found between hearing threshold and serum ferritin level [mean 4046 ng/ml] or DFO dose [mean=38.68mg/kg/day]

RESUMO

Between 2-9 years old which is an important period in body growth, chronic infection diseases with adenotonsilar hypertrophy may cause failure to thrive [F.T.T].Which is the aim of this investigation. 76 children between 2-9 years old who suffered from chronic adenotonsilar hypertrophy and infections for more than two years evaluated and compared with control group, by the results of the developmental index that analyzed by epineuria analysis. According to the revealed results in both groups all of percentile less than 3 are WAP=23.8, WHP=17.8, HAP=18.10. P=0.006 for WAP [meaningful] P=0.045 for HAP [meaningful], Also there is no any meaningful difference for WHP, WHZ, HAZ in both groups. W=Weight, H=Height, A=Age Chronic and recurrent infections of adenotonsilar hypertrophy affecting body growth process and diminishes weight age and height /age percentile

RESUMO

The patient is a seventeen years old girl, with severe pain and swelling in right maxillary sinus region and facial deformity from two weeks prior to referring. CT scan demonstrated a large tumoral mass in right maxillary sinus extended to ptereigomaxillary fossa, orbital floor, lacrimal system and nasoethmoid region. Hard palate had been destroyed and bulged in to the oral cavity. Tumor was resected completely by radical cald-well-luck approach. Hystopathology of tumor was giant cell reparative granuloma [G.C.R.G], which is a very rare tumor in paranasal sinuses. Since the association of these tumors with hyperparathyroidism has been reported in literature, the patient was evaluated for hyperparathyroidism and elevated serum parathyroid hormone [PTH: 390 pg/ml] and other hematological tests confirmed secondary hyperparathyroidism

RESUMO

This case report is a 65-year-old woman from Kerman. Her main complaint is nasal obstruction and recurrent nasal bleeding from five months ago. In clinical examination, there was a large tumor in left nasal cavity. CT scanning revealed a large tumor in left nasal cavity and maxillary sinuse. The tumor is resected completely with negative safe margine by radical Caldwell - luc approach. The origin of tumor was from inferior turbinate. The diagnosis is mesenchymal sarcoma. These sarcomata are very rare in nasal cavity
