RESUMO

Introduction: One of the important aspects in high quality health care system is delivering health services in an appropriate way which can lead to development of the systems. Patient satisfaction is a quality indicator that has the potential to provide valuable information about the care delivered by an Emergency Medical Services system [EMS]. This indicator is considered as an important marker of quality by paramedics


Method: This is a descriptive- analytical study on 1096 patient satisfaction of emergency services suffering from cardiovascular, dyspnea, low level of consciousness and cerebral problems. Variables such as the type of diseases, technicians' and operators' behavior, time of response and outcome of the missions were measured with a valid checklist. ANOVA and correlation Pearson were employed as analytical tests


Results: Considering different types of diseases, 4 categories of diseases had positive correlation with satisfaction of the patients and also there was a meaningful correlation between factors which were measured and satisfied patients.[P<0.001]


Conclusion: Developing some instructions for technicians and operators focusing on interpersonal skills and enhanced sensitivities and behaviors not only toward patients but also toward bystanders and family members is recommended to improve patient satisfaction and thought quality of delivered care in the prehospital emergency medical systems

Due to consuming about 50%-80% of health resources, hospitals are the greatest and costly operational units in Iranian Health system. so allocation of resources specially human and space resources as the most expensive ones is really important for further controlling of costs, analysis of costs and making suitable policies for increasing the profitability and allocation of resources and improvement of quality. This paper intends to describe and analyze any allocation of resources in 530 university hospitals in Iran. The final goal of this research is to provide a data bank according which there is a basis for more scientific budget allocation of state's hospitals from the size and type of application points of view. The relevant index of person to bed was 2.04 for human resources. All hospitals more than 300 beds are located in benefiting areas from which 17 cases are educational and 2 cases are therapeutic. This is necessary to mention that the rate of management group forces to total personnel at deprived areas is about 2.5% more than benefiting areas. Because 60-80% of hospital costs are applied for human forces, all managers of hospitals are obliged to revise their policies in attraction and employment of human force in order to benefit from such a valuable resource and prevent from expensive costs. So any employment of personnel should be based upon real needs of hospital

In Emergency Medical Systems [EMS], decreasing the arrival time for patients is an important factor and as a result, in recent decades motorcycle ambulance [motorlance] is being used in EMS centers for missions called for in crowded, rural and arduous areas. This study was done to evaluate the impact of using motorcycles on arrival time of EMS technicians to the scene. This is a cross-sectional study based on the information of 1727 motorlance and 44426 ambulance missions in Shiraz EMS. One of these motors was settled in the arduous locations and the other in very crowded areas according to traffic statistical information. We compared the results of motorlance and ambulance missions with each other. There was a considerable decrease in referring patients to the hospital in motorlance missions, probably due to the right performance of the dispatch operators, and also the significant decreased arrival time in motorlance missions. Although we can improve the efficiency of the motorcycle ambulance through appropriate screening of age, sex, cause and time of the accident, teaching dispatch operator to select the appropriate cases for dispatching motorlances is one of the principles that should be considered. More precise information is needed to locate proper positions for motorlance missions and timely dispatching
