RESUMO

Anaemia is a severe public health problem amongst all vulnerable age groups in India. The National Nutritional Anaemia Prophylaxis Programme initiated in 1970, was revised and expanded to include beneficiaries from all age groups namely children aged 6-59 months, 5-10 yr, adolescents aged 10-19 yr, pregnant and lactating women and women in reproductive age group under the National Iron Plus Initiative (NIPI) programme in 2011. The dose of iron, frequency and duration of iron supplementation and roles and responsibilities of the functionaries were described. At present, the coverage of beneficiaries with iron and folic acid has been poor at the national level. The prevalence of anaemia has continued to remain high during the last 60 years, and there has been no significant change in the scenario due to various reasons. The constraints in implementation and measures to improve the NIPI programme are discussed in the current article.

RESUMO

Weekly iron folic acid supplementation (WIFS) programme is being implemented by Government of India for Prevention and control of anaemia amongst Adolescents since 2013. Under this program, enteric coated Iron and Folic Acid (IFA) tablet consisting of 100 mg of elemental iron with 500 mcg of folic acid, is given every week to all adolescent girls. Evidence suggests that there is a low absorption of iron from the enteric coated (EC) IFA tablets. The poor clinical response of EC IFA compared to the conventional sugar coated has also been reported The, present review describes the WIFS programme, cost, composition, evidence on absorption, clinical response on Hb from EC IFA tablets. There is an urgent need to take cognizance of available scientific evidence in to consideration and discontinue the EC tablets from WIFS programme.

Anemia is a condition in which the number of red blood cells (and consequently their oxygen-carrying capacity) is insufficient to meet the body’s physiologic needs. Specific physiologic needs vary with a person’s age, gender, residential elevation above sea level (altitude), smoking behavior, and different stages of pregnancy (1). Globally, anemia affects 1.62 billion people (95% CI: 1.50–1.74 billion), which corresponds to 24.8% of the population (95% CI: 22.9–26.7%). The highest prevalence is in preschool-aged children (47.4%, 95% CI: 45.7–49.1), and the lowest prevalence is in adult men (12.7%, 95% CI: 8.6–16.9%). However, the population group with the greatest number of individuals affected is non-pregnant women (468.4 million, 95% CI: 446.2–490.6) (2). The prevalence of anemia is estimated at 9% in countries with high development,in countries with low development, the prevalence is 43% (3).

RESUMO

Anaemia is a serious public health challenge in India with more than 50% prevalence across vulnerable groups such as pregnant women, infants, young children and adolescents. It has adverse effects on health, physical and mental productivity affecting quality of life. Guideline is any document containing recommendations about health interventions, whether these are clinical, public health or policy recommendations. The National Anemia Prevention and control guidelines have been developed taking cognizance of the current scientific evidence. The National Iron+ Initiative guidelines have been developed by the Adolescent Division of the Ministry of Health and Family Welfare (MoHFW), Government of India. Prevention and control of anaemia is one of the key strategies of the Health, Nutrition and Population Sector Programmes for reducing maternal, neonatal and childhood mortality and improving maternal, adolescent and childhood health status. It is estimated that anaemia causes 20 per cent of maternal deaths in India.

Aim: Hand hygiene has now been recognised as one of the most effective intervention to control the transmission of infections in a hospital and education is an important tool to ensure its implementation. In order to convince the users and as a part of education, it is important to generate evidence on the role of hand hygiene in reducing the bacterial fl ora on their hands. The present study was undertaken in a tertiary care hospital to demonstrate the presence of bacterial fl ora on the hands of healthcare workers (HCW) in different categories, to teach them proper hand hygiene technique using alcohol-based hand rub and determine the outcome for reduction of bacteria. Materials and Methods: A total sample size of 60 subjects including resident doctors, medical students, nurses and hospital attendants were included in the study after obtaining informed consent. Each person was educated on the technique of hand hygiene with alcohol-based hand rub and hand impressions were cultured before and after hand hygiene. All the subjects were also given a questionnaire to assess their perception on hand hygiene. The WHO posters on proper hand hygiene were displayed in the appropriate areas of the hospital in addition, as an educational tool. Results: Majority (42 out of 60) of the HCWs had bacterial count up to 100 colonies or more on both hands before the application of hand rub while working in the hospital. After use of alcohol hand rub with a proper hand hygiene technique, it was found that the percentage reduction was 95-99% among doctors and nurses, 70% among hospital attendants and 50% among sanitary attendants. Staphylococcus aureus was present on the hands of eight persons of which three were methicillin-resistant Staphylococcus aureus. Conclusions: The study demonstrates that transient bacteria are present on the hands of HCWs but majority could be removed by proper hand hygiene, which needs continuous education to be effective. It also shows that active education by demonstrating the proper hand hygiene technique and direct observation of the practices, though demanding, are ideal to understand the attitudes and practices of HCWs and helps in planning implementation strategies
